How to Fix Error 0xC000014D on Windows 11 and 10

Error 0xC000014D means STATUS_REGISTRY_IO_FAILED: Windows could not read, write, or flush a file that stores the system registry. When the affected hive or its storage path is needed during startup, Windows may stop before the sign-in screen. The code identifies a registry input/output failure, not the exact failed hive or a specific SSD.

Start with Startup Repair in Windows Recovery Environment (WinRE), then try System Restore. If those options fail, verify the Windows drive letter in WinRE and run offline CHKDSK, SFC, and—when an update is implicated—DISM. Repeated failures require storage, controller, firmware, BIOS, and unexpected-shutdown investigation rather than repeated registry edits.

What error 0xC000014D means

Microsoft’s MS-ERREF specification defines 0xC000014D as STATUS_REGISTRY_IO_FAILED. Its description is: “An I/O operation initiated by the Registry failed and cannot be recovered. The registry could not read in, write out, or flush one of the files that contain the system’s image of the registry.”

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The Windows registry is stored in hive files. During boot, the Windows kernel loads the system registry. If a required hive cannot be read from the volume, or a write or flush operation fails, startup can halt before the logon screen. The code does not tell you whether the underlying cause is a damaged hive, filesystem corruption, a failing disk, a storage controller, a cable or connection, firmware, BIOS behavior, or an unexpected power loss.

This is therefore a storage-path or registry-I/O error, not an activation problem and not a generic application crash. A one-time occurrence after a power interruption can have a different cause from the same code returning after every repair.

Before changing anything

Protect important data

If the computer can still reach Windows occasionally, copy important documents, photos, browser data, and other irreplaceable files to another drive or a trusted cloud location before attempting invasive recovery. If the disk is intermittently disappearing or producing repeated read errors, minimize unnecessary startups and scans and prioritize copying data.

Have recovery access ready

WinRE may ask for the BitLocker recovery key when the system volume is encrypted. Find that key before starting if possible. If the built-in recovery menu does not appear, use a USB flash drive for Windows installation, choose Repair your computer, and open WinRE from there. The USB is only installation media that provides recovery tools; it is not itself a repair utility.

Repair sequence for Windows 11 and 10

1. Enter Windows Recovery Environment

  1. If Windows displays an automatic recovery screen, select Troubleshoot, then Advanced options.
  2. If the computer never reaches that screen, boot from Windows installation media. At the setup screen, select Repair your computer rather than starting a new installation.
  3. When prompted, select the appropriate administrator account and provide the BitLocker recovery key if requested.

Do not choose a reset or reinstall until you have tried the non-destructive options below and secured your data.

2. Run Startup Repair first

  1. In WinRE, open Troubleshoot > Advanced options > Startup Repair.
  2. Select Restart when prompted and let the scan complete without powering off the computer.

Microsoft says Startup Repair looks for and attempts to correct several startup conditions, including missing or damaged system files, corrupted boot configuration data, a damaged master boot record, incompatible drivers, and registry errors. It is the safest first automated attempt because it is designed for an unbootable Windows installation and does not require you to identify a hive manually.

If Startup Repair reports that it could not repair the PC, return to Advanced options instead of repeating it indefinitely. Move to System Restore or the offline commands below.

3. Use System Restore when a suitable restore point exists

  1. Choose Troubleshoot > Advanced options > System Restore.
  2. Select a restore point dated before the first appearance of 0xC000014D.
  3. Confirm the affected Windows installation and allow the restore to finish.

System Restore is Microsoft’s preferred registry-recovery path on current Windows versions when a usable restore point exists. It rolls system files, drivers, installed programs, and registry state back to the selected point; it is not a substitute for a personal-file backup. A restore point created after the failure will not normally help, and no restore points may be available.

Offline checks from WinRE Command Prompt

Use these commands only after opening Advanced options > Command Prompt. WinRE often assigns different drive letters than normal Windows. The partition that is C: during a normal boot may be D: or another letter in recovery.

Identify the actual Windows volume

  1. At the prompt, test a drive by listing its root and Windows directory:
dir C:Windows
  1. If that path is not found, test another letter, such as:
dir D:Windows
  1. Continue until you find the volume containing the Windows directory. Use that confirmed letter in every subsequent command.

The examples below use C:. Replace it consistently if your installation is on another letter. Running a command against the wrong volume can produce an irrelevant result or repair the recovery environment instead of the installed system.

Check the volume with CHKDSK

Start with a filesystem check and repair:

chkdsk C: /f

The /f switch asks CHKDSK to correct logical filesystem errors. If Microsoft’s deeper bad-sector check is justified by the symptoms, use:

chkdsk C: /r

The /r option includes the filesystem repair and searches for unreadable sectors, so it can take substantially longer. Let the operation finish and record whether it reports filesystem errors, bad sectors, or an inability to access the volume. A clean CHKDSK result does not prove that an SSD, hard disk, controller, or connection is healthy; intermittent hardware faults can remain undetected during one scan.

