Some soreness around the incision or implanted pulse generator can occur while a spinal cord stimulation (SCS) system heals. It should gradually improve according to the recovery plan from your implanting team. Call that team promptly for fever, redness, swelling, drainage, worsening or unusual pain, painful or changed stimulation, or a loss of therapy benefit. Do not rub, press, or manipulate the implanted parts through the skin. This guide covers SCS implants; if you have another nerve-stimulation device, follow its own manual and clinical instructions.

What pain around an SCS implant can mean

Pain after implantation has several possible explanations. A patient-facing Medtronic guide says the implant site may hurt and that recovery can take several weeks while the leads settle. Boston Scientific likewise describes incision soreness after surgery. Those statements describe possible recovery experiences, not a guarantee that every pain pattern is harmless or that everyone follows the same timetable.

Healing-related incision or pocket soreness

The cut used to place the system and the pocket that holds the implanted pulse generator can feel tender, tight, bruised, or sore as tissue heals. Movement may make the area more noticeable. Your surgeon’s instructions, rather than a generic calendar, determine what level and duration of discomfort is expected for your device and procedure.

Incision changes that may signal a complication

Redness, swelling, drainage, or fever are reasons to call the doctor’s office immediately under Boston Scientific’s patient guidance. New, worsening, or unusual pain also deserves prompt contact, especially when it appears with a visible incision change. Infection, lead movement, and implant-site pain are listed among potential problems in manufacturer safety information. You cannot confirm or rule out one of these causes by looking at the wound or feeling the implant yourself.

Stimulation-related discomfort or a change in benefit

Uncomfortable, jolting, or shocking stimulation is a recognized potential adverse event. A new sensation, a stimulation pattern that becomes painful, or a sudden change in coverage should be discussed with the device team. Reduced pain relief or a loss of therapy effect also warrants review. Do not assume that changing settings on your own will solve the problem; manufacturer guidance supports contacting the team for instructions.

Pain from another source

Back, limb, muscle, nerve, or unrelated medical problems can occur at the same time as implant recovery. The implanting clinician needs your history and an examination to determine whether the discomfort fits normal healing, a device issue, another condition, or more than one factor. Treat this article as preparation for that conversation, not as a diagnosis.

How to think about the recovery timeline

“Several weeks” is a general description, not a promise that soreness will last exactly that long or that it will disappear on a particular day. Healing can feel uneven: an area may be more noticeable after activity and quieter with rest, or a symptom may change as the leads settle. What matters is the pattern your team told you to expect and whether the overall trend is improving. A symptom that is steadily worsening, becomes distinctly different, or appears after an impact should be reported rather than explained away as normal healing.

Some people expect pain relief as soon as the implant is placed. Medtronic notes that the system may not be turned on until recovery, so the operation and the therapeutic programming phase are not necessarily the same event. Ask when your trial or permanent system will be activated, what sensations are expected during programming, and how the team wants you to report benefit. Do not judge the final effectiveness of treatment from the first postoperative days alone.

Keep the scheduled follow-up even if the incision feels better. The visit allows the clinician to inspect healing, review the lead and pulse-generator sites, discuss your pain pattern, and adjust or confirm programming when appropriate. Contact the clinic sooner if the instructions you received list a specific symptom or if the change is substantial.

What to do during the first days and weeks

Use the pain plan supplied for your operation

There is no single medication, dose, icing schedule, dressing routine, or wound-cleaning method that applies to every SCS patient. Boston Scientific’s patient recovery guidance states: “Discuss the plan for surgical pain management with your doctor.” Ask which medicines are appropriate for you, how to take them, what side effects to watch for, and whom to call if they do not control the pain or cause problems. Do not add, stop, or combine pain medicines unless your clinician or pharmacist tells you to.

Get specific incision instructions before you leave

Ask your team how the incision should be kept clean and dry, whether a dressing should remain in place, when follow-up is due, and what changes require a call. These details can differ by closure method, procedure, device, and local protocol. If written instructions conflict with a general webpage, follow the instructions for your operation and ask the clinic to clarify.

