The most defensible estimate of a multimodal cancer classifier’s performance comes from a patient-level split made before any slides, scans, specimens, visits, or clinical records are turned into training samples. Keep every record from one patient in one partition, fit every preprocessing and selection step on development data only, freeze the model, and then test it on data that played no role in development. Report internal testing on a held-out development population separately from external testing on a genuinely independent cohort.

This design tests whether the classifier works for new patients rather than whether it can recognize a patient, slide, scanner, institution, staining protocol, or processing artifact it has already seen.

What a leakage-resistant evaluation is trying to estimate

A cancer classifier is useful only if its predictions remain reliable for the people and care setting in which it will be used. The target is therefore performance on new patients drawn from the intended-use population, not performance on randomly selected images that may be linked to patients already represented in training.

Multimodal systems make this distinction harder. One patient may contribute whole-slide images, radiology studies, genomic measurements, laboratory values, visits, and outcome records. A sample-level random split can place one patient’s pathology slide in training and another slide from the same patient in testing. The model can then exploit patient-specific morphology, acquisition signatures, or duplicated clinical information. The reported score may look impressive while saying little about generalization.

A sound evaluation keeps three roles separate:

  • Development data: used to fit model parameters and make design decisions.
  • Tuning or internal-testing data: held out within the development population for model selection or an internal performance estimate, depending on the prespecified design.
  • Final evaluation data: untouched until the procedure is locked. This can be an internal holdout or, preferably for transportability claims, a fully external cohort.

TRIPOD+AI states the separation requirement plainly: Evaluation data should be distinct from the data used to train the model, tune hyperparameters, or do model selection, such that there is no overlap in participants between the training and evaluation data. Treat a dataset as development data as soon as its results influence a method change. A supposedly final test that is used to choose a threshold, architecture, feature set, or imputation rule is no longer an unbiased final test.

Define the prediction task before touching the data

Specify what is predicted and for whom

Write a task specification before modeling. State whether the classifier is diagnostic or prognostic, the unit of prediction, the target population, the index time, the outcome definition, and the prediction horizon. A diagnostic task might predict a tumor category from tissue and imaging at a defined clinical time. A prognostic task might estimate recurrence by five years after surgery. These are different estimands and require different labels, follow-up, and evaluation methods.

Define the unit of prediction explicitly. If the intended output is one risk estimate per patient, aggregate slide- or scan-level evidence only after patient-level partitioning. If the intended output is per lesion, explain how multiple lesions from a person are handled and how a patient-level clinical decision will be derived.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Document the reference standard

Use the term reference standard for the process that establishes the target outcome, as recommended by CLAIM 2024. A clinical label is not automatically absolute truth. Explain who assigned the diagnosis, which records or follow-up established an outcome, whether adjudicators were blinded to model inputs, and whether the reference standard was available independently of those inputs.

For prognosis, describe censoring, follow-up duration, competing events where relevant, and rules for incomplete follow-up. For diagnosis, identify the pathology, imaging, molecular, or consensus process used to establish the reference standard. Do not imply that one labeling method is appropriate for every cancer or endpoint.

Build patient-level partitions before creating samples

Create a linkage table first

Start with a master table containing a unique patient identifier and links to every potentially related record: slides, blocks, specimens, scans, series, visits, laboratory results, genomic assays, treatment episodes, and outcome records. Resolve duplicate identifiers and aliases before splitting. A record that cannot be confidently linked should be treated as an unresolved linkage problem, not silently assigned to a random partition.

