Evaluate a drug-response model against a precisely defined prediction task, response endpoint, dataset and intended use. Keep preprocessing and splits identical across models, include simple baselines, report aggregate and per-drug results, and test on independent datasets whenever you claim transfer. Treat cell-line scores as preclinical predictive evidence—not proof that patients will benefit.

Start by defining what the model is supposed to predict

A score has no meaning until its prediction target and use case are explicit. “Drug response” can mean a continuous sensitivity value, a binary sensitive/resistant label, a ranking of drugs for each biological sample, or the effect of a drug combination. These are different tasks and require different labels, splits and metrics.

Specify the biological unit

State whether each prediction concerns a cancer cell line, a patient-derived sample, a tumor biopsy or another unit. Cell-line studies are controlled preclinical experiments; they are not interchangeable with patient-level validation. Also state whether the model predicts for cell lines seen during training, new cell lines, new drugs, or both.

Name the response endpoint exactly

Identify the measured endpoint, such as an IC50 estimate, area under the dose–response curve (AUDRC), viability at a specified concentration, or a derived sensitivity class. Include the assay protocol, dose range and normalization method when available. IC50 and AUDRC values that share a name can still be non-comparable if studies use different concentrations, curve-fitting procedures or response definitions.

Connect the endpoint to a decision

Explain what a correct prediction would enable: estimate a continuous response, rank candidate single agents, classify samples for a treatment threshold, or prioritize combinations for follow-up. A model that has the best correlation may not be the best tool for selecting the top three drugs, and a classifier calibrated for one threshold should not silently be used with another.

Build a fair comparison before training

Model comparisons are credible only when competing methods receive equivalent information and are evaluated on equivalent observations. The 2026 benchmark by Partin and colleagues identifies inconsistent dataset composition, data splits and metrics as major barriers to fair comparisons and standardizes the workflow across seven drug-response models and five public drug-screening datasets.

Freeze the data pipeline

  • Use the same response records, molecular features, omics inputs and drug representations for every compared model unless the comparison explicitly studies input modalities.
  • Apply preprocessing, imputation, scaling, feature selection and dimensionality reduction inside the training portion of each split. Fitting these steps on all data leaks information from the test set.
  • Document duplicate handling, missing responses, failed assays, replicate aggregation and any response-value transformations.
  • Keep the response endpoint and units unchanged across models. If harmonization is necessary, publish the rule and identify which records were excluded.

Include meaningful baselines

Compare complex neural or foundation models with straightforward baselines that use the same split and inputs. A baseline can be a global mean, a drug-wise or cell-line-wise mean, regularized linear regression, or another simple method appropriate to the endpoint. If a sophisticated model does not beat a baseline on the intended test, the extra complexity has not demonstrated value.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prevent hidden information leakage

Check that a cell line, drug, dose-response replicate or near-duplicate molecular profile cannot appear in both training and test data in a way that reveals the answer. Record whether related compounds, repeated assays or measurements from the same study are grouped together. Leakage can produce impressive in-dataset scores while failing on a genuinely new experiment.

Choose a split that matches the claim

The split determines the question your score answers. A random holdout estimates performance on observations resembling the source dataset. It does not establish that the model transfers to another laboratory, assay or study.

Intended claim Appropriate evaluation design What the result supports What it does not support
Predict additional records from the same study Random or stratified holdout, with leakage controls In-domain prediction for similarly sampled records Transfer to a new study, assay or dose range
Predict a new cell line Group the held-out cell line so none of its records enter training Generalization across biological samples Generalization to unseen drugs or protocols
Predict a new drug Hold out all responses for the drug, using a documented drug split Generalization to compounds not represented in training Performance on a new assay context unless separately tested
Transfer to another screening study Train on one dataset and test on an independent dataset; report the relative drop from within-dataset performance Cross-dataset robustness under the tested conditions Universal performance across all laboratories or patient populations
Support a clinical decision Patient-level external validation with a prespecified clinical endpoint Only the clinical association demonstrated by that study Clinical benefit based solely on cell-line benchmarks

State exactly which drugs, cell lines and datasets are held out. If a test set is reused for model selection, it is no longer an untouched final test; use a validation set or nested procedure for tuning and reserve the final test for one locked evaluation.

