Fix a bad AI customer-service answer by tracing it through the whole system: verify the approved answer is current, confirm the system retrieves it for the customer’s actual wording, check whether the model stays faithful to the evidence, and test the change on reviewed examples. A prompt tweak alone will not repair missing policy content or a retrieval failure.
Start by identifying what failed
Save the exact customer question and bot response, the relevant support-policy version, and the passages the assistant received. Label the failure before changing anything: generic answer despite relevant policy, incorrect fact or procedure, missing answer, unsupported claim, stale policy, failure to clarify, or failure to hand off.

This distinction matters because a grounded assistant can only answer from the information it receives. If the approved answer is missing from that context, changing the wording of the prompt cannot make the answer reliably correct. NIST describes retrieval failure as a common cause of retrieval-augmented generation (RAG) failures in its July 2025 initial public draft of NIST IR 8579, which documents an internal cybersecurity-guidance chatbot prototype—not a field study of commercial customer-service systems.

- Confirm the authoritative answer exists in an approved support source and applies to the customer’s product, region, plan, and date.
- Check that instructions and exceptions are explicit enough to answer the question, rather than buried in unrelated material.
- Resolve conflicting or duplicate articles, and clearly identify policies that have been superseded.
These are practical knowledge-management checks, not procedures prescribed by NIST. Its report describes preprocessing and indexing external documents as stages in a RAG pipeline; it does not prescribe a customer-support content-governance process.

Inspect retrieval before changing the prompt
Follow the failed question through the system and inspect what actually reached the model. Retrieval is separate from answer generation: a pipeline can preprocess and index content, find relevant material for a user’s need, and pass retrieved passages to the model as context. A failure at any of those stages can leave the model with the wrong evidence or none at all.
- Check the query. Record the customer’s wording and any query rewrite the system applies.
- Check the indexed content. Confirm the relevant document and its current version are indexed and available to this customer’s access level.
- Check the ranked results. See whether the right passage appears, whether a filter excludes it, and whether other passages outrank it.
- Check the model’s context. Inspect the exact passages supplied to the model—not just what the search interface found.
Test whether a paraphrase, product name, synonym, typo, language, or multi-part question changes the result. If the relevant policy is absent from the model’s context, fix content coverage, indexing, query handling, ranking, or filters before revising the answer prompt.
NIST IR 8579 describes semantic retrieval and ranked chunks, and its prototype selected a top-k value of 3 for its own use case. That is a prototype setting, not a generally optimal value. Choose retrieval settings by evaluating your own corpus and questions.

Check whether the answer is supported by its evidence
If the right policy is present in context, compare the response against it. Check that the assistant answers the question asked, preserves conditions and exceptions, and does not invent specifics. Where the product supports it, expose the source so reviewers—and, where appropriate, customers—can trace the answer.
Set a fallback for absent or conflicting evidence. A validation stage can check whether a draft answer is supported by the passages supplied to the model. NIST’s prototype used a model-based filter to suppress responses classified as hallucinated, while acknowledging that this kind of mitigation does not eliminate hallucination risk. Grounding improves traceability; it does not guarantee truth.
Evaluate changes with reviewed, representative questions
Build a test set from real support interactions, with reviewed expected answers and the policies that support them. Cover frequent intents and paraphrases as well as ambiguous questions, edge cases, current and superseded rules, and questions that should trigger clarification or a human handoff. Review correctness, relevance, evidence support, preservation of exceptions, and appropriate abstention.

The NIST AI Risk Management Framework calls for realistic test sets that represent expected use, documented testing methods, and ongoing testing or monitoring. Results can also be examined across relevant data segments. The framework is voluntary risk-management guidance, not a customer-service implementation recipe; NIST says it is being revised.
NIST IR 8579 reports that its authors created about 100 manually selected and revised question-answer pairs for a small prototype evaluation. They used Ragas with an LLM-as-a-judge for automated scoring and manually reviewed only a very small subset. The report gives a Pearson correlation of 0.7872 between manual and automated assessments, but says the manual sample was too small to support a conclusion with statistical confidence. Its authors conclude: “This led us to conclude that automatic assessment systems can be useful for development purposes, but they cannot replace rigorous human assessment at this time.” Those figures describe this prototype, not a recommended test-set size or a customer-service benchmark.

Define what the assistant may answer, what evidence it needs, when it should ask a clarifying question, and when it must route the customer to a person. Escalate high-impact or policy-sensitive requests according to your organization’s risk assessment. NIST guidance recognizes that human intervention may be needed when an AI system cannot detect or correct its errors; its 2021 NIST IR 8312 also supports operating within intended conditions and only at sufficient confidence.

Monitor answer quality after launch. Rerun representative tests when support content, retrieval configuration, model, prompt, or integrations change. NIST’s AI Risk Management Framework emphasizes context-specific trade-offs and ongoing monitoring rather than treating launch as the end of evaluation.
