An AI agent has recovered from a mistake only when it detects credible evidence that a consequential action or assumption was wrong, diagnoses the cause, performs a valid and executable correction, and leaves the task in a verified goal state without unacceptable side effects. An apology, a confident explanation, or a successful final answer is not enough.
Evaluate recovery as a trajectory, not a final message. Start some trials from realistic failed states, preserve the action history, check the resulting environment independently, and report recovery separately from clean-run performance.

What counts as recovery?
Recovery is a chain of observable events. A useful evaluator should be able to point to the first consequential mistake, the evidence available to the agent, the agent’s diagnosis, the corrective action, and the verified terminal state.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →1. Evidence that something went wrong
The evaluator must identify an observable discrepancy: a failed tool result, an invalid parameter, a violated constraint, an unexpected file or database state, a policy warning, or a user correction. Merely telling the model “you made a mistake” is a weaker test because it removes the detection problem.

2. Recognition of the specific failure
The agent should identify what went wrong in terms that match the trace. “I may have made an error” is not equivalent to locating the bad call, incorrect assumption, or stale observation. Recognition can be implicit if the next action clearly addresses the actual failure, but the evaluator should still record the explanation when one is produced.
3. A diagnosis that leads to a changed action
The correction must be different from the failed action in the relevant way. Repeating the same call with no changed parameter, context, or plan is a retry, not evidence of recovery. A diagnosis is useful only when it produces a correction that can execute under the available tool schema and permissions.
4. Verified task completion
After the correction, inspect the environment or tool output. The intended state must be true, not merely described. For a GUI task, check the actual interface state or an executable assertion. For an API workflow, validate the returned object, resource state, and consistency of related records.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors5. Acceptable cost and side effects
A technically successful repair may still be unacceptable if it caused duplicate purchases, destructive writes, permission violations, unnecessary calls, or other forbidden effects. Record extra turns, redundant retries, and side effects separately from the binary question of whether the goal was eventually reached.
Conversely, an apology is not itself failure. If the subsequent action is valid and the final state satisfies the goal, the trajectory may qualify as recovery even when the agent expresses uncertainty.
Why clean-run success does not measure recovery
A clean trial starts with an intact environment and no prior wrong turn. It measures ordinary task completion, not the ability to recover after state, context, or assumptions have become contaminated. A system can perform well on clean prompts while repeatedly compounding an early error in a conversation.
Microsoft Research reported a 39% average performance drop across six generation tasks when comparing multi-turn with single-turn conversations in a simulated study of more than 200,000 conversations. The analysis attributed much of the degradation to increased unreliability, including early assumptions that models continued to rely on. Microsoft Research summarized the finding as: “when LLMs take a wrong turn in a conversation, they get lost and do not recover.” This result concerns the tested models and tasks; it is not a universal failure rate for every agent.

Use clean performance as a baseline, then add trials that begin after an injected or naturally occurring error. The difference between those conditions is more informative than a single success percentage.
How to design a recovery evaluation
1. Define the task and the verified goal state
Write the success conditions before running the agent. Specify the state that must be true, the evidence that proves it, and the side effects that are forbidden.
- For tool workflows, define valid operation names, required parameters, allowed value ranges, permission boundaries, and checks for consistency between a call and its result.
- For GUI work, define an inspectable or executable end state, such as a particular record value, page state, or file property.
- For conversational tasks, define what counts as a completed commitment and which claims must be grounded in tool output.
Also document the initial context, available tools, model and agent version, and any time or action limits. Without these details, another evaluator cannot interpret the result.
2. Run paired clean and recovery conditions
Use the same task, initial context, and tool access in two conditions. In the clean condition, let the agent begin normally. In the recovery condition, start from a saved state derived from a failed trajectory or inject a realistic error before handing control back to the agent. The paired design makes recovery performance interpretable while preserving clean-run performance as a reference.
3. Preserve the failed state instead of only describing it
Save the conversation, tool calls, tool outputs, environment snapshot, and relevant timestamps. Restoring that state is stronger than adding a message that says the agent made a mistake. The agent must reason from the same evidence that would exist in deployment.
4. Vary error modes and evidence quality
Include mistakes caused by the assistant, the user, and the environment. Vary severity and whether the evidence is explicit or ambiguous.
- Assistant errors: wrong parameter, malformed schema, premature confirmation, incorrect file path, or an assumption that conflicts with the returned data.
- User or environment errors: a changed requirement, revoked permission, stale external record, interrupted operation, or a third-party system that returns an unexpected state.
- Evidence conditions: explicit tool failure, contradictory data, delayed indication, or an ambiguous symptom requiring diagnosis.
- Severity: harmless formatting issue, recoverable state error, costly duplicate action, or a safety and policy violation.
Keep the original action history and resulting state for every condition. Do not collapse all errors into a single “mistake” category.

Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →| Event | What to record | Why it matters |
|---|---|---|
| First consequential mistake | Call or assumption, parameters, available evidence, and resulting state | Defines the recovery starting point |
| Recognition | Agent statement or action that indicates detection | Shows whether the system noticed the relevant failure |
| Diagnosis | Cause identified and evidence cited | Tests whether the explanation matches the trace |
| Correction | Exact follow-up call, parameters, and permissions | Allows executability and validity checks |
| Tool or environment result | Raw response, state transition, and errors | Separates intended repair from actual repair |
| Subsequent retries | Count, changed inputs, and reason for each | Exposes redundant or escalating behavior |
| Terminal state | Independent assertions, tests, and side-effect audit | Determines whether the task really ended correctly |
6. Score separate dimensions
Do not hide all behavior inside one composite score. Report each dimension independently, then explain any weighting used for a deployment decision.
| Dimension | Pass question | Recommended report |
|---|---|---|
| Recovery success | Did the agent repair the seeded or observed failure and reach the goal? | Rate by error type and task family |
| Detection and diagnosis | Did the agent identify the actual failure from available evidence? | Classify correct, partially correct, and incorrect diagnoses |
| Correction validity | Was the proposed action permitted, properly formed, and executable? | Count schema, parameter, permission, and planning errors |
| Task success | Does the independently inspected terminal state satisfy every goal condition? | Report separately from verbal completion claims |
| Recovery cost | How many additional turns or actions were needed? | Give distributions or averages with the protocol |
| Redundant retries | Did the agent repeat an ineffective action? | Count unchanged calls and unnecessary probes |
| Constraint violations | Did it breach policy, schema, authorization, or user constraints? | Report every violation, even when the final goal was reached |
| Side effects | Did recovery create duplicate, destructive, or otherwise forbidden changes? | List severity and reversibility, not just a count |
Use clean-run success as a separate baseline. A system with high clean success and low recovery success has a different operational risk from one with moderate clean success and dependable repair.
7. Validate externally
Whenever possible, use checks outside the model’s own narrative. Validate environment state, tool schemas, executable tests, and policy constraints. A judge model can help classify a trace, but its label should be grounded in an auditable record and, for critical decisions, confirmed by deterministic checks or human review.
How to compare two agents fairly
Give every agent the same tasks, initial context, injected states, tool definitions, and stopping rules. Randomize trial order where practical and preserve identical snapshots. Compare at least the following:

What the reported numbers actually show
PivotOPD and pivotal mistakes
NVIDIA Research’s 2026 PivotOPD page reports that 59% of failed ALFWorld rollouts contained a pivotal mistake. Correcting the pivotal turn lifted replay success from 8% to 59%, while guiding the next two turns reached 58% in the reported analysis. The same page reports 72.7% recovery of replayed pivotal mistakes, compared with 8.3% for the base model and 20.3% for standard on-policy distillation. These figures concern that ALFWorld replay experiment and its three Qwen3 models; they are not general recovery rates.

