Design a clinical escape room backward from observable learning outcomes. Decide what health care students should know or do, then build a clinically coherent scenario and puzzles that make them practice those outcomes. Choose an open, linear, hybrid, or virtual structure that fits the dependencies, space, time, and facilitation available; pilot every challenge; brief the learners; and protect time for an objective-focused debrief and outcome evaluation. A game that is entertaining but disconnected from the curriculum is not a reliable teaching activity.

What a clinical escape room is—and what it is for

A clinical escape room is a timed, team-based simulation in which learners find information, solve puzzles, and complete a clinical quest. The escape-room frame supplies urgency and a shared problem; the educational design determines whether learners actually interpret data, prioritize care, communicate, or make decisions that matter to their curriculum. AORN describes the format as a team game involving clues, puzzles, and a time-limited quest, while the AMEE healthcare-curriculum guide treats the activity as a complete teaching session that includes design, briefing, operation, debriefing, and evaluation (AORN, 2023; AMEE Guide No. 168).

Use the room to address a genuine learning need: for example, recognizing a safety risk, interpreting a handoff, prioritizing actions, consulting another profession, or agreeing on a discharge plan. It can be formative practice, orientation, or a summative assessment, but those purposes require different levels of standardization, documentation, and consequence. State the purpose before selecting props or writing clues.

1. Define learners, constraints, and outcomes first

Specify the learner group

Record the students’ program and level, professional roles, prior knowledge, expected group size, available facilitators, and whether the participants will work as a single team or several teams. A puzzle that is appropriate for a final-year interprofessional group may be inaccessible to beginners who have not learned the terminology. Decide whether discipline-specific knowledge is required or whether all essential information will be supplied in the room.

Set the operating envelope

  • Time: Set a playing period and reserve separate time for briefing, reset, debriefing, and evaluation.
  • Space: Identify the room, accessible routes, surfaces, privacy needs, noise limits, and any simulated equipment that can be used safely.
  • Difficulty: Choose the amount of ambiguity, the number of simultaneous tasks, and the expected independence from hints.
  • Purpose: Mark the activity as formative practice, orientation, or summative assessment before designing scoring and feedback.
  • Facilitation: Assign who observes, gives hints, manages time, resets materials, and handles a technical or simulation problem.

Write outcomes as observable performances

Use verbs that can be seen or scored, such as prioritize actions, interpret a handoff, identify a safety risk, justify a consultation, or agree a discharge plan. Avoid an outcome such as understand discharge planning unless you define what students must say, choose, document, or demonstrate. The AMEE guide and AORN design guidance both place audience and goals before puzzle construction (AMEE Guide No. 168; AORN, 2023).

2. Build a clinically coherent scenario

Give the team a reason to act together

Choose a situation that belongs in the learners’ curriculum and provide a concise patient history, current status, and immediate reason for action. The opening information should orient the team without solving the case. A useful scenario makes collaboration necessary: different roles may hold different pieces of information, or the safest plan may require consultation across professions.

Let information unfold without hiding essentials

Clues can reveal laboratory results, medication history, risk factors, family concerns, or the next decision point. The sequence should reward clinical reasoning rather than arbitrary concealment. If a learner needs a fact to make a safe decision, supply it through the case materials or a discoverable clue; do not make the team guess information that a real clinician would be expected to obtain.

Use an interprofessional example carefully

The University of Minnesota’s healthcare escape-room guidebooks describe a simulated patient and family whose discharge plan requires collaboration and consultation. The listing distinguishes beginner and advanced versions; the advanced design requires discipline-specific knowledge (University of Minnesota, Healthcare Escape Room Design Guidebooks). Treat that as a model for aligning the story with roles, not as a ready-made protocol. Adapt the patient, terminology, and decisions to local curricula and policies.

3. Map every objective to a puzzle

Create an objective-to-puzzle map

Before buying locks or printing clues, create one row for each challenge. Record the objective, information supplied, learner task, correct solution, clue or reward revealed, likely wrong turns, and the hint a facilitator will give. This map exposes puzzles that are decorative rather than educational and shows whether an objective is actually observable.

Design field Question to answer
Objective What observable knowledge, decision, communication, or skill is being practiced?
Clinical information What history, measurement, handoff, policy excerpt, or equipment is legitimately available?
Learner task What must the team interpret, prioritize, match, calculate, communicate, or perform?
Correct solution What answer or behavior demonstrates the objective, and how will it be checked?
Reward What new clue, decision, or scenario change becomes available?
Wrong turns and hint What misconception is likely, and what graduated prompt keeps the team learning?

