Yes, you can collect an H. pylori stool sample at home, but the sample normally must be analyzed by a laboratory and interpreted by a qualified clinician. For suspected active infection, the usual first-line noninvasive choices are a stool antigen test and a urea breath test. Blood antibody testing can show previous exposure but is less useful for proving that infection is active or eradicated. Endoscopy with a biopsy is more invasive and is generally reserved for alarm symptoms or when your doctor needs to investigate another upper-gastrointestinal problem at the same time.
Medication timing matters. Proton-pump inhibitors such as omeprazole, antibiotics and bismuth can suppress H. pylori and produce a false-negative result. Ask the prescriber who manages your medicine when, or whether, it is safe to pause it. Anyone treated for confirmed H. pylori should have a test of cure at least four weeks after therapy.

Which H. pylori test is right for you?
The best choice depends on whether you need an active-infection result, whether you have warning symptoms, which medicines you take and what is available locally. There is no single test that is best for every situation.
| Test | What it detects | Invasiveness and visit requirements | Sample or procedure burden | Use after treatment |
|---|---|---|---|---|
| Stool antigen | H. pylori antigen in feces, indicating current infection when the test is properly prepared and interpreted | Noninvasive; a clinician or authorized laboratory usually orders the test, while collection can occur at home | Collect an adequate stool specimen in the supplied container and return it according to laboratory instructions | Preferred option for a test of cure when performed with the required medication holds |
| Urea breath test | Active urease-producing H. pylori | Noninvasive; performed at a clinic or laboratory equipped for breath testing | Swallow labeled urea, then breathe into a collection bag so labeled carbon dioxide can be measured | Preferred option for a test of cure after the required waiting period and medication holds |
| Blood antibody (serology) | Antibodies showing exposure to H. pylori | Minimally invasive blood draw | One blood sample; local laboratory availability varies | Usually unsuitable for confirming eradication because antibodies may remain after infection is gone |
| Upper endoscopy with biopsy | H. pylori in stomach tissue, plus visible and microscopic upper-GI disease | Invasive procedure performed by a clinician; preparation and any sedation are arranged by the endoscopy service | A scope passes through the mouth and tissue can be collected for histology or rapid urease testing | A biopsy-based test can be used when endoscopy is clinically indicated |
Stool and breath tests are generally the practical noninvasive choices. Endoscopy adds direct visualization and a biopsy, so its value is greatest when your symptoms or medical history already justify looking inside the upper digestive tract. Prices, insurance coverage, laboratory access and regulatory status of home products differ by location; there is no reliable single national cost to quote.

How home stool testing works
“At home” usually describes where you collect the specimen, not where the diagnosis is completed. Mayo Clinic Laboratories identifies feces as the specimen for its H. pylori antigen test and requires an adequate, properly handled sample. The laboratory performs the analysis and a qualified professional must put the result in context.
Step 1: Arrange an authorized test
Ask your doctor, gastroenterology clinic or an authorized laboratory whether a stool antigen test is appropriate. Some jurisdictions allow direct-access laboratory ordering or telehealth ordering, but programs differ in medical oversight and eligibility. Confirm that the service sends the specimen to a recognized laboratory and provides a clinician who can explain a positive or negative result.
Step 2: Review medicines before collecting
Tell the ordering clinician about every prescription, over-the-counter product and supplement. A result collected while an acid-suppressing medicine, antibiotic or bismuth product is interfering may be falsely negative. Do not stop a prescribed medicine on your own; use the medication checklist below and obtain individualized instructions.
Step 3: Obtain and identify the container
Use the container supplied by the laboratory or clinic, not an improvised household container. Read the laboratory’s instructions before collection, including its rules for labeling, storage and transport. An inadequate, contaminated or mislabeled specimen may be rejected or may not provide a dependable answer.

Step 4: Collect the sample exactly as instructed
Keep the specimen separate from toilet water and cleaning chemicals, and follow the supplied directions for transferring the required amount into the container. Do not guess how much is needed or alter the sample with water or other substances. If the collection instructions are unclear, call the laboratory before sending it.
Step 5: Return it promptly
Use the laboratory’s designated drop-off or shipping method and the time and temperature instructions that came with the kit. Keep the accession or tracking information so you can ask about a delayed, rejected or lost specimen.
Step 6: Have the result interpreted
A positive stool antigen result generally supports active infection, but treatment choice depends on your allergies, previous antibiotics, local resistance patterns and other medical factors. A negative result can be unreliable if medication holds were not followed, the specimen was inadequate or symptoms require a different evaluation. A commercial kit that merely displays a result without validated laboratory processing and clinical follow-up should not be treated as a definitive diagnosis.
What happens during a urea breath test?
A urea breath test measures the chemical activity of H. pylori rather than an antibody response. You swallow urea labeled with a detectable form of carbon. If H. pylori urease is present in the stomach, it breaks down the urea and produces labeled carbon dioxide, which is measured in exhaled breath.