Run offline System File Checker

After identifying the Windows volume, check protected Windows files while the installation is offline:

sfc /scannow /offbootdir=C: /offwindir=C:Windows

Substitute the confirmed drive letter in both paths. SFC repairs protected system files when it has valid component files available. It addresses system-file corruption, not every registry-hive failure and not an unreliable storage device. If SFC says it could not perform the requested operation, verify the drive letter and that the volume is accessible, then continue with the recovery options rather than repeatedly issuing the same command.

Revert a pending update when the timing fits

If 0xC000014D began immediately after Windows Update, a pending update action may be preventing startup. Microsoft documents this offline command:

DISM /Image:C: /Cleanup-Image /RevertPendingActions

Replace C: with the confirmed Windows volume. This option is intended for pending actions that block boot; it is not a general registry repair. Microsoft also documents checking the Windows WinSxSpending.xml file as part of pending-update troubleshooting. Do not delete or rename that file casually: follow the documented sequence for the specific pending-update condition and keep the original available for recovery.

Use BOOTREC only when boot code is the symptom

When diagnostics point to damaged boot code, Microsoft documents:

BOOTREC /FIXMBR
BOOTREC /FIXBOOT

These commands repair boot-record code. They do not repair every registry-I/O failure, so using them as a universal response to 0xC000014D can distract from a damaged volume or storage path. If Startup Repair and the registry-focused recovery steps do not indicate a boot-record problem, do not treat BOOTREC as the next automatic step.

If Windows still will not boot

Use the result of each step to choose the next one rather than running every command repeatedly. If Startup Repair completes without resolving the error, try a restore point. If no suitable point exists, confirm the Windows volume and run CHKDSK, followed by offline SFC. Use DISM’s /RevertPendingActions only when the failure follows an update. A volume that cannot be read consistently is a different situation from a volume that is readable but contains logical errors.

After each repair, restart once and note exactly what changed. If the same code returns immediately, repeated hive replacement is unlikely to solve the underlying issue. If the installation media itself cannot read the Windows partition, or the disk disappears between recovery screens, stop software experiments and concentrate on data preservation and the storage path.

Registry-hive recovery: what not to do

Do not blindly copy RegBack files

Older Windows guides often instruct users to replace the files in C:WindowsSystem32config with copies from C:WindowsSystem32configRegBack. Microsoft states that beginning with Windows 10 version 1803, Windows no longer automatically backs up the system registry to RegBack. The folder can therefore contain empty or unusable files.

Use a verified restore point or a known-good system-state backup instead of assuming RegBack contains a valid registry. Manually replacing hives with stale or incomplete copies can make the installation less recoverable.

Preserve the originals

Do not delete the original hive files before you have a verified backup and a recovery plan. If a manual hive replacement is unavoidable in a controlled recovery procedure, first copy the existing files to another location and document which file came from which backup. A corrupted or mismatched set of hives can prevent Windows from booting even when the storage device itself is sound.

How to tell whether hardware or the storage path is involved

The error code alone cannot distinguish an SSD, hard disk, controller, cable, filesystem, or one particular registry hive. Hardware becomes more likely when the failure recurs after a successful restore or when the system shows independent storage symptoms.

Warning signs that justify hardware investigation

  • CHKDSK repeatedly reports bad sectors or new filesystem errors.
  • The disk or Windows volume disappears intermittently from firmware or recovery tools.
  • The same registry-I/O error returns after a clean System Restore or other successful repair.
  • The computer has suffered unexpected shutdowns, power interruptions, or hard resets.
  • System logs, once Windows starts, show disk, controller, or unexpected-shutdown events.
  • The problem began after a firmware, BIOS, storage-controller, or hardware change.

Checks to perform after Windows starts

  1. Open the System event log and look for disk, storage-controller, and unexpected-shutdown events around the time of the failure.
  2. Inspect power and storage connections. For a desktop, reseat the data and power connections only when the computer is fully powered down; for a laptop, use the manufacturer’s service guidance.
  3. Check the device manufacturer’s support page for storage firmware and BIOS updates that match the exact model. Apply updates only with stable power and according to that manufacturer’s instructions.
  4. Back up data again before testing or replacing a suspect drive.

A repair that works once does not establish that the storage path is reliable. If the drive continues to disappear, develops bad sectors, or reproduces the code, prioritize data preservation and storage replacement over repeated registry edits.

Choosing the right recovery action

Recovery option Best use What it does not prove or repair
Startup Repair First attempt when Windows cannot boot It may not resolve a failing disk or controller
System Restore A restore point exists from before the failure It does not replace a personal-file backup or diagnose hardware
CHKDSK /f Logical filesystem errors on the confirmed Windows volume A clean scan does not certify the physical drive or controller
CHKDSK /r Deeper scan when unreadable sectors are suspected It can take substantially longer and still cannot guarantee future reliability
Offline SFC Corrupted protected Windows system files It is not a complete registry or hardware repair
DISM /RevertPendingActions Startup failure that follows a pending update It is not a general-purpose fix for unrelated registry-I/O faults
BOOTREC Evidence of damaged boot-record code It does not address every registry-hive or storage failure

When a reset or reinstall is the remaining option

If Startup Repair, System Restore, offline checks, and update rollback do not restore startup, and a verified backup of personal data exists, Windows reset or a clean installation may be considered. These choices can remove applications, settings, and—in a clean-install scenario—the contents of the Windows partition. They should not be used to hide an unresolved hardware fault: reinstalling Windows on a drive that is failing can lead to another registry-I/O error and further data loss.

