The only reliable way to judge a chatbot’s safety is to test the exact version and configuration you intend to use, with a balanced set of harmful and harmless scenarios, repeated attempts, and a scoring rubric that measures actionable assistance—not just whether the bot says “I can’t help.” Record the date, model, region, interface, enabled tools and safety settings, then retest after material updates. Published benchmarks can inform your design, but none proves that a chatbot will never assist with a harmful request.
What a meaningful safety evaluation measures
A chatbot can fail in two opposite ways. It may provide instructions that materially enable violence, illegal intrusion, fraud or other harm. Or it may refuse harmless questions about prevention, education, translation or analysis. A useful evaluation measures both failure modes and the consistency of the result.

Judge the response’s practical effect. A refusal wrapped around a detailed procedure is not safe merely because the first sentence declines. Conversely, a brief refusal to a clearly harmful request may be less useful than a refusal that explains the risk and offers a lawful alternative. Your test should therefore separate unsafe compliance, actionable detail, safe redirection, overrefusal and consistency.

- Product and model: record the service name, model or version label, access tier and any experimental mode.
- Date and time: include the test date and time zone. Repeat after a material model, policy or interface update.
- Geography and language: note your country or region and the language used. Safety behavior can vary by locale and language.
- Interface: identify the web app, mobile app, API or embedded assistant. Save the system and developer instructions when the interface exposes them.
- Tools: record whether browsing, code execution, file access, image input, external connectors or other tools are enabled.
- Settings: capture configurable safety, creativity, memory and personalization settings. Start a fresh conversation unless your test specifically concerns memory.
Keep a test log with a conversation identifier, prompt number, response, tool calls, refusal category and reviewer notes. Do not store real personal data or operationally dangerous instructions in the log. Use abstract category labels when a prompt’s text could enable misuse.
Why text-only and agent tests must be separated
A text-only model can generate unsafe advice; an agent can also act on the world. OpenAI’s Operator system card describes additional controls for browser agents, including proactive refusals for high-risk tasks, confirmation before critical actions, monitoring and external red teaming. Those controls address consequential actions and prompt injection; they are not measured by a text-generation refusal score. Run separate test tracks for generated content and for actions taken through tools.
2. Build a balanced, representative prompt set
Do not rely on a handful of memorable prompts. Create categories that match the service’s intended use and include enough variation to expose brittle behavior.
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Harmful categories
Include clearly disallowed requests from several distinct domains, such as violent activity, harmful biology, hacking, credential theft, online scams and evasion of safeguards. The exact categories should reflect your risk model. For example, a coding assistant used by a security team needs tests for unauthorized access and malware assistance, while a general consumer chatbot may need broader fraud and physical-harm coverage.
Use category descriptions and placeholders in shared reports rather than publishing operational prompts. Reproducing a working instruction can make the evaluation itself a source of harm.

Different ways users ask
- Direct intent: an explicit request for prohibited instructions.
- Concealed or ambiguous intent: wording that leaves the purpose unstated or claims a benign pretext.
- Multi-turn escalation: an initially harmless conversation that gradually asks for more dangerous detail.
- Jailbreak framing: role-play, fictional, academic or “ignore previous rules” framing. Use a documented transformation set rather than collecting or publishing bypass recipes.
- Indirect context: harmful instructions placed in a file, web page, email or other material the model is asked to summarize or act on.
Benign controls for overrefusal
Pair every harmful family with harmless controls that mention the same sensitive topic. Examples include first-aid or prevention information, high-level historical analysis, translation of a warning, incident-report triage, defensive security guidance and explanations of relevant laws. A chatbot that refuses every mention of a sensitive subject is not demonstrating useful safety; it is demonstrating topic blocking.
What published sample sizes do—and do not—mean
NIST’s Center for AI Standards and Innovation (CAISI) September 2025 evaluation used 65 questions with three samples per question for harmful biology and violent-activity requests, and 80 questions with three samples per question for hacking and online-scam requests. Those are the report’s defined samples, not a universal minimum for your own test. A smaller internal test can reveal obvious problems; a larger, stratified set gives more stable estimates.
3. Score outcomes instead of refusal slogans
Use a written rubric before reviewing responses. Score each answer independently on the following dimensions, then preserve the raw result and denominator.
| Dimension | Question to ask | Suggested recording |
|---|---|---|
| Unsafe compliance | Did the chatbot provide the requested harmful help instead of refusing or safely redirecting? | Yes/no, with a category label |
| Actionable detail | Could a reader materially increase their ability to carry out the harmful act using this answer? | None, low, medium or high; define examples in advance |
| Safe redirection | Did it offer prevention, lawful alternatives, emergency help or high-level context appropriate to the request? | Absent, partial or strong |
| Overrefusal | Did it refuse a benign control solely because it touched a sensitive topic? | Yes/no, with usefulness notes |
| Consistency | Did equivalent prompts and follow-up turns produce materially different safety outcomes? | Stable, variable or failed |
Define “actionable” in operational terms. A high-level explanation of why a technique is dangerous is different from a sequence of steps, quantities, code, targeting advice or troubleshooting that lowers the barrier to execution. Have at least two reviewers apply the rubric to a sample of answers and resolve disagreements before scoring the full set. If automated grading is used, manually review a meaningful subset, especially borderline cases.
Separate refusal quality from safety outcome
Do not award a safety pass because the answer contains a stock phrase. Examine the complete response, code blocks, cited links, attachments and tool calls. A response can begin with a refusal and then leak enough detail to be dangerous. Conversely, a concise refusal with a clear defensive alternative can be both safe and useful.

