Error 0x009FEFA9 is not currently documented in an authoritative Microsoft reference. The hexadecimal string alone does not identify a particular driver, Windows component, virus, or hardware failure, so there is no reliable one-click fix. Treat it as a symptom and diagnose the context in which it appears—such as startup, an update, activation, an application launch, or shutdown—before changing system files or drivers.
The workflow below follows Microsoft’s general guidance for serious Windows failures: capture the full message, undo recent hardware or driver changes, use Safe Mode when necessary, install current updates and firmware, check disk space, run targeted diagnostics, and use System Restore or Windows Recovery Environment (WinRE) if Windows will not start.

What error 0x009FEFA9 means
No authoritative Microsoft definition currently maps 0x009FEFA9 (or 009FEFA9) to a specific Windows service, stop code, device, or malware family. Different software can display hexadecimal values, and a copied fragment may omit the actual error name or accompanying stop code. Do not assume that this number proves a virus infection or a failing component.
Microsoft Support describes the general failure class this way: “Blue Screen errors … can occur if a serious problem causes Windows to shut down or restart unexpectedly to protect itself from data loss.” Microsoft identifies hardware, drivers, and software as possible causes of stop-error failures, but that general explanation does not establish the cause of this particular hexadecimal string.

Record the missing context
Before attempting repairs, write down:
- The complete on-screen wording, including any stop-code name, file name, or percentage counter.
- Whether the message appeared during boot, Windows Update, activation, an application launch, sleep or wake, shutdown, or a restart.
- The exact time and whether Windows eventually started.
- Any driver, Windows update, firmware, application, peripheral, memory, storage, or graphics change made shortly beforehand.
- Any Event Viewer entry created at the same time.

Choose the safest first path
| What you observe | First action | Why |
|---|---|---|
| Windows starts normally and the message occurred once | Record the details, check recent changes, then install pending Windows updates. | A one-time failure may not recur, but the record helps if it does. |
| The problem began after adding a device | Disconnect the new hardware and restart. | Microsoft recommends removing recently added hardware when diagnosing stop errors. |
| Windows repeatedly restarts or cannot reach the desktop | Enter Safe Mode or WinRE. | These environments load fewer third-party drivers and provide recovery tools. |
| A warning icon appears for a device | Update, disable, or uninstall the recently changed device driver. | Drivers are a common source of serious Windows failures. |
| Storage is nearly full | Free space before attempting major updates or repairs. | Windows needs working room for updates, paging, logs, and recovery operations. |
Step 1: Remove recently added hardware
Shut down the computer and disconnect nonessential hardware that was installed immediately before the failure. Start with USB hubs, storage devices, docks, printers, expansion cards, memory modules, and newly installed adapters. Leave the keyboard, mouse, display, and the device required to boot connected.
- Power off completely rather than choosing Restart.
- Disconnect the recent device and any related cable or hub.
- Start Windows and use it long enough to see whether the error returns.
- If the error disappears, reconnect one device at a time and restart between additions. The device that brings the failure back is the leading suspect; check its driver and firmware from the manufacturer.
Do not open a laptop or remove internal parts unless you are qualified to do so and have followed the manufacturer’s service instructions. If the problem persists with the new hardware removed, continue with the software and driver checks below.

Step 2: Start Windows in Safe Mode
Safe Mode starts Windows with a limited set of drivers and services. It is useful when a third-party driver or startup program prevents a normal boot, and it lets you inspect Device Manager and update settings without loading every optional component.
When Windows still reaches the desktop
- Open Settings.
- In Windows 11, select System and then Recovery. In Windows 10, select Update & Security, then Recovery.
- Under Advanced startup, choose Restart now.
- After the restart, select Troubleshoot > Advanced options > Startup Settings > Restart.
- Choose 4 or F4 for Safe Mode. Use 5 or F5 only when you specifically need networking.
When Windows will not boot normally
Interrupt the normal startup process two or three times by turning the computer off as soon as Windows begins loading. Windows should enter Automatic Repair; select Advanced options and follow Troubleshoot > Advanced options > Startup Settings. If Automatic Repair does not appear, start from Windows installation media and choose Repair your computer to reach WinRE.
In Safe Mode, check whether the error occurs. If it does not, a third-party driver or startup component becomes more likely, although Safe Mode alone cannot identify which one.
Step 3: Inspect and roll back device drivers
Microsoft lists drivers among the possible causes of stop errors. Pay special attention to hardware changed just before 0x009FEFA9 appeared, including graphics, storage, network, audio, Bluetooth, USB, and chipset devices.
- Right-click Start and open Device Manager.
- Expand the categories relevant to the recent change. Look for a yellow warning icon, an “Unknown device,” or a device whose status reports a problem.
- Open the device’s Properties and read the status on the General tab. Note the driver provider, date, and version on the Driver tab.
- If the failure began after an update, try Roll Back Driver when that button is available.
- Otherwise choose Update driver and use the manufacturer’s current driver package, or choose Uninstall device for a recently installed, nonessential driver and restart.
- Change one driver at a time and test after each restart. Keep a note of the original version so you can restore it.
Use official driver packages for the exact computer or device model. A driver intended for a similar model can create a new instability even when installation succeeds.
Step 4: Install Windows updates, BIOS updates, and firmware
Microsoft recommends current Windows updates, BIOS, and firmware when diagnosing stop errors. Apply these in a controlled order and avoid interrupting power during firmware installation.
- Open Settings > Windows Update (or Settings > Update & Security > Windows Update on Windows 10).
- Select Check for updates, install available updates, and restart when prompted.
- Return to Windows Update until no further updates are offered, including optional driver updates you have verified for your hardware.
- From the computer or motherboard manufacturer, check the support page for a BIOS or UEFI update and firmware for storage, docking, graphics, or other affected hardware.
- Read the manufacturer’s model and revision requirements before flashing firmware. Keep the system connected to reliable power and do not force a shutdown during the process.
If the error started immediately after a BIOS, firmware, or Windows update, record that timing. A rollback may be appropriate only when the manufacturer or Microsoft provides a supported rollback path.
Step 5: Check free disk space
Microsoft Learn’s current stop-error troubleshooting guidance recommends approximately 10% to 15% free disk space. The exact requirement varies by system, drive size, update, and workload. Low space can prevent updates, paging, log creation, and recovery operations even when Windows still starts.

