A smartphone camera can give you a rough, repeatable estimate of pupil diameter if you record video under controlled conditions and calibrate the image. A casual selfie or single flash photograph is not a clinically reliable measurement. Use the result to document what you observed for an eye-care professional—not to diagnose anisocoria or a neurological condition.
What a phone measurement can—and cannot—tell you
The pupil is the dark opening in the center of the iris. Its diameter changes continuously as the iris responds to light, focus and attention. A phone can capture the apparent edge of that opening, but the camera does not automatically know the real-world scale of the image. Without calibration, you can compare one eye with the other in the same frame, yet you cannot confidently state a diameter in millimetres.

A useful home record has three characteristics:
- Controlled conditions: the same room illumination, camera, distance, angle and gaze for every recording.
- Multiple frames: video or a sequence of images rather than one still photograph.
- A known scale: a safe reference in the same plane as the eye, or a validated geometric-calibration method.
The result is best treated as documentation of a trend or a change. It is not a substitute for a clinical pupillometer examination and cannot establish the cause of unequal pupils, an abnormal light response or a neurological emergency.

Normal pupils oscillate slightly even when lighting is steady. This movement, called pupillary hippus, can be about ±0.5 mm according to the American Academy of Ophthalmology’s EyeWiki (2025). A single frame may capture the pupil at its temporary maximum or minimum, so it is not a dependable estimate of the mean size. Current clinical pupillography systems record many frames and analyze the change over time instead of relying on one snapshot.

Exposure and processing create additional errors. A phone may brighten a dark scene, sharpen the iris edge or alter contrast differently on each model. Autofocus can shift between frames. Eyelid shadows can hide part of the pupil, and an oblique camera angle makes a circular pupil look elliptical. A flash or bright screen can constrict the pupil during capture, meaning the photograph records a light reaction as much as a resting diameter.
Prepare a safe, repeatable setup
Choose the room and adaptation period
Use a dim, steady room rather than changing the lights while recording. Let your eyes adapt for a short, consistent period before starting; use the same waiting time on every session. Do not compare a measurement made in daylight with one made immediately after entering a dark room. Keep the illumination identical for the left and right eyes.
Position the phone
Brace the phone on a stand or another stable support. The camera should face the eye as close to perpendicular as practical and remain at a fixed distance. Do not hold the phone in your hand while leaning toward the lens; small changes in distance and angle alter the apparent diameter. Keep your head still and look at a distant, neutral target so that accommodation and gaze do not change between frames. EyeWiki’s pupillography guidance emphasizes consistent distance and angle for this reason.
Avoid contact with the eye
Never place a ruler, card, calibration marker or other object against the cornea, eyelid or contact lens. If you use a printed target or millimetre ruler as a scale, hold it beside the face, flat and in the same plane as the eye, without touching or obstructing vision. Perspective error remains possible, so describe the reference and its position in your notes.
Record relevant conditions
Write down the phone model, camera used (front or rear), approximate camera-to-eye distance, room-light description, time, and any medication or eye drops. Drugs, decongestants, recreational substances and ophthalmic drops can change pupil size. This context is often more useful to a clinician than a single number.

