This guide focuses especially on LGBTQ+ and gender-identity-related care, but the same process can help you find a therapist who understands other important identities and intersecting needs. The sources behind this guidance are primarily U.S.-oriented; verify licensing, insurance, telehealth availability, cost, and other requirements where you live.
Decide what you need from an affirming therapist
“Affirming” can describe different things. You might want a therapist with basic familiarity who will treat you respectfully, a specialist in a particular identity or concern, a provider who shares an aspect of your identity, or someone who can help with a specific task such as documentation. You might also want your identity understood without making it the focus of every session.

Before searching, write down the concern you want help with and what you need the therapist to understand about your identity. Separate essential requirements from preferences. For example, having experience with your specific identity may be essential, while a shared identity or a particular therapy format may be a preference. This helps you compare candidates on what matters to you rather than on a single profile phrase.
- Your goals: What would you like help with, and what would make therapy useful to you?
- Identity-related needs: Do you need a therapist who can discuss identity-related experiences, a focused specialist, or someone who simply respects your identity while addressing other concerns?
- Practical requirements: Consider cost, insurance, location or telehealth, language, accessibility, scheduling, and any documentation you need.
- Personal preferences: Decide whether provider identity, community experience, or a particular treatment approach matters to you.
A specialist is not automatically necessary for every concern. NAMI notes that specialization may not be needed when the issue a person wants to address is not rooted in sexual orientation or gender identity. The useful question is whether a candidate has the competence and approach your particular goals require.

Where to look for candidates
Use more than one search route. A provider may not appear in every directory, and a listing is a starting point—not proof that a therapist has the experience or approach you need.
- Your insurer: Search the insurer’s provider directory or call the member-help number on your insurance card. Confirm that the therapist is currently in network and accepting new clients; directory details and coverage can change.
- LGBTQ+ directories and organizations: Look at provider directories maintained by LGBTQ+ organizations, as well as local LGBTQ+ community or health centers and community groups. Ask whether they can suggest providers or explain how their listings are compiled.
- Health professionals: Ask a primary-care clinician or another health professional for referrals. You can specify the identity-related experience or practical requirements you are looking for.
- Professional directories: Professional association tools can help you locate clinicians by area and practice interests. The APA Psychologist Locator, for example, lets users search by city or ZIP code and practice area. A directory search does not certify a provider’s competence with a particular identity.
Search terms such as “LGBTQ+ affirming therapist near me” can help surface candidates, but the word “affirming” in a profile is only a lead. Apply the same direct screening questions whether you found someone through an insurer, a community referral, or a professional listing.
Screen a therapist before booking
If possible, contact candidates for a brief call or message exchange before scheduling. Ask questions that invite specific answers about experience and practice, not just a yes-or-no assurance that they are affirming. You can adapt these examples to your own identity and goals:

- Ask about experience: “My identity is ____. What experience do you have working with people with that identity?”
- Ask about preparation: “What training or continuing education have you had in this area?”
- Ask how they work: “How do you make space for identity-related issues while also addressing my other goals?”
- Ask about your specific need: If you need a letter or other documentation, ask whether the therapist understands the applicable insurer or legal requirements and whether they provide that service.
- Ask about practical fit: Confirm availability, session format, fees, insurance, language or accessibility accommodations, and how to raise confidentiality questions.
For a concern about conversion or reparative therapy, ask directly whether the therapist offers it and how they understand the practice. NAMI describes these practices as efforts aimed at changing sexual orientation or gender identity and advises people to ask direct follow-up questions. If the answer is vague, ask what the therapist would do if you wanted support in accepting or exploring your identity rather than changing it.
Pay attention to how a candidate responds, not only to the information they provide. Do they answer plainly, invite questions, and make room for your stated goals? If you feel pressured to disclose more than you want before deciding whether to book, you can end the conversation and contact another provider.
Compare the whole fit, not just identity credentials
Identity-specific understanding matters, but it is one part of choosing a therapist. APA guidance emphasizes credentials and competence as well as rapport and personal comfort. Compare candidates against the same practical criteria so that a polished profile or a single reassuring answer does not outweigh a concern that matters to you.

| What to compare | Questions to ask or check |
|---|---|
| Identity-related experience | Has the therapist worked with people with your specific identity or relevant intersecting needs? Can they describe how that experience informs their work? |
| Training and competence | What relevant training, continuing education, or professional experience do they have? Are their qualifications and license appropriate for the service and location? |
| Your goals and treatment approach | Can the therapist explain how they would address the concern you named? Do they make space for identity without assuming it explains every difficulty? |
| Credentials and experience | What is the provider’s professional role and relevant experience? Verify license status and scope of practice through the appropriate licensing authority for your location. |
| Cost and coverage | Are they in network for your specific plan? What are the session fee, expected out-of-pocket cost, and any charges for services such as documentation? |
| Availability and format | Are they accepting clients? Do they offer in-person or remote sessions that work for your schedule and location? Confirm that remote care is available to you where you are. |
| Communication and access | Can they meet your language, disability-access, or other communication needs? Will they use the name and pronouns you request? |
| Confidentiality and documentation | How do they handle confidentiality, records, and any documentation you need? Ask how relevant rules apply to your circumstances. |
| Comfort and rapport | Did you feel listened to and respected during the first contact? Could you imagine being candid with this person? |
There is no single score that determines the best choice. Some criteria are non-negotiable—for example, a required service or an affordable fee—while others are preferences. If two candidates both appear qualified, consider which one understood your goals more clearly and made it easier to ask questions.

