A credible weight-loss-drug claim tells you exactly who was studied, what dose and comparator were used, how long participants were followed, which outcome was designated primary, how large the between-group difference was, and what harms and uncertainties remained. A percentage printed in a headline is not enough.
Use the protocol and regulatory record to separate weight change from evidence that people feel or function better, live longer, or avoid a serious event. Then verify whether the specific drug and indication are approved, investigational, or compounded, and read current safety information rather than relying on an old summary.

Start with the exact claim, not the drug’s headline
Write down the claim word for word before judging it. “Causes weight loss,” “prevents heart attacks,” “maintains weight after dieting,” and “improves blood sugar” are different propositions requiring different evidence.
Identify the product and use
- Record the generic and brand name, formulation, dose, route and schedule when reported.
- Specify the population: adults or adolescents, baseline weight range, obesity-related conditions, cardiovascular disease, type 2 diabetes, or another characteristic.
- Identify the intended use: weight reduction, maintenance, a complication, or a cardiovascular outcome.
- Check whether the product is approved for that exact population and indication, still under investigation, or compounded.
An approval for one indication does not establish every other claim about the same molecule. The U.S. Food and Drug Administration’s March 2024 announcement, for example, concerned reducing cardiovascular death, heart attack and stroke risk in adults with cardiovascular disease and overweight or obesity, in addition to diet and activity changes. It was not a universal estimate of weight-loss efficacy for everyone who might take the drug.

Find the study that can actually answer the question
A press release or social-media post may omit the design details that determine how much confidence a result deserves. Locate the trial report, protocol or regulatory review and record its basic structure.
Design details to capture
- Phase and randomization: Random assignment helps balance groups, but it does not make every trial question relevant to every patient.
- Masking: Note who knew the assignment. Side effects that reveal treatment can affect behavior, reporting and dropout.
- Comparator: A placebo, lifestyle program, standard care or another drug answers a different question. “Better than baseline” is not the same as “better than a credible control.”
- Sample size and population: A large trial can estimate an effect precisely in its enrolled population; it does not automatically represent people who were excluded.
- Follow-up: Record treatment duration and any post-treatment observation. A short study cannot establish lifelong durability or long-term safety.
- Discontinuations and analysis set: Find out how many stopped treatment, why they stopped and whether the reported analysis included all randomized participants or only those who completed treatment.
The FDA-described cardiovascular-outcomes trial for Wegovy randomly assigned more than 17,600 people. That figure belongs to that particular population, comparator, endpoint and follow-up; it cannot be transferred to a smaller adolescent study or treated as a general performance rating for all weight-loss drugs.
Population determines generalizability
Ask how closely the enrolled participants resemble the person or group in the claim. Age, existing cardiovascular disease, diabetes status, prior treatment, baseline body mass index and access to lifestyle counseling can all change the expected benefit and risk. A result applies first to the people who met that trial’s eligibility criteria and for the time they were observed. Extending it to a different population or duration is an additional inference, not a finding of the trial.

Read the primary endpoint before repeating a number
The primary endpoint is the main outcome specified for the study. It is the result around which the design, sample-size planning and principal statistical test are organized. Secondary and exploratory outcomes may be informative, but they need their own uncertainty and should not be presented as if they were the trial’s main proof.
What FDA recommends for adult weight-loss efficacy
FDA’s January 2025 document on obesity-drug development recommends assessing adult weight-loss efficacy using the mean percentage change from baseline body weight in the drug group versus control. That document is draft guidance: it contains nonbinding recommendations and is explicitly not for implementation as a final requirement.
When reading a study, look for both the change within each group and the difference between groups. The between-group contrast is usually more useful because it accounts for weight changes that also occurred with placebo, counseling, diet, activity or ordinary variation.

