How to Create an Incident Response Plan for Critical Infrastructure Cyberattacks

By Freedom251 Team · · 13 min read

A critical-infrastructure incident response plan must do more than tell an IT team how to remove malware. It must protect people, preserve safe operation of essential services, coordinate IT and operational-technology (OT) decisions, meet applicable reporting duties, and restore dependable service. Build it around essential functions and their dependencies, assign authority before an attack, keep an offline copy, and exercise the complete path from detection through restoration.

What an incident response plan must accomplish

Critical infrastructure includes services whose failure can affect health, safety, security, the economy, or the environment. A cyber incident may involve corporate IT, industrial control systems, engineering workstations, connected equipment, cloud services, suppliers, or the facilities that provide power and cooling. The plan therefore has two simultaneous objectives:

As an Amazon Associate I earn from qualifying purchases.

  • Contain and investigate the compromise without creating an unsafe physical condition.
  • Keep the essential service operating, or move it to a controlled degraded mode, until normal capability is restored.

The plan is a management system as much as a technical document. It identifies who can authorize a shutdown, what evidence must be preserved, which regulator or emergency authority must be notified, and how operators communicate when normal email, phones, or networks are unavailable.

Use the current NIST incident-response model

NIST finalized Special Publication 800-61 Revision 3 in April 2025. It supersedes Revision 2 and aligns incident response with the Cybersecurity Framework (CSF) 2.0. The current model does not attempt to put every technical procedure into one static document. Instead, it treats response as part of continuing cyber-risk management.

The NIST incident-response project overview separates functions this way:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Function How it supports an infrastructure operator
Govern Sets accountability, policy, risk tolerance, legal expectations, and decision authority.
Identify Maps essential functions, assets, dependencies, suppliers, and consequences of disruption.
Protect Provides safeguards, resilience, training, backups, and preparedness that reduce incident impact.
Detect Finds and analyzes signals that may indicate compromise or unsafe activity.
Respond Coordinates triage, communication, containment, evidence handling, and reporting.
Recover Restores trustworthy operation and validates that essential functions are safe and reliable.
Improvement Uses incidents, exercises, and new threat information to improve every other function.

Your plan should show how preparation under Govern, Identify, and Protect enables the operational Detect, Respond, and Recover activities. It should also assign an owner to feed lessons learned back into risk assessments, safeguards, training, and future playbooks.

Step 1: Define essential functions and their dependencies

Start with the service the organization is responsible for delivering, not with a list of security tools. For each essential function, document what happens if it is degraded, interrupted, manipulated, or operated unsafely. Record safety, environmental, public-health, financial, contractual, and regulatory consequences.

Build an essential-function map

For every function, map the systems and conditions required to deliver it:

  • OT networks, industrial control systems, safety systems, sensors, actuators, programmable controllers, and human-machine interfaces.
  • Corporate IT, identity services, remote access, data stores, communications, and cloud services that operators rely on.
  • Important datasets, configuration files, engineering drawings, operating procedures, logs, and backup copies.
  • Facilities and dependencies such as electricity, fuel, water, cooling, telecommunications, physical access, and environmental controls.
  • Suppliers, managed-service providers, equipment manufacturers, contractors, and software or cloud dependencies.

Use the organization’s risk assessment to rank functions and dependencies by consequence. The highest priority is not necessarily the asset with the most vulnerabilities; it is the function whose loss or unsafe behavior would cause the greatest harm. The NCSC’s CAF Objective D, Principle D1 provides a risk-based reference for response and recovery planning.

Record operating limits and degraded modes

For each function, state the safe operating range, the minimum acceptable service level, the maximum tolerable interruption, and the conditions that require a controlled shutdown or emergency response. Identify manual procedures and the people qualified to use them. Do not assume that an action that is safe on an office network is safe for a physical process.

Step 2: Assign authority, roles, and contact routes

Put people-and-process information near the front of the plan so a stressed coordinator can use it immediately. Name a central incident coordinator and a backup, then define decision rights rather than merely listing job titles.

