A badge, a corporate IP address, or a connection from an internal VLAN must not by itself grant access to a server, hypervisor, storage system, management interface, application, or network device.
This guide presents a vendor-neutral sequence based on NIST guidance. Your architecture, risk tolerance, data sensitivity, availability requirements, and applicable obligations determine the exact technologies, policies, review schedules, and retention periods.

Start with the zero-trust access decision
NIST defines zero trust by rejecting implicit trust based solely on physical or network location. Its SP 800-207, Zero Trust Architecture (2020) states: “Zero trust assumes there is no implicit trust granted to assets or user accounts based solely on their physical or network location (i.e., local area networks versus the internet) or based on asset ownership (enterprise or personally owned).”
In practical terms, access is granted only after a policy decision considers the subject, device, requested resource, task, and current conditions. The decision should be made before establishing a session and should be enforceable for both remote and on-site users.

| Access plane | What it controls | Typical decision evidence |
|---|---|---|
| Facility | Building, room, cage, rack, console area | Person’s authorization, visitor status, time, escort requirement |
| Logical identity | Accounts, roles, privileged functions | Identity, MFA evidence, employment or contractor status, task |
| Device and session | Whether a device may initiate or continue a session | Device identity, management state, security posture, session context |
| Network path | Which systems may communicate | Source, destination, protocol, port, segment, policy rule |
| Resource authorization | Server, hypervisor, storage, application, or network-management action | Role, least privilege, change approval, sensitivity, operation |
Physical entry and logical authorization complement each other; neither substitutes for the other. Someone allowed into a server room still needs authorization to log in, and a valid account does not automatically authorize entry to the facility.
1. Define scope and access decisions
Inventory every resource and access point
List the facility areas and technology layers that require protection:
- Buildings, loading areas, secure rooms, cages, racks, and console stations.
- Servers, operating-system administration interfaces, hypervisors, virtual machines, containers, and storage.
- Network devices, out-of-band management, controllers, firewalls, and monitoring systems.
- Applications, databases, automation systems, backup platforms, and administrative control planes.
- Remote-access gateways, cloud consoles, inter-data-center links, and service-to-service APIs.
For each item, write a decision record with five fields: who or what requests access, the exact resource, the permitted task, the evidence checked, and the conditions that cause denial or re-evaluation. Treat human administrators, service accounts, contractors, automation jobs, and emergency identities as different subjects.
Classify the consequence of access
Identify actions that can expose data, interrupt availability, alter security controls, or create persistence. A read-only diagnostic role should not inherit configuration or deletion rights. A network-operations role may need interface status and routing visibility without permission to change security policy. This classification drives MFA strength, approval requirements, session recording, and review depth.
2. Establish identity governance and privileged access
Use a lifecycle, not a static account list
Identity governance covers role management, segregation of duties, logging, access reviews, auditing, analytics, and reporting. Create accounts through an approved joiner process, change roles when duties change, and disable or remove access promptly when employment, contract, or assignment ends. Prefer individually attributable accounts for administration; use service identities only when an application needs them and give each one a defined owner, scope, and rotation process.

