To challenge a hospital bill in India, send a dated written complaint to the hospital’s grievance officer, patient-relations desk or management. Identify the exact charges in dispute and ask for a line-by-line explanation and review. If you cannot pay the balance at once, make a separate written request to the billing or finance team for instalments or another arrangement. Keep copies of the bill, estimates, receipts and every complaint and reply.
If the hospital does not resolve the matter, the National Consumer Helpline (NCH) is a pre-litigation grievance route; a Consumer Commission is a formal route for consumer disputes. The applicable patient-rights and clinical-establishment framework varies by State or Union Territory. PM-JAY beneficiaries and people disputing an insurer’s claim decision may have different complaint channels.

How do I dispute a hospital bill in India?
Start by working out whether the problem is with the hospital’s charge, an insurer’s decision, or a public health scheme. Then make the complaint specific and submit it through the hospital’s published grievance process. The National Human Rights Commission’s 2019 Charter of Patients’ Rights describes patients and caregivers as able to lodge complaints with a designated hospital official and seek redressal. The Ministry of Health and Family Welfare has said that adoption, implementation and grievance arrangements for the Charter sit with States and Union Territories; they are not identical nationwide.
1. Collect the records
Gather the final itemised invoice, any admission estimate or package terms, deposit and payment receipts, discharge papers, and prescriptions or test records that relate to disputed entries. Include relevant insurer or third-party administrator (TPA) communications, authorisations, and messages discussing prices. If you need another document to understand a charge, ask the hospital for it in writing. Keep originals and send copies where possible.

2. List the charges you are challenging
Make a simple table for your own use with the bill date or service date, the line item, amount, why it seems wrong or unclear, and the outcome you want. For example, note whether a charge appears twice, relates to a service you believe was not provided, differs from a written estimate, or appears to bill a package item separately. A confusing bill alone does not establish fraud, so describe what you can verify rather than making unsupported accusations.
Ask the hospital to explain each entry and identify the tariff, estimate, package terms or authorisation it used. If you can calculate the amounts accurately, distinguish the undisputed balance from the amount under review. Request a complaint reference and the name or designation of the person handling it.
3. Send a written complaint to the hospital
Use the hospital’s published process and address the complaint to its grievance officer, patient-relations desk, administrator or management, as appropriate. Include the patient and admission identifiers in the private submission, admission and discharge dates, invoice number, amount disputed, a concise explanation and the specific resolution requested. Attach copies of supporting records and ask for a written response by a reasonable date. There is no nationally mandated response deadline established here, so do not assume a particular number of days applies.

Keep a dated copy and proof that the hospital received the complaint. The NHRC Charter describes an internal complaint to a designated hospital official and further redressal through an appropriate government mechanism, but the applicable state-level route depends on local implementation and rules.
Can I ask a hospital to reduce my bill or let me pay in instalments?
Yes. You can ask the hospital to review a charge, reduce or waive an amount, defer a due date, or accept a payment schedule. Treat these as requests to negotiate: the official sources described below do not establish a general nationwide legal entitlement to a discount or instalment plan.
Make a separate written request to the billing or finance team so that a request for more time does not obscure the bill review. Propose a down payment and instalments that your household can realistically afford. Ask the hospital to confirm in writing:

- the total balance it says is payable and any amount still under review;
- each payment amount and due date;
- whether interest, fees or other charges apply;
- what happens if a payment is late or missed; and
- whether the agreement affects any collection or records process.
Do not assume that paying part of the bill resolves the dispute or means the hospital accepted your complaint. If you need to pay while contesting part of the invoice, get local legal advice about how to record the payment and any consequences. Keep receipts and the written agreement.
Where can I complain about a hospital bill?
The right route depends on who made the decision and what outcome you want. A hospital charge, insurer claim decision and PM-JAY grievance are not the same dispute.
| Situation | First route | Possible next route |
|---|---|---|
| A hospital charge, service or billing explanation is disputed | Use the hospital’s designated grievance process and keep proof of submission. | NCH can provide a pre-litigation grievance route. If unresolved, consider the appropriate Consumer Commission. |
| A PM-JAY beneficiary has a specified grievance about irregularities or denial of treatment at an empanelled hospital | Use the Centralized Grievance Redressal Management System (CGRMS) or call 14555. | Follow the scheme’s grievance process for the case. This route is not for every hospital invoice. |
| An insurer has denied, reduced or otherwise decided a health-insurance claim | Complain to the insurer first and keep the complaint and its decision. | Use the policyholder grievance route described by the Insurance Regulatory and Development Authority of India (IRDAI). |
| The dispute may require formal consumer redressal | Consider whether an NCH complaint may help resolve it before litigation. | File with the appropriate District, State or National Consumer Commission, subject to the applicable rules and facts. |
National Consumer Helpline
The Department of Consumer Affairs describes NCH as a pre-litigation grievance mechanism. Its official information lists toll-free 1915 and online and other channels; contact details can change, so check the current NCH information before using a channel. If the response does not resolve the complaint, NCH says a dissatisfied consumer may approach the appropriate Consumer Commission. NCH is not a guarantee of a refund or of a particular outcome.

Consumer Commission and the filing time limit
The Consumer Protection Act, 2019 establishes District, State and National Consumer Commissions. Department of Consumer Affairs guidance describes relief that may include a specific remedy and, where appropriate, compensation. Which Commission and remedy fit a case depends on current rules and the facts; the Department also describes online filing and jurisdiction connected to a consumer’s residence or work.
Section 69 of the Act generally bars admission of a complaint unless it is filed within two years from the date the cause of action arose. A Commission may admit a later complaint if sufficient cause is shown and it records reasons for condoning the delay. Do not let extended hospital negotiations cause you to miss a possible filing deadline; seek timely case-specific advice if the date is approaching.

PM-JAY beneficiary grievances
For the specified grievances of beneficiaries at empanelled hospitals, Government of India information lists CGRMS and the 24×7 helpline 14555. This is a scheme-specific route for issues such as irregularities or denial of treatment, not a general complaints line for any private hospital bill.

Health-insurance claim disputes
If the contested amount results from an insurer’s coverage or claim decision, IRDAI advises policyholders to complain to the insurer first and describes its policyholder grievance route. Keep this complaint and the insurer’s response separate from the hospital billing complaint: the hospital may explain its charges, while the insurer addresses its own claim decision.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Which patient-rights rules apply in your State or Union Territory?
Health is a State subject, and the Ministry of Health and Family Welfare has placed adoption, implementation, display and grievance mechanisms for the Charter of Patients’ Rights with the relevant State or Union Territory. The NHRC Charter therefore does not mean every state has an identical complaint structure. Check the local health department’s current guidance and whether the Clinical Establishments Act applies where the hospital operates. The route and rules may differ by location.
What to keep throughout the dispute
- A dated copy of the complaint and proof of delivery or submission.
- The itemised bill, estimate or package terms, receipts and relevant medical records.
- Written explanations, complaint references and responses from the hospital.
- Separate insurer or scheme correspondence, where applicable.
- Written terms and payment proof for any agreed payment arrangement.
Do not withhold all payment or delay urgent care based only on general guidance. If a payment is necessary while a charge remains disputed, or if the amount or consequences are significant, obtain advice specific to the applicable state rules and your circumstances.
