In Diablo 4’s 3.1 patch notes, the Horadric Cube recipe crafts a Mythic item from a Unique of the same gear slot. It costs four Pandemonium Fragments, and a Mythic Unique cannot be used as the input. Blizzard changed the recipe label from “Upgrade” to “Craft,” so older guides may use the former name.

Blizzard’s Diablo IV 3.1 patch notes describe a recipe that turns a qualifying Unique into a Mythic for the same gear slot. For example, the result stays in the input item’s slot; it does not let you choose a different slot.

The notes report that the Pandemonium Fragment cost was reduced from five to four in the 3.1.1 update. The 3.1.3 notes also rename the recipe from “Upgrade” to “Craft.” These are the patch-note details available for the 3.1 changes; check the live game for any later revisions.

Where Pandemonium Fragments come from
Blizzard’s 3.1 PTR preview lists the Seasonal Reputation board, Resplendent Caches, and the seasonal Lair Boss Corrupted Reaper as fragment sources. Those routes are described in a seasonal context, so their availability should not be assumed for every current season. Check in-game activity and reward descriptions for the active season.
How to use the recipe
The cited Blizzard patch notes establish the recipe’s cost, input restriction, output slot, and label, but do not document the exact Cube menu sequence or unlock requirements. Use the Cube’s current in-game instructions to locate and perform the recipe; the available official notes do not verify a particular quest path, menu sequence, item-level requirement, or realm restriction.

What the recipe does not guarantee
Blizzard’s 3.1 panel recap refers to a future-season feature for upgrading a Unique to Mythic with all affixes intact. That forward-looking statement does not establish a universal guarantee for the current recipe. Confirm any affix behavior in the current patch notes or the Cube’s in-game description before crafting.
Likewise, a one-Mythic equip limit appears in season-specific preview material and may not describe the active season. The sources cited here do not verify the current season’s limit or all current crafting constraints.