Design roles around tasks and resources
Define access by job or operational task and by resource. Separate ordinary administration from security-sensitive functions such as identity-policy changes, audit-log administration, key management, and firewall-rule approval. Where staffing permits, require different people to request, approve, implement, and review high-impact changes.
Apply multifactor authentication correctly
MFA requires at least two pieces of evidence, such as something the user knows, has, or is. Apply it to remote, privileged, and other high-risk paths according to policy and risk. Do not treat a single password, a badge, or membership in an identity provider as complete authorization. MFA proves an aspect of identity; the resource, task, device state, and session still require separate policy decisions.
Protect virtualized and cloud management layers
Keep management-plane permissions separate from workload permissions. NIST SP 800-210 states that hypervisor access should be restricted to authorized cloud administrators. Apply the same principle to virtualization managers, storage controllers, orchestration consoles, and cloud control planes: tightly limit administrators, require strong authentication, and record management actions even when workloads are hosted on the same physical equipment.
3. Control facility access independently
Authorize people and visitors
Maintain a current physical-access list for employees, contractors, and visitors. Issue credentials only for approved areas, use an escort or time limit where risk warrants it, and remove permissions when they are no longer needed. NIST SP 800-171 Rev. 3 describes physical-access authorization for employees and visitors and cites guards, cameras, and sensors as monitoring methods.
Monitor entry, exit, and anomalies
Record entry and exit events for controlled areas, including the credential or person, location, timestamp, and result. Review access logs for unusual patterns such as repeated denials, access outside an approved schedule, simultaneous use in distant locations, tailgating alerts, or a visitor entering a restricted room without the expected escort. Define review frequency and event-triggered review in your policy; the cited guidance does not prescribe one universal interval.

Connect physical and logical investigations
Use a common time source and a consistent identifier where possible so investigators can correlate a door event with a console login, privileged command, or network-policy change. A physical-access record should not be used as proof that a logical action was authorized, but it can provide context when an account or device is suspected of misuse.
4. Restrict network paths
Write the required communication flows first
Document which systems must communicate, in which direction, over which protocol and port, and for what business or operational purpose. Then deny unlisted flows by default where availability requirements allow. Review management traffic separately from application and data traffic; an administrator’s workstation should not reach every server merely because both are on an internal network.
NIST SP 800-210 states: “Thus, access control for network boundaries and allowlists for network communications are required and may be applied through, for example, dedicated virtual local area networks (VLANs) leveraging automated access control lists (ACLs).” VLANs, boundary controls, and ACLs are mechanisms for enforcing a policy, not evidence that a particular architecture is automatically secure.
Segment according to risk and function
Separate user, server, storage, backup, management, security-tool, and guest or contractor traffic where their trust requirements differ. Use firewalls or equivalent enforcement points between segments, and constrain east-west traffic as well as north-south traffic. For high-value systems, consider per-workload or microsegmentation policies that express identity, device, application, and service conditions rather than relying only on subnet location.
Choose an architecture deliberately
| Approach | Useful strength | Questions to answer before adoption |
|---|---|---|
| Firewalls and boundary controls | Central enforcement of permitted routes, protocols, and ports | Can rules be tied to resource and identity, and can east-west paths be inspected? |
| Microsegmentation | Fine-grained controls between workloads or systems | Can operations maintain accurate dependencies without disrupting availability? |
| VPN | Protected connectivity for remote or inter-site users | After connection, are users still authorized per resource and task? |
| SASE | Distributed policy and security services for users and sites | How will on-premises, remote, and cloud enforcement and logging be integrated? |
| ZTNA | Resource-specific access instead of broad network admission | Does the implementation evaluate identity, device, session, and application context? |
NIST SP 800-215 surveys these and other enterprise-network approaches for environments spanning multiple data centers and cloud services. It does not identify one universally best option. Compare candidates by resource-level policy, narrowness of allowed flows, on-premises and cloud coverage, integration effort, operational burden, audit visibility, and availability impact.

