You do not have to exfoliate keratosis pilaris (KP): the harmless condition needs treatment only if its bumps or symptoms bother you. If you choose to treat it, use a gentle approach and follow it consistently rather than scrubbing hard. KP bumps form when dead skin cells plug hair follicles, commonly on the upper arms and thighs.
Should you scrub KP bumps?
No. Vigorous scrubbing, scraping, and harsh body scrubs can irritate skin and make KP worse. The American Academy of Dermatology (AAD) advises: “Avoid scrubbing your skin, which tends to irritate the skin and worsen keratosis pilaris.”

If you prefer physical exfoliation, try light strokes with a soft washcloth or a similarly gentle tool while bathing. AAD also mentions a loofah or buff puff, but these are optional—not a reason to rub harder. Stop if your skin becomes uncomfortable or more irritated.
Choose a gentle exfoliation approach
There is no established universally best method. Consider your skin’s sensitivity, dryness, and current routine, including whether you use retinoids or benzoyl peroxide. Dry, sensitive, or acne-prone skin may tolerate a washcloth and a mild chemical exfoliator better than mechanical exfoliation; more vigorous friction can cause irritation. General exfoliation guidance also advises against exfoliating over open cuts, wounds, or sunburn. Aggressive exfoliation may cause redness and, for some people—particularly those prone to dark spots—increased pigmentation.

| Approach | What it involves | When to be cautious |
|---|---|---|
| Light physical exfoliation | Gentle strokes with a washcloth or similarly soft tool during bathing. | Skip vigorous rubbing. If your skin is dry, sensitive, or acne-prone, friction may irritate it. |
| Topical keratolytic | A product with an ingredient such as urea, lactic acid, salicylic acid, glycolic acid, another alpha hydroxy acid, or a retinoid. | Use one product as directed rather than combining several exfoliating actives. Too much or too-frequent use can leave skin raw or irritated. |
These ingredients are options, not a combination routine. If you already use retinoids or benzoyl peroxide, be especially mindful of dryness and irritation when considering another active. Follow the product label or your clinician’s directions for the amount and frequency.
A gentle routine for KP-prone skin
- Bathe gently. Use a mild cleanser and warm rather than hot water. AAD’s KP self-care advice suggests keeping baths or showers to 20 minutes or less.
- Use light touch, if tolerated. If you choose a physical tool, move it gently over the skin; do not scrub or scrape the bumps.
- Apply an optional active only as directed. Choose a suitable keratolytic product and follow its label or your clinician’s plan. AAD’s guidance includes several possible ingredients, but does not make one frequency right for everyone.
- Moisturize after bathing. Apply a thick, oil-free cream or ointment to damp skin within five minutes. AAD recommends looking for a moisturizer containing urea or lactic acid; reapply when skin feels dry.
AAD’s self-care guidance describes daily home care until skin looks clearer, while its treatment guidance emphasizes following a dermatologist’s recommended frequency and notes that maintenance may later be as little as twice weekly. Those are not universal schedules: use the instructions for your product or clinician’s plan, and ease off if irritation develops.

What to do if your skin gets irritated
If an active medicine leaves your skin dry or irritated, stop using it for a few days, as AAD advises, and let the irritation settle. When you resume, follow the product or clinician’s directions rather than increasing the amount or frequency. Keep moisturizing, and avoid adding more exfoliating steps. If symptoms persist or you are unsure what is causing them, ask a dermatologist for guidance.
Shaving or waxing can cause more bumps for some people with KP, according to AAD. If you notice a connection, consider avoiding or changing that hair-removal method.

When to check in with a dermatologist
KP treatment does not cure the condition, and maintenance may be needed. If your treatment plan has not improved things after four to six weeks, contact a dermatologist. That interval is guidance for reporting lack of improvement, not a promise that a particular product will work within that time.