Apply the challenge–solution–reward loop

A good puzzle presents a clear challenge, can be solved from the supplied information and logic, and rewards the team by moving the scenario forward. The medical-education puzzle article recommends that each puzzle fit the story, use the information and equipment provided, and accomplish a learning objective (ESCAPE the Boring Lecture). Do not add a lock, code, or riddle solely to extend the clock.

Match cognitive demand to the objective

Use Bloom’s taxonomy as a design check. Recall or matching can support basic knowledge; interpretation, prioritization, and synthesis demand more complex application. One published example has learners match pathogens to confirmatory tests and use the ordered answers to reveal a patient clue (ESCAPE the Boring Lecture). The code is not the learning outcome: the interpretation that produces the code is.

4. Choose the puzzle architecture

Architecture determines which tasks depend on one another and how many learners can work productively at once. There is no evidence in the cited guidance that one format is universally best; select the pattern that fits your objectives, group, room, time, and moderator capacity.

Structure How it works Best fit Main risk and control
Open Teams may tackle puzzles in any order or concurrently. Several objectives can be practiced in parallel and the room supports movement between stations. Learners may miss a dependency; label each station and provide a visible progress system.
Linear Each solution releases the next clue or stage. A tightly evolving case where decisions must occur in sequence. One bottleneck can stop everyone; prepare a timed hint and a facilitator bypass.
Multilinear or hybrid Parallel branches converge into sequential stages. Mixed objectives and larger groups that need simultaneous work. Teams can finish branches unevenly; define convergence checks and assign roles.
Virtual Clues, communication, and submissions are delivered through an online platform. Learners, rooms, or materials cannot be co-located. Connectivity and interface problems can masquerade as knowledge gaps; provide an alternate submission route.

5. Select materials that serve the learning design

Use simple mechanisms deliberately

Possible mechanisms include cryptograms, locked boxes, riddles, black-light markers, combination locks, and clinical skill tasks. AORN lists these categories, and the puzzle-design article uses a combination lock as an example (AORN, 2023; ESCAPE the Boring Lecture). A physical kit is not required. A printable code, envelope opened by a facilitator, or projected clue can provide the same dependency with less reset time.

Plan an inexpensive fallback

If a lock is pedagogically useful, search generically for a combination lock box or use a facilitator-held alternative. UV marker and black-light tools are another optional category. No specific product, current price, stock status, or affiliate listing is established here, so treat props as replaceable components rather than required purchases. Every physical mechanism should have a non-technical backup in the run sheet.

Check accessibility and safety

  • Offer readable print, adequate contrast, alternative formats, and a way to participate without fine-motor, visual, hearing, or mobility barriers.
  • Keep simulated medications, sharps, devices, and patient information clearly identified and controlled under local procedures.
  • Do not require unsafe handling or real clinical intervention to obtain a clue.
  • Follow institutional simulation, accessibility, privacy, and safety policies; the cited sources do not specify one universal policy for every institution.

6. Prepare the facilitator and session run sheet

Write a run sheet another educator could follow without asking the designer what happens next. Include setup photographs or diagrams, materials by station, correct solutions, reset instructions, timing checkpoints, hint wording, anticipated misconceptions, scoring or observation fields, and a backup for every electronic or locked component. Angelilli emphasizes that systematic planning and testing are necessary because of simulation complexity (AORN, 2023).

Brief learners before the clock starts

  1. State the fictional scenario and the learning purpose in plain language.
  2. Explain the time limit, boundaries of the space, what may be handled, and how to request help.
  3. Clarify whether hints affect scoring and whether all team members must agree before a solution is submitted.
  4. Describe accessibility arrangements, stop conditions, and what to do if equipment fails.
  5. Remind learners that the simulation is not permission to improvise real patient care outside the exercise.

Facilitate without solving the puzzle

Observe team communication, role distribution, clinical reasoning, and safety behavior. Use graduated hints: first restate the objective or available information, then point to a relevant relationship, and only then narrow the search. Record notable decisions for the debrief. If a branch is stalled by a faulty prop or ambiguous instruction, preserve the learning task with the planned bypass and document the defect for revision.

7. Pilot, revise, and retest

Pilot with someone who did not write the activity. Watch whether instructions are understood, clues are accessible, answers have accidental shortcuts, clinical content is accurate, the timing is realistic, and every learner has meaningful work. Test both success and failure paths: a team should be able to recover from a wrong answer without losing the entire session.

  • Clarity: Can a new facilitator explain the task without inventing rules?
  • Solvability: Is the solution derivable from supplied information, logic, and equipment?
  • Alignment: Does the puzzle require the target knowledge or skill rather than trivia or dexterity?
  • Flow: Are there avoidable bottlenecks, dead ends, or idle participants?
  • Validity: Are clinical facts, calculations, labels, and expected actions correct for the intended setting?
  • Reset: Can materials be restored consistently between teams?

