How to Evaluate Whether an AI Agent Should Act, Ask for Help, or Stop

An AI agent should act when the task, authority, and consequences are clear enough to stay within an approved boundary. It should ask for help when the missing information is a user preference, intent, or authorization that it cannot safely infer. It should stop or defer when the requested action exceeds its scope, creates unacceptable risk, or encounters an unresolved safety, security, privacy, or policy issue.

That is a context-specific policy, not a universal confidence score. A model’s stated certainty does not grant permission. For consequential, multistep work, have the agent show its intended plan for human review before execution and preserve a way to interrupt or redirect it while it runs.

The three-way decision: act, ask, or stop

Every agent run needs an explicit control policy. The policy should classify the next proposed action, not merely rate the model’s answer. A plausible response can still be unauthorized, harmful, or impossible to reverse.

Decision Use it when Required control
Act The objective and authority are clear, the action is within scope, and foreseeable consequences are bounded and acceptable. Execute only the approved operation, record what happened, and expose an undo, review, or escalation path where practical.
Ask for help The unresolved issue is a preference, intent, authorization, or other judgment the agent cannot safely determine. Ask one focused question that presents the relevant choices and explains what will remain paused.
Stop or defer The action is outside permission, unusually consequential, prohibited, unsafe, or blocked by an unresolved security, privacy, or policy concern. Do not continue the risky operation. State the boundary, preserve evidence, and identify the human or process that must decide next.

These categories are deliberately operational. They tell a team what the system must do at the moment of uncertainty, rather than promising that a model can measure its own reliability perfectly.

Start with context, not confidence

The same action can be safe in one deployment and unacceptable in another. Sending a draft email to a private test account is different from sending a legal notice to a customer. Updating a local copy of code is different from deploying to production. The decision depends on the task, affected people and systems, intended use, and foreseeable impact.

NIST’s AI Risk Management Framework (AI RMF) calls this contextual work the Map function. Mapping establishes what the system is being used for, who may be affected, and which risks matter before an initial go/no-go decision. The framework is voluntary guidance, not a runtime algorithm that produces an automatic answer.

Do not convert a model’s self-reported probability into authority. The primary guidance reviewed here does not define a universal percentage at which every agent should proceed. Your organization must define allowed actions, approval-required actions, and blocked actions for each deployment.

A practical decision sequence

The following six-step sequence is an implementation heuristic synthesized from Anthropic’s agent guidance and NIST’s human-role and risk-management guidance. It is not a formal standard or validated scoring model.

1. Check authority and scope

Identify the principal on whose behalf the agent is acting, the resources it may access, and the purpose for which those permissions were granted. Compare the proposed operation with the actual authorization, not with what the tool technically allows.

  • Is the data source, account, repository, or environment explicitly in scope?
  • Does the instruction authorize reading, editing, sending, purchasing, deleting, or only drafting?
  • Would this operation grant the agent a new privilege or cross a trust boundary?
  • Is another person’s approval required by policy, contract, or law?

If the answer is no or unclear, do not silently widen the scope. Ask for the required authorization or stop and route the task to the designated owner.

2. Understand the consequence and context

List what could change, who could be affected, and how serious an error would be. Consider financial loss, privacy exposure, security impact, operational downtime, legal commitments, reputational damage, and effects on vulnerable people. A small technical change can have a large social consequence if it changes access or eligibility.

NIST’s AI RMF Appendix C describes human–AI configurations ranging from “fully autonomous to fully manual.” The appropriate point on that range depends on the system and its use; some workflows need little human oversight, while others require a human decision at a defined checkpoint.

3. Identify the kind of uncertainty

Separate an information gap from a judgment gap.

  • Factual uncertainty: the agent does not yet know a document’s version, a system’s current status, or a relevant rule. It may investigate if that investigation is itself authorized, read-only where appropriate, and low consequence.
  • Intent or preference uncertainty: several outcomes could satisfy the request, but the user’s priority is unknown. Ask instead of guessing whether speed, cost, tone, privacy, or completeness matters most.
  • Authorization uncertainty: the agent cannot establish that it is permitted to act. Treat this as an approval issue, not as a research task.
  • Safety or policy uncertainty: proceeding could violate a security control, privacy requirement, or internal rule. Stop or escalate until a qualified person resolves it.

