Nausea, vomiting, diarrhoea and constipation are common digestive side effects of GLP-1 medicines. Smaller meals, bland foods and steady fluid intake may make symptoms easier to manage, but persistent or severe symptoms need medical advice. Do not change or stop a prescribed dose without guidance from your prescriber.
Why GLP-1 medicines can upset your digestion
These medicines slow digestion, which can contribute to nausea, feeling full, heartburn, vomiting, diarrhoea or constipation. In UK safety guidance, the Medicines and Healthcare products Regulatory Agency (MHRA) says gastrointestinal effects may affect more than 1 in 10 patients and can last several days. They are more likely near the start of treatment or after a dose increase. That is a broad frequency statement, not a precise rate for every medicine or patient group. MHRA guidance

In October 2024, the MHRA reported 7,228 Yellow Card reports of nausea, vomiting and diarrhoea associated with GLP-1 receptor agonists indicated for weight management, as of 28 October 2024. This is a count of reports, not an incidence rate or proof that a medicine caused each event. MHRA Drug Safety Update
What to eat and drink when you feel nauseated
Guy’s and St Thomas’ NHS Foundation Trust advises that nausea, heartburn and vomiting often ease within 3 to 4 weeks of starting treatment or increasing a dose. This is general patient guidance, not a promise that symptoms will resolve on that schedule. The trust also notes side effects can be more intense during the first six months and feel worse just after a dose. Guy’s and St Thomas’ side-effect guidance

- Help relieve nausea caused by GLP-1 medications with Advanced Herbals Ginger Chews
- Try small meals more often rather than large meals.
- Choose bland, dry foods such as crackers or dry toast. Other plain options include porridge, rice, pasta, potatoes, eggs, plain yogurt, tofu, bananas, cucumber, melon and steamed vegetables.
- If it feels better, choose cold foods rather than hot foods.
- Drink fluids regularly. Avoid fatty or strong-smelling foods if they worsen nausea, and avoid lying down soon after eating.
Managing constipation
To help with constipation, the trust suggests drinking fluids, adding fibre gradually through food, and moving where possible, such as taking a short walk. Fibre-containing choices include fruit, vegetables, whole grains, beans and lentils; it also lists kiwi fruit, chia or flax seeds, and dried fruit. No particular food or amount works for everyone. Contact your care team if symptoms worsen.
Managing diarrhoea and fluid loss
The trust recommends plenty of low-calorie fluids and smaller, more frequent meals for diarrhoea, which may improve as the body adjusts. Vomiting and diarrhoea can cause dehydration, so fluid advice is not a substitute for medical help when you cannot keep fluids down or develop warning signs. When to contact a doctor or seek urgent help
Contact your GP or care team
Get in touch if side effects worsen, are persistent, or make it difficult to eat or drink. Guy’s and St Thomas’ advises contacting a GP or care team for vomiting more than three times a day, being unable to keep fluids down, dark urine or lightheadedness. These may indicate dehydration or that the medicine is not suitable for you.

Do not stop, skip, lower or retime doses as a general self-care measure. The NHS semaglutide information says, “Keep taking your medicine unless you’re advised to stop,” and recommends speaking to a pharmacist or doctor about bothersome or persistent effects.
