How to Debug Missing or Fragmented Traces in Multi-Step AI Agents

Find the exact execution boundary where continuity stops, then determine which of three failures occurred: context was not propagated, the downstream operation was never instrumented, or the span was created but not exported or displayed. Start with one affected request, its trace ID, the last visible span, and the first operation you expected to see. Compare the sender’s injected context with the receiver’s extracted context before changing sampling or backend settings. This method works for agent nodes, tool calls, queues, worker processes and remote graph services.

What a fragmented agent trace actually means

A trace is a connected set of spans. Each span represents an operation such as an agent decision, model call, tool invocation, queue publish, worker task or HTTP request. A downstream span remains part of the same trace only when the upstream execution context reaches the downstream execution unit and is used as the parent context.

OpenTelemetry describes context propagation as the mechanism that correlates signals across execution units. Its documentation summarizes the value this way: “With context propagation, signals (traces, metrics, and logs) can be correlated with each other, regardless of where they are generated.” The important implication for agents is that a trace viewer cannot reconstruct a relationship that the application never transmitted, extracted or recorded.

  • One trace ID, unrelated span IDs: the operations may belong to the same trace, but parent-child links or timing relationships are incomplete.
  • Different trace IDs after a hop: context was probably lost, replaced, or not accepted by the receiving process.
  • No span for an operation: the library, framework path or custom code may not be instrumented.
  • Spans exist locally but not in the central view: exporter, collector, sampling, credentials, routing or project selection may be wrong.
  • Metadata or content is absent while the span exists: recording controls or semantic-convention settings may intentionally suppress those fields.

Use the boundary model before changing configuration

Every cross-process operation has a producer and a consumer. The producer creates or continues a span, injects the current context into a carrier, and sends that carrier with the request or message. The consumer extracts the carrier before starting downstream work and creates a span with the extracted context as its parent.

Boundary point What should happen Evidence to capture
Producer before send The active span and trace context are available; the propagator writes context into the outgoing carrier. Trace ID, span ID, carrier keys and destination.
Transport or intermediary Headers or message properties survive proxies, queues, wrappers and adapters. Raw outbound and inbound carrier values, with secrets redacted.
Consumer before work The receiver extracts context before creating the first downstream span. Extracted trace ID, parent span ID and sampling flags.
Consumer after work Child spans are ended and handed to the exporter. Created span IDs, status, end time and exporter result.
Collector or backend Data is accepted, routed to the intended project and retained under the expected service identity. Exporter response, collector logs, destination and query filters.

For HTTP, OpenTelemetry’s default propagator uses W3C Trace Context. Inspect the traceparent header at both sides of every hop. A present header is not enough: the receiver must extract it and use that context when it starts the downstream span.

A fast triage checklist for one affected run

  1. Choose one request. Record its exact time window, entry point, environment and user-visible symptom.
  2. Copy the trace ID and last visible span ID. Keep the values together; a trace ID finds the overall run while a span ID identifies the boundary operation.
  3. Name the first expected-but-absent operation. For example, the tool service call appears, but the worker’s model span does not.
  4. List every execution boundary. Include threads, asynchronous tasks, queues, containers, remote graph servers, tool adapters and proxies.
  5. Search logs and trace storage by both IDs. A missing child may have been exported to another destination or service name rather than discarded.
  6. Compare the final parent span with the first downstream activity. This is the smallest interval in which continuity can have failed.

Do not assume that a backend dropped a child merely because the parent view ends. First establish whether the operation ran, whether it was instrumented, and where its telemetry was sent.

Step 1: Inspect context at both sides of every hop

HTTP requests

Capture the outgoing carrier immediately before the HTTP client sends it and the incoming carrier immediately after the server receives it. Confirm that traceparent is present, unchanged, and not overwritten by a wrapper. If your deployment uses additional W3C context fields, verify that intermediaries preserve those too.

On the receiving side, extraction must occur before the server creates the first span for the request or tool operation. If a framework starts a span automatically, determine whether extraction happens inside that middleware. If you create spans manually, pass the extracted context as the parent rather than starting from the process’s current default context.

Messaging and queues

Queues do not automatically preserve in-process context. The publisher must inject context into message headers or properties, and the consumer must extract it when a message is handled. Verify that serialization, dead-letter handling, retries and batching do not remove or replace those properties. Decide whether a retry should be a child of the original publish span or a new attempt linked to it, and apply that policy consistently.

