The safest route to a legitimate cancer clinical trial is to start with your oncologist or care team, search the National Cancer Institute (NCI) trial database and ClinicalTrials.gov, and verify each study against its registry record and the sponsoring institution. A registry listing makes a study traceable, but it does not prove that the trial is suitable, safe, or free of conflicts. If artificial intelligence (AI) may be involved, ask exactly what data it uses, what output it produces, whether a clinician can override it, and how that use is described in consent documents.

What a legitimate cancer trial looks like

A real trial has an identifiable study record, a defined scientific purpose, named investigators or a lead organization, stated eligibility criteria, participating locations, and a way to contact the study team. Its protocol should explain what participants receive, what the comparison group receives when applicable, which tests and visits are required, and how outcomes will be measured.

For certain applicable clinical trials, federal law requires registration and results reporting on ClinicalTrials.gov. The FDA says that this creates a transparent public record and supports public trust. Registration is a traceability check, not an endorsement: it does not establish that a study is appropriate for your diagnosis or that its risks are acceptable for you.

  • Traceable identity: a registry identifier, study title, sponsor or lead organization, and named study contacts.
  • Coherent protocol: a purpose, intervention, phase, outcome measures, and eligibility rules that fit together.
  • Verifiable site: a hospital, university, cancer center, company, or other institution that confirms the study independently.
  • Understandable consent: a document covering procedures, risks, possible benefits, alternatives, costs or coverage, privacy, data and specimen use, and contact information.

Be cautious if a listing promises a cure, guarantees acceptance, demands immediate enrollment, asks you to pay an intermediary for access, or requests sensitive financial information before you have reached a verified study contact. Missing sponsor, protocol, location, or contact details are also warning signs.

Prepare the information needed for a useful search

Trial matching works only when the search reflects your actual clinical situation. Before searching, gather the details below. Your care team or medical records department can help confirm them.

  • Cancer type and current stage.
  • Pathology findings, including relevant molecular or biomarker results.
  • Prior treatments, dates, responses, and any treatment you are currently receiving.
  • Current health, organ-function issues, other diagnoses, medications, and performance status as your clinician describes it.
  • Age and the locations where you could realistically attend visits.
  • Your treatment priorities, such as access to a new therapy, fewer trips, a particular biomarker, or a preference about placebo or randomization.

Do not post identifiable medical records in a public form just to obtain a trial lead. Give detailed records to a verified clinical team through its approved process.

Search in the two key registries

1. Ask your oncologist or care team first

NCI advises starting with a clinician. An oncologist, oncology nurse, genetic counselor, or another member of the care team can identify trials that fit your diagnosis, interpret technical eligibility rules, and explain how participation would fit with standard treatment. They can also help distinguish a genuinely relevant study from one that merely uses a similar disease keyword.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Use the NCI cancer-trial search

NCI’s cancer-trial tool provides filters for cancer type, age, keywords, location, prior drugs or treatments, phase, trial ID, investigator, and lead organization. Search broadly enough to catch studies using a different name for the same cancer, then narrow by stage, biomarker, previous therapy, and travel distance.

3. Search ClinicalTrials.gov as a second registry

ClinicalTrials.gov includes NCI-listed trials as well as studies run by pharmaceutical and biotechnology companies that may not appear in NCI’s list. Search by condition, intervention, location, sponsor, investigator, and known trial identifiers. Use the second registry to find records missed by the first and to compare how a study is described.

4. Treat commercial listing sites as leads only

Third-party services can surface possible studies, but NCI warns that some may not be updated regularly, may require registration, or may charge fees. A listing is not proof that recruitment is open or that the intermediary is authorized to enroll anyone. Move from the listing to the official registry and the institution’s own clinical-trials office before sharing documents or making travel plans.

Verify every candidate before contacting the study

Open the complete registry record and any available protocol summary. Save a copy or print it with the date you checked it; records can change. Work through these fields rather than relying on a search-result headline.