Match metrics to the prediction output

Name the metric, its direction (higher or lower is better), the unit or scale, and how values were aggregated. Different studies can report “accuracy” or a correlation while measuring different tasks, making an unexplained headline score unsuitable for comparison.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Task Useful metric families Interpretation to report Important cautions
Continuous response regression Pearson or Spearman correlation; MAE, RMSE or another stated error measure Whether predictions track measured responses and how far they deviate Correlation can be high despite systematic bias; error depends on the response scale
Binary sensitivity classification AUROC, area under the precision–recall curve, sensitivity, specificity, balanced accuracy and calibration measures Discrimination, performance at the chosen threshold and probability reliability Class imbalance can make plain accuracy misleading; thresholds must be prespecified
Drug prioritization or ranking A ranking metric suited to the number of recommendations and a stated top-k rule Whether the model places useful candidates near the top A good global correlation does not guarantee a useful top-ranked list
Combination-effect prediction A metric defined for the chosen synergy or response model Agreement with the combination endpoint and decision threshold Combination prediction is a distinct task; single-agent scores cannot be transferred automatically

Show confidence intervals or variability across repeated splits when feasible, and disclose whether an aggregate is a mean, median, weighted score or pooled calculation. Report the number of test observations behind every summary. A single average can hide unstable or clinically important failures.

Always report overall and per-drug performance

Aggregate performance is necessary but insufficient. The review Foundation models and deep learning for cancer drug response prediction: a framework for data, metrics, and validation states that drug-response prediction models should be evaluated both overall and on a per-drug basis.

What to include for each drug

  • Number of test responses and the response range.
  • The chosen metric with its direction and uncertainty.
  • Performance relative to the same baseline.
  • Whether the drug was represented in training and, if so, at what level.
  • Any assay, dose-range or missingness characteristic that limits interpretation.

Present the distribution across drugs—such as a median, interquartile range and worst-performing subgroup—in addition to the overall value. A model that wins on average because of a few heavily measured drugs may be unsuitable for an underrepresented treatment.

Inspect biological and technical subgroups

Stratify where the intended use requires it: tissue lineage, mutation status, dose range, study site, assay batch or response prevalence. Keep subgroup analyses prespecified when possible. Very small subgroups should be labeled as uncertain rather than treated as definitive rankings.

Test transfer on independent datasets

When the intended use involves new data, train and tune on one dataset, lock the model and evaluate it on an independent dataset that differs in study or assay context. Describe the direction of transfer, the overlapping drugs or cell lines, and the records removed because labels or features were unavailable.

Measure both absolute performance and performance drop

Report the in-dataset score and the external score side by side, then quantify the relative or absolute drop using a clearly stated formula. A large drop is not merely a disappointing number; it reveals sensitivity to protocol, population or feature distribution changes.

Interpret the 2026 benchmark correctly

Partin and colleagues evaluated seven standardized models across five public drug-screening datasets. Their study found substantial performance declines on unseen datasets and no model that consistently led across every dataset. Therefore, an in-domain winner should not be described as the best model for all other studies. The result supports choosing a model for a specified deployment context, not declaring a universal champion.

Use independent or hidden test data when possible

The DREAM drug-sensitivity challenge illustrates the value of reserving hidden gold-standard data from participants. A locked external test prevents repeated tuning against the answers and gives a more honest estimate of transfer. If no hidden set exists, document how often the public test was inspected and treat the resulting estimate as optimistic.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Harmonize response labels before comparing studies

Drug-response measurements are products of experimental protocols, concentration ranges and analysis choices. The 2026 review highlights particular difficulty when comparing IC50 or AUDRC values across datasets collected under different conditions.

Document curation decisions

  • Record the original assay, concentration points, exposure time and response normalization.
  • Keep track of curve-fitting failures, censored IC50 values and extrapolated estimates.
  • Specify whether replicate measurements were averaged, median-combined or otherwise summarized.
  • Use an explicit conversion or normalization rule only when its assumptions are justified.
  • Retain the original values and provenance so another analyst can audit the harmonized label.

Do not treat similarly named endpoints as interchangeable merely because their columns have the same label. If cross-study comparability is uncertain, state that limitation and restrict the claim to the conditions actually tested.

Keep single-agent and combination prediction separate

Single-agent response predicts how one drug affects a biological sample. Combination prediction estimates a joint effect or synergy under a particular combination model. Their labels, experimental designs and decision thresholds differ, so results from one task should not be used as evidence for the other.

The 2019 cancer drug-combination community assessment analyzed 11,576 experiments covering 910 combinations and 85 molecularly characterized cancer cell lines. Twenty percent of combinations were poorly predicted by all participating methods in that challenge dataset. That figure describes those experiments and methods; it is not a general failure rate for every combination model or current dataset.

Use the CTRPv2 figures with their scope attached

In the five-dataset 2026 benchmark, the dataset table lists CTRPv2 with 494 drugs, 720 cell lines and 286,665 responses. Those counts describe that benchmark’s curated records, not every version or download of CTRPv2. Cite the dataset version and curation rules whenever you reproduce the comparison.

