Better OCR starts before recognition: capture a sharp, evenly lit, correctly oriented page, then use a parser that understands document structure instead of exporting one undifferentiated text string. Preserve the parser’s page, span, bounding-region, paragraph, table and confidence data; route uncertain or high-consequence content to human review; and validate both characters and reading order against representative originals. No single resolution setting, model, or confidence cutoff works for every document collection.
Why OCR errors and layout damage happen
Optical character recognition converts pixels into characters, words and higher-level elements. Every stage can introduce a different failure. A blurred scan can turn a 3 into an 8; a skewed page can split lines; a faint stamp can disappear; and a parser that sees two columns as one region can interleave their sentences even when every individual word is recognized correctly.

Microsoft’s OCR guidance summarizes the input problem directly: “Document scan quality, resolution, contrast, light conditions, rotation, and text attributes such as size, color, and density can all affect the accuracy of OCR results.” These variables interact, so a document that works at one setting may fail after a change in paper stock, camera angle, language, font size or page design.
Accuracy also has two meanings that should be measured separately:

- Transcription accuracy: whether the recognized characters, words and numbers match the page.
- Structural accuracy: whether headings, paragraphs, columns, tables, checkboxes, figures, footnotes and page boundaries remain associated and in the correct reading order.
A plain-text export can score well on transcription while destroying the structure needed for search, data entry or downstream automation. Improving a workflow therefore requires better input, a layout-aware extraction path and a review policy based on measured risk.
1. Diagnose and improve the source before tuning OCR
Inspect representative pages at their original size before changing a model or threshold. Include the worst pages, not only clean examples. Record the document type, page dimensions, language or script, capture method and whether the PDF contains digital text, scanned images or both.
Check the defects that most often cause recognition errors
- Blur: motion or focus blur softens character edges. Recapture the page with the camera held steady or rescan it.
- Skew and rotation: a tilted page or sideways image changes line detection and can alter reading order. Align the page and correct orientation before parsing.
- Low contrast and faint print: light gray text, carbon copies and faded receipts can merge with the background. Improve lighting or obtain a clearer source rather than assuming software enhancement will recover missing strokes.
- Glare and shadows: reflections from laminates and shadows near a book spine hide characters. Use diffuse, even lighting and flatten pages where practical.
- Cut-off edges: cropping can remove leading digits, table columns, checkboxes or footnotes. Reframe the capture with a margin around the entire page.
- Small or dense text: crowded lines and tiny type leave less separation for line and word detection. Capture at a higher quality and test the smallest text that occurs in production.
- Variable text appearance: color, font, handwriting, stamps and mixed sizes can behave differently on the same page. Include those variants in your sample set.
There is no universal resolution number that fixes all of these conditions. Microsoft recommends testing on a sample dataset representative of the real content because performance varies by implementation. If a source is visibly poor, rescanning or recapturing it is usually a more reliable intervention than repeatedly adjusting post-processing.

A PDF may contain an embedded digital text layer, image-only scanned pages, or a mixture. Google’s Enterprise Document OCR documentation describes native PDF parsing that extracts digital text directly and applies OCR to non-digital text. Check the file before rasterizing every page: preserving an existing text layer can avoid unnecessary recognition errors, while scanned pages still require OCR.
Mixed PDFs need page-level inspection. A digitally generated cover, a scanned signed form and a photographed attachment may require different handling in one file. Confirm that the resulting page associations remain intact after extraction.
2. Choose an OCR path that matches the document
Use the simplest path that preserves the information your application needs. A flat text result may be sufficient for a short, single-column memo. Forms, invoices, reports with multiple columns and archival collections generally need a layout-aware document model.
| Document situation | Suitable path | What to verify |
|---|---|---|
| PDF with a reliable digital text layer | Native PDF text extraction | Characters, page boundaries and reading order against the rendered page |
| Image-only scan or photograph | OCR on the image or scanned page | Image quality, orientation, language support and low-confidence words |
| PDF containing digital and scanned pages | Native PDF parsing with OCR for non-digital text | That both paths preserve page association and do not duplicate text |
| Tables, forms, columns, figures or sections | Layout-aware document analysis | Paragraphs, lines, words, selection marks, tables, figures, sections and reading order |
Microsoft’s Azure Document Intelligence v4.0 layout model documents extraction of pages, paragraphs, lines, words, selection marks, tables, figures and sections, with Markdown output. Its listed input formats include PDF, image formats and several Office formats. Those are documented capabilities, not a promise that every visual arrangement will be reconstructed perfectly.