Manual propagation when automatic instrumentation is absent

OpenTelemetry provides a Propagators API for explicit injection and extraction. The exact method names depend on the SDK and language, but the sequence is stable:

Step 2: Separate propagation from instrumentation

Propagation answers, “Does the downstream execution know which trace and parent it belongs to?” Instrumentation answers, “Does the operation create a span at all?” They are independent failure surfaces.

Observed symptom Most likely surface What to verify
Downstream spans have a new trace ID Propagation Injection, transport preservation, extraction order and parent selection.
No span is created for a known operation Instrumentation Library support, enabled integration, code path and manual span coverage.
Span exists but prompt, tool arguments or response fields are absent Recording controls Content capture settings, privacy filters and GenAI semantic-convention opt-in.
Span appears in local logs but not in the viewer Export or backend Exporter endpoint, credentials, collector routing, sampling and project selection.
Trace is visible under an unexpected project or service Resource or metadata configuration Connection string, resource attributes, environment variables and backend filters.

A correctly propagated context cannot create spans for an uninstrumented database call, tool wrapper or model client. Conversely, an instrumented span can still be unrelated to its caller if context extraction failed.

Step 3: Check asynchronous tasks and remote agent execution

Creating a task, handing work to a worker pool or calling a remote agent often leaves the caller’s active context. The framework or transport must explicitly carry context into that new execution unit. Inspect task-submission code, worker middleware and any serialization boundary instead of assuming that thread-local or process-local state follows the work.

RemoteGraph and client/server runs

LangChain documents a LangGraph Server RemoteGraph pattern in which a client-side run and a server-side run can appear as separate traces. Its support guidance recommends enabling distributed tracing and configuring the server to accept propagated trace context. Because settings vary by framework and deployed version, verify the precise client and server configuration in your environment rather than copying a setting from another release.

When diagnosing this pattern, capture the client’s outgoing headers, the server’s received headers, the server’s extracted parent, and the first server span. If the server creates a fresh root trace, the break is at acceptance or extraction. If the server continues the trace but expected node spans are absent, investigate node instrumentation separately.

Tool services and worker processes

For a tool call that crosses a service boundary, record the span around the caller’s request, the transport carrier, and the tool service’s entry span. A tool adapter may create a span for dispatch while the actual tool implementation remains uninstrumented. That produces a plausible-looking trace with a missing interior operation, not necessarily a propagation failure.

Step 4: Investigate “Missing Project or Metadata” symptoms

If a run is absent from the expected project, first verify the telemetry destination rather than changing agent code. In Microsoft Foundry setups, the documented checks include the Application Insights connection string. Confirm that the deployed process actually receives the intended connection string and that no environment-specific value overrides it.

  • Check exporter startup logs for authentication or endpoint errors.
  • Confirm collector routing rules and destination project names.
  • Check whether sampling excludes the affected operation.
  • Verify resource attributes such as service name, deployment environment and instance identity.
  • Search across projects or workspaces using the trace ID before concluding that data is missing.

Metadata can also be intentionally absent. Disabled content recording removes prompts, responses or tool payloads even when the span is healthy. Missing GenAI semantic-convention opt-in can leave AI-specific attributes unset. An operation without instrumentation will not acquire those attributes merely because a surrounding agent span exists.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 5: Test exporter, collector and sampling behavior

Once you have confirmed that spans are created and correctly parented in process, follow them through export. Add temporary diagnostic logging that reports span creation, export success or failure, and the destination chosen by configuration; never log secrets or full model content just to prove that export works.

  1. End a known test span at the boundary where the real trace disappears.
  2. Observe whether the SDK queues it for export or reports a rejection.
  3. Inspect collector logs for authentication, schema, payload-size or routing errors.
  4. Check backend filters, retention windows and clock ranges used by the trace query.
  5. Compare a sampled-in request with one that is sampled out, if your policy makes that distinction.

Sampling can make a complete execution look incomplete. Parent-based policies may retain or drop descendants according to the root decision; tail-based policies may make a trace visible only after collection. Use a deliberately retained diagnostic request, within your organization’s privacy policy, to distinguish sampling from propagation.

Step 6: Build a minimal reproducible path

Reduce the failing workflow to one request and the smallest number of nodes that still reproduces the break. Compare a direct local call with the asynchronous or remote path. At each boundary capture four artifacts: outgoing carrier, extracted context, created span IDs and exporter result.

This experiment tells you which class of failure you have:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Carrier differs before and after transport: a proxy, queue, wrapper or serializer removed or changed context.
  • Carrier arrives intact but extracted context is empty: propagator mismatch, extraction order or unsupported carrier format.
  • Context is extracted correctly but no child span appears: missing instrumentation or a code path that never ran.
  • Child span is created and exported but not searchable: collector, destination, sampling, retention or query issue.

