Rural patients can make specialty care more reachable by coordinating early with a local clinic, checking which referral options are covered and available, and matching telehealth or travel to the clinical need. Distance is only part of the problem: local provider shortages, cost, transportation, connectivity, and referral coordination can all block access. In the United States, solutions depend on local services, insurance rules, state licensing, and available technology.
Why specialty care can be hard to reach in rural areas
Rural residents may face long drives to hospitals, clinics, or universities that provide specialized care, alongside too few local clinics and clinicians. Limited access to medical technology and a stretched workforce add further constraints. The U.S. Department of Health and Human Services describes long distances as potentially prohibitive for some patients (HHS guidance on getting started with rural telehealth).

The Centers for Medicare & Medicaid Services (CMS) also identifies fragmented care, workforce shortages, unaffordable insurance, and limited specialty services as barriers. Transportation and difficulty engaging with health information can make it harder to complete a referral even when a specialist is available (CMS Rural Health Strategy).

Start by mapping the care need and available routes
Before choosing between a remote consultation and a trip, ask the local care team to clarify the clinical need and the practical options. Do not assume there is a universal wait time or travel-distance threshold; availability varies by specialty and location.

- What kind of assessment is needed? Ask whether the specialist can begin with a remote visit or whether an examination, imaging, lab work, or procedure requires in-person care.
- Which referral destinations are realistic? Confirm that the specialist accepts the patient’s insurance, is taking referrals, and can see the patient within a clinically appropriate timeframe.
- What will the full burden be? Consider appointment and travel costs, the number of likely trips, transportation options, and whether local testing or follow-up can reduce repeat travel.
- Can the patient use remote care comfortably? Check internet or cellular reliability, device access, privacy, language and accessibility needs, and whether a clinic can provide support or a private connection point.
- Will the information reach the local team? Ask how records, test results, and the specialist’s recommendations will be shared with the clinician coordinating ongoing care.
These questions are a practical way to compare choices, not a validated scoring tool. The treating clinician should guide decisions about urgency and whether remote assessment is clinically suitable.
Use the local care team to coordinate specialist input
A rural primary-care clinic may help prepare records, send a complete referral, track whether it was received, and coordinate local tests or follow-up. Some clinics can also connect patients with distant specialists through telehealth. Local clinicians can support continuity, but they do not replace specialty judgment when a specialist assessment is needed.
Provider-to-provider consultation and clinician education can extend a rural team’s access to expertise. HHS describes a Harvard Medical School Center for Primary Care hub-and-spoke project linking rural clinics with telemedicine and telementoring for education and quality improvement. These are project goals; they do not establish that every program reduces referrals or improves outcomes (HHS rural telehealth guidance).
Choose a telehealth format that fits the situation
Telehealth is not limited to a video appointment. HHS describes phone calls, remote patient monitoring, secure patient-portal messages, and video visits between patients and clinicians or between clinicians. A patient may be able to connect from home or, where available, from a local clinic or community center (HHS rural telehealth guidance).
Ask the care team which format is appropriate for the concern and what to do if the connection fails. A phone or message may suit some follow-up, while another concern may require video, an in-person examination, or testing. A local clinic connection can help when home service, privacy, equipment, or technical confidence is a problem.
Telehealth does not remove the need for staffing, dependable connectivity, suitable equipment, payment arrangements, or compliance with state licensing rules. CMS identifies reimbursement, cross-state licensure, and implementation costs as barriers. Coverage and requirements vary by payer, state, service, and date, so confirm them with the clinic and insurer before relying on a virtual visit (CMS Rural Health Strategy; CMS FY 2024 rural health year in review).

Plan for transportation, cost, and follow-through
Ask the clinic, insurer, health system, or local health department whether transportation assistance, referral coordination, or financial counseling is available. CMS identifies transportation as a patient barrier, but there is no single program available everywhere. Confirm local eligibility, service area, scheduling requirements, and any cost directly with the organization offering assistance.

1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhen a trip is needed, ask whether appointments can be coordinated on the same day, whether tests or follow-up can happen closer to home, and how the patient should contact the clinic if symptoms change while waiting. These arrangements depend on the clinicians and services available locally.
What clinics and policymakers can do
Individual workarounds cannot solve shortages or infrastructure gaps on their own. Rural clinics and health systems can strengthen access by building reliable referral workflows, maintaining telehealth staffing, supporting record-sharing, and partnering with regional specialty centers. Workforce recruitment and retention, broadband and equipment, patient education, and sustainable financing also shape whether services can be used.
Digital integration is one specific challenge. CMS reported that 48% of rural hospitals could fully participate in electronic health record (EHR) interoperability in 2024. That figure concerns rural hospitals’ ability to exchange health information—not household internet access or specialty access in general. CMS points to investment and maintenance costs, workflow automation, and staff participation as challenges (CMS FY 2024 rural health year in review).

Check the rules that apply to the specific visit
Federal telehealth policy and payer coverage can change. CMS’s FY 2024 report describes Medicare and Medicare Advantage changes for calendar years 2024 and 2025, including behavioral-health telehealth provisions. Those dated details should not be treated as a current eligibility or payment guarantee. Patients and clinics should confirm current coverage, billing requirements, and any in-person visit rules with the payer and the relevant state licensing authorities (CMS FY 2024 rural health year in review).
