Enforce an autonomous AI agent as if it were a separate, potentially privileged software principal: give it a unique identity, narrowly scoped and short-lived authority, and access only through tools that a trusted policy service can mediate. Let the model propose an action, but never let its prompt, plan, or self-assessment decide whether that action is authorized.
Automate only bounded, low-risk work. Require explicit approval for external sends, code execution, production changes, permission changes, payments, deletion, deployment, and other high-impact or irreversible operations. Bind each approval to the exact actor, tool, target, normalized parameters, expiry, and execution instance; consume it atomically immediately before execution; fail closed when a check, policy lookup, or audit write fails.

What least privilege means for an AI agent
Least privilege is not simply giving an agent a read-only API key. It means limiting every dimension of authority to what the current task requires:
- Identity: a distinct agent identity and credential set, separate from a human administrator and from other agents.
- Tools: an allowlist of specific functions, not a general shell, unrestricted URL fetcher, wildcard API, or unused plugin.
- Operations: separate read, preview, and commit capabilities. A tool that can draft a change should not automatically be able to apply it.
- Data: resource, tenant, record, environment, and field-level scopes where practical; keep sensitive and administrative data outside ordinary task scopes.
- Time: short-lived credentials and task windows that expire when the job ends or its risk changes.
- Autonomy: a defined limit on retries, recursion, concurrency, transaction size, and unattended execution.
OWASP describes excessive agency as the combination of excessive functionality, excessive permissions, or excessive autonomy. Unexpected, ambiguous, hallucinated, or prompt-injected output becomes dangerous when all three are present. Least privilege reduces the blast radius even when the model is wrong.

Why model instructions are not an authorization boundary
A system prompt can tell an agent not to delete data, but it cannot reliably enforce that rule. Prompt injection, tool-output manipulation, a confused-deputy path, or a model error can cause a forbidden call. Authorization must therefore be implemented in downstream systems, as OWASP recommends for its LLM06:2025 guidance.
Place the decision in a policy decision point, API gateway, tool proxy, or execution service that the model cannot override. That component independently evaluates the authenticated principal, delegated human or organizational principal, requested tool, target resource, normalized parameters, risk class, approval state, and time validity. The model supplies a proposal; trusted code supplies the decision and performs the operation.
Reference architecture
1. Define the principal and trust boundaries
Document who initiated the task and on whose behalf the agent acts. Record the human account, service account, tenant, business process, and environments the task may touch. Mark every boundary where the agent crosses from untrusted model output into a privileged system: retrieval, tool invocation, workflow transition, deployment, payment, messaging, and identity administration.

Pick the symptom - the matching free tool is one click away.
For multi-agent workflows, identify each agent separately. Do not treat a planner, researcher, executor, and reviewer as one indistinguishable identity. A downstream service must be able to tell which component requested an action and which human principal delegated authority.
2. Issue a separate, delegated identity
Create a unique identity for each agent or deployment. Bind its authority to the initiating human or organization, while keeping the agent credential distinct so it can be revoked, rate-limited, and audited independently. Prefer short-lived, scoped credentials over long-lived secrets. Rotate or revoke them when the task ends, a model or tool changes, or the risk classification rises.
Delegation should carry the minimum claims needed by the execution service: agent identity, human principal, tenant or project, allowed resources, allowed operations, issue time, expiry, and policy version. A human token copied into an agent runtime destroys the separation needed for investigation and rapid revocation.
3. Start with deny-by-default scopes
Begin with no tools and no data access. Add one operation at a time to an allowlist. Separate:

- read from write;
- preview or dry-run from commit;
- ordinary records from sensitive or administrative records;
- test, staging, and production environments;
- single-object changes from bulk operations.
If a narrow function can meet the requirement, do not expose a general shell, broad network fetcher, unrestricted database connection, wildcard API scope, or unused extension. Unknown or newly discovered tools must be denied until a policy owner classifies them.
4. Mediate every call outside the model
Route tool requests through a trusted execution component. It should authenticate the agent, resolve the human delegation, normalize the request, classify its risk, check policy and approval, enforce quotas, execute the operation, and write an audit event. Direct model-to-system connections create paths that bypass complete mediation.
Normalization matters. Convert aliases, alternate encodings, reordered parameters, and implicit defaults into one canonical representation before authorization. The approval should cover that canonical action, not merely a natural-language description.
5. Limit blast radius
Apply rate limits, transaction and spend caps, concurrency limits, retry and recursion limits, sandboxing, and network-egress restrictions. Use circuit breakers that stop an agent after repeated failures or an unusual burst of high-risk calls. Prefer idempotent operations so a retry cannot duplicate an external effect. Where idempotency is impossible, require duplicate confirmation or a unique transaction token.

