An independent review of an AI use case in financial services is a documented, risk-proportionate challenge of the entire decision process—not a quick check of model accuracy. Define what the system does and where it is used, assess materiality, appoint reviewers with real objectivity and authority, test design, data, implementation and outcomes, document limitations and evidence gaps, then decide whether to approve, restrict, defer or reject the use. Attach owners, deadlines, monitoring measures and change triggers to the decision.
There is no single universal legal checklist. Requirements depend on the country, regulator, institution, product and use case. The US interagency guidance, NIST resources, Financial Stability Board consultation and European supervisory commentary below are useful reference points, but your legal, compliance and policy teams must identify the binding requirements for the actual deployment.

What an independent AI review is—and is not
The review asks whether a particular AI system is fit for a stated purpose, used within defined boundaries and controlled throughout its life. It covers the model or service, the data and technology around it, human decisions, downstream rules and the effects on customers, counterparties, the institution and its obligations.
Independence is a working condition rather than a job title. The Federal Reserve, OCC and FDIC describe effective challenge as work by people with appropriate expertise, sufficient independence to remain objective, and organizational standing to make changes. A reviewer can sit in a second-line function and still lack authority, or sit outside the development team while having incentives that compromise objectivity. Both situations require safeguards.

A review also is not the same as internal audit. The April 17, 2026 interagency Supervisory Guidance on Model Risk Management says internal audit generally evaluates the rigor and effectiveness of model-risk practices rather than duplicating development or validation. The review should therefore state its purpose—pre-deployment approval, a material-change assessment, periodic validation, a control review or an incident re-review—and avoid overlapping roles without a reason.
1. Set a written mandate before testing
Start with a short mandate that fixes the object of review and the decision it will support. Record:
- the accountable business owner, system owner, model or AI owner, reviewers, approvers and remediation owners;
- the review type and decision date;
- the system, model, prompt or configuration versions and deployment environment covered;
- the start and end dates, jurisdictions, products, populations and channels in scope;
- whether the review covers only the model artifact or the full workflow in which its output is used; and
- the escalation route when evidence is missing or a material disagreement remains.
Define what is outside scope rather than allowing the boundary to drift. The mandate should require a record of recommendations, management responses, exceptions and remediation. If an urgent business need means use might start before full validation, require a separate approval that states why, what restrictions apply, who accepts residual risk and how often the use will be checked.

Do not begin by interpreting a test score. First describe the decision and the operating context in plain language. A use-case statement should answer:
- What business objective or customer problem is being addressed?
- What decision, task or recommendation does the system support?
- Which uses are intended, and which are expressly prohibited?
- Who uses the output and who is affected by it?
- Is the output a rank, recommendation, summary, screen, approval, decline, price, route or trigger?
- Where does human judgment enter, and can a person override or escalate?
- What happens when the output is wrong, unavailable, contradictory or uncertain?
Map the output through every downstream step. A model that merely prioritizes cases may indirectly determine which customers receive an investigation, while a recommendation may become an automatic decision if staff have no time or authority to challenge it. Materiality depends on exposure and purpose. A technically sound model can create high risk when applied outside its intended population or decision.
3. Classify risk and set proportionate depth
Use a documented materiality rationale to choose the amount of review. The interagency guidance takes a risk-based approach tailored to model risk profile, organizational size and complexity; it does not prescribe one structure or a universal numeric threshold. Assess at least the following dimensions.

