The safest osteoarthritis (OA) plan is usually a combination of joint-appropriate movement, strengthening, practical support, and carefully selected symptom treatment. The right mix depends on the affected joint, your diagnosis, other health conditions, current medicines and supplements, daily goals, access and cost—not on a universal “best” treatment.
Before discussing a new option, bring a short symptom record and a complete medicine list. Ask what benefit to expect, how and when to try the treatment, which side effects or interactions matter for you, what it will cost, and when you will review the plan. Never combine anti-inflammatory medicines or exceed label directions without professional advice.

What safe osteoarthritis management involves
Osteoarthritis is diagnosed from your symptoms, examination and, when useful, imaging or other tests. The National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS) states: “There is no single test for osteoarthritis.” That matters when a new treatment is proposed: the plan should fit the joint involved, the certainty of the diagnosis, the pattern of pain and stiffness, and the effect on your function.

Pick the symptom - the matching free tool is one click away.
Tell the clinician what you want to regain or preserve. A goal might be walking to work, climbing stairs, sleeping through the night, opening jars, returning to a sport, or reducing reliance on a support. Pain intensity is important, but so are stiffness, swelling, endurance, balance, confidence and the activities you have stopped doing. A treatment that lowers pain but does not improve a meaningful activity may need adjustment; a modest pain change that restores function may be worthwhile.

OA care is generally staged and revisited. Movement and self-management are foundations. Weight management may help some people, especially when knee or hip symptoms are affected by load. Supports, heat or cold and selected medicines can complement those measures. Injections or other prescription options may be considered for particular joints and circumstances. Your clinician should explain the intended trial, likely benefit, risks, alternatives and follow-up rather than presenting any medicine as risk-free.
Start with movement, strengthening and self-management
NIAMS describes exercise as a way to reduce pain and stiffness and improve flexibility, muscle strength and endurance. A well-rounded plan can include several types of activity, introduced gradually:
- Range-of-motion exercises to maintain or improve joint movement.
- Strengthening with weights or exercise bands to support the joint and improve function.
- Water exercise or water aerobics, which can make movement easier for some people.
- Low-impact activities such as walking, cycling, swimming or tai chi.
- Balance activities when steadiness is a concern.
Start below the level that reliably triggers a flare and build slowly. A physical therapist can adapt the movements to the joint, your strength and any balance or medical limitations. Ask your doctor or therapist how to distinguish expected muscle soreness from a symptom that means you should stop and seek advice. Exercise does not require a particular branded product; the useful choice is the activity you can perform safely and continue.

Make the plan fit real life
Ask for a schedule you can attach to existing routines, such as a short mobility session after waking and strengthening on selected days. Track whether the plan changes walking distance, stair use, sleep or hand function, not only whether pain changes immediately. If pain or fatigue repeatedly prevents participation, report that rather than abandoning the plan silently; the intensity, exercise selection or diagnosis may need review.
Consider weight management without blame
If you are overweight or have obesity, weight loss may reduce stress on weight-bearing joints and can improve pain and mobility, particularly for knee or hip OA. It is not automatically relevant or appropriate for every person. A respectful discussion should cover your goals, nutrition, mobility, medical conditions, eating history and access to support—not assume that body size explains every symptom.