Risk classification should be explicit and machine-enforced. Reversibility is a useful test: an action that can be undone safely is not automatically low risk, but an irreversible action should never be treated as ordinary automation.
| Action class | Typical examples | Default mode | Required controls |
|---|---|---|---|
| Low risk, reversible | Search, read-only retrieval, bounded calculation | Autonomous | Narrow scope, output validation, rate limits, logging |
| Medium risk | Create or edit a draft, write a noncritical file, open a ticket | Preview plus approval by policy | Exact target and parameter binding, audit trail, rollback |
| High risk | Send an external message, execute code, modify production configuration, change an access policy | Explicit human approval | Independent policy check, short-lived approval, step-up authentication where appropriate |
| Critical or irreversible | Delete data, transfer funds, grant privilege, deploy to production | Two-person or designated-approver workflow where organizational policy requires | Fail closed, idempotency or duplicate confirmation, immutable audit evidence, rollback or circuit breaker |
This matrix is a design baseline, not a universal compliance rule. Tune thresholds to resource classification, safety and financial impact, reversibility, and organizational policy. A low-value action against a crown-jewel system may still require approval.

Show the exact action
Present a preview that a person can understand and verify. It should identify the agent and delegating principal, the tool, target resource, normalized parameters, expected effect, risk class, timing, and rollback or irreversibility. “Continue?” is not an adequate confirmation for a destructive or externally visible operation.

Bind approval to what will execute
Store an approval record containing the approving actor, agent identity, human principal, tool name, target, normalized parameters, timestamp, expiry, policy version, and a unique approval identifier. Include the intended execution instance or transaction identifier. If any material field changes, require a new approval. A permission to “send an email” must not authorize a different recipient, attachment, message body, or account.
Use short validity and atomic consumption
Check and consume the approval atomically immediately before execution. Mark it used in the same transaction that reserves the execution, or use an equivalent one-time mechanism. This prevents replay and closes the race in which two workers observe the same unused approval. Expired, already-consumed, malformed, or policy-version-mismatched approvals fail closed.
Require stronger authentication for exceptional impact
Use step-up authentication for privilege changes, account recovery, payments, bulk deletion, and production deployment. Organizations that require separation of duties should route critical actions to two people or to a designated approver group. The approver should not be the same automated component that proposed the action.
Separate proposal from execution
The agent may draft a command, transaction, configuration diff, or message. A trusted service then validates the structure and semantics, applies policy, obtains or verifies approval, and performs the operation. Keep the proposal and the executed result linked by an immutable request identifier.
A minimal decision flow looks like this:
- Authenticate the agent and resolve its delegated human or organizational principal.
- Verify that the requested tool and operation are allowlisted for this identity and task.
- Normalize the target and parameters; reject ambiguous or unsupported values.
- Classify impact and reversibility. Unknown tools and unclassified actions fail closed.
- For approval-gated classes, display the exact preview and collect the required approval or approvals.
- Immediately before execution, re-check policy, scope, expiry, and one-time approval state.
- Reserve and atomically consume the approval, then execute with limits and an idempotency key.
- Record the decision, execution result, retries, errors, and any rollback.
If policy lookup, classification, approval validation, or audit logging is unavailable, do not proceed. A temporary outage must not turn into an implicit allow.

Approval patterns for common operations
Sending an external message
Keep drafting autonomous, but gate the send. Show the sender identity, recipients, subject, body, attachments, and external domains. Recalculate those fields after any edit and invalidate the approval if they change. Enforce recipient and attachment limits and log the final payload hash.
Deleting data
Require an explicit target set, deletion method, retention impact, and recovery path. Prefer a reversible quarantine or soft-delete step. Bulk deletion should have a separate threshold, a transaction cap, and a second approver where policy demands it.