Role Typical responsibility Decision or information needed
Incident coordinator Declares the incident, runs the response rhythm, assigns tasks, and maintains the authoritative record. Escalation threshold, call schedule, status format, and authority to request resources.
IT security and infrastructure Performs detection, triage, identity and network actions, malware analysis, and IT recovery. Approved containment options, logging sources, and change-control boundaries.
OT operations and process engineering Assesses process consequences, operating limits, control-system behavior, and restoration safety. Process-specific isolation, manual-operation, and restart criteria.
Safety and environmental leads Evaluates hazards to people, the environment, and the public. Stop-work, evacuation, emergency-management, and safe-state triggers.
Executive decision-maker Authorizes service degradation, major expenditure, public statements, or shutdowns. Impact, options, uncertainty, legal advice, and estimated recovery time.
Legal, privacy, and compliance Determines notification, privilege, contractual, privacy, and evidence requirements. Applicable jurisdiction, sector rules, deadlines, and required content.
Communications and public affairs Coordinates staff, customer, media, regulator, and stakeholder messages. Approved facts, spokesperson, update cadence, and rumor-control process.
Suppliers and external responders Provides specialist expertise, equipment, threat intelligence, or emergency support. Activation terms, access rules, confidentiality, and 24-hour contact route.

For every key role, list a primary and alternate person or group. Provide at least two independent contact methods and details for two or more people or groups, as recommended by NCSC. Include an after-hours route and a method that does not depend on the organization’s normal identity system.

Define escalation and decision authority

State who may:

  • Declare an incident and move from local handling to enterprise coordination.
  • Disconnect a device, segment a network, disable remote access, or block a supplier connection.
  • Authorize a controlled shutdown, degraded service, manual operation, or restart.
  • Approve external notifications, public statements, law-enforcement contact, and specialist support.
  • Accept residual risk when evidence is incomplete or recovery cannot be immediate.

Document alternates for every authority. A plan that depends on one unavailable executive or engineer is not executable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 3: Make the first hour executable

Create a one-page flow that a coordinator can follow from an alert to review. Keep detailed procedures in linked sections, but make the first actions visible without searching.

  1. Receive and record the alert. Capture time, reporter, affected location, systems, observed symptoms, and immediate safety concerns. Do not alter evidence unnecessarily.
  2. Check for immediate danger. Ask operations and safety personnel whether people, the environment, physical equipment, or essential service are at risk. Use emergency procedures first if they are.
  3. Triage and classify. Determine whether the event is a suspected cyber incident, a process fault, or both. Estimate scope, confidence, essential-function impact, and potential spread.
  4. Declare and escalate. Activate the incident coordinator and the required technical, OT, safety, executive, legal, and communications roles when thresholds are met.
  5. Open the incident record and call. Start a time-stamped log for decisions, actions, findings, communications, evidence, and outstanding questions. Use the prearranged incident bridge or conference method.
  6. Preserve evidence. Record volatile information and relevant logs according to the organization’s evidence procedure. Avoid rebooting, wiping, or reconfiguring systems unless safety or containment requires it.
  7. Choose an initial containment option. Compare safety, service continuity, spread, evidence, recovery time, and reporting consequences with the people who understand the affected process.
  8. Set the next decision time. Assign owners, define what information is needed, and schedule the next update rather than allowing the response to become an unstructured conversation.

Keep the plan available during a system outage

Store a current, access-controlled copy outside the systems most likely to be compromised. Maintain printed or otherwise offline copies of the contact page, first-hour checklist, essential-function map, decision authorities, reporting matrix, and communications alternatives. Identify how to reach the response team if corporate email, single sign-on, internal phones, or internet access fail.

Use a consistent incident record

The record should contain:

  • Incident identifier, classification, start time, current status, and affected functions.
  • People involved, decisions made, approvals, assumptions, and alternatives rejected.
  • Actions with owner, time, expected result, actual result, and rollback or follow-up.
  • Evidence collected, its source, time, handler, storage location, and integrity controls.
  • Internal, supplier, regulator, emergency, law-enforcement, and public communications.
  • Open risks, dependencies, next decision point, and recovery acceptance criteria.

Step 4: Set decision gates for containment, eradication, and recovery

Write decision gates before an incident so the team is not inventing authority under pressure. Each gate should consider six axes:

  4. Recovery time and ability to operate in a degraded mode.
  5. Reporting, contractual, privacy, and legal obligations.
  6. Authority, skills, equipment, and external support available to execute the action.

Containment

List approved options for the environments you operate: disabling a compromised account, restricting remote access, isolating a corporate segment, blocking a command-and-control route, separating a supplier connection, or moving a service to a known-good standby. For each option, document prerequisites, process consequences, approval authority, evidence to capture first, and how to reverse the action.