Step-by-step smartphone workflow
- Set the light. Turn off changing sources such as a television or moving lamp. Use one dim, stable room-light arrangement and allow the same adaptation period before each recording.
- Mount the phone. Secure it so the lens is level with the eyes, perpendicular to the face and at a measured distance. Use the same camera and camera settings for both eyes and for later sessions.
- Set a distant target. Place a small, non-glowing target several metres away. Ask the person being recorded to look at it without shifting gaze. A nearby phone screen encourages accommodation and can alter pupil size.
- Start video. Record several seconds rather than taking one photograph. If possible, include both eyes in the same clip so they share lighting and exposure. Do not use a flash or shine a bright screen into the eye.
- Keep the face still. Blink normally, but avoid turning the head, squinting or moving closer to the camera. If an eyelid covers the pupil, discard that portion of the clip instead of forcing an estimate.
- Repeat for the other eye. If the camera cannot see both eyes clearly, reposition it without changing the room light, distance or target. Do not switch from a front camera to a rear camera halfway through a comparison.
- Add a scale when millimetres matter. Place a flat, known-size reference beside the face in the same plane as the eye, keeping it away from the cornea. A printed calibration target or ruler can be affected by perspective; record exactly where it was held. A validated app may perform geometric calibration, but its accuracy depends on the phone model and setup.
- Review paused frames. Step through the video and identify frames in which the pupil edge is visible and the gaze is steady. Estimate the dark opening from edge to edge, not the outer iris.
- Use many observations. Measure a series of frames for each eye. Report the median or a typical range, not the largest or smallest frame. Note frames affected by blinking, glare, eyelid shadow or focus hunting.
- Repeat on another day. For a question about asymmetry, repeat the same protocol under matched conditions. A difference that appears only once may reflect lighting, gaze, camera geometry or normal hippus.
How to estimate diameter from recorded frames
Pause the clip at a frame where the pupil boundary is sharp. Measure horizontally across the visible opening, or use two perpendicular measurements if the pupil is clearly elliptical. If the image includes a reference of known width, calculate a scale from the same plane: divide the reference’s known millimetres by its pixel width, then multiply the pupil’s pixel width by that scale. This simple proportion is valid only when the reference and eye are at essentially the same distance from the lens and the camera is not strongly angled.
When a reference is absent, you can still compare the two pupils in one frame as a relative ratio. For example, if one opening spans 180 pixels and the other 170 pixels under identical geometry, the first appears about 6% wider in that frame. Do not convert that ratio into a clinical threshold or assume the pixel-to-millimetre scale remains valid after moving the phone.
Make a small table in your notes with the frame time, left-eye estimate, right-eye estimate and any quality problem. Use the median of usable frames for each eye. Keep the individual values because a clinician may want to see whether the difference is stable or simply oscillation.
Interpreting an apparent difference between pupils
Compare eyes only when illumination, gaze, distance, angle and camera processing are matched. A pupil can look larger because one eyelid is casting a shadow, one eye is closer to the lens or the person is looking slightly to one side. Reflections from glasses and tear film can obscure the edge. Correct those technical issues and repeat before drawing attention to a small difference.
A persistent difference on repeated recordings is a reason to arrange an eye-care assessment, especially if the light response looks abnormal. The phone cannot determine whether the cause is benign physiological anisocoria, medication, an eye disorder or a neurological problem. Bring the original clips and your conditions log rather than relying on a transcribed number.

Accuracy limits you should expect
Lighting and light reflex
Changing illumination changes the pupil itself. A bright lamp, camera flash or phone screen can trigger constriction while the frame is being captured. Infrared illumination may be used by some front-facing facial-identification cameras, but consumer phones differ in whether and how they use infrared light. Therefore two models can produce different apparent sizes under the same room conditions.

The pupil is approximately circular, but an off-axis view projects it as an ellipse. A small tilt of the head or phone can make one eye appear different from the other. Autofocus, digital zoom, stabilization and computational sharpening can move or reshape the apparent boundary. Fixed mounting and a consistent distance reduce—but do not eliminate—these errors.
Biological variation
Hippus means that the true opening is moving while you measure it. Fatigue, attention, emotional state and near focusing can also alter the diameter. A median from many frames is more representative than a single value, but it remains an estimate under one set of conditions.
What the evidence says about smartphone systems
The paper “At-Home Pupillometry using Smartphone Facial Identification Cameras” describes a research system that combines an RGB selfie camera with a front-facing near-infrared facial-identification camera to track absolute dilation. It demonstrates a research direction, not proof that every consumer phone can make a clinical measurement.
UC San Diego’s DigiHealth Lab reports sub-millimetre accuracy for its prototype under controlled conditions. That performance should not be generalized to an uncalibrated phone, a different handset or a casual home setup.

