How to Manage a Mild Fever and Other Common Vaccine Side Effects at Home

A mild fever, sore arm, tiredness or headache after vaccination can usually be managed at home with rest, fluids, comfortable clothing and simple local care. Most minor reactions improve within a few days, but the expected pattern depends on the vaccine, the person’s age and health, and the country’s guidance. Monitor symptoms, ask a clinician before giving medicine—especially to a child—and treat any suspected severe allergic reaction as an emergency.

What is normal after a vaccination?

Vaccines can produce a temporary immune response. The Centers for Disease Control and Prevention (CDC) describes common minor effects such as a sore arm or low-grade fever that generally go away within a few days. Depending on the vaccine, people may also notice redness or swelling where the injection was given, fatigue, headache, muscle aches, chills, nausea or a mild rash.

Those symptoms are possibilities, not a checklist for every vaccine. A reaction that is expected after one vaccine may not occur after another, and timing can differ. Follow the information given at the appointment and the current instructions for the specific vaccine.

The NHS says injection-site redness, swelling and soreness commonly last two to three days. Feeling unwell or having a high temperature often lasts one or two days. Common symptoms usually pass after a few days. Serious allergic reactions are rare and usually happen within minutes; vaccination staff are trained to respond.

For COVID-19 vaccines specifically, CDC guidance also says effects generally go away in a few days, although some people find that symptoms temporarily interfere with normal activities. That timeframe is COVID-19-specific and is not a promise about every vaccine or every person.

The CDC’s general side-effect information, dated July 31, 2024, describes possibilities across vaccines rather than a single universal timetable. Read the handout or digital instructions you received at the visit alongside general advice. If the vaccine provider gave a different expected time frame, use that vaccine-specific information.

Typical duration by symptom pattern

Reaction What official guidance says about duration Scope
Redness, swelling or soreness at the injection site Commonly two to three days NHS general vaccination guidance
Feeling unwell or a high temperature Often one to two days NHS general vaccination guidance
Minor vaccine effects overall Usually within a few days CDC general guidance
Effects after a COVID-19 vaccine Generally a few days CDC COVID-19 guidance

Before leaving the vaccination appointment

Take a moment to ask what reactions are expected for the vaccine just given and how long they commonly last. Confirm whom to call if the person becomes unwell, and check whether the provider has written instructions for a child, an older adult or someone with a health condition. Keep the vaccine name, date and time available; those details help a clinician interpret symptoms later.

Vaccination sites commonly observe people for a short period according to local policy. If symptoms begin while you are still there, tell staff immediately rather than trying to manage them yourself. NHS guidance notes that vaccinators are trained to respond to serious allergic reactions.

First steps for a mild fever

If the person is otherwise alert and drinking, concentrate on comfort rather than trying to force the temperature to a particular number. A thermometer can help a caregiver check a child’s temperature and watch its direction over time. In England’s guidance for babies and children under five, a fever is defined as a temperature over 38°C. That definition and the appropriate response may differ elsewhere or for older people, so use local advice when deciding whether a reading needs assessment.

  1. Let the person rest. Keep the day quiet and allow extra sleep if needed. There is no need to maintain normal activity while someone feels unwell.
  2. Offer fluids regularly. Water and other usual drinks can help someone stay hydrated. For a child, offer small amounts often if a large drink is unappealing.
  3. Choose comfortable clothing. Avoid overdressing. Light, comfortable layers make it easier to respond if the person feels hot or chilled.
  4. Check how the person is doing, not just the number. Note alertness, drinking, urination and whether symptoms are easing. A temperature reading is useful context, not a diagnosis by itself.

Do not use a cold bath or extreme cooling as a routine response. The practical advice from CDC is rest, plenty of fluids and comfortable clothes for fever discomfort. Do not bundle someone in heavy clothing simply because they have chills; adjust layers for comfort.

Using a thermometer sensibly

Use the same thermometer and measurement method when checking a trend, and record the time with each reading. Avoid repeatedly checking when the person is comfortable, since a single number does not show whether the overall illness is improving. If the reading seems inconsistent with how the person looks or behaves, repeat it according to the thermometer instructions and ask a healthcare professional for advice.

Relieving a sore, swollen arm

For pain, redness or swelling where the shot was given, place a clean, cool, wet washcloth over the area. Use the arm gently rather than keeping it completely still; normal light movement can help relieve discomfort. Avoid pressing hard on the injection site.

A simple comfort routine

  1. Wet a clean washcloth with cool water and wring it out.
  2. Place it gently over the sore area for comfort.
  3. Remove it when it is no longer comfortable, and repeat as needed.
  4. Encourage gentle use or movement of the arm within what feels comfortable.
  5. Observe the area over the next day. For a COVID-19 vaccination, CDC advises contacting a healthcare provider if redness or tenderness gets worse after 24 hours.