Changing permissions
Display the subject, old and new permissions, affected resources, duration, and downstream consequences. Use step-up authentication and short-lived grants. Never allow the agent to approve its own privilege elevation.
Deploying or changing production
Require a reviewed diff, environment and version identifiers, health checks, rollback procedure, and a maintenance window if applicable. The deployment service must independently verify that the artifact and target match the approved values.
Spending or transferring money
Bind approval to amount, currency, payee, account, purpose, and expiry. Enforce transaction and daily caps outside the model. Use duplicate confirmation or an idempotency key to prevent retries from creating multiple transfers.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Prompt injection and confused-deputy defenses
Treat retrieved documents, web content, email, tickets, and tool output as untrusted data. They may contain instructions aimed at changing the agent’s behavior. The execution service must ignore any instruction that is not represented in the authenticated request and policy context.
- Do not let retrieved text grant tools, widen scopes, alter recipients, or suppress approval.
- Keep credentials and policy decisions out of model-visible text where possible.
- Re-authorize after every handoff between agents or workflow stages.
- Bind the human principal and resource scope across the entire chain, not just at the first prompt.
- Reject requests that attempt to disable logging, bypass a gate, or call an unregistered endpoint.
These controls address the confused-deputy problem: an agent with broad authority must not be tricked into using that authority for someone else’s instruction or target.
Logging, monitoring, and revocation
Capture enough evidence to reconstruct every decision. At minimum, record:

Alert on repeated approval-bypass attempts, unusual privilege use, abnormal tool frequency, sudden increases in high-risk actions, unexpected destinations, and repeated failures. Keep denial, timeout, rollback, and residual-risk records, not only successful calls.
Best Value
Provide an emergency revocation path that can disable an agent identity, delegated token, tool, environment, or policy scope quickly. Reassess privileges after changes to the model, prompt, tools, retrieval sources, memory, workflow, or downstream systems. Remove unused permissions and expire stale credentials.
Testing before production
Test the control plane, not just the agent’s conversational quality. Preserve the agent version, tool policy, model and provider configuration, expected results, and observed approval or denial behavior for each test run.
- Prompt injection: place instructions in documents, webpages, email, and tool output that request secret disclosure or a forbidden action.
- Parameter substitution: obtain approval for one target, then alter the target, recipient, amount, or command before execution.
- Replay: reuse an approval after consumption, expiry, policy change, or task completion.
- Unauthorized calls: request an unallowlisted tool, wildcard resource, production endpoint, or administrative operation.
- Privilege escalation: attempt to grant the agent or its principal broader access.
- Data exfiltration: send sensitive records to an unapproved destination or encode them in an apparently harmless request.
- Failure modes: interrupt policy lookup, approval storage, audit logging, or the execution service and verify that the result is denial, not allow.
- Abuse limits: exceed rate, concurrency, retry, recursion, transaction, and spend caps.

What the major guidance says
OWASP’s Agentic AI AAI9 guidance says agents must operate under the same change-management controls applied to human administrators, with additional automated guardrails appropriate to autonomous operation. It recommends minimum necessary tool access, explicit approval for security-relevant changes, and reversibility-based gating.
OWASP’s LLM06:2025 guidance calls for human-in-the-loop control for high-impact actions and authorization in downstream systems rather than relying on an LLM to decide whether an action is allowed.
NIST’s COSAiS project defines AI-agent systems as capable of autonomous decision-making and action with limited human supervision to achieve complex goals, and explores adapting SP 800-53 controls through overlays for single-agent and multi-agent systems. NIST’s NCCoE notes that the scale and range of actions taken by software and AI agents has the potential to increase exponentially while it explores standards-based identity and authorization approaches. Anthropic’s framework, published August 4, 2025, likewise identifies keeping humans in control as a central design tension for agents that direct their own processes and tool use.
Governance over the agent’s lifetime
Approval gates are not a one-time feature. Assign an owner for each agent, tool, policy, and resource scope. Review access after every material change to prompts, models, retrieval, memory, plugins, workflows, or downstream APIs. Re-run abuse tests and compare observed decisions with the documented policy.
Recommended Free Tools
Retain evidence of approvals, denials, timeouts, rollbacks, revocations, and residual risk for the period required by your organization. When an agent is retired, revoke credentials, disable its tools, close outstanding approvals, and preserve the audit trail needed to explain prior actions.
