If a Safari extension is missing or not working on an iPhone or iPad, check its main permission, its setting for the active Safari profile, and its access to the affected website—in that order. On a school- or work-managed device, an administrator may control those settings. Apple does not document one universal fix for extensions that still fail after these checks.
Check whether Safari has enabled the extension
- Open Settings > Apps > Safari > Extensions.
- Tap the extension you want to use.
- Turn on Allow Extension.
Apple documents this as the main extension control in its iPhone guide to Safari extensions in iOS 27. If the extension does not appear in the list, check whether it has been installed through its associated App Store app. Apple describes App Store extensions as installed through the App Store.

Make sure it is enabled for the Safari profile you use
Safari profiles can have separate extension settings. If you use profiles, enable the extension for each profile where you expect it to work; turning it on for one profile does not mean it is on for another. Apple’s guide says, “If you have Safari profiles set up, turn on the extension for each profile you want to use it with.”
Check the extension’s setting on the affected website
Safari can control extension access for an individual site separately from the main extension setting. Open the affected site in Safari, open the Page Menu, then choose Manage Extensions. Check that the extension is enabled for that website.

- This phone is unlocked and compatible with any carrier of choice on GSM and CDMA networks (e.g. AT&T, T-Mobile, Sprint, Verizon, US Cellular, Cricket, Metro, Tracfone, Mint Mobile, etc.).
- Please check with your carrier to verify compatibility.
- The device does not come with headphones or a SIM card. It does include a generic (Mfi certified) charging cable.
- Tested for battery health and guaranteed to have a minimum battery capacity of 80%.
The scope of the problem can help you decide which control to inspect first: if the extension fails across sites, check its general setting and profile; if it fails on just one site, check that site’s setting. These are useful troubleshooting clues, not proof of a single cause.
Check for organization-managed restrictions
On a supervised work or school device, a management policy may determine which extensions are allowed, force an extension on or off, or restrict the website domains it can access. Apple documents these controls in its Safari extensions management guide for Apple devices. If a setting is missing, locked, or does not stay changed, ask the device administrator to check the policy. Apple notes that an extension’s host app must be present for the extension to be managed.

Know which kind of custom extension you have
“Custom extension” can refer to different things. Apple’s iOS 27 guide covers extensions installed through the App Store as well as a Safari creation flow launched from the Page Menu with Describe Extension. In that flow, Safari checks the App Store for a match to the requested feature; if it finds none, it may offer the option to create an extension that can be tried, edited, and saved.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchThe creation flow requires Apple Intelligence and is subject to device, language, and regional availability limits, so it is not available to every iPhone or iPad user. The removal steps also differ: an App Store extension is associated with its app, while an extension created through Describe Extension can be deleted from Page Menu > Manage Extensions.

If it still does not work
If the extension is enabled globally, enabled for the profile you are using, and allowed on the site, Apple’s cited iOS guide does not prescribe a universal cache-clearing, reset, reinstall, or restart fix. Check with the extension’s developer about compatibility, known bugs, or service requirements, or contact Apple Support for help diagnosing the specific case.
Do not apply Mac-only troubleshooting instructions to an iPhone or iPad. Apple’s separate Safari extension troubleshooting article for Mac discusses macOS and its own steps; it is not evidence that Mac Safe Mode applies to iOS or iPadOS.