Anthropic’s practical guidance makes the same distinction: an agent can often research a factual gap, while a question about what the user wants may require the user. Its warning is two-sided: an agent that stops at every possible question creates friction, while one that always pushes through risks misunderstanding.

4. Consider reversibility and reviewability

Reversibility is a useful design heuristic, not a formula prescribed by NIST or Anthropic. A bounded action that can be previewed, rolled back, and audited can usually tolerate more autonomy than an irreversible action affecting external parties.

  • Highly reversible: create a draft, run a read-only query, produce a report, or edit an unshared branch.
  • Conditionally reversible: change a configuration with a tested rollback, update a record with version history, or send an internal notification that can be corrected.
  • Hard to reverse: delete data, publish a statement, transfer funds, revoke access, cancel a service, or make a high-stakes eligibility decision.

When an operation is difficult to undo, require a stronger checkpoint even if the model appears confident. Review should occur before the irreversible step, not only after the log has been written.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Choose the control

There are four practical controls: proceed within approved scope, ask one focused question, present a plan for approval, or stop and defer. Select the least burdensome control that still addresses the risk. A read-only lookup may proceed; a multistep production change may need plan approval; a prohibited data disclosure must stop.

6. Revisit the policy

Agent oversight is a lifecycle responsibility. NIST’s AI RMF organizes work into Govern, Map, Measure, and Manage. Governance sets roles and accountability; mapping defines context; measurement tests performance and risk; management prioritizes responses and improvements. Reassess after incidents, material system changes, new tools, or changes in the people affected.

When an agent should act

Acting is appropriate only when the proposed operation is both technically feasible and authorized in context. A useful allow-list describes the exact operation, resource, environment, data class, and maximum impact. “Can edit files” is too broad; “may update documentation in this repository, never production configuration, and must leave a reviewable diff” is testable.

Conditions for autonomous action

  • The objective has a single reasonable interpretation or the permitted choice is explicit.
  • The agent has the minimum access needed and cannot escalate privileges on its own.
  • The operation is within a documented risk tier and does not cross an approval boundary.
  • Inputs can be validated and outputs can be inspected, logged, or rolled back.
  • A human can interrupt the run or review the result before an external commitment.

Acting does not mean hiding the work. The agent should report the operation it selected, the resources it touched, assumptions that mattered, and any checks that failed. Transparency lets a reviewer detect drift before a small exception becomes a systemic problem.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Example: a bounded engineering task

An agent asked to update a project’s documentation may inspect the repository, edit the specified files, run the approved checks, and present a diff. It should not deploy the change, rotate credentials, alter access controls, or edit unrelated systems unless those permissions and steps were separately authorized.

When an agent should ask for help

Ask when the human must choose among legitimate outcomes or confirm a consequential interpretation. The question should be narrow enough to answer quickly and specific enough to prevent a second round of ambiguity.

Good clarification questions

  • “The report can be sent today with preliminary figures or tomorrow after reconciliation. Which do you want?”
  • “This change affects all users in the production environment. Should I prepare the rollout plan only, or request the release approval?”
  • “Two records match the name. Which account identifier is authorized for this action?”

Explain the consequence of each option, what the agent has already verified, and what remains paused. Do not ask the user to repeat facts the agent can safely obtain itself.

Ask once, at the right level

For a long workflow, asking about every individual command can overwhelm the reviewer. Anthropic’s 2026 practical guidance contrasts action-by-action permission prompts with upfront approval of a proposed plan. A plan review lets a human judge strategy while retaining the ability to intervene during execution. It is a design pattern, not proof that every workflow benefits from it.

A useful plan contains the objective, ordered steps, resources, assumptions, approval boundaries, expected side effects, rollback method, and conditions that will cause the agent to pause. If the plan changes materially, request approval again rather than treating the original approval as unlimited.

When an agent should stop or defer

Stopping is the correct behavior when continuing would require the agent to invent authority, bypass a control, or accept a risk that the policy has not approved. A stop is not a failure of intelligence; it is a boundary enforced by design.

Stop conditions

  • The requested action is outside the account, environment, data set, or purpose specified in the authorization.
  • The action would expose sensitive information to an unverified recipient or cross a security boundary.
  • A required approval is missing, expired, or supplied by someone without the right role.
  • The agent detects conflicting instructions, suspicious prompt content, or an attempt to disable logging or oversight.
  • The operation is irreversible or high impact and no required human checkpoint is available.
  • Inputs are inconsistent enough that proceeding could produce a materially different outcome.
  • The tool reports an error that makes the resulting state unknown, especially during writes or transactions.