| Dimension | Questions to answer | Effect on review depth |
|---|---|---|
| Complexity and assumptions | How difficult are the methods, features, prompts, rules, integrations and assumptions to understand or challenge? | More complex or opaque designs need stronger conceptual review, alternative analysis and specialist expertise. |
| Exposure and impact | Which customers, counterparties, portfolios, financial amounts, operations or regulatory decisions can be affected? | Broader or more consequential exposure warrants deeper outcome testing, scenario analysis and senior approval. |
| Purpose | Does the use support a significant business line, a regulatory obligation, risk management or a high-consequence customer decision? | Important purposes require tighter permitted-use rules and clearer evidence that objectives are met. |
| Data and implementation risk | Are inputs reliable, representative and permitted? Do deployment transformations and integrations match the reviewed design? | Material data limitations require targeted quality, lineage, bias and production-environment testing. |
| Dependency and aggregate risk | Does the system share data, assumptions, vendors, infrastructure or downstream rules with other models? | Review interactions and concentration, not just the individual model’s score. |
Record why the chosen depth is reasonable. Do not turn a low-risk classification into a permanent exemption: a new product, population, vendor, data source, market condition or automation level can change the assessment.

List everyone who developed, configured, selected, procured, implemented, operates or benefits from the system, then compare those roles with the proposed review team. Ask whether a reviewer:
- made design or procurement decisions now being judged;
- has performance incentives tied to launch, volume, savings or approval rates;
- reports to the owner whose decision is under challenge;
- has unrestricted access to evidence and production information; and
- can escalate an unresolved issue to an approver who is not dependent on the project.
Document conflicts and safeguards. Safeguards can include an independent assignment, a second reviewer, a separate approver, recusal from a particular test, direct escalation to a risk committee or a time-limited use restriction. Independence is not proved by an organizational chart alone. The 2026 guidance states that validation quality depends on the rigor and effectiveness of review rather than on the structure of the risk-management function.
Match expertise to the use case. A team may need statistical or machine-learning knowledge, data engineering, cybersecurity, cloud or application architecture, business-process expertise, consumer-protection or conduct knowledge, operational resilience, privacy, and accounting or prudential context. Give reviewers organizational standing to require changes, impose conditions or recommend stopping use. The ECB has highlighted fragmented ownership, unclear accountability, weak senior oversight and inadequate challenge as supervisory concerns; its February 24, 2026 speech, “Technology is neutral, governance is not: AI adoption in the banking sector,” is a supervisory perspective, not a universal rule.
5. Build an evidence inventory
Request evidence before agreeing that a test can be performed. Create an inventory with the document owner, version, date, scope and any restrictions. At minimum, seek:

| Use and architecture | Use-case record, architecture diagram, workflow map, interfaces, version history and deployment topology. | Which boundary cannot be verified and what temporary use limit compensates. |
| Purpose and design | Objective, target or labels, methodology, assumptions, feature or prompt rationale, decision thresholds and prohibited uses. | Unresolved assumptions, design choices and their effect on the conclusion. |
| Data | Lineage, provenance, permissions, quality checks, representativeness, transformations, retention and production-versus-development differences. | Population or quality gaps and any compensating sample, sensitivity or restriction. |
| Testing and outcomes | Development and validation results, benchmark or alternative methods, out-of-sample or out-of-time tests, stress or scenario analysis, subgroup results, back-testing and incident history. | Tests that could not be reproduced, data limitations and confidence limits. |
| Operations and people | Runbooks, user training, access controls, human-review procedures, override records, alerting, incident and complaint logs. | Controls that rely on informal practice or cannot be evidenced. |
| Third parties | Vendor methodology, service description, contract, service levels, security information, customizations, parameters, support and change notices. | Proprietary constraints, unverified claims and the resulting use restriction. |
The inventory should support an institution-wide view of individual and aggregate risk. For a third-party product, assess conceptual design, development data, performance, customizations, parameters, overlays and ongoing outcomes as far as evidence permits. A vendor’s assertion is not a substitute for validation. Retain responsibility and oversight even when an outside party performs testing or operations. The GAO report on artificial intelligence use and oversight in financial services (published May 19, 2025, with a March 2026 recommendation update) is useful context for credit-union oversight and third-party examination limits; it does not establish that every regulator has the same gap.

