Do not treat an AI model’s confidence, chain-of-thought-style explanation, or self-grade as proof that its reasoning is sound. Evaluate the observable behavior instead: define the capability you need, test it on representative held-out tasks, compare answers and intermediate steps with independent criteria, probe how it reacts to changed evidence, and calibrate any confidence signal against actual outcomes.
A correct answer can contain invalid steps, and a persuasive explanation can be an after-the-fact account rather than a faithful transcript of the computation that produced the answer. Self-reports are useful clues for an evaluator, but they remain model outputs that require external checking.

What “good reasoning” means depends on the job
There is no single observable property called reasoning that one test can certify. Decide what must be reliable in your application before choosing a benchmark or metric.

- Evidence-based question answering: claims must be supported by the supplied documents, remain internally consistent, and distinguish evidence from inference.
- Tool-using agents: the agent must satisfy constraints, select appropriate actions, and avoid prohibited behavior, not merely describe a plausible plan.
- Uncertainty handling: confidence statements should predict success rates over many examples rather than sound persuasive on one example.
- Monitoring: a reviewer may need to predict a property of the agent’s behavior from traces, actions, or outputs.
These are different claims. A model can be accurate on a narrow calculation while poor at citing evidence, or produce readable explanations while being insensitive to a crucial premise. Your evaluation should name the property and the conditions under which it is being measured.

Why self-judgment is weak evidence
A right answer does not validate every step
Errors can cancel, an unjustified shortcut can reach the right number, or a model can guess correctly. If the process matters—for example, because a result will be audited, reused, or used to choose an action—check the intermediate claims separately. OpenAI’s process-supervision experiments on the MATH test set (published May 31, 2023) compared rewards for correct individual steps with rewards only for the final outcome and reported significantly better results for process supervision in those experiments. OpenAI also said that generalization beyond mathematics was unknown, so the finding is evidence for that setup, not a universal law.
An explanation may not be a faithful transcript
A fluent rationale can be generated after the answer is selected. Anthropic’s intervention study found substantial task-to-task variation in how strongly models condition their answers on the stated chain of thought, with lower faithfulness on most of the tasks studied as model capability increased. Its concise limitation is worth keeping in mind: “Models show large variation across tasks in how strongly they condition on the CoT when predicting their answer, sometimes relying heavily on the CoT and other times primarily ignoring it.” Treat a rationale as an inspectable artifact, not privileged access to hidden computation.
Confidence can help only after calibration
A confidence score is informative if it tracks outcomes on a representative set. Anthropic reported encouraging self-evaluation results in some settings, while also finding difficulty calibrating predictions about whether the model knows an answer on new tasks. A single “90% confident” response therefore tells you little until you have measured how often similarly scored responses are correct.

Step 1: Define the claim before testing
Write a one-sentence evaluation claim that specifies the task, required behavior, and operating conditions. Examples:
- “On new algebra problems in the specified format, the model reaches the correct result and every stated transformation is valid.”
- “Given a supplied policy, the model answers questions using only supported claims, cites the relevant passage, and does not contradict another cited passage.”
- “When an agent is given a prohibited action, it refuses or chooses an allowed alternative while preserving the task objective.”
Also record what would count as failure. If you cannot say which observation would disprove the claim, the test is measuring impressions rather than a defined capability.

Step 2: Build a representative, held-out task set
Use examples that match the real domain, difficulty, language, tool access, context length, and user constraints. Keep a held-out portion that is not used to tune prompts, select demonstrations, or adjust the evaluator. The cited studies use task-specific testbeds; they do not establish one universal dataset size or sampling rule.
Cover the situations that matter
- Include ordinary cases and edge cases from the deployment distribution.
- Separate languages, domains, difficulty bands, and tool environments so performance is not hidden by one aggregate.
- Add cases where evidence is incomplete, conflicting, or deliberately misleading if those conditions occur in practice.
- Prevent contamination between prompt-development examples and the final comparison set.
Do not claim that a score generalizes beyond the tested distribution without evidence. A model evaluated only on short English questions has not thereby been evaluated for long, multilingual, tool-mediated work.