Assign patients to development, tuning or internal testing, and final evaluation partitions before extracting tiles, image patches, embeddings, tabular rows, or modality-specific datasets. Every item linked to a patient stays in that patient’s partition. If grouped cross-validation is used, every record for a patient stays in one fold.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the right unit for each split

Design element Correct grouping unit What it protects against What to report
Train/holdout split Patient Cross-patient duplication and correlated samples Number of patients and all linked records in each partition
Cross-validation Patient, with all records in one fold Leakage from repeated slides, scans, visits, or specimens Grouping rule and fold construction
Site or institution assessment Site, when testing transportability Shortcut learning from local acquisition or workflow Sites, dates, protocols, and patient counts
Temporal assessment Time period, with patient grouping Use of future information and changing practice effects Cutoff dates and whether preprocessing was refit

TRIPOD+AI asks authors to explain how multiple samples from one person were handled and whether individuals stayed within a single fold. Include a flow diagram or table that makes the partitioning unit auditable.

Do not let a test result become a tuning signal

The final evaluation set must not influence feature selection, threshold selection, preprocessing choices, architecture choice, augmentation policy, or hyperparameter search. If investigators inspect a result and revise the method, that result has entered development. A fresh, untouched evaluation set is then needed for an unbiased estimate.

Keep a decision log with dates, dataset versions, and the exact procedure frozen before final evaluation. This is especially important when many candidate models are tried: selecting the best score from the final set is a form of overfitting even when the model weights themselves were not retrained.

Prevent leakage in preprocessing, feature extraction, and linkage

Fit transformations on development data only

Calculate normalization statistics, imputation values, feature-selection rules, dimensionality-reduction components, vocabulary or codebooks, and calibration parameters using development data only. Apply the frozen transformations to tuning, internal-test, and external-test patients. A pipeline that standardizes every record before splitting can leak information through means, variances, missingness patterns, or batch structure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When cross-validating, fit each transformation inside the training portion of each fold and apply it to that fold’s held-out patients. Repeating the entire model-building process, including tuning and preprocessing, is necessary for an honest cross-validation estimate.

Audit duplicates and near-duplicates

Check for duplicate slides, repeat scans, re-exported images, overlapping tiles, shared specimen blocks, and near-identical augmented images across partitions. A tile extractor can create thousands of apparently independent samples from one slide; those tiles remain correlated observations from one patient and one acquisition. Keep the parent slide and patient identifiers with every derived sample so leakage checks can be rerun after feature extraction.

Look for acquisition shortcuts

Record institution, scanner, camera, staining or preparation protocol, sequence settings, software version, and acquisition date. A model can learn a site logo, scanner noise pattern, stain batch, compression signature, or label workflow when those factors correlate with the outcome. The examples are practical audit targets derived from the transparent data-handling and acquisition-reporting principles in CLAIM 2024 and TRIPOD+AI; they are not an exhaustive checklist of every possible leakage mechanism.

Batch correction, stain normalization, scanner harmonization, and artifact removal must be specified. If a correction model is trained on pooled data, determine whether information from evaluation patients influenced its parameters. If so, refit it using development data or justify an unsupervised procedure that cannot use outcome or patient identity information.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make modality availability explicit

For each modality, report how many patients have usable data, why measurements are missing, and whether missingness is related to disease, site, or clinical workflow. If deployment must work when one modality is absent, define that behavior and evaluate it deliberately. Do not silently drop patients for one model while retaining them for another; differing denominators can make a fusion model appear better simply because it was evaluated on an easier subset.

Internal testing and external testing answer different questions

Evaluation type Dataset Question answered Limitation
Internal testing Held-out patients from the development setting, or grouped resampling within it How well does the locked procedure perform within the population and workflow used for development? It may not reveal changes in prevalence, protocols, referral patterns, or site-specific shortcuts.
External testing A distinct cohort not used in development, ideally another institution or later time period Does the fixed model transport to a new but relevant population and acquisition context? Performance can differ because of case mix, prevalence, reference standards, or technical shifts; those differences must be described.
Tuning data Development-population data used to choose settings or models Which configuration should be frozen? It is part of model development and cannot serve as an unbiased final test.

Internal testing

Internal testing can use a patient-level holdout, grouped cross-validation, or bootstrapping. Describe whether tuning occurred inside each resampling loop and whether the entire model-building process was repeated. A single random split at the image or tile level is not a valid substitute for patient-level grouping.