Ask what outcome would count as success for your joint and how progress will be assessed. Weight management can be one part of care alongside strengthening, activity and symptom treatment; it should not be used to dismiss pain or delay evaluation of another condition.
Use practical supports for a specific task
Supports work best when linked to a clearly identified functional problem:
Recommended Free Tools
- A brace or orthotic may help selected joint problems when prescribed and fitted by a qualified professional.
- A cane or walker can improve stability and mobility when balance or load tolerance is limited. A clinician or therapist can check height, technique and whether the device is appropriate.
- Heat or cold may ease symptoms for some people. Ask how to use it safely for your skin, circulation and sensation.
- Hand-grip aids such as jar openers can reduce strain during a painful task.
Tell the clinician which activity you want the support to enable. A device that is uncomfortable, poorly fitted or used for the wrong joint can add burden without improving function.
Discuss medicine options as individualized trials
Medicines may include oral pain relievers or anti-inflammatory medicines, topical creams, rubs or sprays, selected injections and other prescription options. The choice depends on the affected joint, previous response, other medical conditions, allergies, current medicines and your priorities. A clinician or pharmacist should determine whether an option is suitable and explain the current label instructions.
| Option category | Where it may fit | What to clarify at the visit |
|---|---|---|
| Oral pain reliever | May be considered for broader or episodic symptoms when the clinician judges it appropriate. | Expected change in pain or function, duration of a trial, organ-related risks, interactions and what monitoring is needed. |
| Oral anti-inflammatory medicine (NSAID) | May reduce inflammatory pain for selected people. | Heart, blood-pressure, kidney, stomach and bleeding risks; interactions; shortest appropriate trial; and what to do if symptoms or side effects occur. |
| Topical medicine | Can be useful when the painful joint is accessible to topical treatment, such as a knee or hand for products specifically labeled for OA. | Exact joint and label directions, skin precautions, whether another NSAID is being used, and whether your medical history changes the balance of benefit and risk. |
| Injection or other prescription option | May be considered for selected joints and situations after discussing diagnosis, goals and alternatives. | How the procedure works, expected duration of benefit, procedure risks, cost, access and the next step if relief is limited. |
Ask for a defined review point rather than taking a treatment indefinitely without checking its value. The review should consider function, side effects, adherence, affordability and whether the original goal was met.

Understand NSAID risks before taking an anti-inflammatory
The American College of Rheumatology (ACR) patient guidance lists possible NSAID harms including stomach upset, ulcers or bleeding, kidney problems, high blood pressure or heart problems, fluid retention, rashes and allergic reactions. Each NSAID has its own dosing instructions. Do not combine NSAIDs or take more than the recommended amount unless a healthcare professional specifically directs you.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesFDA medication guidance warns that NSAIDs can increase the chance of heart attack or stroke. Risk can rise with longer use and in people with heart disease. NSAIDs can also cause ulcers and bleeding in the stomach or intestines at any time, potentially without warning symptoms. Gastrointestinal risk is affected by factors such as longer use, older age, poor health, smoking, alcohol, and use of corticosteroids or anticoagulants; this list is not exhaustive.
Bring the information that changes the risk assessment
Tell the doctor or pharmacist about heart disease, high blood pressure, kidney problems, previous ulcers or bleeding, allergies, asthma reactions, pregnancy or plans for pregnancy when relevant, and any prior reaction to an anti-inflammatory. Provide every prescription, over-the-counter medicine, vitamin, herb and supplement, including products you take only occasionally. Ask specifically whether an existing pain reliever, blood thinner, steroid or another NSAID changes the plan.
Do not assume an over-the-counter product is automatically safer for you. Do not start, stop, combine or extend an NSAID course based only on a friend’s experience or an old prescription. If you develop a concerning reaction, follow the product’s current safety instructions and contact a healthcare professional promptly.
What to know about topical diclofenac
Topical diclofenac is one clinician-selected option for OA pain. FDA-approved labeling for Voltaren Gel identifies diclofenac sodium topical gel 1% for OA pain in joints amenable to topical treatment, including knees and hands. The label still carries serious cardiovascular and gastrointestinal warnings because topical administration does not mean zero systemic risk.

Before using it, confirm that the painful joint is one covered by the current label and that the product is appropriate for you. Check the complete, current instructions rather than relying on a remembered schedule. Tell the clinician or pharmacist about oral or topical NSAIDs, allergies, heart or stomach history, kidney problems, anticoagulants, corticosteroids and other medicines. Ask what to do if it does not help or if you notice a reaction. Availability without a prescription does not remove the need for a personal risk check.
Prepare a concise record for the appointment
NIAMS advises tracking symptoms and taking a list of medicines and supplements to the doctor. A brief record for one or two weeks can make the visit more productive:
- Which joint hurts, and whether one or both sides are involved.
- Pain, stiffness and swelling, including when they occur and how long stiffness lasts.
- Activities you cannot do, do more slowly or avoid.
- Effects on sleep, work, exercise, self-care and other daily tasks.
- What worsens or eases symptoms, including movement, rest, heat or cold.
- Every treatment already tried, how long you tried it, the benefit and any side effect.
- Your priorities, such as improved walking, grip, sleep or ability to participate in a valued activity.
Bring medicine containers or an up-to-date list with the name, strength, how often you take it and why. Include supplements and occasional products. If possible, bring someone who can listen and take notes, as ACR communication guidance suggests.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Questions to ask about a new treatment
Use questions that turn a general recommendation into a safe, measurable plan:
- What is the goal of this treatment for my specific joint and symptoms?
- What improvement should I look for, and when should we reassess?
- How should I use or try it, and for how long before deciding whether it helps?
- Which side effects, allergies or interactions matter given my health history and medicines?
- Could this interact with another pain reliever, anti-inflammatory, steroid, anticoagulant or supplement I use?
- What alternatives are reasonable if I prefer to avoid this option, cannot afford it or cannot access it?
- How does this fit with exercise, strengthening, weight management or a brace?
- What should I do if it does not help, causes a problem or my function worsens?
- What will the total cost be, including visits, therapy, equipment or monitoring?
- Who should I contact between visits, and what symptoms require prompt advice?
Ask the clinician to explain unfamiliar terms in plain language and to write down the agreed plan. Confirm which treatment you are stopping, continuing or adding so that you do not accidentally duplicate medicines.