Google documents configurable layout behavior and an optional legacy layout algorithm. Its guidance warns that enabling the legacy option might change text ordering. Compare both configurations on your own pages before adopting one, especially for multi-column documents, sidebars and footnotes.
Verify language and script support
Check the exact model and workflow’s supported languages before deployment. Google documents language hints, while Microsoft directs users to model-specific language-support documentation. Do not infer support from a vendor’s general OCR product name: a language may be available in one model or API version but not another. Test mixed-language pages, numerals, punctuation and diacritics in the same way they occur in production.
3. Preserve layout as structured data
Request and retain the structured response instead of storing only the final text string. At minimum, keep the page identifier, text spans, coordinates or bounding regions, recognized words and lines, and any paragraph, table, figure, section or selection-mark objects the model returns. This lets you reconstruct context, inspect an error in its original location and reprocess a page without losing the source mapping.
Keep page and location relationships
Associate every extracted element with its page. Preserve bounding regions or polygons when available so a reviewer can jump from a low-confidence word to the exact location on the image. Store offsets or spans into the recognized text alongside those coordinates; a character offset without page context is difficult to audit in a multi-page file.

Handle tables as tables
Do not flatten a table into a paragraph if columns matter. Retain row and column indices, cell spans and header relationships when supplied. Check merged cells, wrapped lines, totals, blank cells and values that align only by position. A Markdown table can be useful for a text workflow, but it is not proof that visual alignment, merged cells or every border has survived.
Use Markdown for text workflows, not pixel-perfect reproduction
Azure’s layout model and the Azure AI Search Document Layout skill document Markdown or text output. Markdown can represent headings, paragraphs and simple table rows in a way that downstream search systems can consume. It does not preserve every visual detail. Compare figures, complex columns, footnotes, annotations and irregular tables with the original page whenever those details affect the task.
The Document Layout skill also notes that DOCX and PDF inputs may yield different results because their image handling differs. If both formats are accepted, test each format separately rather than assuming identical layout output.

Preserve figures and selection marks explicitly
Selection marks such as checkboxes are data, not decoration. Keep their location and state, and verify that a checked box is not confused with a printed square or a nearby mark. For figures, retain the figure object and its page location; a text-only export may omit the information a reader needs to interpret a chart or illustration.

4. Use confidence to prioritize review, not to declare truth
When the service provides word-level confidence, capture it with the recognized text. Use low values to find pages and spans for inspection, but do not treat confidence as a universal accuracy guarantee. Microsoft describes word confidence as a transcription-confidence score and recommends sampling your own documents to understand how values relate to actual extraction quality.
For extracted fields, inspect both field-level confidence and the underlying OCR. Microsoft explains that field confidence reflects confidence in the position of the extracted value; a field can be located confidently even when one character inside it was misread. Review the OCR text itself, especially for identifiers, dates, totals and other values where one character changes the result.
Set thresholds by consequence
A threshold should reflect the cost of an error and the cost of human review. A misspelled word in an internal search index may be tolerable; a wrong account number, legal clause, medical instruction or invoice total may not be. Microsoft’s Content Understanding guidance gives illustrative thresholds for critical, important and non-critical fields, while explicitly saying thresholds must be determined experimentally for each use case. Treat those figures as examples of a risk-based process, not plug-in defaults for every OCR engine.
Microsoft’s OCR capabilities guidance gives 0.80 as an example cutoff for straight-through processing. That is an illustrative workflow value, not a population statistic or a universal recommendation. Validate any cutoff on documents that match your real languages, layouts and image conditions.
Combine confidence with other signals
- Flag words or fields below the locally validated threshold.
- Flag values that violate known formats, such as an identifier with the wrong number of characters.
- Flag totals that do not reconcile with line items when arithmetic checks are appropriate.
- Flag text located in an unexpected region, such as a value appearing outside the form field.
- Escalate pages with image-quality warnings, severe rotation, glare or missing edges even when individual word scores look acceptable.
Confidence scores should determine where people look first. They should not be presented as a measured accuracy percentage unless you have established that relationship on a labeled evaluation set.
5. Build a validation set and measure what matters
Create a small, labeled set before changing production settings. It should cover document types, page designs, languages, scripts, print quality, capture devices, rotations and the full range of table and form layouts. Include representative failures, not only ideal pages.

Compare transcription separately from structure
For text, compare recognized characters and words with a trusted transcription of the original. For structure, inspect reading sequence, headings, paragraph boundaries, table rows and columns, selection marks, figures and page breaks. A workflow can improve character recognition while making column order worse, so one aggregate score can hide the regression.
Track task-specific errors
- Character substitutions in numbers, dates and identifiers.
- Missing or duplicated lines.
- Column interleaving on multi-column pages.
- Rows shifted into the wrong table column.
- Lost headings, footnotes or page associations.
- Incorrect checkbox state or figure location.
After every change to capture quality, preprocessing, model, language hint, layout option or threshold, rerun the same set. Keep a record of which errors changed and whether review volume increased. Microsoft recommends representative sampling and evaluation; the available vendor documentation does not establish an independent, vendor-neutral head-to-head accuracy benchmark for OCR in general.