Revise and run the pilot again after substantial changes. Keep a version number and change log so facilitators know which answer key and clues belong together.

8. Design the debrief as part of the lesson

The debrief is where game behavior becomes clinical learning. Protect it on the timetable instead of using all available minutes for play. A practical sequence is:

  1. Description: Ask what happened, what the team noticed, and where it became stuck.
  2. Analysis: Examine the data interpretation, prioritization, communication, consultation, and safety decisions that shaped the result.
  3. Application: Revisit each objective and ask how the reasoning would transfer to the learners’ clinical setting.

Discuss the game only as a route to the objectives. The puzzle-design article warns that talking about the mechanics can crowd out the intended learning, so facilitators should explicitly connect decisions and team processes to the clinical content (ESCAPE the Boring Lecture). Ask every role to contribute, including learners who opened few locks but supplied important clinical reasoning.

9. Evaluate what students actually learned

Match measures to outcomes

Use pre- and post-activity knowledge items when the objective is knowledge. Use a standardized checklist or rubric for observed communication, prioritization, technical, or teamwork behaviors. Collect learner feedback about clarity, workload, psychological safety, and usefulness as usability evidence—not as proof of competence. AORN recommends evaluation matched to learning outcomes and cautions that enjoyment alone does not demonstrate knowledge or skill change (AORN, 2023).

Separate formative and summative decisions

For formative use, give immediate feedback and allow a second attempt or a debrief-based correction. For summative use, standardize clues, hint rules, observation criteria, and scoring, and document accommodations. Do not claim durable clinical competence or transfer from a single satisfaction survey. The reviewed guidance offers evaluation methods but no population-wide effect size for clinical escape rooms.

Novice and advanced versions need different assumptions

Create separate pathways when prior knowledge differs instead of merely giving beginners more time. Compare the versions on the dimensions below.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Dimension Beginner design Advanced design
Prior knowledge Essential terminology and reference information supplied. Discipline-specific knowledge may be required, as in the advanced version described by the University of Minnesota.
Clinical ambiguity Fewer competing interpretations and clearer decision criteria. More incomplete or conflicting information, with justification required.
Parallel work Fewer simultaneous branches so facilitators can coach foundational reasoning. More concurrent tasks and role interdependence.
Assessment Emphasis on feedback and correction. More structured observation of prioritization, consultation, and rationale.

Common failure modes and practical fixes

The room tests trivia instead of clinical reasoning

Replace obscure facts with a supplied reference, a realistic handoff, or a decision that requires interpretation. Recheck the objective-to-puzzle map and remove any challenge with no educational purpose.

One lock stops the whole team

Convert the bottleneck to a parallel branch, add a time-triggered hint, or give the facilitator a safe bypass that preserves the decision the learners must make.

Only one learner does the work

Design role-specific information, parallel stations, or a required team handoff. Observe participation and discuss role distribution during debriefing rather than assuming that being in the room means collaborating.

Clues are hidden arbitrarily

Make the search rule explicit and ensure the clinical information needed for a safe choice is discoverable. A surprise should reward observation or reasoning, not guesswork about the author’s habits.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The debrief runs out of time

Shorten or combine low-value puzzles, set a hard stop for play, and prepare three objective-linked prompts in advance. Learning discussion should not be sacrificed to let a team finish a code.

A prop fails

Use the documented backup, record the incident, and assess the clinical task rather than the device. Reset and retest before the next group.

A reusable design checklist

  • Target learners, roles, group size, prior knowledge, purpose, time, space, and facilitator capacity are documented.
  • Each learning outcome uses an observable verb and has an assessment method.
  • The scenario is clinically coherent, relevant, and safe for the intended setting.
  • Every puzzle has an objective, supplied information, solvable logic, answer, reward, wrong-turn plan, and hint.
  • The chosen architecture matches task dependencies and allows meaningful participation.
  • Materials have accessible formats and a non-technical backup.
  • The run sheet covers setup, briefing, timing, observation, reset, and contingencies.
  • An uninvolved pilot has checked clarity, content, accessibility, timing, and accidental shortcuts.
  • The debrief explicitly revisits clinical reasoning, teamwork, and every objective.
  • Evaluation measures knowledge or performance directly and reports limitations honestly.

Bottom line for educators

The strongest clinical escape rooms are compact simulations with a game interface, not collections of locks. Start with the learners and the behavior you need to observe; let the patient story determine the information flow; make every puzzle earn its place; choose a dependency pattern the team and facilitators can manage; and treat briefing, piloting, debriefing, and evaluation as core teaching work. That approach preserves the motivation of an escape room while keeping the activity accountable to health-professions learning.
