Mild, period-like cramps for a few days and light spotting can be normal after an intrauterine device (IUD) is fitted. Bleeding patterns can then change during the first three to six months, depending on whether the IUD is copper or levonorgestrel-releasing. Severe or worsening pain, very heavy bleeding, fever, unusual discharge, possible pregnancy, or strings that are missing or feel different need prompt medical advice.
What is usually expected after an IUD is fitted?
During the first few days
The NHS says period-like pain may last for a few days after fitting, and light bleeding or spotting may occur. Cramps can feel similar to menstrual cramps because the uterus has just undergone a procedure. Light spotting can also be part of the immediate aftercare period. These symptoms can be uncomfortable without indicating a complication, but population guidance cannot predict exactly how long they will last for one person.

The important distinction is between short-lived symptoms soon after fitting and the bleeding-pattern changes that may occur during the first months of IUD use. A person may have mild cramps immediately after placement and later notice intermittent spotting as the body adjusts. Those are separate time frames, so a symptom that begins after the first few days should be considered in the context of the IUD type and any other symptoms.
There is no single number of days or universal amount of bleeding that is safe for everyone. Your IUD type, medical history, possibility of pregnancy and other circumstances affect how symptoms should be interpreted. Pain that is severe, worsening, persistent or accompanied by other warning signs should not be dismissed simply because the device was fitted recently.

During the first three to six months
The bleeding pattern after the immediate fitting period is different from the short-lived cramps and spotting many people notice right away. The CDC’s 2024 U.S. Selected Practice Recommendations describe spotting or light bleeding as common during the first three to six months with both copper and levonorgestrel IUDs. Copper IUDs can also cause heavier or prolonged bleeding during this period, while heavy or prolonged bleeding is uncommon with levonorgestrel IUDs. Bleeding generally decreases with continued use.
“Common” describes what is seen across groups of users; it does not guarantee that a particular episode is harmless. Likewise, a change in bleeding does not automatically mean that the IUD has moved. The amount, direction and accompanying symptoms matter. A pattern that is gradually settling is different from bleeding that suddenly becomes very heavy or pain that steadily intensifies.
| IUD type | Pattern commonly described in the first 3–6 months | Trend with continued use |
|---|---|---|
| Copper IUD | Spotting or light bleeding is common; heavier or prolonged bleeding can also occur. | Bleeding generally decreases over time. |
| Levonorgestrel IUD | Spotting or light bleeding is expected; heavy or prolonged bleeding is uncommon. | Bleeding generally decreases over time. |
These are population-level patterns, not a promise about an individual experience. Neither type eliminates the possibility of cramps or spotting soon after insertion. If your pattern differs markedly from what you were told to expect, contact the clinician or service that fitted the device rather than trying to interpret it from timing alone.

Ways to manage ordinary post-fitting cramps
Use an over-the-counter pain reliever only if it is suitable for you
NHS aftercare guidance names paracetamol and ibuprofen as painkillers that can help period-like pain for a few days after fitting. The American College of Obstetricians and Gynecologists (ACOG) also notes that over-the-counter pain relief such as ibuprofen may help with discomfort around placement.
Follow the product label rather than copying someone else’s dose or schedule. Check with a pharmacist or clinician first if you have allergies, a stomach or kidney condition, take medicines that could interact, might be pregnant, or are unsure whether a particular painkiller is appropriate. These general options are not individualized medical advice. A pharmacist can help you compare the active ingredients in combination products and avoid accidentally taking more than one product containing the same medicine.

Pain medicine should not be used to mask severe, worsening or otherwise concerning pain while delaying assessment. If a recommended over-the-counter option does not help, that itself is a reason to seek advice when the pain is significant. Do not take someone else’s prescription medicine, and do not assume that a medicine suitable for a previous period or procedure is suitable now.
ACOG’s clinical guidance on pain control for office uterine and cervical procedures is available at Pain Management for In-Office Uterine and Cervical Procedures.
Observe the direction of symptoms
Notice whether cramps are easing, staying unchanged or becoming stronger. A short symptom record can help you tell a clinician when the pain began, whether bleeding is light or becoming heavier, and whether fever, discharge or pregnancy concerns are present. Record the IUD fitting date and whether the device is copper or levonorgestrel-releasing. This makes it easier to distinguish the initial aftercare period from the first-month bleeding pattern.
Do not use improvement in one symptom to disregard a new warning sign. For example, cramps that are easing do not make a new fever or unpleasant-smelling discharge unimportant. Similarly, light spotting does not rule out a problem if the IUD threads suddenly feel different or pregnancy is possible.
Do not manipulate the device
The threads are there so a clinician can check the IUD, but they are not a handle for self-treatment. Do not pull, push, retrieve, reposition or remove the IUD yourself. If you are worried about its position, arrange an examination through an appropriate healthcare service.
How copper and levonorgestrel IUD bleeding differs
Copper IUD
Copper IUD users commonly experience spotting or light bleeding in the first three to six months, and heavier or prolonged bleeding can occur in that same period. The CDC says this pattern generally decreases with continued use. This early change can be frustrating, but it does not provide a universal timetable for when an individual’s bleeding will settle.
A heavier pattern that is sudden, extreme, worsening or associated with significant pain still warrants clinical advice rather than being assumed to be a normal copper-IUD effect. Contact a clinician if bleeding is very heavy, if pain is not relieved by appropriate over-the-counter medicine, or if fever, unusual discharge or possible pregnancy occurs.