Injection-site discomfort can occur alongside tiredness, headache, aches or a mild fever. Consider the whole pattern and whether it is improving, rather than treating one symptom in isolation. Comfortable sleeves can prevent fabric from rubbing the area. Do not massage, squeeze or repeatedly examine the injection site if doing so increases pain.

Headache, tiredness, aches and other common effects

Fatigue, headache, muscle aches, chills, nausea and a mild rash are reported with some vaccines. The absence of one symptom does not indicate that a vaccine failed, and the presence of another does not automatically mean there is a complication. Vaccine-specific information is the best guide to what is expected.

  • Fatigue: Plan a lighter schedule and allow rest.
  • Headache or muscle aches: Rest and fluids are first-line comfort measures; ask a healthcare professional before using medicine.
  • Chills: Use comfortable layers without bundling the person in excessive clothing.
  • Nausea: Offer usual fluids in small amounts and avoid forcing food.
  • Mild rash: Check the vaccine’s information and contact a clinician if it is worrying, worsening or accompanied by other concerning symptoms.

These effects are not universal. A vaccine’s official information may describe a different timing or additional expected reactions, so do not apply a symptom list from one vaccine to another. If a symptom was not mentioned at the appointment and concerns you, call the provider instead of assuming it is vaccine-related.

Should you give pain or fever medicine?

Medicine decisions depend on age, weight, health conditions, the vaccine and local guidance. Do not give a child a dose based on an adult label or on advice intended for a different vaccine.

Before vaccination

CDC advises not taking pain relievers or other medicines before a COVID-19 vaccination solely to prevent side effects. This is COVID-19-specific advice; it should not be rewritten as a universal rule for every vaccination programme without checking the relevant instructions.

After vaccination

After a COVID-19 vaccination, CDC advises talking with a doctor about over-the-counter pain relievers. For a child, ask the child’s healthcare provider about a non-aspirin pain reliever and other comfort measures. A clinician or pharmacist can check the product, formulation, dose and whether the child’s medical history creates a reason to avoid it.

Keep the medicine’s package and the vaccination record available when asking for advice. Tell the clinician the person’s age, the vaccine received, when it was given, the temperature or symptoms, and any medicines already taken. Do not combine products or repeat a dose unless the label or a professional specifically tells you to. If you are unsure whether a product contains a pain reliever, ask a pharmacist.

England’s infant MenB programme: a specific exception

England has programme-specific advice for infants receiving MenB vaccine at the two- and three-month visits. Government guidance says to give three doses of infant paracetamol after those vaccination visits: the first as soon as possible, followed by subsequent doses four to six hours apart. The cited instructions specify 2.5 ml of a 120 mg/5 ml suspension.

  • Note: Before using the health care thermometer, carefully read the product manual. Start measuring when “Lo℉” shows up. To ensure the accuracy of the measurement, avoid touching the probe with your hands or other hot objects prior to taking the temperature.

This schedule is for that English infant MenB programme only. Do not transfer the dose or timing to another vaccine, age, country, schedule or paracetamol formulation. For any other situation, follow the current instructions from the vaccination provider or local health authority. Formulations can have different concentrations, so the volume in one bottle cannot safely be assumed for another.

How to monitor a child at home

Children can be unsettled after a vaccination, and a caregiver may need to combine a thermometer reading with observation. Keep a simple note of when the vaccine was given, the temperature readings, fluids taken, urination, sleep and any medicine recommended by a professional.

  • Check the temperature with a thermometer when the child feels hot or seems unwell.
  • Offer fluids frequently and watch whether the child is drinking and urinating normally.
  • Use comfortable clothes and avoid overheating.
  • Use a cool, damp cloth for a sore injection site and allow gentle arm movement.
  • Follow the child’s specific vaccination instructions rather than a general internet schedule.

England’s under-five guidance uses more than a temperature threshold when assessing a child. If you are worried about the child’s behavior, hydration or overall condition, contact a healthcare professional even if the number does not seem high. A child who is unusually difficult to wake, is not drinking or is passing much less urine needs prompt professional advice rather than repeated home temperature checks.

Planning the next day

Keep plans flexible for the first day or two. Arrange a quiet place to rest, have usual drinks available and make sure another adult knows when the vaccine was given if the child will be cared for by someone else. Share the written instructions and the clinician’s contact details. This avoids guessing about a dose or schedule when the primary caregiver is unavailable.

When to contact a healthcare professional

Contact a healthcare professional when symptoms are worsening, worrying or not improving after a few days. For a COVID-19 vaccine, CDC specifically advises seeking advice if redness or tenderness at the injection site gets worse after 24 hours, or if side effects are worrying or do not seem to be going away after a few days.