The other benchmark datasets should likewise be identified by name, release and number of usable responses in your own report. If a value is not stated or cannot be verified for the version used, label it as not stated rather than filling the cell with an estimate.

Make the experiment reproducible

A result is useful only if another team can reconstruct the data selection, features, splits and scoring. The 2026 benchmark reports versioned code and an archived benchmark dataset containing response data, omics features, drug representations and predefined splits. Check the article’s archive and version tags before relying on those resources, because software and availability can change.

Publish a minimum record

  • Dataset names, versions, access dates and inclusion or exclusion rules.
  • Feature sources, preprocessing order and random seeds.
  • Exact train, validation and test identifiers, or a deterministic split script.
  • Model configuration, hyperparameters, stopping criteria and hardware-relevant settings.
  • Metric definitions, aggregation rules and confidence-interval procedure.
  • Per-drug predictions or sufficient summaries to reproduce subgroup analyses.

Use a locked configuration for the final test and separate exploratory plots from prespecified results. Version the code and environment so a future rerun can distinguish a genuine model change from a dependency or dataset update.

Interpret scores as preclinical evidence

Cell-line benchmarks measure predictive agreement with laboratory response labels. The available evidence does not establish that a higher cell-line score improves patient outcomes, treatment selection or survival. Patient tumors contain microenvironmental, pharmacokinetic and clinical factors that are not represented fully in a cell-line assay.

Use careful language: “the model predicted the held-out cell-line endpoint under these assay conditions” is supported by a benchmark; “the model will identify the most effective treatment for patients” requires separate patient-level evidence. A clinical claim needs an external patient cohort, a prespecified endpoint and an evaluation design appropriate to that clinical decision.

A step-by-step evaluation protocol

  1. Write the use-case statement. Name the biological unit, drug scope, endpoint, prediction output and decision the model is intended to support.
  2. Freeze the dataset. Record release, assay context, response units, feature sources and curation rules.
  3. Audit leakage risks. Group duplicates, replicates and related records according to the generalization claim.
  4. Select the split. Use in-dataset, leave-cell-line-out, leave-drug-out or cross-dataset testing as appropriate, and state what is held out.
  5. Define preprocessing. Fit transformations only on training data and apply the frozen parameters to validation and test data.
  6. Choose baselines. Include simple predictors and use identical inputs and splits for every model.
  7. Prespecify metrics. Define direction, threshold, units, aggregation and uncertainty before inspecting test results.
  8. Train and tune. Use validation data or nested procedures; never tune repeatedly on the final test set.
  9. Report detail. Show overall, per-drug and relevant subgroup results with sample counts and uncertainty.
  10. Run external validation. Lock the model, test on an independent dataset and quantify the performance drop.
  11. Analyze failures. Investigate drugs, assays, doses and biological subgroups with weak predictions instead of hiding them in an average.
  12. State the boundary. Explain that the result is preclinical unless patient-level clinical utility has actually been tested.

Troubleshoot common evaluation failures

The score is excellent in-dataset but poor elsewhere

Check for study-specific signatures, batch leakage, overlapping cell lines or drugs, and inconsistent response labels. Re-run with grouped splits and report the independent-dataset result as the transfer estimate.

Two papers report incompatible “best” models

Compare endpoint, assay, dataset composition, split, metric direction and aggregation before comparing model names. Different experimental questions can legitimately produce different winners.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The aggregate hides a weak treatment

Publish per-drug values, test counts and the performance distribution. Consider whether heavily sampled drugs dominate a pooled score.

Classification accuracy looks high but decisions are poor

Inspect class prevalence, precision–recall performance, calibration and threshold-specific sensitivity and specificity. Reconsider whether the threshold reflects the intended decision.

IC50 values disagree across sources

Compare dose ranges, exposure times, curve fits, censoring and normalization. Preserve provenance and avoid merging values that are not demonstrably comparable.

A model is called clinically useful from cell-line results

Remove the clinical claim unless an appropriate patient-level external evaluation supports it. Describe the result as preclinical predictive performance and specify the assay conditions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What a high-quality report should let a reader answer

  • What exactly was predicted, for which biological unit and for what decision?
  • Which endpoint, assay and dose range produced the labels?
  • Could information from the test set enter preprocessing or model selection?
  • What does the split represent, and which drugs or cell lines were unseen?
  • How does the model compare with simple baselines under the same conditions?
  • What are the overall, per-drug and subgroup results?
  • How much performance was lost on an independent dataset?
  • Are the response labels comparable across studies?
  • Can the code, data version, splits and metric definitions reproduce the result?
  • Is the conclusion limited to preclinical prediction, or is there genuine patient evidence?