Check What to confirm Why it matters
Study identity Exact title, registry identifier, phase, and version or update date Prevents confusion between similarly named studies and shows when the information was last revised.
Purpose and design Scientific question, intervention, comparator, randomization, masking, and primary outcomes Explains what the trial is testing and what participation may involve.
Status Recruiting, not yet recruiting, active but not recruiting, suspended, completed, or withdrawn A search page can remain visible after enrollment has stopped.
Eligibility Cancer type and stage, biomarkers, prior therapies, age, laboratory values, organ function, and exclusions General disease matching does not establish that you qualify.
Locations Specific facility, city or region, and whether that site is currently recruiting A study can be open at one location and closed at another.
People and organizations Sponsor or lead organization, principal investigator, site investigator, and study coordinator Provides accountable parties you can verify independently.
Contact route Telephone, email, or web contact shown in the registry and on the institution’s own site Lets you detect copied, altered, or outdated contact details.

Then cross-check the study on the sponsoring hospital, university, cancer center, or company website. Call that institution’s clinical-trials office using contact information you obtained independently, not only a phone number supplied by an advertisement or social-media post. Ask the office to confirm the registry identifier, recruiting status, location, and coordinator.

Confirm eligibility without assuming you are enrolled

A registry can show broad inclusion and exclusion criteria, but it cannot determine your eligibility from a short description. During pre-screening, the coordinator can answer questions from you and your doctor. NCI notes that final eligibility is usually decided after you meet the trial doctor and the site reviews your records, examination, and required tests.

  1. Send only the records the verified study team requests, through its approved secure channel.
  2. Ask which criteria are preliminary and which require laboratory, imaging, pathology, or genomic confirmation.
  3. Request a written list of screening tests, expected timing, and who pays for each test.
  4. Ask what happens if a test is borderline, delayed, or unavailable at your current hospital.
  5. Do not stop or change treatment to qualify unless your treating clinician and the trial doctor jointly discuss that decision.

Discuss the trial with your treating clinician

Bring the saved registry record and protocol summary to your oncologist. A useful discussion covers more than whether your name appears to match a search filter.

  • What happens if the study closes, the intervention is ineffective, or you decide to stop?

How to determine whether AI is involved

AI can be the treatment under investigation, or it can operate in the background. A study might use machine learning for image analysis, eligibility abstraction, participant assignment, trial operations, data collection, monitoring, or study design without describing “AI” in a prominent title.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Search the record and consent materials

Review the registry title, brief summary, detailed description, intervention names, outcomes, keywords, protocol, and consent documents for terms such as artificial intelligence, AI, machine learning, algorithm, deep learning, automated image analysis, prediction model, natural-language processing, chatbot, and computer-aided detection. Also look for a named vendor, software product, model, or data platform.

The absence of those words does not prove that no AI is used. A coordinator should be able to explain whether software makes or supports decisions and whether an outside organization receives study data.

Ask what the system does

Question What a clear answer should identify
What is the intended purpose? Whether AI is the treatment, a diagnostic aid, a research measurement, an administrative tool, or something else.
Who uses the output? The treating clinician, radiologist, pathologist, coordinator, automated system, or another role.
What data go in? Images, pathology slides, electronic records, genomic data, symptoms, laboratory values, audio, or other measurements.
What comes out? A classification, risk score, recommendation, alert, eligibility flag, or automated action.
Can a clinician override it? Where human review occurs, whether it is mandatory, and how disagreements are recorded.
Does it affect care or trial decisions? Whether it influences treatment, eligibility, randomization, monitoring, or only research administration.
Which version is being evaluated? The model or software version, whether it is fixed during the study, and how updates are controlled.
How are failures handled? How errors, bias, missing data, unsafe outputs, and performance differences between patient groups are detected and reported.
Where does data go? Whether data remain at the institution, move to a vendor or cloud service, are coded or identifiable, and may be shared onward.

Use protocol-transparency principles

SPIRIT-AI recommends that a protocol identify the AI intervention and intended use, describe the clinical setting and users, specify inputs and outputs, explain human–AI interaction and downstream decisions, state technical integration and access restrictions, and plan how performance errors will be identified and analyzed. If a registry summary is short, ask the coordinator where these details appear in the full protocol or consent discussion.

NIH guidance for institutional review boards says researchers should clearly explain AI use in the study plan so the board can review it. That does not eliminate risk, but it gives you a reasonable basis for asking how the system was evaluated and supervised.

Review consent, privacy, and your right to leave

NCI identifies informed consent, institutional review board (IRB) review and approval, and ongoing monitoring as central participant protections. Consent should be a conversation in understandable language, not merely a signature page. Keep your own copy and note the version or date.