The stop message should state what was not done, what was completed, the exact boundary encountered, evidence available for review, and the next decision owner. Preserve logs and partial outputs without leaking secrets into the explanation.

Choosing an oversight design

Compare oversight patterns against consequence, authorization, uncertainty, reversibility, reviewer workload, and lifecycle risk. The following table is a practical design aid, not a prescribed standard.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Pattern Best fit Strength Risk or cost
Autonomous within an allow-list Routine, low-impact, reversible operations with clear authority. Low friction and fast execution. Bad boundaries can allow silent scope drift; requires strong logging and periodic review.
Approval per action Distinct high-impact operations where each step has different consequences. Fine-grained human control. Many prompts can cause fatigue or automatic approval without meaningful review.
Upfront plan approval Complex, multistep work with a stable strategy and several routine substeps. Human judgment is applied to the overall approach instead of every command. Unexpected changes during execution still require interruption and re-approval.
Human executes, agent advises High-stakes, ambiguous, or poorly understood operations. Keeps the final commitment with a qualified operator. Slower and vulnerable to human overreliance on persuasive suggestions.
Hard stop and escalation Prohibited actions, unresolved security issues, or missing authority. Prevents the agent from normalizing an unacceptable risk. Work remains blocked until the responsible process responds.

Anthropic’s framework says humans should retain control over how goals are pursued, particularly before high-stakes decisions. Its example includes requiring approval before canceling subscriptions or downgrading service tiers. Treat those product examples as illustrations of a control approach, not as a universal rule for every agent.

Permissions, roles, and intervention

Define human and agent responsibilities separately. The person who sets the objective may not be the person authorized to approve a production change. Document who can configure the agent, who can approve a plan, who can intervene during execution, who investigates incidents, and who can revoke access.

Anthropic describes Claude Code as using read-only access by default, seeking human approval before actions that modify code or systems, and allowing persistent permissions for routine trusted tasks. These are controls in one product, not an industry standard. The general lesson is to begin with least privilege, make write access explicit, and expand it only for operations that have been tested and assigned to a trusted scope.

Intervention must be usable. Provide a visible pause or cancel control, timeouts for stalled tasks, a way to revoke tokens or sessions, and a recovery procedure for partially completed work. Test interruption during reads, writes, external calls, and retries; a button that stops the interface but not the underlying job is not meaningful control.

A risk-tier policy you can adapt

Organizations can translate the decision sequence into tiers. The labels below are a synthesis for implementation and should be mapped to your own legal, security, and operational requirements.

Tier Typical examples Default behavior Evidence to retain
Low Read-only research, drafts, local analysis, reversible formatting. Act within an allow-list; disclose assumptions. Inputs, tool calls, output, and validation result.
Moderate Edits to shared content, internal notifications, configuration changes with rollback. Act only after scope checks; use review or plan approval for batches. Diff or change set, approver where used, rollback status, and affected resources.
High External publication, access changes, financial commitments, customer-impacting decisions. Ask for explicit approval before the commitment; keep intervention available. Decision rationale, identity and role of approver, final payload, and outcome.
Blocked Unauthorized data disclosure, privilege escalation, prohibited content, unknown transaction state. Stop and escalate; do not seek a workaround. Boundary triggered, relevant logs, containment action, and assigned owner.

Measure whether the policy works

Do not evaluate oversight only by completion rate or the number of prompts. Track whether the controls produce correct, authorized outcomes without training people to click through warnings.

  • Scope adherence: how often the agent touched only approved resources and permissions.
  • Escalation quality: whether questions identified genuine intent or authorization gaps rather than facts the agent could have checked safely.
  • Approval usefulness: whether reviewers had enough context to detect a bad plan before execution.
  • Interruption effectiveness: whether pause and cancellation stopped the relevant work within the expected time.
  • Recovery: how often failed operations were rolled back or contained without secondary damage.
  • Near misses and incidents: cases in which a boundary prevented harm, as well as cases in which the agent acted too far or stopped unnecessarily.

