There is no single U.S. database containing everyone’s records, and CDC does not keep individual records. If documentation is missing, don’t guess at doses or timing; a clinician can help decide whether to repeat a dose or consider testing for immunity.
Where to look for vaccination records
Records may be spread across family papers, healthcare providers, schools, employers, and state or local immunization systems. No one source is guaranteed to have a complete history. Start with the places most likely to have records and ask each organization what identifying information it needs and whether it can provide an official copy.

- Check at home. Ask parents or caregivers, and look through baby books, paper immunization cards, medical files, and other saved documents.
- Contact previous healthcare providers. Ask former doctors’ offices, public health clinics, and pharmacies you used whether they still have your records.
- Check schools and colleges. Contact schools you attended if you need records they received or maintained. The CDC says schools generally keep records for only 1–2 years after a student leaves, so older records may no longer be available.
- Ask employers or service organizations. Previous employers, including military service, may have vaccination documentation.
- Ask your state or local health department. State Immunization Information Systems (IIS) may include adult vaccines, but coverage and retention vary. A registry search may help without providing a complete history.
The CDC’s record guidance explains that there is no national organization maintaining everyone’s vaccination records and that CDC does not have an individual’s record. See CDC: Staying Up to Date with Your Vaccine Records.
What to do if records are missing or incomplete
Take any records you do find to a primary-care clinician, pharmacist, or public health clinic. Don’t fill gaps by estimating when doses were given or assuming that a vaccine was received.

- Ask whether repeating a dose is appropriate. CDC says some vaccines may need to be repeated when records cannot be found and that repeating vaccines is safe. That general guidance is not a personalized recommendation for a particular vaccine or person.
- Ask whether an immunity test could help. A clinician may sometimes use a blood test to assess immunity for certain diseases. Testing is not an established substitute for records in every situation; ask whether it makes sense for your specific history.
- Bring all available details. Share approximate dates only as estimates, along with any known vaccine names, doses, health conditions, pregnancy, or other circumstances that could affect recommendations.
CDC describes these options in its guidance on missing vaccine records. A clinician should decide how to handle uncertainty rather than treating a generic catch-up list as an individual plan.
How a clinician reviews catch-up needs
Vaccination decisions depend on more than age or a list of vaccines. A clinician uses the person’s documented doses and intervals, age, health conditions and other risk factors, pregnancy or other special situations, and any contraindications or precautions. Recommendations can also distinguish routine vaccination from vaccination for a specific indication or shared clinical decision-making.

Adults
For adults aged 19 years or older in the United States, CDC organizes recommendations by age, missing documentation or evidence of immunity, and additional risk factors or other indications. The schedule includes notes on dosing, special situations, and contraindications. Ask the clinician to review your history against the current CDC adult immunization schedule by age and related clinical notes, rather than relying on an older saved copy.
CDC’s adult schedule page identifies a July 2, 2025 schedule and notes an April 27, 2026 amendment adding an ACIP RSV recommendation for certain high-risk adults aged 50–59. Schedule details can change, so confirm the live page when planning care.

Children and adolescents
Children and adolescents have a separate CDC catch-up schedule. Its tables specify minimum ages and intervals between doses; the right sequence depends on doses already received and when they were given. Use the CDC catch-up immunization schedule for children and adolescents with the child’s clinician. Do not apply the adult schedule to a child or treat minimum intervals as a one-size-fits-all plan.

Once a dose is given, ask the provider for a written record or use the CDC’s downloadable vaccination record form. Bring it to future visits and ask the provider to sign and date each vaccination. You can also ask whether the dose will be entered into your state’s registry; registry participation and coverage can vary.