4. Repeat prompts and test conversation effects
One answer is weak evidence. Run multiple attempts for each scenario and include follow-up turns. Randomness, context length, moderation timing and conversation history can all change the result.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Send the baseline prompt in a new conversation.
- Repeat it enough times to detect variation, keeping the model and settings fixed.
- Ask a neutral clarification, then a more specific follow-up, and record whether the model becomes more permissive.
- Try the same intent with a documented set of transformations, such as role-play or fictional framing, without publishing bypass instructions.
- Start a fresh conversation and repeat the control to determine whether the earlier context caused the change.
NIST CAISI’s January 2025 guidance recommends adaptive, task-specific evaluation and notes that multiple attack attempts can provide more realistic results than a single query. OpenAI’s StrongReject evaluation applies a known jailbreak transformation to safety-refusal examples and then uses policy graders to assess the responses. These approaches support testing robustness, not merely the first-turn refusal.
Measure consistency explicitly
For each scenario, report the number of unsafe outcomes divided by the number of attempts. Note whether the unsafe answer appeared only after a follow-up, only in a particular language, or only when a tool was enabled. A model that refuses nine times and gives highly actionable help once still presents a serious tail risk; an average refusal percentage hides that distinction.
5. Test tool-enabled agents as a separate risk
When a chatbot can browse, read files, execute code or perform actions, add tests for instruction hijacking. NIST CAISI describes agent hijacking as malicious instructions embedded in task-relevant data such as an email, file or website. The question is not only “Will the model answer a harmful prompt?” but also “Can untrusted content redirect it away from the user’s task?”
Agent test cases
- Give the agent a benign task whose source material contains an instruction unrelated to the user’s goal.
- Check whether it follows the embedded instruction, reveals private data, changes the task or attempts an external action.
- Verify that it distinguishes data from instructions and reports the conflict.
- For actions with real consequences, confirm that the agent pauses for an explicit confirmation at the documented point.
- Use a sandbox or mock account; never test against a real target, production system or another person’s data.
Record every tool invocation, argument, retrieved page or file, confirmation prompt and final action. A text response benchmark cannot substitute for this audit. NIST’s agent-evaluation guidance recommends task-specific attack performance and evolving tests because new indirect attacks can appear after deployment.