Respect movement restrictions

Manufacturer guidance advises avoiding activities that stress the implanted system, including bending, twisting, and lifting during early healing. Boston Scientific describes particular restrictions for a period after surgery, while other manufacturers and clinicians may use different limits. Do not adopt a universal number of days or a weight limit from someone else’s experience. Ask exactly when you may resume driving, work, exercise, housework, bathing, and other activities, and whether each needs a gradual return.

Restrictions are about more than avoiding pain. Repeated bending, twisting, lifting, impact, or reaching can place stress on healing tissues and the implanted system even when the incision looks closed. Break questions into specific tasks: carrying groceries, caring for a child or pet, getting in and out of a car, sleeping on one side, or sitting for a long period. Your team can tell you which movements are acceptable now and how to progress.

Understand when therapy may start

Medtronic notes that the system may not be turned on until recovery and that the leads need time to settle. Therefore, pain relief may not begin immediately after implantation. Follow the activation and programming schedule given by your team. A lack of immediate relief after surgery is not, by itself, proof that the implant has failed; report your experience at the planned review or sooner if the team told you to call.

Plan for practical comfort without improvising treatment

Arrange help with tasks that involve lifting, bending, twisting, or prolonged activity until you have clearance. Keep frequently used items within easy reach so you are not repeatedly stretching toward them. Choose clothing and seating that do not press on the pocket, and change a position that puts sustained pressure on the implant. These are ways to follow movement and pressure precautions, not substitutes for the instructions from your surgeon.

If discomfort makes sleep, walking, eating, or basic self-care difficult, tell the clinic. Do not wait for a routine visit simply because the incision looks normal; the team may need to review your medication plan or examine you. Conversely, do not borrow another patient’s medication, dressing method, exercise plan, or device settings.

Protecting the implanted system

Keep pressure and friction off the implant

Do not rub, massage, press, or repeatedly poke the pulse generator or lead path through the skin. Abbott warns that pressure can dislodge a lead, alter implant position, or damage the skin and create further problems. Avoid tight equipment or activities that push directly on the pocket unless your clinician has approved them. If a strap, waistband, chair edge, or sleeping position repeatedly presses the area, ask the team for a safe modification rather than testing it yourself.

Watch for a new prominence, change in the shape or position of the pocket, skin that becomes thin or irritated over the device, or pain that follows repeated pressure. These findings do not prove that anything has moved, but they are useful details to report. Never try to push the generator back into place or reposition a lead yourself.

Handle the programmer and charger as directed

Programming, charging, and other controls are device-specific. Use the manual supplied for your model and the instructions from the device team. If a setting, charge, alert, or controller behavior is unfamiliar, call for guidance. Do not open, reposition, or attempt to repair any implanted or external component.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep the device identification card and clinic contact information accessible. When you call, provide the model or system name if you know it, the program or setting in use, and whether the change began after charging, programming, movement, or an impact. This information helps the team give precise instructions.

Return to activity only with clearance

Healing restrictions protect the incision and the leads. Even if the skin looks better, deeper tissues may still be recovering. Ask whether a planned activity involves bending, twisting, lifting, reaching, impact, or sustained pressure near the implant. Increase activity only in the sequence your clinician approves, and report pain that rises rather than settles after an activity.

Keep a simple symptom record

Write down when the pain began, where it is, what it feels like, its trend, and what you were doing when it changed. Note temperature if your care team has asked you to monitor it, visible incision changes, stimulation settings or programs, and whether therapy still helps. A short record can help the team distinguish a steady healing pattern from a new change and can make a telephone assessment more efficient.

A practical decision path when discomfort changes

  1. Look for warning signs. Check for redness, swelling, drainage, fever, or a rapidly worsening symptom. Call immediately when your clinic’s instructions say to do so.
  2. Stop the provoking pressure or activity. Move away from a tight strap, hard edge, strenuous movement, or direct contact with the pocket. Do not manipulate the implant.
  3. Follow only your prescribed plan. Use medicines and wound care exactly as instructed. Do not add a product or change a dose because a friend or online post recommended it.
  4. Record what changed. Note timing, location, severity, stimulation quality, recent activity, and any change in therapy benefit.
  5. Contact the implant team. Ask whether you need an examination, programming review, or another form of assessment. Use urgent care or emergency services for severe or rapidly changing symptoms.

