How to Manage Nausea and Other Common Side Effects of Semaglutide

Nausea, vomiting, diarrhea and constipation are common gastrointestinal effects reported with semaglutide. The safest approach is to use simple food and eating adjustments, watch for fluid loss, and involve your prescriber or pharmacist when symptoms are severe, persistent or difficult to manage. Do not raise, lower, skip or repeat a dose on your own, and do not assume advice for one semaglutide product automatically applies to another.

Which side effects are common with semaglutide?

Semaglutide products can cause gastrointestinal symptoms, particularly nausea, vomiting, diarrhea and constipation. Other reported effects include abdominal pain, upset stomach, bloating, belching, gas and heartburn. Fatigue, dizziness and other effects are also listed in product information. This list is not exhaustive, and a symptom alone cannot establish its cause.

Product labeling is not interchangeable. Wegovy, Ozempic and other semaglutide products have different approved uses, instructions and patient materials. The practical suggestions below draw partly from the U.S. Wegovy patient guidance, so treat them as discussion points rather than universal instructions for every formulation, dose or country.

Symptom General discussion points Contact a healthcare professional
Nausea Consider bland, low-fat foods, slower eating, foods containing water, fresh air and avoiding greasy foods or lying down after meals. Ask whether smaller meals or stopping when full could help. If it is severe, persistent, interferes with eating or drinking, or bothers you and does not go away.
Vomiting Focus on the possibility of fluid loss and tell your clinician if you cannot keep fluids down. Do not create a fixed rehydration or anti-vomiting regimen without individualized advice. Promptly for persistent or extended vomiting, difficulty maintaining fluids, or symptoms that concern you.
Diarrhea Recognize that ongoing diarrhea can deplete fluids. Ask your clinician or pharmacist what is appropriate for your medical history and other medicines. Promptly for persistent or extended diarrhea or possible dehydration.
Constipation Ask what dietary priorities, such as fiber, and activity changes are suitable for you. There is no single fiber target or laxative plan for everyone. When it persists, becomes severe, causes significant pain or does not respond to the plan your clinician gives you.
Other gastrointestinal effects Record abdominal discomfort, bloating, belching, gas, heartburn or upset stomach and discuss patterns with your care team. For severe or persistent symptoms, or any effect that bothers you or does not go away.

What to do when symptoms start

  1. Identify the symptom and its trend. Note whether you have nausea, vomiting, diarrhea, constipation or another effect, when it began and whether it is improving, stable or worsening.
  2. Check whether you can eat and drink. Vomiting and diarrhea can cause fluid depletion. Tell a healthcare professional if you cannot keep fluids down or if ongoing losses make drinking difficult.
  3. Use conservative food and behavior measures. For nausea, the U.S. Wegovy patient page suggests discussing bland, low-fat foods such as crackers, foods containing water such as soup or gelatin, slower eating, fresh air, avoiding greasy foods and not lying down after meals. These are examples to discuss, not guaranteed treatments.
  4. Keep the prescribed schedule unless your prescriber changes it. Do not take an extra dose, repeat a dose, lower it, raise it or skip it to manage a side effect without product-specific medical advice.
  5. Contact the right professional. Your prescriber or pharmacist can interpret symptoms in light of your product, dose, other medicines, kidney health, digestive history and hydration status.
  6. Escalate persistent or severe symptoms. The U.S. prescribing information advises prompt contact for persistent or extended nausea, vomiting or diarrhea and warns that fluid depletion can contribute to kidney problems.

Managing nausea while taking semaglutide

Try small, gentle eating changes

Novo Nordisk’s U.S. Wegovy guidance frames nausea questions for discussion with a healthcare professional. Examples include choosing bland, low-fat foods such as plain crackers, eating more slowly, stopping when you feel full and asking whether smaller, more frequent meals might be appropriate. A food that is tolerated at one stage may be less comfortable later, so use your symptoms and your clinician’s advice rather than forcing a particular menu.

Foods that contain water, including soup or gelatin, are also named as examples in the Wegovy guidance. They are not a substitute for professional assessment when vomiting or diarrhea is causing fluid loss. If you have dietary restrictions, diabetes-related concerns, kidney disease or another condition that affects food or fluid choices, ask your clinician or dietitian what fits your plan.

Reduce common meal-related triggers

The same patient guidance suggests avoiding greasy foods, getting fresh air and not lying down after eating. These steps are low-risk discussion points, but they are not promises that nausea will stop and they do not establish a treatment for every semaglutide formulation. Tell your care team if nausea continues despite practical changes or prevents normal intake.

Do not assume a fixed duration

The manufacturer says nausea was generally transient in medical studies. That observation does not predict how long symptoms will last for a particular person, dose or product. Report nausea that is severe, persistent, worsening or simply bothersome. Novo Nordisk’s patient guidance states: "Talk to your healthcare professional about any side effect that bothers you or does not go away."

Vomiting, diarrhea and the risk of fluid depletion

Why hydration concerns matter

Vomiting and diarrhea remove fluid from the body. The U.S. Wegovy prescribing information warns that dehydration or fluid depletion can contribute to kidney problems and advises precautions to avoid it. This warning is especially important if you already have kidney problems or another condition that makes fluid balance more difficult.

When to call

Contact a healthcare professional promptly for persistent or extended vomiting or diarrhea, for symptoms that prevent you from maintaining fluids, or when you are worried about dehydration. Do not wait for a self-selected number of hours or attempt to manage ongoing losses with a universal fluid formula. The appropriate advice depends on your health, medicines, kidney function and the severity of the illness.

What not to self-prescribe

The evidence used for this guide does not establish a safe, universal antiemetic or antidiarrheal regimen. Ask a prescriber or pharmacist before taking an over-the-counter product, particularly if you use other medicines, are pregnant or breastfeeding, have kidney or digestive disease, or have been told to restrict fluids.