How to assess emerging OA treatments in 2026
Treatment developments should be interpreted in the context of your joint, diagnosis, health history and goals. A September 24, 2026 ACR news summary describing an updated 2026 OA guideline for knee, hip and hand OA reports continued support for regular exercise, self-management education, weight loss when appropriate, anti-inflammatory medicines and corticosteroid injections for selected patients. It also reports conditional recommendations for GLP-1 receptor agonists for some people with knee OA who may benefit from weight management, and methotrexate for erosive hand OA.
That summary is not a substitute for a personal assessment or for the complete guideline text. It does not establish that either newer option is appropriate, effective or available for you. Ask whether your diagnosis and health profile match the population considered, what evidence supports the option, what monitoring is required, how cost and access work, and what established alternatives remain available.
The 2026 VA/DoD clinical practice guideline for nonsurgical hip and knee OA compares topical diclofenac and oral NSAIDs and characterizes topical NSAID gastrointestinal and cardiovascular risk as lower than oral NSAID risk. “Lower” is a relative comparison, not a guarantee of safety; FDA labeling still warns about serious adverse events with NSAIDs. Your clinician should use the comparison as one factor alongside your own history and medicines.

| Decision question | Why it matters | What to ask |
|---|---|---|
| Is it suitable for this joint? | A treatment may be designed for an accessible hand or knee joint, a weight-bearing problem, or a particular diagnosis; suitability is not interchangeable. | Is this option studied, labeled or recommended for my joint and diagnosis? |
| What benefit is realistic? | Pain relief, stiffness, walking, grip and sleep may change at different rates. | What specific function should improve, and by when? |
| What risks and interactions apply? | Age, heart, kidney, stomach, blood-pressure and bleeding history—and other medicines—can change the balance. | Which personal factors make this safer or less suitable? |
| What is the burden? | Daily application, exercise visits, fitting, injections, monitoring and travel affect whether a plan is sustainable. | How much time, equipment, follow-up and assistance will it require? |
| What will it cost and how will I access it? | Insurance, prescription status, therapy availability and transport can determine whether you can follow through. | What is covered, what alternatives cost less, and who can help with access? |
| How does it support function? | The strongest plan usually helps you keep moving and performing valued activities safely. | How should I combine it with strengthening, mobility work or a support? |
This framework avoids ranking treatments for everyone. It helps you and the clinician choose an option that is useful, tolerable and practical for your circumstances.
Set a follow-up and adjust safely
Before leaving, agree on what you will monitor, when you will review it and what happens if the first plan fails. Bring the same symptom measures to the follow-up so you can compare function, sleep and activity with your starting point. If the treatment is unaffordable, difficult to use or incompatible with your routine, say so; those are treatment-design problems, not personal failures.
Contact the prescriber or pharmacist when you are unsure whether two products overlap, when a new medicine is added by another clinician, or when a side effect appears. Keep an updated medicine list for every appointment. If the diagnosis, swelling pattern, function or response does not fit the expected OA course, ask whether the assessment should be revisited rather than repeatedly adding treatments.
Safe OA care is an ongoing conversation: define the function you want back, combine movement with appropriately selected supports, check medicine risks before starting, and review whether the plan is delivering enough benefit to justify its burden.
