If a Copilot Notebook answer cites an old file or makes a claim your source does not support, do not search for a universal “refresh all” command. Open the citation, inspect the notebook’s References list, remove superseded material, add the latest authoritative source you can access, and rerun a narrowly worded prompt that names the intended file. Ask Copilot to confirm the publication date and quote the exact supporting section, then open the new citation yourself before relying on the answer.
Why Copilot Notebook can cite an old or wrong source
Notebook answers are grounded in the websites, folders and files attached to that notebook and available under your account’s permissions. Copilot does not automatically search every document in your organization merely because you can find it elsewhere. If an earlier PDF, an archived web page or a duplicate file remains in the reference set, it can be selected for an answer even when a newer version exists.

- The source is outside your permissions, private, paywalled or otherwise unsupported for that Copilot experience.
- A large reference set makes it harder for the intended source to be used within the product’s grounding limit.
- Your account, license, app surface or feature rollout differs from the instructions you are following.
Fix an outdated citation: the reliable sequence
Work through these steps in order. Editing the references before changing the wording of your question removes the most common cause first.
- Open the citation. Select the citation attached to the answer and read the source title, publication date or version, and the passage Copilot used. Compare that passage with the actual claim. If the passage is missing, inaccessible or unrelated, record that before changing anything.
- Audit the notebook’s References list. Confirm that the exact current file, folder or website is included. A source that exists in OneDrive, SharePoint or a browser tab is not automatically part of the notebook.
- Remove superseded or duplicate material. Delete the old file or obsolete web page when it should no longer influence answers. If both versions must remain for historical work, keep them clearly labeled and identify the required version in your prompt.
- Add the authoritative current source. Use the latest approved document or URL, not a search result that redirects to an archive. For Microsoft 365 content, add supported OneDrive or SharePoint files or locations that your account can open.
- Check access and support. Make sure the account used by Copilot can read the source. In Microsoft’s documented Microsoft 365 Copilot experience, external web links must be public; authenticated or paywalled pages and linked subpages are not supported. - Ask a diagnostic question. Name the exact file and version, ask for the publication date, and require the section that supports each important claim. A precise prompt helps reveal whether Copilot can actually use the source; it is not a guarantee of correctness.
- Rerun and verify. After editing references, run the question again. Open the returned citation, compare its text with the answer, and check dates, numbers, definitions and exceptions yourself.
- Send feedback if the result is still wrong. In the Copilot experience, use Settings and more > Feedback and describe the source, the incorrect claim and the expected version.
Step 1: inspect the citation instead of trusting its label
Start with the answer that raised the concern. Selecting a citation should lead you to the referenced file, page or passage. Check four items:
- Identity: Is the title the document you intended, or a similarly named copy?
- Version: Does the file name, revision number or date identify the latest edition?
- Authority: Is it the owner’s approved source, rather than an unofficial copy or summary?
- Support: Does the quoted section actually establish the answer, including its conditions and exceptions?
An answer can be wrong even when the citation opens successfully. For example, a policy may state a rule that applies only to one region or date range, while Copilot omits that qualifier. Treat the cited passage as text to audit, not as an automatic fact-check.
Step 2: repair the References list
When an old file is still attached
Remove the earlier file if it has been replaced and has no role in the question. Then add the current file from its supported storage location. Give versions unambiguous names such as “Travel policy — 2026-09-01” rather than leaving several files called “final” and “final2.” If historical comparison is the purpose of the notebook, keep both but state the target version every time.
When the current file is missing
Add it explicitly to the notebook. Copilot’s access is limited to references you provide and can open under your permissions; it does not infer that a newer document should supersede an attached one. After adding it, inspect the References list again so you know the operation completed.
When a website changed
Replace an old URL with the publisher’s current public page. A link to a page that requires a sign-in, subscription or a second linked page may not be usable in the documented Microsoft 365 experience. If the authoritative content is private, add an accessible approved file instead, subject to your organization’s policy.
When several sources are all technically current
“Current” does not always mean “controlling.” A regional procedure, a draft announcement and a corporate policy can carry different authority or effective dates. Keep the source that governs the decision, label the others by status, and tell Copilot which hierarchy to follow. Ask it to report conflicts rather than silently choosing one: “If the documents disagree, list the conflict, identify each effective date and do not decide which rule controls.”
Step 3: verify account, permissions and product surface
Instructions can differ depending on whether you are using Copilot Chat, Microsoft Copilot for work or school, a personal plan, or a OneNote-integrated surface. Confirm which account is signed in and whether your organization has enabled the Notebook feature. Microsoft notes that capabilities can roll out gradually, so a missing control may reflect account, license or rollout differences rather than a bad prompt. For Microsoft 365 files, test the source outside Copilot with the same account. If you cannot open the file, Copilot cannot reliably ground an answer in it. Do not paste confidential content into a different account merely to make a citation work; resolve the permission or sharing issue through your organization’s approved process.
Also check whether the notebook owner and the person asking the question have the same access. A file may appear in a shared notebook but remain unreadable to another user. Reconfirm permissions after a SharePoint move, ownership change or link replacement. If a public web page redirects to a sign-in page, treat it as inaccessible for this purpose even if the original URL looked public.
Copilot Chat and Microsoft Copilot do not have identical grounding limits
Microsoft documents different reference behavior for the two broad contexts below. The limits are product constraints, not accuracy scores.

