The PDF file
Have the source PDF available locally or in a location you can open from the Mac. You can drag the file into PDF Expert or use its file-selection control. If the document is a scan made from paper, expect to run OCR before attempting text edits.
A decision about the intended result

Add, replace, move, rotate, or delete an image
Choose Image from the editing toolbar. PDF Expert documents this tool as supporting image insertion, replacement, movement, rotation, and deletion. Review OCR before trusting it
OCR is a recognition step, not a guarantee that every character was interpreted correctly. Compare the recognized text with the page image before relying on names, figures, dates, addresses, or sensitive information. Tables, unusual fonts, handwriting, low contrast, skewed pages, and damaged originals deserve especially close inspection. When text selection still does not work
If a word cannot be selected, do not keep trying the Text tool against the image. Return to the OCR controls, select the correct language and page range, and apply recognition. Then test search and selection on that page. A document can contain both native-text pages and image-only pages, so inspect each section rather than assuming that OCR on one page covered the entire file.

Recognition can also produce characters that look plausible but are wrong. Compare names, dates, decimal points, account numbers, and legal wording against the page image. If the recognized layer is not reliable enough for a critical correction, use the image as the authority and make a clearly reviewed change rather than silently trusting an unverified transcription.
