For a parent in the United States, establish Medicare eligibility and then compare Original Medicare with Medicare Advantage, supplemental coverage and prescription arrangements. For a parent in India, start with actual residence, public-program eligibility, employer or private insurance, local hospitals and the policy wording. If the parent travels between countries, treat travel medical care and evacuation as a separate coverage problem.
No reliable, current source establishes a like-for-like U.S.–India price or outcome ranking. Use the framework below to compare coverage, access, daily-care needs and cross-border risks for one specific parent.

Start with the decision that changes everything: where will care be delivered?
Write down the parent’s usual residence, citizenship or immigration status, expected travel dates, and the places where routine, specialist and emergency care would actually occur. A U.S. insurance plan generally governs care in the United States; an Indian policy generally follows its own eligibility, network and claims rules. Neither label alone tells you what will be paid.

Parent who lives in the United States
Confirm Medicare eligibility and enrollment dates, then identify whether the parent has Original Medicare, Medicare Advantage, Medigap, employer or retiree coverage, Medicaid, Veterans benefits or another payer. Gather the plan’s Evidence of Coverage, drug formulary, provider directory and latest premium and deductible notices. Eligibility cannot be inferred from age alone: immigration, work history, disability status and enrollment periods can matter.

Parent who lives in India
Document the state and city of residence, age, citizenship or other status required by a public scheme, employer coverage, existing private policy, preferred hospitals and treating doctors. Read the policy schedule and exclusions rather than relying on a product name. The NITI Aayog Senior Care Reforms in India report (2024) describes the system and gives examples of senior-focused insurance, but it is not a current ranking of plans, premiums, waiting periods, exclusions or claim performance. Verify every current term directly with the insurer or public program.
Parent who moves or visits between countries
Separate three questions: routine care in the home country, planned treatment in the other country, and an unexpected emergency while travelling. A domestic plan may answer only the first question. Obtain written confirmation for any planned treatment abroad before booking it.
Compare U.S. Medicare choices on the same checklist
Medicare.gov describes a fundamental difference between Original Medicare and Medicare Advantage. Original Medicare generally lets a beneficiary use any U.S. doctor or hospital that accepts Medicare. Medicare Advantage plans are offered by private insurers and may require network providers and a service area for non-emergency care.
| Feature | Original Medicare | Medicare Advantage |
|---|---|---|
| Provider choice | Any U.S. doctor or hospital that accepts Medicare. | Plan network and service area commonly apply for non-emergency care. |
| Referrals and prior authorization | Rules are generally less restrictive, although particular services can still have Medicare requirements. | Plan-specific referral and prior-authorization rules can apply. |
| Part B cost sharing | Usually 20% coinsurance after the Part B deductible; Original Medicare has no annual out-of-pocket limit unless supplemental or other coverage applies. | Cost sharing varies by plan, with an annual limit for covered Medicare services. |
| Prescription coverage | Usually requires a separate Part D plan or other drug coverage. | Many plans include Part D, but formulary, pharmacy and utilization rules differ. |
| Extra benefits | Coverage is defined by Medicare; separate supplemental policies may add benefits. | Plan benefits and extras vary and must be checked in the plan documents. |
Ask the parent’s clinicians whether they accept the exact plan, not merely “Medicare.” Check every specialist, hospital, rehabilitation facility, laboratory and pharmacy the parent uses. A network directory can be outdated, so confirm participation with both the provider and the plan.
Calculate the parent’s real annual exposure
- Monthly Part A, Part B, Part D, Medicare Advantage, Medigap or employer-plan premiums.
- Annual deductibles and the copay or coinsurance for primary care, specialists, emergency departments, inpatient stays, imaging, rehabilitation and durable medical equipment.
- The Medicare Advantage annual maximum out-of-pocket amount for covered Medicare services, and which services count toward it.
- For Original Medicare, the absence of a general annual limit unless another policy supplies one.
- Drug tiers, deductibles, quantity limits, step therapy, mail-order requirements and the pharmacies available near the parent.
- Non-covered services, transportation, dental, vision, hearing and other extras only when the plan document expressly includes them.
Do not compare a premium with a premium and stop there. A plan with a lower monthly charge can expose the family to higher cost sharing, narrower networks or more authorization work.