Keep the reproducer free of real credentials and sensitive prompts. If the failure occurs only under concurrency, retain the same boundary captures while varying worker count; do not infer causality from timing alone.

Correlate traces with logs and agent events

Include the current trace ID and span ID in structured logs emitted by each agent node, tool adapter and worker. Log an operation’s logical name, attempt number, queue message ID or remote run ID as separate fields. Trace IDs answer which execution a record belongs to; span IDs identify the exact operation; a domain-level run ID can connect retries or business-level sessions that intentionally use separate traces.

When a trace is fragmented, search logs from both sides of the suspected boundary. A receiver log containing the caller’s trace ID proves that the carrier arrived, while a new trace ID in the same log indicates that the receiver started a new root. A log with no trace fields may indicate that logging instrumentation, not tracing propagation, is missing.

Protect propagated context and baggage

Trace headers arriving from outside your trust boundary are untrusted input. OpenTelemetry warns that forged headers can manipulate telemetry or target parser vulnerabilities. Validate header syntax through the SDK, enforce normal request-size limits, and avoid custom parsing that accepts ambiguous formats.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Baggage can carry application-defined key-value data across services. Do not put credentials, personal information, prompts, proprietary identifiers or other sensitive values in baggage. Avoid sending internal topology or deployment details to untrusted services. Apply an allowlist for keys that may leave your environment, and strip baggage at trust boundaries when no downstream use is justified.

Choose an observability approach that fits the failure

Instrumentation and propagation should be evaluated separately from the trace viewer. OpenTelemetry can provide a common propagation and export layer, while a vendor interface may add agent-specific views. Compare options against the boundaries your system actually uses.

Decision axis Questions to ask
Process and remote-worker propagation Does context survive HTTP, messaging, task queues, remote graphs and retries?
Language and framework coverage Are your model clients, tool libraries, orchestration framework and worker runtime instrumented?
OpenTelemetry and collector routing Can spans use your existing propagators, collectors, sampling and storage controls?
Span and content controls Can you record or suppress prompts, tool arguments and responses independently of structural spans?
Deployment and retention Where are telemetry data and sensitive content stored, for how long, and under whose controls?
Debugging views Can the interface show parent-child links, service boundaries, retries and multi-turn agent threads?

LangChain describes LangSmith as offering native tracing for common agent frameworks and OpenTelemetry support. Treat that as a product description, and verify current framework compatibility, data-retention terms and deployment model for your stack before adopting it.

Common traps and precise fixes

“The header is present, so propagation works.”

A header can arrive but never be extracted, or be extracted after the server’s root span was already created. Instrument the receiver’s entry middleware and verify the parent of its first span.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“The parent span ended, so the child was lost.”

Parent completion does not prove child loss. The child may be uninstrumented, exported elsewhere, filtered by sampling or delayed in a collector. Search by trace ID across destinations and inspect exporter status.

“Adding more logging will reveal the missing span.”

Logs cannot create telemetry for an operation that has no instrumentation. Add a deliberate span around the operation, then use logs to correlate its IDs and outcome.

“Record all prompts and tool payloads while debugging.”

Content capture can expose secrets and personal data. Prefer structural spans and redacted metadata first; enable narrowly scoped content recording only under an approved policy.

“A remote call is always a new trace.”

Remote execution may be a child of the caller when context is accepted and propagated. A separate trace is evidence to investigate, not an expected requirement.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical incident worksheet

For each broken run, fill in one row per boundary:

Field Record
Boundary name For example, planner to tool gateway.
Producer trace/span IDs and timestamp when the carrier was created.
Carrier Transport type and redacted propagation keys.
Consumer extraction Extracted trace ID, parent span ID and sampling decision.
First consumer span Span ID, operation name and parent relationship.
Export result SDK and collector outcome, destination and project.
Expected evidence Node, tool, model or worker operation that should appear next.
Disposition Propagation, instrumentation, recording, export, sampling or query issue.

Keeping this worksheet for one complete request prevents a broad configuration change from obscuring the original failure. It also gives platform and application teams a shared vocabulary: producer, carrier, consumer, span and exporter.

When the trace is still incomplete after these checks

Escalate with the smallest reproducible trace, the exact SDK and framework versions deployed, sanitized boundary captures, exporter and collector errors, and the project or workspace queried. Include whether the missing data is structural (no span), relational (wrong parent or trace ID), descriptive (missing attributes or content) or visual (backend query or retention). That classification directs the next owner to the relevant layer without treating every gap as a propagation bug.