Anthropic reports that, on complex tasks, users interrupt Claude only slightly more often than on simple tasks while Claude’s own check-ins roughly double. The passage provides no baseline counts, sample, or precise percentage, so it should be read as a qualitative observation rather than an industry-wide rate. It supports a practical point: a well-designed agent can increase its requests for guidance as task complexity rises without forcing a human to approve every routine action.

Worked decisions

Drafting and sending a customer email

Reading the approved customer record and drafting a response can be an act if the agent has a defined tone, template, and data scope. Sending the message is a separate decision. If the message creates a legal commitment, discloses sensitive information, or changes a customer’s service, ask for approval before sending. If the recipient cannot be verified, stop rather than guessing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Changing production configuration

Inspecting the current configuration and preparing a diff may proceed under read-only permissions. Applying the change requires a defined maintenance window, rollback plan, and authorized approver. If the observed state differs from the plan, pause and present the discrepancy; do not improvise a repair in production.

Canceling a subscription or downgrading a service

The agent may gather account details and show the consequences, but the commitment should require explicit approval when the user’s business priority is unknown or the action affects access, billing, or data retention. Anthropic uses this type of high-stakes decision as an example of where humans should retain control.

Deleting records

Before deletion, verify the data owner, retention rule, exact record set, and recovery method. If any are uncertain, stop. A reversible quarantine or archival step may be an approved alternative, but it must not be substituted silently for the requested outcome.

Common policy failures and fixes

Using a confidence threshold as permission

Failure: “Proceed above 90 percent confidence.”
Fix: Use authority, consequence, reversibility, and approval requirements. Confidence can inform investigation, but it cannot authorize an action that policy forbids.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Asking about every factual gap

Failure: The agent pauses for information it could obtain safely from an approved source.
Fix: Permit bounded, read-only investigation and reserve questions for intent, preference, authorization, or unresolved risk.

Letting the agent push through ambiguity

Failure: The agent chooses a plausible preference and commits externally.
Fix: Ask one focused question with the meaningful options and consequences.

Approving a plan once and ignoring changes

Failure: The agent encounters a different system state but treats the original approval as unlimited.
Fix: Define change thresholds that require a new review, and make material deviations visible.

Creating approval fatigue

Failure: Reviewers receive so many prompts that they approve without reading.
Fix: Group routine steps into a plan, keep high-impact actions separate, and include concise evidence that lets a reviewer make a real decision.

Free tools Windows power users keep installed

Stopping without a recovery path

Failure: The agent says “cannot continue” and leaves the operator to reconstruct the state.
Fix: Report completed and uncompleted steps, preserve logs, identify the boundary, and name the owner or approval process for the next action.

Implementation checklist

  1. Define the agent’s purpose, principal, environments, data classes, and maximum permitted impact.
  2. Write allow-lists for routine actions and explicit blocks for prohibited operations.
  3. Map affected people, systems, and foreseeable harms before deployment.
  4. Classify uncertainties as factual, preference or intent, authorization, or safety and policy issues.
  5. Choose autonomous execution, per-action approval, plan approval, human execution, or hard stop for each risk tier.
  6. Require least privilege, separate read and write permissions, and prevent silent privilege escalation.
  7. Specify pause, cancellation, timeout, rollback, and escalation behavior.
  8. Log prompts, tool calls, resources, approvals, outputs, errors, and final state without exposing secrets unnecessarily.
  9. Test normal runs, ambiguous requests, conflicting instructions, tool failures, partial writes, and interruption.
  10. Review incidents, near misses, unnecessary escalations, and policy changes throughout the system lifecycle.

What the published guidance does—and does not—establish

Anthropic’s agent framework, published August 4, 2025, and its practical guidance published in 2026 provide product and design examples for permissions, clarification, plan review, and human control. NIST’s AI RMF 1.0 dates to 2023; its Playbook page, updated June 10, 2026, presents voluntary guidance organized around Govern, Map, Measure, and Manage. OpenAI’s governance paper landing page is dated December 14, 2023 and presents an early contribution on lifecycle responsibilities and safety practices while acknowledging open operational questions.

None of these sources supplies a validated universal numeric rule for deciding when every agent should act, ask, or stop. The defensible approach is to define context-specific authority and risk boundaries, make human roles explicit, use the least intrusive control that fits the consequence, and keep a human able to review or intervene before a high-stakes commitment.
