How to Find Specialty Care When You Live in a Rural Area

Start with the clinician or rural clinic already managing your care. Ask which specialty you need, whether the first visit can be by telehealth, and what referral, insurance approval, records, and costs must be settled before you book travel. Then work with the clinic, health plan, or local navigator to identify regional options and any transportation help that actually serves your area.

Start with the clinician who knows your case

Contact your primary-care clinician, Rural Health Clinic (RHC), community health center, or hospital clinic. Medicare describes RHCs as providers of outpatient primary and preventive services in non-urbanized areas with provider shortages; an RHC can be a useful local starting point, but it does not mean a needed specialist practices nearby. Medicare’s RHC overview explains the clinic role.

Ask the clinician or referral staff:

  • Which specialty is needed, and how urgent is the consultation?
  • What records, test results, or images should go with the referral?
  • Does the specialist need to examine you in person, or could an initial or follow-up visit be remote?
  • Who in the office handles referrals or care coordination?

Referral rules depend on the health plan and the service. Ask the referring office to send the referral and confirm that the specialist received it, but verify the plan’s requirements yourself before scheduling.

Search across the region, not just your town

Ask the referring office about specialist clinics and regional hospital systems that accept your plan. A care coordinator may know which facilities handle referrals, what records to send, and whether follow-up tests can be arranged closer to home. Confirm directly with each office that the specialist is accepting patients and that the office participates in your specific insurance network; directory listings and general recommendations do not guarantee either.

When comparing available routes, consider the factors that affect whether care is practical as well as clinically appropriate:

  • Whether the specialist can address your condition remotely or needs an in-person exam or test.
  • How far you must travel and how many trips may be needed for consultation, testing, and follow-up.
  • Network status, referral and prior-authorization requirements, and your estimated out-of-pocket cost.
  • Appointment availability and how follow-up will work.
  • Whether a telehealth visit requires video, or whether an appropriate audio-only option is offered.

Ask whether telehealth is appropriate and covered

Ask the specialist and referring clinician whether a remote appointment is suitable for your condition. Some consultations or follow-ups may be possible by telehealth; others may require an exam, imaging, laboratory work, or another in-person service. Confirm the arrangement with both the provider and your insurer, including whether any in-person follow-up would mean a separate trip.

If you have Medicare

Medicare telehealth policy is time-sensitive, and the rules for a particular service matter. HHS’s Medicare telehealth policy summary, last updated February 5, 2026, describes several temporary flexibilities through December 31, 2027, including home access without geographic restrictions for non-behavioral/mental telehealth and audio-only availability. For the specific provision allowing Rural Health Clinics and Federally Qualified Health Centers to serve as distant-site providers, later CMS guidance takes precedence: CMS guidance MM14468, issued May 28, 2026, says that flexibility was extended until January 1, 2028, with payment through calendar year 2027. HHS also describes permanent Medicare flexibilities for behavioral and mental telehealth, including home access and RHC/FQHC participation.

These Medicare dates do not establish coverage for every specialist, service, or payer. Ask the plan and specialist whether the exact service is currently covered, whether the provider participates, and whether audio-only care is appropriate and available. Check current rules when booking because policy can change.

If you have Medicaid or private insurance

Telehealth coverage and other plan rules vary. HHS’s coverage and payment resource links to state Medicaid and private-insurance information and notes that Medicaid policies differ by state. Contact your plan for the rules that apply to your specific service and provider.

Confirm coverage, approvals, and likely cost before booking

Call the insurer using the member-services number on your card, and contact the provider’s billing or referral office. Ask about the particular specialist, location, and appointment type—not just whether the clinic accepts your insurance.

  • What are the expected copay, coinsurance, deductible, or facility charges?
  • Could tests or follow-up visits create separate charges or require another trip?

Ask both the insurer and provider for an estimate before care. For Medicare, the RHC services page says most preventive services have no charge and that, after the Part B deductible, patients generally pay 20% of covered charges. That is general Medicare information, not a quote for a specialist visit or an individual bill: actual costs can depend on other insurance, the clinician’s charges and assignment, facility type, and service location. Medicare advises patients to ask their clinician what a service will cost.

If travel is difficult, ask about local help

Long distances and limited public transportation are documented rural access barriers. The 2023–2027 Rural Health Care Coordination Program Directory describes those challenges and profiles ten coordination initiatives funded in ten states. Its examples show approaches used in participating communities, not services guaranteed in every rural area.

Ask the clinic care coordinator, insurer, county or community social-service navigator, and the specialist’s patient-services office whether a local ride, lodging resource, or other support is available for your appointment. Verify eligibility, scheduling, service area, and any cost directly; do not assume a plan or program will cover travel.

Use rural health resources to find a navigator

HRSA’s rural health resources page, last reviewed in April 2025, points patients and communities to the Rural Health Information Hub (RHIhub). RHIhub offers state guides, a resource library, toolkits, program models, and free help by phone or email. These resources can help you investigate regional contacts and support, but they cannot confirm that a particular specialist, appointment, or ride is available to you.