When to contact the implant team

Call immediately for incision or systemic warning signs

  • Redness at the incision.
  • Swelling at or around the incision or pocket.
  • Drainage from the incision.
  • Fever.

Boston Scientific’s patient guidance specifically advises calling the doctor’s office immediately for these findings. Use the contact route your clinic supplied; do not wait for a routine appointment if the instruction says to call immediately.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Call promptly for a concerning pain pattern

Contact the team for pain that is new, worsening, unusually intense, or different from the soreness you were told to expect. Pain that starts after a bump, strenuous movement, or pressure over the implant should also be reported. Explain the timing and trend rather than trying to label the cause as a displaced lead or infection.

Call when stimulation changes or becomes painful

Report jolting, shocking, uncomfortable, or newly painful stimulation; a change in where stimulation is felt; an unexpected increase or decrease in intensity; or a loss of the usual therapy benefit. The team may need to review programming, examine you, or assess the hardware. Manufacturer guidance does not support a blanket instruction to turn settings up, turn them down, or switch programs without advice.

Use urgent care for severe or rapidly changing symptoms

If symptoms are severe or changing rapidly, contact your treating team or appropriate urgent care rather than relying on general information. If you believe you are experiencing an emergency, use local emergency services. Bring your device identification and medication information if it is safe to do so, and tell clinicians that you have an implanted nerve-stimulation system.

What the clinical team may evaluate

A clinician can inspect the incision and pocket, ask about the pain’s timing and quality, review stimulation changes, and check whether therapy is still helping. Depending on the findings, the team may consider healing, infection, lead movement, implant-site pain, programming issues, or a separate medical cause. Manufacturer safety information lists these as possibilities, not as a way for patients to diagnose themselves. The appropriate next step depends on the examination and your specific system.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Be prepared for the clinician to ask about recent activity, pressure, falls, medication use, and exactly when stimulation changed. An assessment may focus on the wound, the position and condition of the pocket, neurologic symptoms, or programming. The team—not an online checklist—decides whether observation, a programming adjustment, testing, or another intervention is appropriate.

Why your device manual matters

Restrictions and safety instructions can vary by manufacturer, model, region, and manual revision. The patient pages used for this overview come from Medtronic, Boston Scientific, and Abbott, and their wording is not a substitute for the instructions supplied with your system. Keep the manual, implant identification card, clinic phone numbers, and programming information where you can find them. At follow-up, ask whether any updated instructions apply to your model.

If you received a trial system before a permanent implant, do not assume the same care rules apply. Confirm which components are temporary or permanent, how to manage the external equipment, and which number handles after-hours problems. Tell every clinician who treats you that you have an implanted SCS system, especially before a procedure or imaging appointment; follow the device-specific instructions they provide.

Questions to ask before leaving the clinic

  • Which pain symptoms are expected for my operation, and what trend should I look for?
  • What is my exact medication plan, including what to do if it is ineffective or causes side effects?
  • How should I care for the incision, dressing, and skin around the pocket?
  • Which movements, lifting tasks, sleeping positions, and activities are restricted, and for how long?
  • When will the system be activated, and whom do I call if stimulation is painful or changes?
  • What findings require an immediate call, a same-day call, or urgent care?
  • What information should I have ready when I contact the device team?
  • How should I manage charging, programming alerts, or a controller that does not behave as expected?
  • Who handles questions after office hours, and where should I go if symptoms become severe?