Ask specifically whether AI receives identifiable or coded information; whether images, tissue, blood, or genomic specimens can be reused to train or validate models; whether data leave the institution; who can access them; how long they are retained; and whether future sharing is optional. If an AI-enabled activity is ancillary rather than necessary to the main trial, ask what happens if you decline that activity.

Clarify whether an automated output can influence your care, even when a clinician reviews it. Ask how you will be told about a serious error, a change in the software, or a new use of your data. FDA’s required ClinicalTrials.gov disclosure includes the sentence: “This Web site will not include information that can identify you.” That public-record statement does not mean the research team never handles identifiable information; the consent form should explain how the study protects the data it collects.

You can leave a trial at any time. Ask whom to notify, what follow-up is recommended after withdrawal, whether already collected data or specimens can still be used, and whether stopping affects access to ordinary medical care.

Money or identity requests

A legitimate study may discuss insurance, travel, reimbursement, or billing, but an intermediary demanding payment for access is not the same as an authorized study fee. Do not send bank details, government identification, or a full medical record until you have independently confirmed the institution and secure contact method.

Inconsistent records

Stop to reconcile differences in sponsor, locations, recruitment status, intervention, or contact names between an advertisement, registry, and institutional website. If the institution cannot explain the discrepancy, do not proceed through that listing.

Unverifiable AI claims

A promoter may use “AI” as a marketing label without explaining the model, data, output, or oversight. Ask the technical and consent questions above. If no one can answer them in plain language, treat that as a safety and transparency concern.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare two plausible trials systematically

When more than one study fits, make a side-by-side worksheet with the same fields for each candidate. This prevents an exciting intervention from overshadowing practical burdens or unanswered safety questions.

Comparison area Record for each trial
Scientific purpose and phase What question is being tested and how established the intervention is.
Intervention and comparator Drug, device, procedure, behavioral program, usual care, placebo, or other control.
Eligibility Biomarkers, prior treatment, age, laboratory limits, and exclusions.
Burden Visits, tests, biopsies, infusion time, monitoring, travel, and total expected duration.
Risk and uncertainty Known adverse effects, procedure risks, unknowns, and stopping rules.
AI role and data governance Purpose, inputs, outputs, human oversight, software version, sharing, and reuse.
Sponsor and site Organizations, investigators, site experience, and independently verified contacts.
Costs and coverage Routine-care charges, sponsor-covered items, reimbursement, and possible out-of-pocket costs.
Alternatives and exit plan Other treatment choices and what happens if you decline or withdraw.
Consent clarity Whether answers are understandable, complete, and consistent with the registry.

A practical call checklist

Use this script with a verified coordinator or clinical-trials office:

  1. “Please confirm the registry identifier, the exact site, and whether this location is recruiting now.”
  2. “Which eligibility requirements are most likely to determine whether I qualify?”
  3. “What treatment, tests, visits, travel, and time commitment should I expect?”
  4. “What costs are covered by the sponsor, billed to insurance, or potentially mine?”
  5. “Is AI or machine learning used anywhere in the treatment, diagnosis, eligibility review, randomization, monitoring, data analysis, or administration?”
  6. “What data does that system receive, what output does it create, and who reviews or can override it?”
  7. “What software or model version is being evaluated, and can it change during the trial?”
  8. “Are my images, records, or specimens used to train or validate a model, shared outside the institution, or retained for future research?”
  9. “Where are those details explained in the consent form, and may I take a copy home?”
  10. “Who should my oncologist contact with medical questions, and what happens if I decide not to enroll or later withdraw?”

Getting free help with the search

NCI’s Cancer Information Service offers a free, tailored clinical-trial search by phone, email, or chat. You still need your treating clinician and the study site to assess medical fit and eligibility, but the service can help identify candidates to discuss. Keep the registry record, protocol summary, consent form, and your notes together so every person involved is reviewing the same version of the study information.

Before signing: a final decision check

  • I can identify the registry record, sponsor, site, investigator, and verified coordinator.
  • The recruitment status and location have been confirmed directly with the institution.
  • My oncologist and the trial doctor have discussed benefit, risk, alternatives, timing, and eligibility.
  • I understand visits, tests, travel, insurance handling, reimbursement, and possible out-of-pocket costs.
  • I know whether AI is involved, what data it uses, what it produces, and who supervises it.
  • The consent form explains privacy, data or specimen reuse, AI-related activities, contacts, and withdrawal.
  • I have had enough time to ask questions and take the consent form home before deciding.