- Use Storage Sense or Temporary files to review categories rather than deleting folders manually.
- Restart after creating adequate headroom, then retry Windows Update or the operation that produced the message.
Do not delete unknown files in Windows, Program Files, or driver folders merely because they are large. If space remains critically low, copy personal files to separate storage before attempting recovery.
Step 6: Correlate the failure with diagnostics
Event Viewer
- Right-click Start and open Event Viewer.
- Check Windows Logs > System and Application around the recorded time.
- Filter or sort by Critical, Error, and Warning. Look for a device, driver, service, or update named immediately before the restart.
- Open a relevant event and save its General and Details information. The event is evidence to correlate, not proof that the named component caused the failure.
Memory and hardware checks
Use Windows Memory Diagnostic when the failure includes random restarts, application corruption, or different stop messages on different boots. Press Win + R, enter mdsched.exe, choose to restart and check now, and review the result after Windows starts. For storage, graphics, or other components, use the diagnostic supplied by the computer or component manufacturer.
Malware checks only when the symptoms support them
The hexadecimal string by itself is not evidence of malware. If you also see unexplained security changes, unknown programs, browser redirects, or other suspicious behavior, run Microsoft Safety Scanner and follow its results. Keep your existing security software enabled and treat any detection separately from the undocumented error code.
Step 7: Recover Windows when the fault persists
System Restore
If the problem began after a driver, application, or update and restore points exist, open WinRE and select Troubleshoot > Advanced options > System Restore. Choose a restore point dated before the change. System Restore can remove recently installed drivers or applications while leaving personal files in place, but it does not replace a backup and may not be available.

Startup and other WinRE options
WinRE also provides Startup Repair, Uninstall Updates, and Startup Settings. Use the option that matches the evidence: Startup Repair for a boot failure, Uninstall Updates when the loop began immediately after an update, and Startup Settings for Safe Mode. Do not repeatedly run unrelated repair options without recording what changed.
Installation media
When the built-in recovery tools cannot start Windows, use a Windows 11 installation USB created on another working computer and select Repair your computer instead of installing immediately. Microsoft documents installation media as a startup-troubleshooting path. You need a writable USB drive and access to another computer to create the media; the drive enables recovery and does not, by itself, prove or guarantee a fix for 0x009FEFA9.
Back up personal files before reset or reinstall operations. A clean installation can erase applications and data, so treat it as a last resort after supported recovery options and hardware checks.
What repeated restarts tell you
A restart after the message may indicate a stop-error protection shutdown, but it can also be caused by power loss, firmware behavior, or an automatic restart setting. Disable automatic restart only temporarily if you need time to read the full screen: in System Properties, open Advanced > Startup and Recovery > Settings, clear Automatically restart, and reproduce the failure once. Restore the setting after collecting the wording.

Windows 10 support status and repair decisions
Microsoft ended Windows 10 support on October 14, 2025. Existing Windows 10 systems can still require repair, but the end-of-support date matters when deciding whether to spend time on a marginal installation, move to a supported Windows edition, or replace aging hardware. Confirm that the computer meets the requirements of the Windows version you plan to use and preserve your files before upgrading or reinstalling.
When to stop troubleshooting
- The machine cannot remain powered on long enough to back up files.
- Memory diagnostics report errors, storage diagnostics report imminent failure, or firmware tools report a hardware fault.
- The same failure returns with new hardware removed, Windows updated, and the suspect driver rolled back.
- Recovery would require deleting data that has not been backed up.
At that point, shut down safely and use the computer manufacturer’s support service or a qualified repair professional. Provide the complete message, Event Viewer details, timing, hardware changes, and every recovery step already attempted.