Levonorgestrel IUD
Spotting or light bleeding is expected during the first three to six months. Heavy or prolonged bleeding is uncommon, and bleeding generally decreases over time. Some users may therefore notice an initially irregular pattern before it becomes lighter, but the expected trend does not make every episode of heavy bleeding normal.

What both types have in common
Both devices can be followed by period-like cramps and light spotting immediately after fitting. Both can also be associated with spotting or light bleeding during the first three to six months. The main difference in the CDC description is that heavier or prolonged bleeding is common with copper IUD use during that early period but uncommon with levonorgestrel IUD use.
The urgent warning signs are the same regardless of IUD type, and symptoms that suggest a problem require assessment rather than a change in self-care. Do not switch, remove or attempt to reposition an IUD on the basis of bleeding alone without discussing your options with a qualified clinician.

- IUD threads that cannot be felt or feel different from usual.
The NHS lists these symptoms in its IUD side-effects guidance. The CDC also advises prompt care for heavy bleeding, cramping or pain, abnormal vaginal discharge, or fever in relevant IUD-related situations.
Use your local emergency service for severe or rapidly deteriorating symptoms. NHS 111, a GP or a sexual-health clinic are UK routes for urgent advice; those services are not universal, so readers elsewhere should use their local urgent-care or emergency pathway. Official guidance does not establish one pad-per-hour rule or a fixed number of days that applies to everyone. The combination, severity and trajectory of symptoms matter more than an arbitrary cutoff. A clinician may need to assess you even when the amount of blood does not fit a particular numerical threshold.
What to do if the IUD strings are missing or feel different
The threads normally extend through the cervix into the vagina. If you cannot feel them or they suddenly seem shorter, longer or otherwise different, the IUD may not be in its intended position and contraceptive protection may be affected.
Recommended Free Tools
- Contact a clinician promptly for advice and an examination. In the UK, the NHS advises contacting a sexual-health clinic, NHS 111 or a GP straight away.
- Use another contraceptive method until a clinician has checked the IUD, following NHS advice. This precaution is important because you might not be protected against pregnancy if the device has moved or is no longer in place.
- Do not try to pull, push, retrieve, reposition or remove the device yourself.
- Tell the clinician about any unprotected sex, pain, bleeding, fever or pregnancy possibility so they can advise you appropriately.
Do not wait for the threads to reappear before seeking advice. A change in threads is a reason to check the device even if cramps and bleeding are otherwise mild. Continue using the additional contraceptive method until the clinician gives you advice based on the examination.
Understanding the rare but important complications
ACOG states that serious complications from IUDs are rare. Its patient FAQ gives uterine-wall perforation during placement as occurring in about 1 out of every 1,000 placements. That figure describes a rare placement complication; it is not a threshold for deciding whether your current pain is safe. A person with severe or worsening symptoms still needs assessment even though serious complications are uncommon overall.
ACOG also describes IUDs as over 99% effective contraception. Effectiveness does not mean that every episode of bleeding is normal, and it does not remove the need to act on possible pregnancy, severe pain or a change in threads. If pregnancy is possible, tell the healthcare professional when the IUD was fitted and whether you have had unprotected sex so the situation can be assessed promptly.

