Start with the Apache error-log entry matching the failed URL and time. Use that message to decide whether to check configuration syntax, an .htaccess rule, a module, a virtual host, or an application handler.
1. Find the error-log entry for the failed request
Record the URL, the time of the failure, and any request ID or reverse-proxy details available. Reproduce the request once if it is safe and useful, then inspect the error log at that time. Apache’s error log is its primary record of runtime problems; as the project’s Log Files guide puts it, “Troubleshooting any problem without the error log is like driving with your eyes closed.”
Do not assume a universal log path. The active ErrorLog directive determines the location, and a matching <VirtualHost> can have its own error log. Check the global configuration and the virtual host that handled the request. Apache’s documented source-build default is /usr/local/apache2/logs/error_log, but installation choices can change it. Distribution packages, containers, control panels, and managed hosts may use different paths.
Read the detailed message and nearby entries, including the module and severity information. Log formats can be customized, and CGI, PHP, or another handler may write its own messages to Apache’s error log. If the entry points to an application or handler, investigate that component rather than treating every 500 as an Apache configuration fault. Apache documents its logging behavior in the error-log reference.

2. Check recent configuration changes for syntax errors
If the error began after a configuration edit, run a syntax check before restarting the server. Use the command matching the installed Apache binary or service environment:
apachectl configtesthttpd -t
These commands check configuration syntax without starting the server. Fix any reported error first. If Apache is using a non-default configuration file, pass its actual path with the -f option to the installed httpd binary. Configuration files can include other files, so note the file and line identified by the test. A successful syntax test does not prove that the application or every request path will work; it only rules out configuration syntax errors. Main server configuration changes take effect when httpd starts or restarts. Follow the service controls used by your deployment rather than assuming a particular restart command.
3. Inspect .htaccess and directive permissions
A mistake in .htaccess can produce an Internal Server Error. Check the file for misspelled directives, malformed syntax, and directives whose required module is not available. Also check whether the parent directory configuration permits the directive through its AllowOverride settings. Apache documents errors from invalid or disallowed directives in its .htaccess guide and the applicable rules in its core directives reference.

Do not broadly relax AllowOverride as a blind fix. Apache warns that access-control directives ignored under an unsuitable override configuration can undermine restrictions the site depends on. Confirm the required directive and its allowed context for the relevant directory before changing the parent configuration.
4. Verify modules and virtual-host selection when the log points there
If the error names a module, check whether it is loaded and inspect that module’s configuration. If the request appears to be reaching the wrong site configuration, check the parsed virtual hosts. These commands answer different questions:

Use the installed binary and the same configuration context as the running service; command paths and configuration files vary by installation. Apache lists these and other options in the httpd command-line documentation.

For a problem that remains unclear, Apache supports higher startup log verbosity with the -e option and per-module log levels, including targeted rewrite tracing. Increase detail only for the relevant module or investigation, then return logging to its normal level: trace output can be high-volume. 6. Apply a targeted fix and verify the original request
- Use the matching error-log entry to identify the failing configuration, module, handler, or application.
- Change only the identified cause. If you changed server configuration, run
apachectl configtestorhttpd -tagain before applying it. - Reload or restart only as required by your deployment and by the type of change.
- Repeat the original URL or request. Confirm the client response and inspect the corresponding log entries for a new failure or a successful request.
Apache can include the same %L log ID in access- and error-log entries, which can help correlate a request across the two logs when that format is configured. Details are in the logging reference.
Why a custom error page does not fix the cause
An ErrorDocument changes what is returned for an error; it does not repair the configuration, module, or application that caused the 500. When its target is a remote URL, Apache redirects the client, so the response is a redirect status rather than the original error status. Treat a custom error page as presentation, not diagnosis or recovery. Apache explains this behavior in the core directives reference.
Version and installation caveats
The commands and references here follow the Apache HTTP Server 2.4 documentation. Confirm your installed version, loaded modules, and deployment-specific service controls before applying examples. Apache directives are module-dependent and may be valid only in particular configuration contexts; configuration can also be scoped to the server, a virtual host, a directory, a file, or a URL. The configuration guide explains these scopes.
An HTTP 500 is defined broadly as an unexpected server condition that prevented fulfillment of the request, not as a diagnosis of an Apache-specific fault. See RFC 9110, HTTP Semantics (published June 2022). The exact correction depends on the matching log entry and local configuration.