A screening call can reveal a lot, but the first conversation or session gives you more evidence about how the therapist actually works. You do not need to wait for a crisis or a major disclosure to assess the relationship.
- Notice whether the therapist listens before interpreting what you say.
- See whether they use the name, pronouns, and language you request, and respond constructively if they make a mistake.
- Notice whether you are expected to teach basic identity concepts that the therapist should already understand for the work they claim to do.
- Check whether identity can be discussed when relevant without being treated as the presumed cause of every problem.
- Ask whether the therapist can explain how sessions connect to your goals and whether you can revisit that plan together.
These are practical observations, not a formal test or guarantee of future care. You can tell the therapist what is or is not working, ask for a change in approach, or decide to look elsewhere.

Finding a therapist for a child or teenager
When searching for a child or adolescent, ask specifically about experience with that age group. Experience treating adults does not by itself establish that a provider is qualified to work with a young person. For gender-related care, ask about training in gender identity development, how the clinician works with both the young person and family, and whether they collaborate with other providers when appropriate.
- Ask what experience the therapist has with the child’s developmental stage and the concern bringing the family to therapy.
- For gender-related questions, ask how the therapist’s training and approach align with current professional guidance, including relevant APA guidance and the WPATH Standards of Care.
- Ask how the therapist balances the young person’s participation with family involvement and what confidentiality means in that setting.
- If several professionals are involved, ask how the therapist coordinates care when appropriate and with the necessary permissions.
- After an initial visit, ask the young person how they felt heard and respected; their response matters when deciding whether to continue.
APA guidance cautions that a gender-diverse identity alone is not necessarily a reason for psychotherapy. Therapy should address the young person’s actual needs rather than treating identity itself as a problem. The APA’s statement is specific to gender-diverse children and adolescents; do not assume that guidance for this population answers every question about every identity or age group.
What “affirming” means—and what a label cannot tell you
The American Psychiatric Association’s introductory guide describes gender-affirming therapy as “a therapeutic stance that focuses on affirming a patient’s gender identity and does not try to ‘repair’ it.” The guide describes practices such as affirmation, space to process and understand, connection with social supports and other services, creating a safe zone, allowing for diversity, reflection, and empathy.

That guide is dated November 2017 and calls itself an introductory guide. It can help explain the term, but it should not be treated as the sole or current authority on professional practice. Ask candidates how they work and consult current professional guidance relevant to your situation.
A directory label, stated interest, or referral can help you find someone to contact; none alone establishes competence. Likewise, a therapist does not have to share your identity to provide respectful care, and sharing an identity does not by itself establish clinical fit. Screen for the combination that matters to you: relevant understanding, sound professional qualifications, an approach suited to your goals, and a working relationship in which you feel respected.
If the therapist is not the right fit
It can take several calls to find a suitable provider, and the first therapist you meet may not be the right one. A mismatch does not mean you asked the wrong question or that therapy cannot help. If the concern feels manageable, you can tell the therapist what you need to change and see how they respond. If you do not feel comfortable continuing, you can stop and contact another candidate.

A practical search sequence
- Write a short brief for yourself. Note the concern you want help with, the identity-related understanding you need, and your non-negotiables for cost, access, format, language, or documentation.
- Gather names from several sources. Check your insurer, relevant LGBTQ+ organizations or community services, referrals from health professionals, and professional association directories.
- Verify the basics. Confirm the provider’s qualifications, location or remote-care availability, current openings, fees, and insurance status directly rather than relying only on a listing.
- Contact more than one plausible candidate if possible. Ask about identity-specific experience, training, approach, and any service you need. For a concern about conversion or reparative therapy, ask about it directly.
- Compare responses against your priorities. Consider competence, approach, practical access, and how comfortable you felt asking questions.
- Reassess after the first contact or session. Notice whether the therapist listens, respects your requested language, and can address your goals without making assumptions about what your identity means.
- Continue, discuss a change, or keep searching. You can revisit the approach with the therapist or choose a different candidate if the fit is wrong.
Provider eligibility, licensing rules, insurance coverage, telehealth availability, fees, and appointment openings depend on your location and can change. Confirm the details that apply to you with the provider, insurer, and relevant licensing authority before relying on them.