When a notebook approaches a cap, do not assume the newest items are prioritized. Organize the reference set so the files needed for the immediate task are clearly included within the usable range. A smaller, curated notebook is easier to test than one containing every historical document.
Use a diagnostic prompt that exposes the source Copilot chose
After correcting the references, avoid a broad request such as “What is the current policy?” Use a prompt that constrains the source and asks for evidence:
Using only “EXACT FILE NAME — VERSION OR DATE,” answer: [your question]. Confirm the document’s publication or revision date. For every key claim, identify the exact heading or section that supports it. If the file does not contain the answer, say so instead of using another reference.For a web source, replace the file name with the exact public page title and URL. For a multi-part answer, ask for a claim-by-claim format: claim, citation, quoted section, effective date and any exception. This exposes unsupported leaps that can disappear in a polished paragraph. If Copilot says the file does not contain the answer, accept that limitation instead of broadening the prompt until an unrelated source supplies a guess.
What to do when the answer remains wrong
The citation still points to the previous version
Return to References and confirm the old item was actually removed, not merely hidden from a view. If the old and new files have similar names, rename them and rerun the diagnostic prompt with the full version identifier. Keep the reference set small enough that the current source is unambiguous.
The citation is current but the conclusion is incorrect
Read the surrounding paragraphs, tables and footnotes. Copilot may have selected a genuine passage but missed a condition, exception or definition elsewhere in the same document. Ask it to quote the complete subsection and list any stated limits. If the source itself is ambiguous, have a subject-matter owner resolve the policy rather than asking repeated prompts to guess.

This can happen when no relevant accessible source is available. Recheck that the reference was added, that you can open it, and that the content is supported in your Copilot surface. Then ask the question using the exact source name. If Copilot still cannot ground the response, treat it as general information and locate the answer directly in the approved document.
The source cannot be opened from the citation
Check whether the link is public and whether you are signed in with the account that has permission. External authenticated or paywalled pages and linked subpages are not supported in the documented Microsoft 365 scenario. Replace the link with an accessible, approved source or resolve the sharing configuration; do not assume the citation is valid merely because a title appears.
The References or refresh controls are missing
Verify the app, account and license, then check whether the feature is available to your organization. Microsoft documents a refresh action for the OneNote Overview Page after references are added or removed. That instruction is specific to the Overview Page; it is not evidence of a universal command that re-indexes every Notebook answer. For a persistent product issue, submit feedback through Settings and more > Feedback.
The answer changes between identical runs
Save the prompt, reference names and citations from the first run. Confirm that the reference list, permissions and account did not change, then repeat with one authoritative source if possible. Variation is a reason to increase verification, not a reason to treat the later answer as automatically newer or better.

There is no universal “refresh all answers” switch
Editing references and rerunning a prompt is the supported correction path described for outdated or incorrect grounding. A separate OneNote Overview Page refresh may update that page after reference changes, but it should not be presented as a global Notebook reset. No refresh action guarantees that every answer will immediately use a new source or become correct.

Do you need to buy anything to fix a stale citation?
No physical product, accessory or repair tool solves a source-grounding error. The practical question is whether your signed-in account and license include the Notebook surface and reference types you need. A Microsoft Copilot or Microsoft 365 subscription may affect access, but availability varies by plan, account and rollout. Check Microsoft’s current plan documentation or your administrator rather than purchasing a generic office product or assuming a paid tier guarantees accurate answers. Prevent stale-source answers in shared notebooks
- Use a source-of-truth naming convention. Include owner, effective date and revision in file names.

- Keep references focused. A shorter set of authoritative sources is easier to audit than a large collection of duplicates.
- Record effective dates. Ask questions that include the date or jurisdiction when a rule changes over time.
- Review permissions after reorganizations. Moving a file or changing sharing can make a previously usable reference inaccessible.
- Verify before high-impact use. Check citations for legal, financial, medical, security, employment or operational decisions, and have a qualified person review consequential conclusions.
- Assign a notebook owner. Give one person responsibility for retiring superseded files and documenting which source controls.
- Keep a change note. When replacing a reference, record the old and new version, effective date and reason so collaborators know why an answer may differ.
How to judge whether a corrected answer is usable
Before sharing or acting on the result, use this final check:
- The cited item is the intended current file or public page.
- The date, revision and scope match the question.
- The quoted section supports the claim without an omitted exception.
- Every important number, threshold and deadline matches the source.
- No inaccessible or superseded reference was silently substituted.
If any item fails, use the source directly or ask its owner for clarification. Citations improve traceability, but they do not make Notebook answers infallible.