Typical appointment sequence
- The laboratory confirms your preparation and collects a baseline breath sample.
- You drink the labeled urea solution, following the site’s timing instructions.
- After the specified interval, you breathe into one or more collection bags.
- The laboratory measures labeled carbon dioxide and reports whether the pattern is consistent with active H. pylori.
The test itself is noninvasive, but the preparation is important. Mayo Clinic Laboratories’ instructions for post-treatment testing call for waiting at least four weeks after antibiotics and withholding proton-pump inhibitors for two weeks. Antibiotics should also be avoided for four weeks before testing; bismuth and H2 blockers may affect performance. Use the exact instructions from the laboratory performing your test, because preparation rules can differ by assay.
When a blood antibody test helps—and when it does not
The National Institute of Diabetes and Digestive and Kidney Diseases lists a blood sample as one way to look for signs of H. pylori. Serology can be useful when stool or breath testing is unavailable or unsuitable, but antibodies do not reliably distinguish a current infection from an earlier one. A positive antibody result therefore does not, by itself, prove that H. pylori is still present.
Because antibodies may remain after successful treatment, serology is generally not the preferred way to confirm cure. If active infection needs to be established, ask whether a stool antigen or urea breath test is available and medically appropriate.

Upper endoscopy lets a clinician inspect the esophagus, stomach and first part of the small intestine while collecting tissue for histology or rapid urease testing. It is more invasive than stool or breath testing, so it is not usually the first step for an otherwise stable person who simply needs an H. pylori answer.

Your clinician may choose endoscopy when there are alarm features or another condition that requires direct examination, including:
- Evidence of gastrointestinal bleeding or black, tarry stools
- Unexplained anemia
- Persistent vomiting
- Unintentional weight loss
- Difficulty or pain with swallowing
- Known or suspected ulcer disease, severe gastritis or another upper-GI disorder
- Symptoms that persist despite an appropriate initial evaluation
During the consultation, ask which biopsy method will be used, how medicines should be managed, whether sedation is planned and when pathology results will be released. Endoscopy may answer more than the H. pylori question, but it also requires procedure-specific preparation and recovery.
Medication and timing checklist
Interfering medicines can suppress bacterial activity enough to make an active-infection test appear negative. Before changing anything, ask the prescriber whether a hold is safe for you, particularly if you have severe reflux, a previous ulcer or another condition requiring continuous treatment.

How to get tested through a doctor
- Describe the reason for testing. Explain your symptoms, prior ulcers, family history, previous H. pylori treatment and any recent antibiotics or bismuth use.
- Review warning signs. Mention bleeding, anemia, weight loss, persistent vomiting or swallowing difficulty immediately, because these may change the plan from a noninvasive test to endoscopic evaluation.
- Provide a complete medicine list. Include over-the-counter acid reducers and combination cold or stomach products, not just prescriptions.
- Choose the test together. For a stable patient without alarm features, the clinician will commonly consider stool antigen or urea breath testing. A blood test may be considered when other methods are unavailable. Endoscopy is selected when direct evaluation is needed.
- Follow collection and preparation instructions. Ask when to stop or switch medicines, where a stool sample must be delivered and how long results usually take at that facility.
- Make a results plan. Know who will contact you, what a positive result means and what to do if symptoms continue despite a negative test.
How to interpret a result
Positive stool antigen or breath test
These results generally indicate active infection when preparation and specimen handling were adequate. Contact the ordering clinician rather than self-treating with leftover antibiotics. The 2024 American College of Gastroenterology guideline highlights state: “All patients found to be infected with H. pylori should be treated.” The regimen should account for allergies, previous antibiotic exposure, interactions and local clinical guidance.
Negative stool antigen or breath test
A negative result is most useful when the medication intervals and collection instructions were followed. If you tested soon after antibiotics, while taking a proton-pump inhibitor or bismuth, or with an inadequate sample, ask whether the test should be repeated after proper preparation. Persistent or severe symptoms deserve clinical review even when H. pylori is not detected, because reflux, ulcers, gallbladder disease, medication effects and other conditions can cause similar complaints.
Positive blood antibody result
Discuss it with a clinician who can decide whether an active-infection test is needed. The antibody result alone should not be used to declare that treatment worked or failed.

Biopsy result
Your gastroenterology or pathology team will combine the biopsy finding with the endoscopy report and your symptoms. Ask whether the tissue was examined by histology, rapid urease testing or both, and whether another test is needed after treatment.
How to confirm that H. pylori is gone
Do not assume that symptoms disappearing means the infection has been eradicated. The 2024 American College of Gastroenterology guideline highlights require a test of cure for treated infection: “Complete test of cure at least four weeks after therapy with either: Urea breath test; Fecal antigen test; Biopsy-based test.”
- Finish the prescribed eradication regimen unless your clinician changes it because of a reaction or another safety concern.
- Schedule the follow-up test for at least four weeks after therapy.
- Use a urea breath test, stool antigen test or biopsy-based test, not routine antibody testing.
- Apply the medication holds in the checklist above, including the two-week proton-pump-inhibitor hold and four-week antibiotic and bismuth intervals when clinically appropriate.
- Wait for the documented result and contact the treating clinician if infection persists; a different regimen may be required.
Choosing between home collection and an appointment
- Choose home stool collection when you have no alarm symptoms, can obtain an authorized laboratory kit and can return a properly handled sample on time.
- Choose a clinic-based breath test when you want a noninvasive active-infection test but prefer staff to manage the collection and timing.
- Choose a doctor visit first when you are pregnant, testing a child, taking medicines that cannot be paused, have significant medical conditions or are unsure whether H. pylori explains your symptoms.
- Ask about endoscopy when alarm features or another upper-GI problem make direct visualization and biopsy useful.
Home collection can reduce travel and appointment time, but it does not remove the need for laboratory quality control or medical interpretation. Availability, age rules, shipping requirements and regulatory status for direct-to-consumer kits vary by jurisdiction, so verify those details with the laboratory or clinician before paying for a kit.
