Limited rural internet does not automatically rule out a telehealth appointment. Ask the clinic which format fits your health concern—video, phone, secure messaging, or an exchange of files or images—and agree in advance what to do if the connection fails. A hotspot may help only where cellular coverage is strong.
Start by asking whether this visit needs video
Contact the clinic before the appointment and explain what your connection can and cannot handle. Ask the clinician whether your particular concern can be addressed by phone, secure messaging, or asynchronous care—where you send information such as documents or images for review—rather than a live video call. These formats can make care possible where broadband is unavailable, but they are not interchangeable: the clinician should decide what is appropriate for the issue.

Telehealth.HHS.gov describes phone calls, secure messages, text, asynchronous file or image sharing, and connecting from a local clinic as options in rural settings. Its rural telehealth guide says, “Even in communities without access to broadband, some health care services can be provided by phone, secure messaging, and asynchronous care.” Read the HHS rural telehealth guide.
Compare the options for your location and appointment
| Option | When it may fit | What to check |
|---|---|---|
| Phone call | The clinician agrees the specific visit can be handled by audio. | A phone visit may not work for every health problem. Ask the clinician first. HHS patient guidance. |
| Secure message or asynchronous exchange | The issue can be managed through messages, documents, images, or follow-up. | It may not replace a real-time assessment when the clinician needs one. Confirm the right format with the clinic. HHS rural internet options. |
| Video from home | You have a video-capable device and a connection that works with the clinic’s platform. | Rural connections can drop. Confirm what to do if video or audio fails. HHS preparation guidance. |
| Mobile hotspot | Your fixed internet is unreliable, but cellular signal is strong at the place you will use it. | A hotspot depends on usable cellular coverage; check any service cost or data limit that applies to your plan. HHS says a hotspot may be considered when a strong cell signal is available. HHS hotspot guidance. |
| Local clinic or community site | Home connectivity is unavailable and a supported location is accessible. | Ask whether the site can host your appointment and whether a private space and required platform are available. HHS patient guidance. |
Choose based on clinical suitability, connection reliability at the actual location, privacy, accessibility, available technical help, and any device or service costs. No single option is best for every patient or visit.

Prepare if the clinic says video is needed
- Confirm the setup. Ask which platform the clinic uses and whether you need an app or account. Use a smartphone, tablet, or computer with a camera and sound if required; do not assume you need to buy a new device.
- Set it up ahead of time. Install or open the app, complete any account steps, and check the camera and microphone. Ask the clinic for step-by-step help if you are new to telehealth. HHS preparation guidance.
- Test from the appointment location. Try the device on the same connection and in the place where you plan to sit. Follow the clinic’s platform instructions: official rural guidance does not set one minimum internet speed that applies to every platform and visit type. HRSA also lists the Telehealth Broadband Project for broadband mapping, speed-test data, and technical assistance; check current local availability. HRSA telehealth resources.
- Choose a private setting. If you need to use a library or another community internet location, arrange a private space and confirm with the clinic that its platform and privacy requirements can be met. A public connection does not remove the need for a private place and a safe, private communication method. HHS patient guidance.
- Agree on a fallback. Before the appointment, establish which number to call, whether the clinician will call you, or whether the visit should be rescheduled if either side loses connectivity. HHS preparation guidance.
During the visit, switch to the plan if the connection breaks
Tell the clinician early if the sound cuts out or the picture freezes. Use the fallback you agreed on rather than repeatedly trying to reconnect without knowing how to proceed. Before ending the call or disconnecting, confirm the next steps and how to contact the clinic afterward with questions. HHS recommends preparing patients for technical problems and clarifying follow-up contact and care instructions. See HHS guidance for preparing rural patients.