In OT, never make a universal assumption that network isolation, controller shutdown, or firmware replacement is safe. OT operations, engineering, safety, and security must evaluate the specific process, timing, interlocks, fail-safe behavior, and manual alternatives together. NIST’s OT guidance supports this tailored approach but does not provide one isolation rule for every facility.

Eradication

Define when the team has enough confidence to remove malicious persistence, rebuild systems, rotate credentials, revoke certificates, or replace equipment. Require a documented understanding of the entry path and affected scope where feasible. If the root cause is uncertain, state what compensating controls are required before reconnecting a system.

Recovery

Specify who verifies that restored systems are authentic, patched or otherwise protected, correctly configured, synchronized, and safe for the process. Require staged reconnection, monitoring, functional tests, and an explicit acceptance decision from the appropriate operations and safety authority. Recovery is complete only when the essential function—not merely the server—is operating at the required level.

Step 5: Create scenario playbooks

A core plan explains governance and common workflow. Playbooks provide detailed instructions for likely or high-consequence scenarios. NCSC examples include malware infection, phishing, and data breach; an infrastructure operator should add scenarios drawn from its OT and essential-service risk assessment.

Playbook section Questions it must answer
Activation What symptoms trigger the playbook, who declares it, and what immediate safety checks are required?
Contacts Which IT, OT, engineering, safety, executive, supplier, legal, and external responders join?
Triage What is the first evidence to collect, how is scope estimated, and which essential functions may be affected?
Containment options What can be isolated or disabled, what process risks result, and who approves each option?
Communications What staff, customers, suppliers, regulators, emergency services, or law-enforcement messages may be needed?
Continuity What manual procedure, alternate site, mobile equipment, failover, or degraded service is feasible?
Recovery validation What tests demonstrate that systems and the physical process are trustworthy and safe?
Closure Who accepts residual risk, preserves the record, and assigns post-incident actions?

Basic instructions should cover at least the first several hours. Keep them short enough to use during a crisis and link to technical runbooks, evidence procedures, safety instructions, and restoration checklists rather than duplicating every command in the playbook.

Step 6: Build a jurisdiction-specific reporting matrix

Reporting duties vary by country, sector, incident type, contract, and the organization’s designation. Do not insert a universal deadline into a generic plan. Instead, create a matrix that your legal and compliance teams maintain for each operating jurisdiction.

Matrix field What to specify
Threshold What impact, data exposure, safety issue, service disruption, or attempted compromise requires notification?
Authority Which sector regulator, national cyber authority or CERT, law-enforcement body, emergency-management authority, insurer, customer, or contract counterparty receives the report?
Timing Applicable initial, update, and final-report deadlines, including conditions that change the clock.
Content Required facts, affected services, known or suspected cause, actions taken, indicators, contact person, and uncertainty statement.
Owner The role that prepares, approves, sends, and tracks the notification, with an alternate.
Evidence Records supporting the notification and restrictions on sharing sensitive operational or personal information.

Map each row to the operator’s own regulator and legal counsel. NCSC guidance discusses thresholds, report detail, authorities, and voluntary reporting in the UK context; those recommendations should not be treated as universal duties. NIST’s Incident Response: Preparation Resources points to specialized material, including resources for water and wastewater operators and emergency managers. The Federal Government Cybersecurity Incident and Vulnerability Response Playbooks can help structure terminology and workflow, but each operator must confirm which rules apply.

Step 7: Connect cyber response to continuity and restoration

Cross-reference the incident plan with business-continuity, disaster-recovery, emergency-management, safety, and communications plans. A cyber response that contains an attacker but leaves an essential service without fuel, cooling, staff, or validated backups is incomplete.

Define continuity triggers

State when to activate continuity arrangements, who can authorize degraded service, and what conditions require a controlled shutdown. For each essential function, record:

  • Recovery time objective and the maximum tolerable interruption established by the business and safety owners.
  • Recovery point objective or an explicit decision that a particular dataset cannot be recreated from backups.
  • Backup mechanism, location, retention, restoration owner, and offline or immutable protections where appropriate.
  • Manual workarounds, mobile equipment, mobile sites, alternate facilities, failover sites, or mutual-aid arrangements that are actually feasible.
  • People, tools, spares, access permissions, and specialist skills required to execute the workaround.
  • Criteria for returning from degraded operation to normal service.