If the installation media cannot consistently read the Windows volume, or the disk vanishes during recovery, stop attempting software repairs and address the storage path first. A replacement drive and a clean installation may be necessary, but preserve data from the original device before wiping it.

Frequently overlooked details

Drive letters are not permanent in WinRE

Always verify the Windows directory before running CHKDSK, SFC, or DISM. The recovery environment’s letter assignment is independent of the letters you normally see after logging in.

BitLocker can block offline access without the key

Encrypted volumes may require the recovery key before WinRE can unlock and repair them. Repeatedly entering commands without unlocking the correct volume will not repair the installation.

A successful boot is not the end of diagnosis

After Windows starts, complete a backup and inspect event logs if the code was preceded by a power loss, disk warning, or intermittent disappearance. Recurrence is the signal to investigate the storage path, controller, firmware, BIOS, and power conditions rather than to keep replacing registry files.

Frequently asked questions

What does error 0xC000014D mean?

It is Microsoft’s STATUS_REGISTRY_IO_FAILED status. A registry I/O operation could not be recovered because Windows could not read, write, or flush a file containing its system-registry image. The status does not identify a particular hive or prove that the SSD is defective.

Windows 11 will not boot after 0xC000014D. What should I try first?

Enter WinRE and run Startup Repair. If that fails, use a restore point from before the failure. Only then move to Command Prompt for drive-letter verification, CHKDSK, and offline SFC. This order starts with the least invasive Microsoft recovery options.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can CHKDSK fix 0xC000014D?

CHKDSK can correct logical filesystem errors that prevent registry files from being read. It cannot repair every damaged registry hive, and a clean result does not certify the physical disk, controller, cable, or firmware. Use /r when a deeper unreadable-sector scan is warranted, understanding that it takes longer.

How do I repair the registry from WinRE?

Use System Restore first when a restore point exists. If you use Command Prompt, verify the Windows volume with dir C:Windows or another drive letter, then run offline checks. Do not blindly overwrite hives from RegBack; automatic registry backups are not provided there by default on Windows 10 version 1803 and later.

Is my SSD failing?

The code alone cannot tell. Suspicion increases if CHKDSK repeatedly finds bad sectors, the disk disappears intermittently, the error returns after a clean restore, or logs show disk and controller events. Back up immediately and investigate the storage path before repeating registry repairs.

Why does the Windows drive have a different letter in recovery?

WinRE assigns letters independently of a normal Windows session. The installed system may be D: or another letter while recovery is running. Using the wrong letter can check or repair the recovery environment instead of the actual Windows installation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What if BitLocker asks for a recovery key?

Provide the key to unlock the encrypted system volume before attempting offline repairs. Without access to the correct volume, CHKDSK, SFC, and DISM cannot operate on the intended Windows installation.

Should I use BOOTREC for this code?

Only when symptoms or diagnostics indicate damaged boot-record code. BOOTREC /FIXMBR and BOOTREC /FIXBOOT address boot records; they are not universal fixes for registry-I/O or storage failures.

When is reinstalling Windows appropriate?

Consider reset or clean installation only after recovery options fail and personal data is backed up. If recovery tools cannot consistently read the volume or the drive vanishes, resolve the storage problem first; reinstalling onto failing media can cause another failure and additional data loss.

Frequently Asked Questions

What does error 0xC000014D mean?

It is STATUS_REGISTRY_IO_FAILED: Windows could not recover a registry read, write, or flush operation.

Can CHKDSK fix this error?

It can repair logical filesystem errors, but it cannot prove that the disk, controller, connection, or firmware is healthy.

Is a failing SSD the cause?

Possibly, but the code alone cannot identify the failed component. Repeated bad sectors, disappearing disks, or recurrence after restore warrant storage investigation.

Is copying RegBack hives safe?

Not by default. Windows 10 version 1803 and later do not automatically populate RegBack with usable registry backups, so use System Restore or a verified system-state backup instead.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why do drive letters change in WinRE?

WinRE assigns letters independently. Confirm the Windows directory before running CHKDSK, SFC, or DISM.

When should I reinstall Windows?

Only after non-destructive recovery fails and personal data is backed up; address an unreadable or disappearing drive before reinstalling.

The Bottom Line

Error 0xC000014D is a registry I/O failure. Start with Startup Repair, then System Restore, and use confirmed offline CHKDSK, SFC, or update rollback commands when appropriate. If the error returns, treat it as a possible storage-path, controller, firmware, BIOS, or power problem and protect your data before considering a reset or reinstall.