Common situations and the safest next step

Situation What it might represent Recommended action
Mild incision or pocket soreness during the expected recovery period Healing after surgery is possible; manufacturer guidance does not establish that every pain pattern is normal. Follow the operation-specific pain and activity plan, and mention the symptom at follow-up or sooner if it worsens.
Redness, swelling, drainage, or fever A possible incision complication, including infection. Call the doctor’s office immediately as directed by Boston Scientific’s patient guidance.
New, worsening, or unusual pain Could be healing, implant-site pain, lead movement, infection, or another cause. Contact the implant team promptly; do not attempt to diagnose or reposition anything.
Jolting, shocking, or painful stimulation A recognized potential adverse event or a programming or hardware issue. Stop guessing at settings and contact the device team for instructions.
Less relief or a sudden loss of therapy benefit A therapy or device change that needs clinical review. Report when it began, what changed, and the program or setting in use.
Pain after pressure, rubbing, a bump, or strenuous movement Possible irritation or stress on the healing system. Remove the source of pressure, avoid further strain, and call for personalized advice.

How to describe the problem on the phone

Start with the procedure date and device type if known. Then give the location of the pain, whether it is superficial at the incision or deeper at the pocket, its severity and direction of change, and any redness, swelling, drainage, or fever. Describe stimulation precisely: where it is felt, whether it is painful or shocking, and whether the therapy benefit changed. Mention recent bending, twisting, lifting, pressure, or impact, along with the medicines you have taken under your prescribed plan. This information helps the team decide how quickly you need assessment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What not to do

  • Do not press, rub, massage, or manipulate the implanted components.
  • Do not assume a universal recovery timetable, lifting limit, or wound-care routine applies to your device.
  • Do not change programming or stimulation intensity independently when stimulation becomes painful or unusual unless your team has given you a specific protocol.
  • Do not treat a fever, drainage, increasing redness, or swelling as routine soreness.
  • Do not use another patient’s medication or recovery advice in place of your own instructions.

Key takeaway for SCS recipients

Incision and pocket soreness can occur for several weeks as an SCS system heals, and therapy may not be activated immediately. Protect the system by following your team’s movement limits and avoiding pressure over the implant. The threshold for a call is low: report warning signs at the incision, fever, worsening or unusual pain, painful or changed stimulation, or reduced benefit. Your implanting team is the only reliable source for a diagnosis and for device-specific instructions.

Frequently Asked Questions

Is pain around a spinal cord stimulator implant normal?

Incision or pulse-generator soreness can occur while the surgical sites heal, and Medtronic notes that recovery may take several weeks. Pain that is worsening, unusual, or accompanied by redness, swelling, drainage, fever, stimulation changes, or reduced benefit needs prompt clinical review.

How long should soreness last after SCS surgery?

Manufacturer guidance describes recovery over several weeks but does not set one timetable for every person or device. Follow your surgeon’s plan and report a symptom that is not improving, changes character, or becomes more intense.

Can I massage or press the implanted pulse generator?

No. Abbott warns that pressure can dislodge a lead, alter implant position, or damage the skin. Avoid rubbing, massage, and repeated poking, and ask the device team how to modify clothing, seating, or sleeping positions that press on the pocket.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should I do if stimulation feels shocking or painful?

Contact the device team for instructions. Jolting, shocking, or uncomfortable stimulation is a recognized potential adverse event, and you should not independently change intensity, programs, or other settings unless your team has given you a specific protocol.

Can I expect pain relief immediately after implantation?

Not necessarily. Medtronic notes that the system may not be turned on until recovery and that leads need time to settle. Ask your team about activation and programming, and report a loss of benefit or other concerning change.

Which incision symptoms require an immediate call?

Boston Scientific’s patient guidance says to call the doctor’s office immediately for redness, swelling, drainage, or fever at the incision. New or worsening pain, especially with one of these findings, also warrants prompt contact.

What if I have a different nerve-stimulation implant?

Use that device’s manual and your own implanting team’s instructions. Activity limits, programming, charging, and warning signs can vary by device, model, region, and manual revision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What information should I have ready when I call?

Give the procedure date and device type if known, pain location and trend, incision findings, temperature if your team asked you to monitor it, recent pressure or strenuous movement, stimulation changes, program or setting, therapy benefit, and medicines taken under your prescribed plan.

The Bottom Line

Some SCS incision or pocket soreness can occur during healing, but worsening pain, fever, incision changes, painful stimulation, or reduced benefit should prompt a call. Protect the implant from pressure and follow your own team’s medication, wound-care, activity, and programming instructions.