For an unexpected reaction after any vaccination, the World Health Organization advises: “If you or your child has an unexpected reaction after vaccination, consult with your health-care worker.” Use the vaccination provider, primary-care clinician, pharmacist or local health service according to your country’s system.

Information to have ready

  • The person’s age and relevant health conditions.
  • The vaccine name, date and time it was given.
  • When each symptom began and whether it is improving or worsening.
  • Temperature readings and how they were measured.
  • Fluids, urination and ability to carry out usual activities.
  • Any medicine already taken, including strength and amount.

Ask for advice sooner if you cannot tell whether a symptom is expected, if the person takes regular medicines, or if a previous reaction makes you concerned. A call can clarify whether home observation is appropriate and what change should trigger reassessment.

When it is an emergency

A suspected severe allergic reaction requires immediate medical care. Do not wait at home to see whether it resolves. Serious allergic reactions are rare and usually occur within minutes, but urgent assessment is still required if one is suspected after leaving the vaccination site.

Emergency numbers and care pathways vary by country. Use your local emergency number or emergency service. If the reaction begins while still at the vaccination site, alert the vaccination staff immediately; NHS guidance notes that vaccinators are trained to respond.

Do not rely on a text message, online forum or a later routine appointment when an emergency reaction is suspected. Tell emergency staff which vaccine was given and when, if you know. Bring the vaccination record if doing so does not delay calling for help.

Why vaccine-specific and local advice matters

There is no single aftercare schedule that applies to every vaccine. CDC’s general information describes common possibilities, while vaccine-specific pages may give different timing, symptoms or precautions. COVID-19 aftercare guidance should be read as COVID-19 guidance, not as a universal medicine or duration rule.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Age and location also change the advice. The infant MenB paracetamol instructions above apply to England’s programme, and the cited England page covers babies and children up to age five. Another country may use a different product, dose, schedule or fever definition. If local instructions conflict with a generic list, follow the current instructions from the vaccination provider or health authority and ask a clinician to explain the difference.

Keep the local leaflet even after the first day. It may explain a delayed, vaccine-specific reaction and provide the correct contact number. General internet advice cannot replace the instructions for the exact product, formulation and schedule used at the appointment.

A practical decision guide

Home care is generally reasonable when

  • Symptoms are mild and match the information supplied for the vaccine.
  • The person can drink and is reasonably alert.
  • Soreness, tiredness or fever is stable or beginning to improve.
  • There are no signs of a suspected severe allergic reaction.

Call a clinician or health service when

  • Symptoms are getting worse or are worrying.
  • They have not begun to improve after a few days.
  • COVID-19 injection-site redness or tenderness is worse after 24 hours.
  • You are unsure whether a medicine is suitable, particularly for a child.
  • The reaction is unexpected for the vaccine or the person has a condition that changes usual advice.

Seek immediate emergency care when

  • A severe allergic reaction is suspected.
  • The person needs urgent assessment rather than observation at home.

When in doubt, contact the vaccination provider or a healthcare professional rather than guessing at a medicine dose or waiting through a potentially serious reaction.

Should I take pain medicine before a vaccine?

CDC advises not taking pain relievers before a COVID-19 vaccination solely to prevent side effects. Advice can differ for other programmes, so follow the vaccine provider’s instructions.

Can I give my child paracetamol after vaccination?

Ask the child’s healthcare provider or pharmacist about the product, strength and dose. England has a specific three-dose paracetamol schedule for infants receiving MenB vaccine at two- and three-month visits; it should not be applied to other vaccines, ages or countries.

When should I call a doctor about vaccine side effects?

Call if symptoms are worsening, worrying or not improving after a few days, or if you are unsure whether a medicine is suitable. Contact a clinician sooner for an unexpected reaction.

What temperature counts as a fever in a young child?

England’s guidance for babies and children under five defines fever as a temperature over 38°C. Definitions and assessment advice can differ by location and age, so use local guidance and consider the child’s alertness, drinking and urination.

Are chills, headache or nausea normal after every vaccine?

No. These effects are reported with some vaccines, but not every vaccine causes every listed symptom. Check the information for the specific vaccine and call a clinician if the symptom is unexpected or worrying.

When is a vaccine reaction an emergency?

A suspected severe allergic reaction requires immediate medical care. Serious allergic reactions are rare and usually occur within minutes; do not wait at home to see whether one resolves.

The Bottom Line

Most mild vaccine side effects settle within a few days. Rest, fluids, comfortable clothing, a cool damp cloth and gentle movement are usually enough. Use medicine only with vaccine- and person-specific advice, and contact a healthcare professional for worsening, worrying or persistent symptoms. Treat any suspected severe allergic reaction as an emergency.