Constipation: questions to take to your clinician

Constipation is among the commonly reported semaglutide side effects. The Wegovy patient page suggests asking a healthcare professional what to prioritize in your diet, including whether fiber is appropriate, and what activity changes may help. It does not provide a universal fiber target, laxative choice or dosing plan for every patient.

Bring practical details to the discussion: how often you are passing stool compared with your usual pattern, whether stools are hard, whether you have abdominal pain or vomiting, what you are eating and drinking, and which medicines or supplements you use. Your clinician can decide whether a dietary change, activity adjustment, medication review or further assessment is appropriate. Do not repeatedly add laxatives or supplements without advice, because the right choice depends on the cause and your medical history.

Abdominal pain, bloating, heartburn and other effects

Abdominal pain, upset stomach, bloating, belching, gas and heartburn appear among the patient-facing effects described for Wegovy, along with fatigue and dizziness. Semaglutide product information lists additional possible reactions. These symptoms can have many causes, so do not diagnose a complication from a symptom alone or assume that every new symptom is a medication effect.

Record when the symptom occurs, its severity, what you ate, whether it follows an injection and whether it is improving. Share that record with your prescriber or pharmacist. Report any side effect that is severe, persists, interferes with daily activities or bothers you and does not go away. A clinician can decide whether the symptom needs examination, a medication review or another response.

Should you change or skip a semaglutide dose?

No. Do not raise, lower, skip or repeat a dose as a self-treatment for nausea, vomiting, diarrhea, constipation or another side effect. Follow the regimen prescribed for your specific product and ask the prescriber what to do if symptoms occur around an injection, if a dose was missed or if you cannot take it as scheduled. Instructions differ by product and situation.

Compounded injectable semaglutide creates an additional dosing concern. The U.S. Food and Drug Administration says compounded drugs are not FDA-approved and are not reviewed by FDA for safety, effectiveness or quality before marketing. FDA has received reports of dosing errors with compounded injectable semaglutide, including some serious events. Those reports do not mean every compounded product or every event has the same cause, but they make exact measurement and professional guidance important. Never convert a dose between products or concentrations yourself.

How product-specific guidance changes the answer

Semaglutide is the active ingredient in multiple products, but each product has its own labeling, indications, titration instructions, formulations and patient materials. A suggestion printed on the U.S. Wegovy patient page should not silently be presented as universal advice for Ozempic, another formulation or a compounded preparation.

The U.S. Wegovy safety information says Wegovy is not recommended for patients with severe gastroparesis. If you have been diagnosed with delayed stomach emptying, have a history of significant digestive motility problems or develop unusually persistent fullness and vomiting, discuss the product with your prescriber rather than trying to manage the symptoms alone. The statement is product-specific and does not replace an assessment of your condition.

  • Could smaller, more frequent meals or stopping when full help with my nausea?
  • What foods should I eat, such as bland foods or crackers, while I feel nauseated?
  • Should I change how I drink fluids, and what should I do if vomiting or diarrhea makes that difficult?
  • What should I prioritize in my diet, such as hydration or fiber, for my particular health conditions?
  • Does the timing or severity of my symptom require a review of my prescribed regimen?
  • Which over-the-counter medicines, if any, are safe with my other medicines and conditions?
  • How should I handle a scheduled dose if I am vomiting, unable to eat or have missed an injection?
  • Could another medicine, supplement or medical condition be contributing to this symptom?

A simple symptom record can make the conversation more useful

A brief record is an organizational tool, not a substitute for care. Write down the product name and formulation, the dose and injection date, the time symptoms began, what happened after meals, episodes of vomiting or diarrhea, whether you could keep fluids down, bowel changes, and any other medicines or supplements taken. Note whether the symptom is improving or worsening.

Share the record with your prescriber or pharmacist, especially if the symptom returns after each dose or is interfering with eating, drinking, sleep or normal activities. Do not use the record to calculate a new dose or to decide independently that a serious symptom is harmless.

When symptoms need prompt medical attention

Contact a healthcare professional about severe or persistent gastrointestinal symptoms. The U.S. Wegovy labeling specifically emphasizes persistent or extended nausea, vomiting or diarrhea and the risk that fluid depletion may contribute to acute kidney injury. Call promptly if ongoing symptoms make it hard to maintain fluids or if you have kidney concerns.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Seek advice rather than waiting for a side effect to become unbearable. The manufacturer’s patient guidance is explicit that a side effect that bothers you or does not go away should be discussed with a healthcare professional. If symptoms are severe or you feel unable to manage safely at home, use the urgent-care or emergency pathway recommended in your area.

What the evidence does and does not establish

The available patient guidance supports practical nausea discussions, recognition of common gastrointestinal effects, and early attention to fluid loss. It does not support a universal prediction for symptom duration, a single diet that works for everyone, a fixed hydration amount, a universal fiber target, or a standard over-the-counter antiemetic, antidiarrheal or laxative plan.

Study figures from the European Medicines Agency are formulation- and trial-specific. They should not be converted into a general probability for every semaglutide user, product or population. For a personal estimate, ask the clinician who knows your prescription, dose, medical history and response.

Key safety reminders

  • Use the prescribing information and Medication Guide for your exact product.
  • Do not change, repeat or skip doses to control side effects without prescriber instructions.
  • Take persistent vomiting or diarrhea seriously because fluid depletion can affect the kidneys.
  • Ask before using an over-the-counter medicine for nausea, diarrhea or constipation.
  • Report any side effect that is severe, persistent or bothersome and does not go away.
  • Remember that compounded semaglutide is not FDA-approved, and FDA has reported dosing errors involving compounded injectables.