6. Challenge the system and its evidence
Purpose and conceptual soundness
Determine whether the method, objective, labels, prompts, assumptions and decision threshold fit the stated task. Ask whether the output has a meaningful interpretation for the decision-maker and whether important qualitative judgments are documented. Check that the reviewed version is the version actually used. A high benchmark score does not cure a mismatch between the task and the decision.
Data and implementation
Test whether inputs are relevant, sufficiently reliable and representative of the intended operating population and conditions. Trace critical fields from source to production, including transformations, missing-value handling, feature or retrieval updates, permissions and logging. Compare development, validation and live environments for material differences. Investigate whether a proxy, stale field, selective sample or undocumented filter changes who is assessed.
Testing strategy
Challenge whether the tests answer the use case’s real risks. Depending on the system, use out-of-sample or out-of-time testing, benchmarks or alternative methodologies, stress and scenario analysis, subgroup or segment analysis, and focused data-quality tests. Set thresholds before interpreting results where practicable and explain why they are appropriate. Examine failures, borderline cases, abstentions and missing outputs, not only average performance. The rigor should reflect complexity and materiality; a single accuracy or loss number is rarely sufficient.
Outcomes and performance in operation
Compare outputs with corresponding real-world outcomes and the original objectives. Review back-testing, error patterns, persistent deviations, drift, changes in products or exposures, client and data populations, and market conditions. Establish how often performance is monitored, which ranges are expected and who receives reports. Define when recalibration, an overlay, adjustment, redevelopment or a pause is required. Investigate complaints, overrides and operational incidents as outcome evidence rather than treating them as separate from model performance.
Limitations, explanations and human use
State where the system is unreliable, what users may and may not do with its output, how uncertainty or anomalies are identified, and when a user must override, escalate or stop. An explanation is useful only when decision-makers can understand the drivers, risk teams can challenge them and auditors can reconstruct how the output was used. Provide training and a usable override path; nominal human review is not a control if staff cannot disagree or lack time to investigate.
Aggregate and workflow risk
Review interactions with other models, shared assumptions and data, downstream rules, human decisions, external services and operational dependencies. Consider concentration—for example, many decisions relying on one data supplier or one common feature—and correlated failure during a stressed market. Controls must cover the end-to-end decision process, not only the model file.

Special treatment for generative and agentic systems
The April 17, 2026 US interagency model-risk guidance defines a model as a complex quantitative method, system or approach applying statistical, economic or financial theories to input data to produce quantitative estimates. It excludes simple arithmetic, deterministic rule-based processes and software without those theoretical underpinnings. The guidance expressly excludes generative and agentic AI and says its principles apply to traditional statistical and quantitative models and non-generative, non-agentic AI. Do not silently expand that defined scope.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
For a generative or agentic use, use the organization’s broader governance and applicable requirements to design the review. Document prompts or system instructions, retrieval and context sources, output review, tool calls or autonomous actions, access boundaries, logging, escalation, data leakage controls, version changes and rollback. These are practical lifecycle controls, not a claim that the interagency guidance prescribes a particular generative-AI test.
NIST’s AI Risk Management Framework FAQs describe the framework and Playbook as voluntary resources that organizations may use to the extent useful. NIST also says the framework is a living document and notes that a 2025 White House action tasked it with revising AI RMF 1.0; verify the status of any revision before calling it complete.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.7. Write findings that can be acted on
Each finding should stand on its own. Include:
- the condition observed and the evidence supporting it;
- the expected control, objective, policy or permitted-use requirement;
- the affected decision, population and risk;
- the cause, if established, separated from assumptions;
- the severity rationale and whether the issue blocks deployment or requires a restriction;
- the specific action, accountable owner and due date; and
- the evidence that will demonstrate closure.
List unresolved questions, unavailable evidence and out-of-scope items separately from confirmed findings. Let the business owner correct factual errors, but do not give that owner veto power over the independent conclusion. If reviewers disagree, preserve both positions and identify the approver who decides whether residual risk is acceptable.
8. Make a decision and constrain use
Use an explicit disposition:
- Approve for the stated use: evidence supports the objective and controls are operating.
- Approve with conditions: specify populations, thresholds, human review, reporting, expiry dates and remediation deadlines.
- Defer: evidence or controls are insufficient; no production use occurs until named conditions are met.
- Reject or stop: the method, purpose, exposure or control environment is not acceptable.
Name the approver and the person authorized to accept residual risk. If a limited launch precedes complete validation, document the urgent reason, use limits, stakeholder communication, compensating controls, heightened monitoring and a firm date for completing the review. Conditions should be enforceable in workflow and access settings, not merely written in a memo.
9. Monitor, re-review and close the lifecycle
Approval is not the end of validation. Attach every condition to an owner and deadline, then monitor performance against expected ranges and relevance as products, exposures, data, customers, market conditions, vendors or versions change. Define re-review triggers in advance:
Best Value
- material model, prompt, data, product or workflow changes;
- drift, persistent error or a breached performance limit;
- an incident, complaint, privacy or security event;
- failure of human oversight, override or escalation;
- a new jurisdiction, customer population or decision purpose; and
- vendor changes, outages, subcontractors or altered service terms.
Set a reporting cadence, recipients, thresholds and escalation times. Track exceptions, remediation evidence and closure approval. The Financial Stability Board’s June 10, 2026 consultation report proposed 12 sound practices for organization-wide AI governance and lifecycle management; its July 22, 2026 response deadline has passed, so treat it as a consultation source unless a final report is separately verified: FSB consultation report.