Validate backups and alternatives

Document how to test that backups are complete, unaltered, usable with available hardware and software, and free of the compromise. Include configuration, engineering, and process data—not only business documents. Identify an alternate source of critical parts, specialist labor, communications, and power or cooling support if the primary supply chain is affected.

Step 8: Prepare an OT-aware forensics and response capability

OT environments have long equipment lifecycles, proprietary protocols, strict availability requirements, and physical consequences that ordinary enterprise procedures may overlook. Prepare an OT response team that includes operators, control engineers, safety personnel, security specialists, and people familiar with the site’s process history.

NIST’s Digital Forensics and Incident Response (DFIR) Framework for Operational Technology (OT) (NISTIR 8428) was published on June 22, 2022, by Eran Salfati and Michael Pease. It extends traditional response with event escalation, OT-team preparation, and OT digital-forensics techniques. Treat it as supporting guidance to adapt to the facility’s safety and process constraints, not as a procedure to copy unchanged.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before an incident, identify safe ways to collect controller, historian, engineering-station, network, and physical-process evidence; determine which collection actions can affect timing or availability; and obtain approval from operations and safety authorities. Document vendor access rules, time synchronization limitations, logging gaps, and the chain of custody for evidence that may be used in legal or regulatory proceedings.

Choose a response-capability model deliberately

There is no universally superior staffing model. Compare the options against the organization’s actual risk and operating constraints.

Model Strengths Questions to resolve
Primarily internal team Deep knowledge of the site, process, architecture, and business priorities. Is there enough 24-hour coverage, specialist expertise, tooling, and independence for a major incident?
Internal lead with external augmentation Retains site knowledge while adding surge capacity, specialist forensics, threat intelligence, or legal support. Are activation terms, access permissions, confidentiality, evidence handling, and response-time expectations already agreed?
External-led response with internal operations Provides rapid specialist capacity when internal capability is limited. Can the external team understand the process quickly, obtain safe access, and coordinate with the authority that controls shutdown and restart decisions?

NCSC expects arrangements to obtain external specialist help when necessary. Evaluate any potential support by OT experience, sector familiarity, site and process knowledge, availability during the incident, evidence-handling capability, and ability to work within safety and regulatory constraints. The plan should state exactly how external responders are activated; it should not rely on finding a provider after compromise.

Exercise the plan through restoration of essential functions

Exercises should be risk-informed and realistic. Use the organization’s risk assessment, relevant incidents affecting the organization or peers, and current threat information. Test more than the initial alert:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Inject a plausible cyber event and verify that the report reaches the right on-call route.
  2. Test declaration, role activation, alternate contacts, offline plan access, and the incident bridge.
  3. Introduce loss of normal email, identity, phones, or remote access to test communications alternatives.
  4. Force a decision about containment that could affect an OT process or essential service.
  5. Require evidence preservation, internal updates, regulator or emergency coordination, and executive approval.
  6. Activate continuity arrangements and operate in a degraded mode.
  7. Restore systems, validate the process and service, and obtain an explicit return-to-normal decision.
  8. Record findings, assign corrective-action owners and due dates, and track closure.

NCSC’s Cyber Assessment Framework Principle D1 was published April 18, 2024, reviewed August 6, 2025, and is version 4.0. It expects routine exercising and use of findings, but it does not establish one universally correct exercise interval. Set the interval according to risk, change, regulatory expectations, and the results of previous exercises. Review contacts, dependencies, reporting criteria, and procedures after every material change and every real incident.

Maintain the plan as an operational system

Assign an owner for the plan and a separate approval authority. Keep a change record showing what changed, why, who approved it, and which exercise or incident prompted the change. At each review, verify:

  • Essential functions, system dependencies, suppliers, facilities, and contact details are current.
  • Primary and alternate decision-makers can be reached through at least two methods.
  • Offline copies, printed quick references, call bridges, and alternate communications work.
  • Containment and shutdown authorities match current safety, engineering, and change-control rules.
  • Backups, manual procedures, failover sites, mobile resources, and restoration tests remain feasible.
  • Reporting thresholds, authorities, deadlines, contracts, and notification templates reflect the operator’s jurisdictions.
  • OT evidence procedures account for current devices, protocols, logging, time sources, and vendor arrangements.
  • Every exercise and incident corrective action has an owner and a tracked completion status.

A plan is ready when an unfamiliar coordinator can find the right people, make a safe first decision, preserve evidence, maintain essential service, notify the correct authorities, and lead recovery without depending on a compromised system or one unavailable expert.