GUI-RobustEval
The 2026 Proceedings of Machine Learning Research paper presents 1,216 executable test cases for GUI-agent error recovery. Its value for evaluation design is the executable, error-mode-specific structure: a claimed repair can be checked against the interface state rather than judged only from language.
AgentRx
Microsoft Research’s AgentRx framework uses 115 manually annotated failed trajectories from τ-bench structured API work, Flash incident management, and Magentic-One web and file tasks. It seeks the first critical failure step with guarded, evidence-backed checks. This makes diagnosis auditable, but a correct diagnosis still needs a separate repair and terminal-state test.

The Findings of ACL 2026 benchmark contains 968 annotated dialogues across 10 domains and 88 APIs. It separates a model judging another trajectory from the model reflecting on its own trajectory and scores detection, classification, correction accuracy, and explanation quality. The paper reports that performance drops sharply for self-reflection, which is why self-reports should not replace external validation.
Structured reflection for tool interactions
The Findings of ACL 2026 paper by Su and colleagues describes a reflection process in which the agent diagnoses an error from evidence in the previous step and then proposes a correct, executable follow-up call. Tool-Reflection-Bench operationalizes this sequence with checks for structure, execution, parameters, and result consistency. These miniature trajectories are useful unit tests, but they do not represent every problem in a long-running agent.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Interpreting difficult outcomes
The agent says it was wrong, but changes nothing
Classify this as recognition without correction. The trace contains an admission, but no executable action repaired the state.
The correction executes, but the goal remains false
Classify the correction as executable but the recovery as unsuccessful. This distinction reveals whether the failure was in planning, parameter selection, or verification.
The final state is correct by luck
Inspect the path. If an unrelated retry or an external change produced the desired state, record task success only if your protocol allows it, but mark diagnosis or correction validity as failed when the agent did not repair the actual cause.
Best Value
The agent repeats a call until it works
Count unchanged retries and examine side effects. A later success does not erase redundant calls, rate-limit exposure, duplicate writes, or an absence of diagnosis.
The user or environment caused the failure
Do not penalize the agent merely because it did not cause the initial error. Test whether it recognized the changed condition, adapted within its permissions, and restored the goal without violating the new constraints.
The evidence is ambiguous
Require the agent to gather a discriminating observation before making a consequential correction. Score the information-gathering action, its cost, and whether the eventual diagnosis is supported by the trace.
Setting a pass threshold
No shared pass threshold, standard recovery-cost unit, or universal benchmark is established by the reviewed sources. Set limits for the intended deployment and publish them with the evaluation protocol.
- For low-risk tasks, you may tolerate extra turns while requiring a verified final state.
- For high-impact workflows, treat policy violations, unauthorized actions, and irreversible side effects as failing outcomes even when the nominal goal is reached.
- For latency-sensitive systems, define a maximum number of recovery turns or calls and report how often that limit is exceeded.
- For safety-critical automation, require deterministic state checks and an escalation path when evidence remains ambiguous.
State the task set, model and agent version, injection method, number of trials, scoring rules, stopping criteria, and uncertainty. A benchmark-specific percentage should never be presented as a universal statement about an agent’s recovery ability.
A practical evaluator checklist
- Write observable goal conditions and forbidden side effects.
- Run clean trials to establish a baseline.
- Create paired recovery trials from saved failed states or realistic injected errors.
- Include assistant, user, and environment error sources, with explicit and ambiguous evidence.
- Capture the full action history and environment state.
- Mark the first consequential mistake and the evidence available at that moment.
- Assess recognition, diagnosis, correction executability, and actual tool results separately.
- Verify the terminal state with tests, schemas, and policy checks outside the agent’s narrative.
- Count extra turns, redundant retries, violations, and side effects.
- Report results by error type and task family, with clean performance shown separately.
- Publish the protocol and its limits before interpreting the percentages.
The central test is simple to state but demanding to implement: after a consequential wrong turn, did the agent use evidence to make a valid change and restore the verified goal state at an acceptable cost? Only a trajectory-aware evaluation can answer that question.