How to compare two review designs
When choosing between internal review approaches, compare them on the same axes rather than on labels such as “independent” or “AI validation.” The following dimensions synthesize supervisory and governance guidance; they are not a regulator-issued scoring scale.
| Comparison axis | Questions |
|---|---|
| Independence, competence and authority | Can reviewers challenge the owner, understand the methods and require a change or escalation? |
| Scope | Does the work cover the artifact, data, technology, human decisions and downstream workflow? |
| Proportionality | Is depth tied to materiality, complexity, exposure and aggregate dependencies? |
| Conceptual and outcome testing | Are assumptions, implementation, real-world outcomes, stress cases and failure modes examined? |
| Data and vendors | Are lineage, representativeness, customizations, proprietary limits and concentration risks addressed? |
| Decision usefulness | Can users understand limitations, uncertainty, drivers and override obligations? |
| Traceability | Can every finding be followed to a restriction, owner, deadline, monitoring measure and re-review trigger? |
Regulatory boundaries to state in the file
The Federal Reserve-hosted interagency guidance dated April 17, 2026 replaces earlier interagency model-risk guidance. It is supervisory guidance, not a prescriptive enforceable standard; the Federal Reserve says non-compliance with the guidance itself will not result in supervisory criticism, although violations of law or unsafe or unsound practices remain separate matters. It is expected to be most relevant to banking organizations with more than $30 billion in assets, while potentially relevant to smaller organizations with significant model-risk exposure.
Do not present the guidance as a universal checklist or apply its defined model scope to generative and agentic AI. NIST’s framework is voluntary. The FSB material is a consultation. The ECB speech is a supervisory perspective. GAO’s credit-union discussion provides context about oversight differences and an open recommendation to broaden NCUA model-risk guidance, not a blanket statement about every financial regulator. Record the institution’s actual jurisdictional and policy requirements alongside these reference points.
What a defensible conclusion looks like
A defensible conclusion connects purpose, evidence, risk and action in one chain: the system was reviewed in a stated version and workflow; reviewers had appropriate expertise, independence and authority; material assumptions, data, implementation, outcomes and dependencies were challenged; limitations and missing evidence were disclosed; and the approval or restriction is tied to named owners, monitoring and change triggers. As the ECB speech puts it, "AI does not dilute responsibility. If anything, it raises the bar." The review record should make that responsibility visible long after launch.