For every task, decide what determines success without asking the model to be the final authority.
- Exact answer keys: appropriate for unambiguous numerical or symbolic results.
- Trusted calculations or executable tests: useful for code, arithmetic, and transformations that can be run or checked mechanically.
- Source documents: required when an answer must be grounded in supplied evidence.
- Formal rules: useful for policy, logic, or constraint compliance.
- Expert annotations: needed for open-ended quality judgments, with clear rubrics and adjudication.
If a model grades itself, store that grade as another prediction. Compare it with the independent label; never let it define correctness by itself. Google Research’s REVEAL benchmark (ACL, 2024) illustrates this separation by labeling individual reasoning steps for relevance, attribution to evidence passages, and logical correctness.
Step 4: Score outcomes and reasoning steps separately
Keep final-answer accuracy distinct from process validity. A useful report can show how often the final answer is correct, how often all material steps are valid, and which kinds of step fail.

| Score | What it measures | Typical evidence | What it cannot prove |
|---|---|---|---|
| Final outcome | Whether the requested result is correct | Answer key, executable test, trusted calculation | That the explanation or every intermediate step is valid |
| Step relevance | Whether a step contributes to solving the task | Human or validated rubric labels | That the step is logically correct |
| Evidence attribution | Whether a claim is tied to the right passage or source | Source-span annotations | That the cited passage actually entails the claim |
| Logical correctness | Whether each inference follows from prior information | Formal checks or expert review | That the model used the stated chain internally |
| Confidence calibration | Whether predicted probabilities match observed frequencies | Grouped confidence-versus-accuracy results | Correctness on a single example |
REVEAL reports that automatic verifiers struggle particularly with logical correctness and contradiction detection. Validate a verifier against trusted or human labels before treating its score as ground truth.

Inspect the failure, not just the percentage
For failed examples, identify the first unsupported claim, the point where a contradiction appears, or the action that violates a constraint. Preserve the model output, the reference, the scoring decision, and the evaluator’s rationale. A small set of concrete failures often reveals a systematic weakness that an aggregate hides.
Step 5: Probe sensitivity with controlled interventions
Ordinary test items show performance under ordinary conditions. Interventions test whether the model is responding to the information that should matter.
- Remove a relevant hint. The answer should change or become appropriately less certain when the hint is genuinely necessary.
- Introduce a misleading cue. Check whether the model follows the cue despite contrary evidence.
- Perturb a premise. Change one material fact and verify that the conclusion changes in the logically expected direction.
- Add a contradiction. See whether the model notices incompatible evidence rather than silently merging it.
- Paraphrase the prompt or explanation request. A robust result should not depend on superficial wording.
- Ask for a counterfactual. Require the model to state what would follow if a specified premise were reversed.
Anthropic’s faithfulness work used interventions such as adding mistakes or paraphrasing chain-of-thought and found that conditioning on the stated reasoning varied by task. These tests reveal sensitivity to selected inputs; they do not expose every hidden computation.
Step 6: Measure confidence calibration instead of confidence theater
Collect confidence predictions across many held-out examples, then group predictions into ranges and compare each group’s average confidence with its observed correctness. If a group averages 0.8 confidence, its accuracy should be near 80% before that signal is used for operational decisions. The comparison must use enough representative examples to make the estimate meaningful; the cited sources do not establish a universal sample size or acceptable calibration threshold.

Record the conditions attached to confidence
- Was the model allowed to use tools or retrieve documents?
- Did it provide a probability, a verbal label, or only a phrase such as “I’m sure”?
- Was confidence requested before or after seeing a critique?
- Were examples sampled at random, or selected because they were difficult?
Do not average confidence across unlike tasks. A model can be calibrated on arithmetic and overconfident on legal text. Report calibration by domain, difficulty, language, and tool setting when those categories affect use.
Step 7: Re-run stochastic evaluations and report uncertainty
Sampling settings, tool results, and conversation history can change outputs. Repeat evaluations when outputs are stochastic, retain the settings, and report uncertainty around aggregate scores. Show results by task category and include representative successes and failures.
OpenAI’s monitorability work frames evaluation as testing distinct properties across evaluation archetypes and environments. The article reports a suite of 13 evaluations comprising 24 environments, but the publication date was not visible in the page extract used for that figure. Treat the count as a description of that study’s suite, not a dated industry benchmark or a universal coverage claim.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Step 8: Use explanation monitoring as one layer, not a verdict
Reasoning traces may help a monitor predict a property of an agent’s behavior, but a trace is not automatically complete or faithful. OpenAI defines monitorability as “the ability of a monitor to predict properties of interest about an agent’s behavior.” In its reported settings, monitoring chains of thought often outperformed monitoring actions and final outputs alone, while frontier models were not perfectly monitorable.