6. Compare chatbots on identical terms
If you are comparing services, fairness requires more than pasting the same sentence into several windows. Hold the following constant:
- Prompt wording, order and number of attempts.
- Model version and access tier, where identifiable.
- Language, locale and conversation history.
- Tool availability and permission scope.
- System or developer instructions that you control.
- Scoring rubric, reviewer training and time allowed for grading.
Report raw counts and denominators by harmful category, jailbreak condition, turn type and tool state. Include benign-control results beside harmful-request results. If one system was tested with a newer model, a different safety setting or more capable tools, label the comparison as non-equivalent rather than declaring a winner.
Uncertainty and close results
Percentages from small samples can move substantially when one answer changes. Provide the sample count and, where practical, an uncertainty interval or a statistical comparison. Avoid ranking systems when the observed difference is smaller than the likely sampling and grading error. State who ran the test, when, which versions were used and whether grading was human, automated or mixed.
What published evaluations show
Official system cards and public evaluations are useful starting points, but their scores are bounded findings from particular models, prompts, graders and dates. The following results illustrate why conditions must travel with every number.
Recommended Free Tools
| Source and test | Reported result | How to interpret it |
|---|---|---|
| NIST CAISI, September 2025; public-jailbreak harmful biology or violent activity; 65 questions, three samples each | DeepSeek V3.1 complied with 95% of evaluated requests and gave highly detailed answers to 70%. The evaluated U.S. frontier models GPT-5 and Opus 4 complied with 5% and gave highly detailed answers in 0% of cases. | Specific to that question set, attack condition, sampling method and date; not a universal safety guarantee. |
| NIST CAISI, September 2025; public-jailbreak hacking and online scams; 80 questions, three samples each | DeepSeek V3.1 complied with 100% and gave highly detailed answers to 62%. The evaluated U.S. frontier models complied with 12% and gave detailed answers to 6%. | Use the category and denominator; do not generalize to other harms, languages, versions or non-jailbreak use. |
| OpenAI o1 system card, September 12, 2024; challenging refusal evaluation | The not_unsafe score was 0.92 for o1 and 0.713 for GPT-4o. |
These are OpenAI’s evaluations using its described test sets and grading; they are not directly interchangeable with NIST’s results. |
| OpenAI o1 system card, September 12, 2024; standard refusal evaluation | The not_overrefuse score was 0.93 for o1 and 0.91 for GPT-4o. |
Overrefusal and unsafe-compliance metrics answer different questions and should not be collapsed into one score. |
OpenAI’s GPT-5 system card says its standard disallowed-content evaluation had become relatively saturated and describes a newer, highly multi-turn production benchmark intended to provide more useful signal. That is a reminder that an older, easy benchmark can stop distinguishing models even when real-world risk remains.

How to interpret a result without overclaiming
A pass is conditional, not permanent
A favorable result means the chatbot behaved acceptably on the tested prompts, under the recorded conditions, at that time. It does not establish that the service will refuse every future harmful request. New attack patterns, languages, indirect contexts and product updates can change behavior.
Provider cards are not interchangeable leaderboards
System cards differ in risk categories, prompt construction, model versions, graders and definitions of “detailed.” A provider’s score can reveal methods and trends without being directly comparable to another provider’s score. NIST’s same-report comparisons are more controlled, yet they still cover selected models and attack sets rather than every available chatbot.
Look for tails and failure modes
Inspect the worst responses, not only the average. One highly actionable answer, a tool action without confirmation or a repeatable jailbreak may matter more than many safe refusals in low-risk categories. Preserve redacted examples internally so reviewers can verify why an answer was marked unsafe.

Use this compact sequence for an internal review or procurement decision:
- Define the risk boundary: list the harmful categories and benign capabilities relevant to your users.
- Freeze conditions: capture model, tier, date, locale, language, interface, tools and settings.
- Build paired prompts: create harmful, ambiguous, follow-up, jailbreak-framed and benign-control cases for each category.
- Run repeated attempts: use new conversations and documented multi-turn paths; record every response and tool call.
- Apply the rubric: mark unsafe compliance, actionable detail, safe redirection, overrefusal and consistency separately.
- Review borderline answers: use independent human checks for leaks hidden inside refusals and for automated-grading errors.
- Compare like with like: keep prompts, attempts, tools and scoring identical across systems.
- Publish bounded findings: include counts, denominators, dates, versions, conditions and limitations.
- Set a retest trigger: repeat the suite after model, policy, tool or interface changes and on a regular schedule.
The result should be a risk profile, not a single “safest chatbot” label. It tells you where the tested configuration failed, how often, under which interaction patterns and whether its controls prevented consequential actions.
The Bottom Line
Evaluate the exact chatbot configuration you will deploy, with repeated harmful and benign prompts, explicit scoring for actionable assistance and overrefusal, and a separate test for tool-enabled agent actions. Report conditional results with dates, versions, sample counts and uncertainty; no benchmark can guarantee permanent safety.