Do not promote a secondary endpoint to the headline
FDA’s adolescent Saxenda example shows why labels matter. In that 56-week study of 251 adolescents, the primary endpoint was change in body-mass-index standard-deviation score (BMI SDS). The agency reported an average 0.23 reduction in BMI SDS in the treatment group and no average reduction in the placebo group. Body-weight change was secondary: the treatment group averaged a 2.65% loss while the placebo group averaged a 2.37% gain, and FDA said that secondary comparison was not evaluated for statistical significance. Those weight figures therefore cannot be presented as a statistically confirmed primary finding.
Separate weight from health outcomes
Blood pressure, lipid levels, fasting glucose and A1C can provide useful context, including for participants with type 2 diabetes. They do not automatically prove that patients live longer, feel better or function better. A claim about fewer heart attacks requires a trial designed to count heart attacks; a claim about quality of life requires an appropriate patient-reported or functional measure.
Express the effect in comparative and absolute terms
Relative percentages can sound dramatic when the underlying absolute difference is small. For an event outcome, report the event rate in each group and the absolute difference whenever the source provides it. For a continuous outcome such as body weight, report the mean change in each group, the between-group difference and a measure of uncertainty if available.
Worked example: a cardiovascular endpoint
In the Wegovy trial supporting the cardiovascular-risk indication described by FDA, major adverse cardiovascular events occurred in 6.5% of participants receiving Wegovy and 8% receiving placebo. More than 17,600 people were randomized, and both groups received standard-of-care treatment and lifestyle counseling. The absolute difference was 1.5 percentage points in that trial. These are event rates for adults with cardiovascular disease and overweight or obesity under those study conditions, not weight-loss rates and not a guarantee for an individual.
Questions to ask of any percentage
- Is it a change from baseline, a difference between groups, a relative risk reduction, an absolute risk difference or the proportion reaching a threshold?
- Was it calculated among everyone randomized or only among people who remained on treatment?
- What time point does it represent, and was follow-up long enough to show whether the effect persisted?
- Does the source report a confidence interval, p-value or another indication of statistical uncertainty?
- Could missing data, treatment switching or early discontinuation have changed the estimate?
Distinguish clinical outcomes from surrogate endpoints
FDA defines a clinical outcome as a measure of whether people feel or function better, or live longer. A surrogate endpoint is used instead of that direct outcome because it is expected to predict it. A lower scale weight, blood pressure or laboratory value may be biologically meaningful without proving a direct benefit on survival, symptoms or daily function.
How much validation does a surrogate have?
FDA says extensive evidence is generally needed to establish that a surrogate predicts clinical benefit in the context where it is used. A validated surrogate has stronger support than a candidate or “reasonably likely” surrogate. Accelerated approval can rely on a surrogate reasonably likely to predict benefit, with post-marketing studies required to determine whether the predicted benefit is confirmed.

FDA also reports that 45% of new drugs approved between 2010 and 2012 relied on a surrogate endpoint. That is a historical figure for that period, not a current approval-rate estimate.
“Clinical trials are conducted to demonstrate that new medical products deliver a positive balance of benefit and risk.”

When a story moves from “the drug lowered weight” to “the drug improved health,” identify the intermediate endpoint and ask what evidence connects it to the claimed clinical outcome.
Read harms, tolerability and body-composition evidence alongside efficacy
A benefit estimate without its adverse events is incomplete. Use the same trial and time period when comparing side effects, serious adverse events, treatment interruptions and discontinuations.
What a safety table should include
- Common adverse events and their frequency in each group.
- Serious adverse events and deaths, with the follow-up period stated.
- Discontinuations caused by adverse events.
- Whether the comparison was placebo-controlled and whether participants received other standard treatments.
- Any imbalance that could affect masking or adherence.
A preapproval trial’s size and duration do not settle every possible harm. FDA’s safety-monitoring program continues after approval because broader use, longer exposure and less-selected patients can reveal effects not found before marketing. Check the current prescribing information and FDA safety communications for the particular product and indication.

Do not infer body composition from the scale alone
FDA’s draft obesity-drug guidance discusses measuring body composition in a representative sample to determine whether weight reduction is primarily fat rather than lean mass, using baseline and follow-up DXA or a suitable alternative. That is a drug-development recommendation. A person’s scale change alone does not reveal the proportion of fat, muscle, fluid or other tissue lost.

Verify regulatory status before repeating “approved”
Use precise regulatory language:
| Status | What it means | What it does not establish |
|---|---|---|
| FDA-approved for a stated indication | FDA has determined that the product’s benefits outweigh known risks for the labeled use, based on the submitted evidence. | Approval for every dose, population, outcome or off-label claim. |
| Under investigation | The product or use is being studied and its benefits and risks remain uncertain. | That a promising result will be confirmed or that the use is authorized. |
| Compounded | A pharmacy or outsourcing facility prepares a product under applicable compounding rules. | FDA review of safety, effectiveness or quality before marketing. |
FDA says compounded GLP-1 products are not FDA-approved and are not reviewed for safety, effectiveness or quality before marketing. Its consumer alert identifies warning signs such as claims that a compounded product is identical to an FDA-approved drug, no screening or prescription by a licensed clinician, suspicious pricing or packaging, and confusing dosing instructions. Patients should obtain a prescription from a doctor and use a state-licensed pharmacy.
Do not use “FDA-approved” as shorthand for “contains the same ingredient,” and do not treat a compounded product’s label or promotional claim as evidence equivalent to a reviewed clinical trial.