Use error costs to choose automation
Calculate the practical effect of errors in your workflow. If a false acceptance can release an incorrect payment or alter a legal record, require review even when confidence is high. If the output is only used to locate pages for a person who will read the original, a lower automation threshold may be reasonable. Document the policy by field and document class so it can be audited and revised.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Common failure modes and targeted fixes
Words are recognizable, but two columns are interleaved
Use a layout-aware parser and inspect its reading-order configuration. Compare the standard and optional legacy layout behavior where the service provides both; Google warns that the legacy option can change ordering. Preserve coordinates so a reviewer can verify which column each span came from.
The PDF output contains duplicate or missing text
Determine whether the PDF has an embedded text layer, scanned images or both. On mixed files, ensure that native text is not extracted again as an image and that OCR is applied only to non-digital content. Compare page counts and text locations with the rendered PDF.
Tables become a stream of numbers
Switch from plain text extraction to a layout model that returns table objects. Verify row and column assignments, merged cells, wrapped text and totals. Keep the original page image available for values whose position carries meaning.
Headings and paragraphs run together
Retain paragraph and section objects, not only line text. Check whether the model’s Markdown or structured output marks headings consistently across page breaks, and inspect pages with unusual typography or decorative titles.

Confidence is high but a critical value is wrong
Do not lower the threshold blindly. Add format and reconciliation checks, review the source crop, and include similar examples in the validation set. Microsoft recommends adding representative variations to training data when appropriate; a new font, stamp, language or field layout can expose a gap that confidence alone will not reveal.
Changing the scan setting helps one batch and hurts another
There is no single setting that optimizes every source. Segment documents by capture condition or type when necessary, and evaluate each segment separately. If pages are still visibly blurred, shadowed or cropped, fix acquisition rather than relying on model tuning.
DOCX and PDF produce different layout results
Test the formats independently. Microsoft’s Document Layout skill notes that image handling differences can change results between DOCX and PDF. Keep the source format in your evaluation metadata and avoid assuming that a configuration validated on one format transfers unchanged to another.

The Azure AI Search Document Layout skill documents a five-minute processing limitation for the called layout model. Limits like this are version- and service-dependent, so verify current documentation before deployment and design a retry or page-splitting strategy where appropriate.
Do not label one provider categorically more accurate from capability documentation alone. The documented services expose different outputs and controls, but those descriptions do not supply a comparable independent benchmark. Your own representative evaluation is the evidence needed for a provider decision.
When a dedicated document scanner is justified
A scanner is a conditional solution for paper workflows whose existing photos or scans are consistently blurred, skewed, shadowed or poorly framed. The OCR guidance supports the premise that source quality affects recognition, but it does not establish that buying a particular scanner improves every workflow or identify a best model.
Choose equipment only after measuring the work:
- Expected pages per day or batch.
- Whether originals are single-sided or duplex.
- Largest page size and the condition of the paper.
- Whether pages contain staples, receipts, cards or fragile media.
- How much automatic feeding and alignment reduce manual handling.
- Whether the resulting files retain enough contrast and detail for the smallest text.
Test a representative stack, including damaged and faint pages, before standardizing the capture process. A scanner cannot restore text that is physically missing, and a camera can be adequate when it produces evenly lit, sharply focused, uncropped pages.
Deployment checklist
- Collect representative pages covering every document class, language, layout and capture condition.
- Inspect each source for blur, skew, rotation, glare, shadows, low contrast, small text and cropped edges.
- Determine whether each PDF page contains digital text, an image, or both.
- Choose native extraction for reliable digital text and OCR for image-only regions.
- Use a layout-aware model when tables, columns, forms, figures, sections or selection marks matter.
- Retain page IDs, spans, coordinates, confidence values and structural objects with the extracted text.
- Test language hints and layout options on the actual documents, including any legacy ordering mode.
- Validate transcription and structure separately against the original pages.
- Set review thresholds by field criticality and measured error cost; treat example values such as 0.80 as starting hypotheses only.
- Add format checks, arithmetic checks and region checks for high-consequence fields.
- Rerun the validation set after every capture, preprocessing, model, language or configuration change.
- Keep the original page image available for review and for disputes involving legal, medical, financial or identity information.
Bottom line
Reliable OCR is a controlled pipeline, not a single “accuracy” switch. Improve the page first, select extraction that matches whether text is digital or scanned, preserve layout-aware structures and locations, and use confidence only to prioritize review. Measure character errors and layout errors separately on representative documents, then automate only the fields and document classes whose risk and review thresholds you have validated.