A 2025 PubMed-indexed study, “Using smartphone pupillometer application to measure pupil size and light reflex: An unsuccessful prototype and analysis of the causes of failure,” found statistically significant disagreement between its prototype, penlight testing and a commercial pupillometer. App output should therefore not be treated as interchangeable with clinical equipment.

| Option | Calibration and repeatability | What it can measure | Main limitations |
|---|---|---|---|
| Manual phone video | Uses your camera geometry and a reference you provide; repeatability depends on mounting, lighting and frame selection. | Approximate diameter and a documented trend. | No universal scale, substantial exposure and perspective error, and manual measurements. |
| Consumer smartphone app | May automate detection or calibration, but accuracy is model- and app-dependent; no single app is established by the cited sources as universally validated. | Some apps attempt diameter or light-response estimates. | Validation, privacy practices, supported phones and data handling vary. An app result is not automatically clinical-grade. |
| Dedicated handheld pupillometer | Designed for controlled illumination and repeatable optical measurement; specifications and calibration differ by model. | Pupil size and, on suitable devices, dynamic light responses. | Cost, availability, regulatory status and calibration must be checked for the specific device. |
When evaluating any option, ask whether it has published comparison data against a clinical instrument, how it establishes a traceable scale, whether it measures both diameter and response dynamics, how repeatable it is across days, and what happens to recorded images. A low price or a polished interface is not evidence of medical validity.
Do not confuse pupil diameter with pupillary distance
Pupillary distance (PD) is the distance between the centers of the pupils, usually measured in millimetres for eyeglass fitting. It is not the diameter of either pupil. Cleveland Clinic notes that PD can be estimated at home with a millimetre ruler, mirror or smartphone app, while professional measurement is more accurate. A PD tool therefore cannot answer how wide an individual pupil is.
What common apps and attachments actually do
WHOeyes is an official World Health Organization app for near- and distance-visual-acuity checks. It is not a pupil-diameter meter, and the World Health Organization says it does not replace regular eye checks by an eye-care professional, even when vision seems good.

A smartphone fundus-photography attachment is also a different tool. EyeWiki’s smartphone funduscopy material concerns specialized retinal imaging with controlled alignment and lighting; it does not validate that attachment for measuring pupil diameter. Do not treat a generic ruler, a PD app, a fundus camera or a visual-acuity app as a validated pupillometer.

Troubleshooting poor or inconsistent measurements
The pupil edge disappears
Increase room light only if you can keep it stable and document the change; otherwise move the phone slightly, remove glare-producing glasses and wait for focus to settle. Never compensate by using a flash. Discard frames with blinking or eyelid shadow.
Left and right values change when you repeat the clip
Check that the phone has not moved, the person is looking at the same distant target and the room light has not changed. Measure a longer clip and use medians. If the discrepancy remains under matched conditions, preserve the clips and seek professional assessment.
The result changes with the camera
Different lenses and processing pipelines are not interchangeable. Choose one camera, disable automatic switching or digital zoom where possible, and use it for every session. Record the exact phone and camera so later measurements are not mistaken for a calibrated series.
A calibration ruler gives implausible numbers
The ruler may be closer to the lens than the eye, tilted, curved or partly hidden. Reposition a flat reference beside the face in the eye’s plane without touching the eye. If the geometry cannot be controlled, report a relative comparison instead of a millimetre value.
When a phone measurement should not wait for a routine appointment
Arrange an eye-care assessment for pupils that remain unequal when repeated under matched conditions, an unusual or unequal reaction to light, or a new change that you cannot explain. Seek urgent care for a new pupil-size change accompanied by severe headache, eye pain, drooping eyelid, double vision, confusion, weakness, or a recent head or eye injury. Do not spend time refining a phone measurement when those symptoms are present; the recording can support the history you give the clinician but must not delay evaluation.
What to bring to an appointment
Save the original video files, not only screenshots. Add the date and time, phone model and camera, camera-to-eye distance, room-light description, adaptation time, target distance, reference object and its placement, medication or eye-drop use, and the median or range you calculated for each eye. This lets the clinician judge the conditions and repeat a proper examination if needed.