External testing

CLAIM 2024 recommends the term external testing rather than external validation for testing on data external to development, such as data from another site. Report the external cohort’s institutions, dates, patient mix, disease prevalence, acquisition protocols, and reference-standard process. An internal random split does not establish performance at a new hospital.

An external cohort should be representative of the intended use, not merely convenient. If the study excludes patients with missing modalities but deployment will encounter them, the external estimate does not describe real-world operation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Freeze the complete procedure before the final evaluation

  1. Write the protocol: define the task, reference standard, index time, horizon, patient eligibility, modalities, missing-data rules, metrics, and subgroup analyses.
  2. Construct the linkage table: connect every record to a patient and resolve duplicates or uncertain identities.
  3. Assign partitions: place patients, not samples, into development, tuning or internal-test, and final-test groups. Record the assignment seed or deterministic rule.
  4. Build the development pipeline: fit preprocessing, feature selection, fusion architecture, and hyperparameters without accessing final-test patients.
  5. Select thresholds: choose operating thresholds on development or tuning data according to a prespecified clinical objective. Do not choose a threshold after seeing final-test outcomes.
  6. Lock artifacts: save model weights, preprocessing parameters, feature lists, imputation rules, threshold, software versions, and inference code.
  7. Run the final evaluation once the procedure is locked: apply the frozen pipeline to internal holdout or external patients, preserving patient-level independence.
  8. Report the denominator and uncertainty: include excluded patients, missing modalities, class counts, confidence intervals, and every deviation from the protocol.

Report discrimination, calibration, and clinical utility

Discrimination

For binary outcomes, report a discrimination measure such as the c statistic or area under the receiver-operating-characteristic curve with uncertainty. Include class counts and the number of patients contributing to the estimate. For time-to-event outcomes, use an appropriate time-to-event measure such as the c index and explain censoring and the evaluation horizon.

A single AUC does not show whether predicted probabilities are trustworthy or whether using the model improves decisions. Avoid presenting a score from one cohort as a universal benchmark; no disease-independent performance figure exists for multimodal cancer classifiers.

Calibration

Show whether predicted risks match observed frequencies. A calibration plot, calibration-in-the-large estimate, and calibration slope can reveal systematic overprediction or underprediction that discrimination metrics miss. State how calibration was estimated and whether any recalibration was performed. Recalibration on an external test set changes the model and should be treated as development for a subsequent evaluation.

Clinical utility

Where a threshold-based decision is intended, report clinically meaningful operating points and, when appropriate, decision-curve analysis. Explain the consequences of false positives and false negatives, the threshold-selection objective, and whether the threshold was fixed before testing. Utility depends on the intended decision and cannot be inferred from AUC alone.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Uncertainty and missing data

Provide confidence intervals or other uncertainty estimates, not just point estimates. State the resampling or statistical method, handling of missing outcomes, and whether intervals account for patient clustering or repeated measurements. Give denominators for every metric and subgroup.

Compare multimodal and single-modality models fairly

A fusion model should be compared with each available modality-specific model on the same patient-level cohort, under the same outcome definition, partition, preprocessing isolation, threshold rules, and final evaluation process. This matched design is a practical synthesis of general evaluation guidance; it is not a universal fusion-specific checklist item.

Model Patients included Inputs Fair comparison requirement
Fused model Patients with all required modalities, or a prespecified missing-modality strategy All modalities available to the model Use the same locked patient set and evaluation rules as comparators.
Modality-specific model A The matched patients used for the fused model when possible Modality A only Do not evaluate on a larger, easier population unless that analysis is labeled separately.
Modality-specific model B The matched patients used for the fused model when possible Modality B only Report any additional patients with B and keep that analysis distinct.
Missing-modality model Patients following the deployment missingness rule Available inputs at inference time State how missingness is represented and evaluate it as its own operating condition.