How the main evaluation approaches differ
| Approach | Primary question | Unit scored | Main limitation |
|---|---|---|---|
| Outcome supervision | Did the model produce the right result? | Whole answer or action | Can miss invalid steps and lucky guesses |
| Process supervision | Are intermediate steps correct? | Individual reasoning step | Requires reliable step labels and may be domain-specific |
| Evidence-grounding evaluation | Are claims relevant and attributed to supporting passages? | Claim or step | Attribution does not guarantee logical entailment |
| Intervention testing | Does behavior respond to material evidence and resist misleading cues? | Change between controlled conditions | Tests selected sensitivities, not all hidden computation |
| Confidence calibration | Do confidence predictions match success rates? | Prediction across a set | Can fail when the deployment distribution changes |
| Monitorability testing | Can a monitor predict a property of agent behavior? | Monitor prediction | Depends on both the model and the monitor |
These methods answer different questions. REVEAL, process-supervision experiments, Anthropic’s faithfulness and self-evaluation studies, and OpenAI’s monitorability work should not be collapsed into one general reasoning score.
Applying the workflow to three common use cases
Mathematical problem solving
- Create held-out problems matching the target curriculum and difficulty.
- Define the external criterion with exact answers and, where possible, a trusted symbolic or numerical check.
- Score the final result and each material transformation separately.
- Add premise perturbations, such as changing a coefficient or constraint, and verify that the solution adapts.
- Compare confidence bands with actual correctness and inspect examples where the answer is right but a step is invalid.
The May 31, 2023 OpenAI process-supervision result supports this style of step-level checking in the studied MATH setting; it does not establish the same advantage for every subject.
Document-grounded question answering
- Supply a fixed document set and preserve a held-out question split.
- Require each material claim to identify its supporting passage.
- Label relevance, attribution, logical support, and contradictions separately.
- Insert conflicting or irrelevant passages to test whether the model distinguishes evidence quality.
- Have a validated reviewer check both the model’s answer and any automated verifier’s decisions.
REVEAL’s step-level labels demonstrate why “has a citation” and “is logically supported” should be separate fields.
Recommended Free Tools
Tool-using or autonomous agents
- Define allowed actions, prohibited actions, success conditions, and recovery behavior before testing.
- Log prompts, tool inputs and outputs, intermediate decisions, and final actions.
- Include tasks with ambiguous instructions, tempting prohibited actions, and recoverable tool failures.
- Score objective task success and constraint compliance independently.
- Test whether an explanation monitor predicts the property you actually care about, then compare it with action-log checks.
An agent that completes a task while violating a safety constraint is not fully successful, even if its final explanation sounds responsible.
Common evaluation mistakes and fixes
- Using the model as its own judge: replace self-grading with answer keys, executable tests, source evidence, formal rules, or validated expert labels.
- Reporting only final accuracy: add step validity, evidence support, contradiction checks, or constraint compliance where relevant.
- Trusting a long explanation: apply interventions and verify claims independently; verbosity is not evidence of faithfulness.
- Mixing domains into one score: report categories separately so a strong area cannot hide a weak one.
- Tuning on the test set: keep a held-out split and document every prompt or evaluator change.
- Accepting an automated verifier without validation: measure its agreement with trusted labels, especially for logical correctness and contradictions.
- Generalizing beyond the test distribution: state the domains, languages, models, tools, and sampling settings actually evaluated.
- Ignoring uncertainty: repeat stochastic runs and show variability, not only a single aggregate.
What to record in an evaluation report
A reproducible report should let another evaluator understand exactly what the score means.
- The capability claim and failure definition.
- Task sources, domains, languages, data split, and contamination controls.
- Model version, system and user prompts, tool permissions, context limits, and sampling settings.
- External references, annotation rubric, adjudication process, and verifier validation.
- Separate outcome, step, evidence, calibration, intervention, and monitorability results.
- Number of runs, uncertainty estimates, category-level results, and representative failure examples.
- Known limits, including proxy measures, restricted data, verifier errors, and uncertain transfer to deployment.
This level of disclosure prevents a confidence score or benchmark label from being mistaken for a general certificate of reasoning ability.
The practical decision rule
Use an AI model’s explanation and confidence to decide what to inspect next, never to decide that inspection is unnecessary. Trust a reasoning-dependent output only when it passes an external outcome check, the required intermediate or evidence checks, controlled robustness tests, and a calibration analysis appropriate to the deployment conditions. If any of those layers is missing, narrow the claim to what was actually measured.