A practical decision guide
If symptoms are mild and improving
- Period-like cramps for a few days and light spotting fit the NHS description of common immediate aftercare symptoms.
- If you can safely take them, use paracetamol or ibuprofen according to the product label and your pharmacist’s or clinician’s advice.
- Keep observing the pattern, especially whether pain is easing and whether bleeding remains light.
- Keep the fitting details available, including the date and IUD type, in case you need to contact the service that inserted it.
If symptoms are continuing or changing
- Remember that spotting or light bleeding can occur during the first three to six months with either IUD type.
- With a copper IUD, heavier or prolonged bleeding is also common in that period; with a levonorgestrel IUD, it is uncommon.
- Compare the current pattern with its direction: gradually decreasing bleeding is different from bleeding that becomes suddenly heavier.
- Arrange clinical advice if bleeding becomes very heavy, pain worsens or persists, or another warning sign appears.
If a warning sign appears
- Stop relying on self-care alone and contact an appropriate healthcare service promptly.
- Use emergency services for severe or rapidly worsening symptoms.
- If threads are missing or different, use another contraceptive method until the IUD has been checked.
- Do not remove or reposition the device yourself while waiting for advice.
Questions to take to a clinician or pharmacist
When seeking advice, be ready to state when the IUD was fitted, whether it is copper or levonorgestrel, how the pain and bleeding have changed, whether you have a fever or unusual discharge, whether the threads feel different, and whether pregnancy is possible. Also mention medicines, allergies and health conditions before taking an over-the-counter painkiller. This information helps the professional decide how urgently you need assessment and which symptom-relief options are appropriate.

You can also ask which local service should handle urgent symptoms outside normal clinic hours, how to obtain a check of the IUD if the threads change, and when to use emergency services. Local pathways differ, so a UK NHS route should not be assumed to apply in another country.
Key points to remember
- Short-lived, period-like cramps and light spotting can occur after fitting.
- Spotting or light bleeding is common during the first three to six months with both copper and levonorgestrel IUDs.
- Copper IUDs may also cause heavier or prolonged bleeding early on; this generally decreases. Heavy or prolonged bleeding is uncommon with levonorgestrel IUDs.
- Paracetamol or ibuprofen may help some people when used according to label directions and personal medical advice.
- Severe or worsening pain, very heavy bleeding, fever, unusual discharge, possible pregnancy, or missing or changed threads require prompt clinical advice.
- If the threads cannot be felt or feel different, use another contraceptive method until a clinician checks the IUD.
- Do not use a fixed day count or pad-per-hour rule as a substitute for clinical judgment.
Frequently Asked Questions
How long do cramps last after IUD insertion?
The NHS says period-like pain may last for a few days after fitting. There is no universal safe duration for every person. Seek prompt advice if pain is severe, worsening, does not go away, or is not relieved by an appropriate painkiller.
Is spotting normal after getting an IUD?
Light spotting can occur immediately after fitting. Spotting or light bleeding is also common during the first three to six months with both copper and levonorgestrel IUDs. Very heavy bleeding, worsening pain, fever or unusual discharge needs medical advice.
Which IUD causes heavier bleeding?
During the first three to six months, heavier or prolonged bleeding is common with copper IUD use and generally decreases with continued use. Heavy or prolonged bleeding is uncommon with levonorgestrel IUDs, although spotting or light bleeding can still occur.
Can I take ibuprofen or paracetamol for post-insertion cramps?
NHS guidance names paracetamol and ibuprofen as options for period-like pain for a few days, and ACOG notes that over-the-counter relief such as ibuprofen may help around placement. Follow the product label and ask a pharmacist or clinician if you have allergies, health conditions, medicine interactions or possible pregnancy.
What should I do if I cannot feel my IUD strings?
Contact a clinician promptly. Until the IUD has been checked, use another contraceptive method because protection may be affected. Do not pull, push, retrieve, reposition or remove the device yourself.
When is bleeding after an IUD an emergency?
Use your local emergency service for severe or rapidly worsening symptoms. Very heavy bleeding, severe or worsening pain, fever, unusual or smelly discharge, possible pregnancy, or missing or changed strings all require prompt clinical assessment.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Do copper and hormonal IUDs have the same warning signs?
Yes. Although their usual bleeding patterns differ, both types can be followed by cramps and spotting, and the same warning signs require assessment: severe or worsening pain, very heavy bleeding, fever, unusual discharge, possible pregnancy, or changed or missing strings.
How effective are IUDs?
ACOG describes IUDs as over 99% effective contraception. That statistic does not make every episode of bleeding normal and does not remove the need to seek care for warning signs or possible pregnancy.
The Bottom Line
Mild cramps for a few days and light spotting can be expected after IUD insertion. During the first three to six months, spotting is common with both IUD types; copper devices may also cause heavier or prolonged bleeding, while this is uncommon with levonorgestrel devices. Use only pain relief that is suitable for you and follow the label. Seek prompt care for severe or worsening pain, very heavy bleeding, fever, unusual discharge, possible pregnancy, or missing or changed strings, and use backup contraception until a clinician checks a device whose strings feel different.