- Copy the exact claim. Mark every implied outcome: weight, maintenance, symptoms, function, complications or cardiovascular events.
- Name the product and indication. Add formulation, dose, population and approval status.
- Locate the primary evidence. Prefer the trial report, protocol and regulatory review over a promotional summary.
- Map the design. Record phase, randomization, masking, comparator, participant count, duration, discontinuations and analysis population.
- Find the prespecified primary endpoint. Check whether the reported number is primary, secondary or exploratory.
- Calculate the right comparison. Use between-group differences; for events, include absolute rates as well as relative effects.
- Check uncertainty. Look for confidence intervals, statistical tests, missing-data methods and whether a subgroup or post-hoc analysis is being emphasized.
- Read the harms. Include adverse events, serious events, discontinuations and the length of safety follow-up.
- Classify the endpoint. State whether it directly measures feeling, function or survival, or is a surrogate expected to predict one.
- Verify current status. Check the exact label, indication and latest safety communications; distinguish approved, investigational and compounded products.
- Limit the conclusion. State who was studied, under what conditions and for how long. Do not generalize beyond those boundaries without saying that you are making an inference.
How to compare two weight-loss drugs without creating a false ranking
A larger percentage from one trial is not automatically a larger effect than a smaller percentage from another. Different populations, comparators, durations, adherence patterns, endpoints and analysis methods can produce numbers that are not directly comparable.
| Comparison axis | What to record | Why it matters |
|---|---|---|
| Studied and approved population | Age, baseline characteristics, conditions and labeled indication | Shows whether the evidence applies to the person or claim. |
| Endpoint | Primary outcome and whether it is clinical or surrogate | Prevents a laboratory or scale measure from being mistaken for a direct health benefit. |
| Effect size | Group-specific results, between-group difference and absolute event rates | Places percentages in a clinically interpretable frame. |
| Durability | Treatment and follow-up length, including after discontinuation | Reveals whether the result is temporary or sustained. |
| Safety and tolerability | Adverse events, serious events and discontinuations | Shows the cost of achieving the reported benefit. |
| Evidence source | Published trial, registry, regulatory review or promotional summary | Signals how much methodological detail can be checked. |
| Regulatory status | Approved indication, investigational use or compounded preparation | Separates established labeling from unresolved claims. |
If a named candidate is the subject of a story, match its trial-registry entry to the published protocol and report. Check registration dates, prespecified endpoints, amendments and any changes between the protocol and publication. A registry record alone is not proof of efficacy, and a published result that omits the original endpoint plan deserves additional scrutiny.

Reporting only the treatment group’s change
People can lose or gain weight in a placebo group. Without the control result, a treatment-group percentage exaggerates what the drug contributed.
Best Value
Calling a secondary result “the trial’s finding”
Secondary outcomes can be useful, but they were not necessarily the basis for the study’s power calculation or principal test. Preserve the endpoint hierarchy and any qualification about statistical testing.
Turning a surrogate into a promise
A lower BMI, glucose value or blood-pressure reading is not interchangeable with fewer hospitalizations, better functioning or longer life. Explain the evidence connecting the surrogate to the claimed benefit.
Ignoring treatment discontinuation
If many participants stop because of adverse events or burden, an efficacy estimate among completers may not describe ordinary use. Report both the result and how many remained on treatment.

A cardiovascular-outcomes trial in adults with established disease is not evidence for adolescents, people without that disease or every form of weight management. Keep the population and duration attached to the conclusion.
Treating draft guidance as a rule
The January 2025 FDA obesity-drug document is nonbinding draft guidance and not for implementation. It can explain FDA’s recommended development approach, but it is not a final legal requirement.
How to write a trustworthy conclusion
End with a bounded statement rather than a superlative. Identify the studied population, intervention, comparator, primary endpoint, effect estimate, follow-up and principal harms. Then say what remains unknown: durability, effects in excluded groups, confirmation of a surrogate’s predicted benefit, or post-market safety with wider use.
A careful conclusion might say that a randomized, placebo-controlled trial found a specified difference in a named endpoint over a stated period in its enrolled population, while the evidence did or did not establish a direct clinical benefit and the safety record remains limited to the available follow-up. That sentence gives readers a result they can use without turning one study into a universal promise.

Bottom line
Evaluate the claim at the level of the endpoint and the evidence, not the drug’s reputation. Verify who was studied, what the control received, how long follow-up lasted, whether the number was primary, and how benefits compare with harms. Keep weight change separate from direct clinical outcomes, attach every statistic to its population and time frame, and use current regulatory and safety information before deciding what the result really means.