5. Design useful logging and review
Capture decisions and actions across planes
At minimum, plan records for:
- Authentication attempts, MFA results, authorization decisions, session creation, elevation, and termination.
- Privileged commands, configuration changes, account and role changes, and policy approvals.
- Network-rule changes, denied and allowed management flows, segmentation changes, and remote-access events.
- Facility entry and exit, visitor activity, alarms, and physical-access changes.
- Changes to logging itself, including destination, retention, access permissions, and attempted deletion or alteration.
Make each event attributable
When the system supports it, associate an event with a distinguishable identity, the device or session, resource, action, timestamp, policy decision, and result. Synchronize clocks across facility, identity, network, and workload systems. Protect collected logs from unauthorized modification, restrict who can read them, and separate log administration from the people whose actions are being monitored.
Set retention and alerting from risk
The cited NIST material does not establish one universal retention period or alert threshold. Choose coverage, retention, storage protection, review frequency, and alert conditions according to data sensitivity, threat exposure, investigative needs, contractual requirements, and applicable law. A short-lived authentication record may need different treatment from a privileged configuration change or a facility incident.
Turn alerts into an operating process
Define who receives alerts, what severity means, how quickly each class is reviewed, and when an event becomes an incident. Useful detection combinations include a denied facility entry followed by a successful remote login, an administrator changing a policy from an unmanaged device, an unexpected hypervisor action, or a burst of denied east-west connections. Logging without ownership, review, and escalation is an archive rather than a control.
6. Protect system changes and reduce exposed services
Control who may change systems
Restrict physical and logical changes to qualified, authorized personnel. Document the requested change, affected resources, risk, approval, implementation window, rollback method, and evidence of completion. Use controlled workflows or change windows when they reduce operational risk. Emergency changes should still identify the actor, reason, scope, and subsequent review.
Apply least functionality
Configure systems to provide mission-essential capabilities only. Disable unnecessary or nonsecure ports, protocols, connections, services, interfaces, and administrative tools. NIST SP 800-171 Rev. 3 identifies scanning, intrusion detection and prevention, and endpoint protection as methods that can identify or prevent prohibited functions and connections. Recheck the baseline after upgrades, migrations, and troubleshooting because temporary exceptions frequently become permanent exposure.

7. Validate the controls and maintain them
Test allowed and denied paths
- Test an authorized identity on an approved, policy-compliant device and verify that only the documented resource and task are available.
- Test the same identity from an unmanaged or out-of-policy device and confirm the expected denial, step-up challenge, or restricted session.
- Test an unauthorized identity, expired role, disabled account, and visitor credential against facility and logical controls.
- Attempt prohibited inter-segment connections and confirm that boundary devices log and block them.
- Perform a privileged action through the approved workflow and verify attribution, approval evidence, and log generation.
- Alter a logging or policy setting through an authorized change and confirm that the change itself is protected and recorded.
- Run an incident scenario that correlates physical, identity, network, and workload events and verify escalation to the response team.
These tests are implementation practices derived from the purposes of the controls; the cited sources do not prescribe one universal test script. Record expected results, observed results, exceptions, and corrective owners.
Reassess after change
Repeat reviews after role changes, facility expansion, architecture or segmentation changes, new cloud services, major incidents, and material changes in obligations. Keep policy documentation, diagrams, inventories, and access reviews synchronized with the deployed configuration.
Control checklist for day-to-day operations
- Every protected resource has an owner, an access policy, and a defined business or operational purpose.
- Facility permissions and logical permissions are maintained as separate records.
- Privileged access uses individually attributable identities and multifactor authentication appropriate to risk.
- Hypervisor, storage, network, identity, and logging administration are explicitly scoped.
- Allowed network flows are documented, segmented, and reviewed for necessity.
- Devices and sessions are evaluated for policy compliance instead of trusted because of location.
- Logs identify the subject, resource, action, time, decision, and result where technically possible.
- Log access and alteration are themselves controlled and monitored.
- Unnecessary services, ports, protocols, and connections are disabled.
- Access reviews, visitor reviews, anomaly handling, and retention decisions have named owners.
- Denied-path tests and recovery exercises are performed after significant changes.
How the NIST material fits your program
SP 800-207 supplies the resource-centered zero-trust principle. NIST’s SP 1800-35 implementation guide, finalized June 10, 2025, documents 19 example implementations developed with 24 collaborators; those figures describe demonstrations and mappings, not measured security outcomes, certification, or endorsement of any commercial technology. SP 800-210 is cloud access-control guidance, so apply its hypervisor and virtual-network examples to applicable cloud or virtualized layers rather than assuming every physical facility is an infrastructure-as-a-service environment.