Report how many patients have each modality and whether excluding incomplete cases changes disease prevalence or site composition. If the fused model is tested on 500 complete cases while a single-modality model is tested on 900 cases, their metrics are not a controlled comparison. Provide matched-cohort results and then, if useful, clearly labeled availability analyses.

One breast-cancer prognostic study combining digital pathology and clinical characteristics evaluated its model in five external cohorts. That is an example of an ambitious external-evaluation design, not evidence that the same architecture or performance will generalize to another cancer, endpoint, or health system.

Examine transportability, sites, and clinically relevant subgroups

Quantify how performance varies across institutions or other meaningful clusters. TRIPOD+AI asks authors to explain how heterogeneity in estimates across clusters such as hospitals or countries was handled and quantified. Report site-specific discrimination, calibration, class counts, and uncertainty when sample sizes permit.

Describe differences between development and external cohorts in referral patterns, disease severity, prevalence, demographics, treatment pathways, acquisition hardware, protocols, and reference standards. A performance drop may indicate a change in case mix, a technical shift, label inconsistency, or genuine transport failure; the report should not attribute the cause without evidence.

Where justified by the clinical task and supported by data, provide subgroup results for relevant patient groups. Prespecify subgroups, show uncertainty, and avoid turning a small exploratory subgroup into a definitive claim. There is no universal subgroup list for every cancer type.

Make the report auditable with TRIPOD+AI and CLAIM 2024

Common leakage patterns and how to correct them

Randomly splitting tiles or patches

Symptom: very high test performance with many samples but few patients. Correction: return to the patient linkage table, regenerate partitions, and extract tiles separately within each partition.

Using test performance to choose the model

Symptom: repeated architecture or threshold changes after viewing final-test metrics. Correction: classify that dataset as development data and reserve a new untouched cohort for the final estimate.

Global normalization or imputation

Symptom: means, variances, missing-value replacements, or feature lists were calculated before splitting. Correction: fit each operation within development data, or within each training fold, then apply frozen parameters forward.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Hidden duplicates and repeat acquisitions

Symptom: the same specimen, scan, or near-identical image appears under different identifiers. Correction: deduplicate at patient and acquisition levels and retain parent identifiers through feature extraction.

Different cohorts for fusion and baseline models

Symptom: the fused model is evaluated only on complete cases while a baseline uses all available patients. Correction: report matched-patient comparisons and disclose the availability trade-off.

Calling an internal split external validation

Symptom: a random holdout from one hospital is described as proof of external performance. Correction: label it internal testing and reserve external testing for a dataset outside development, such as another institution or a later period.

A practical end-to-end example

Suppose three patients contribute the following records: Patient A has four pathology slides and two scans; Patient B has one slide and three visits; Patient C has two scans and a clinical record. First assign A, B, and C—not their individual records—to partitions. If A is in development, all six of A’s records remain there. Tile extraction, scan preprocessing, and clinical feature generation happen after that assignment, and every derived record keeps its patient identifier.

Fit stain normalization, scan intensity scaling, missing-value rules, and feature selection using only development patients. Use grouped folds if tuning within development. Select the fusion architecture and operating threshold without consulting the final cohort. Save the resulting pipeline. On the final cohort, run inference once, calculate discrimination, calibration, and utility with uncertainty, and report the number of patients with each modality. If an investigator changes the pipeline after seeing those results, the original final cohort has become development data and cannot support the next unbiased claim.

What a credible conclusion can and cannot say

A leakage-resistant internal result supports a claim about the development population under the stated protocol. An external result supports a claim about the evaluated external cohort and its acquisition and reference-standard context. Neither result automatically proves safety, benefit, or performance in every cancer, hospital, scanner, or subgroup.

The strongest report makes independence visible: patient-level grouping, isolated preprocessing, a locked procedure, clearly labeled internal and external testing, matched multimodal comparisons, and performance reported across discrimination, calibration, utility, uncertainty, sites, and relevant subgroups.