What Medicare does—and does not—do outside the United States
Medicare.gov’s plain-language warning is: “Medicare generally doesn’t cover medical care outside the U.S.” The agency describes narrow exceptions for certain foreign inpatient hospital, physician or ambulance services. A foreign hospital does not have to submit a Medicare claim, so the family may need to pay and handle documentation itself. Check the exact circumstances with Medicare before treatment.
Some Medigap policies may cover emergency care abroad, subject to the policy’s limits, deductible and lifetime maximum. That benefit is not automatic for every Medigap policy. Medicare.gov also warns that travel insurance does not necessarily include health insurance.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesBuild a written cross-border answer
- Ask the domestic insurer whether emergency and planned care in India or the United States is covered.
- Ask whether authorization is required before admission, surgery, dialysis, oncology treatment or medical transport.
- Confirm who pays the foreign provider, in which currency, and what claim documents are required.
- Check whether evacuation to the parent’s home country is covered or whether the policy only pays treatment at the nearest suitable facility.
- Keep the insurer’s international assistance number, policy number and claims instructions with the parent and caregiver.
Travel medical insurance is a separate purchase decision
For a temporary visit, compare travel medical insurance and medical-evacuation coverage rather than assuming ordinary Medicare or an Indian domestic policy will follow the traveller. The U.S. Department of State says air-ambulance evacuation to the United States can cost $20,000 to $200,000, depending on location and medical condition. This is a general estimate from undated State Department guidance accessed in 2026, not a quote for a particular route.
The State Department’s travel-insurance checklist, last updated August 11, 2025, says to verify:
- Whether the destination and every transit country are valid under the policy.
- The maximum trip length and whether extensions are possible.
- Emergency medical treatment, hospital admission and physician services.
- Transportation to the United States or the parent’s home country when medically necessary.
- How the policy treats current or pre-existing medical conditions.
- Exclusions for planned activities, alcohol, adventure travel or destinations subject to government warnings.
- The deductible, maximum benefit, claim deadlines and required records.
- A 24-hour assistance line that can coordinate hospitals, payment guarantees and evacuation.
Policy wording varies significantly. Ask for a written answer about the parent’s diagnoses, medications, recent admissions, pending tests and planned procedures; a generic “covered” statement is not enough.

Medicine rules for the journey
State Department guidance says to check medicine rules with the embassy or other official authority for every destination and transit country. Carry enough medicine for the trip plus a few extra days, keep it in the original packaging, and bring prescription copies and a doctor’s letter listing generic names. Rules can differ for controlled substances, injectables and quantities. A pill organizer can be useful after arrival, but it is not a substitute for travelling with medicines in their original labelled containers.
Compare Indian options without treating an old report as a live product table
The NITI Aayog Senior Care Reforms in India report (2024) is useful for understanding need and insurance access, but its figures and named products are report-era evidence. Before buying or renewing a policy, obtain the current brochure, policy wording, schedule, network list and claim procedure from the insurer.
Questions for a public scheme
- Does the parent meet the scheme’s current age, residence, income or identity requirements?
- Which hospitals in the parent’s city participate, and can the parent use a preferred specialist?
- Are referrals, pre-authorization, package limits or room-category limits imposed?
- Does the scheme cover outpatient medicines, diagnostics, rehabilitation and home care, or mainly hospital treatment?
- How are emergency admissions handled when the parent is outside the home district or state?
- What documents must a family member present when coordinating a claim?
Questions for private or employer insurance
- What are the entry-age rules, renewal guarantees and maximum age, if any?
- Which pre-existing conditions have waiting periods, and how is a pre-existing condition defined?
- Are there disease-specific sub-limits, co-payments, room-rent caps, non-payable consumables or restoration conditions?
- Does the policy pay cashless treatment at the hospitals the parent would actually use?
- How are day-care procedures, home treatment, ambulance transport, rehabilitation and mental-health services handled?
- What is the claim-intimation deadline, and can a distant caregiver submit documents electronically?
Do not infer current availability or superior benefits from a product name mentioned in the NITI Aayog report. Insurer terms change, and the report does not establish a current premium, exclusion, waiting-period or claim-service comparison.
Budget daily assistance separately from medical insurance
Medical treatment and help with daily living are different expenses. Medicare distinguishes skilled nursing-facility care from long-term custodial care. Most long-term care helps with activities of daily living such as dressing, bathing and using the bathroom; it can also include meals, adult day health care and transportation. Medicare and most health insurance, including Medigap, do not pay for most non-medical long-term care.
Map the assistance the parent actually needs
- List each task: bathing, dressing, toileting, transfers, eating, medication management, shopping, cooking, finances and transport.
- Record how often help is needed and whether a second person or skilled clinician is required.
- Identify the safest setting: the parent’s home, a community day program, assisted living, a nursing facility or a temporary rehabilitation stay.
- Price staffing, meals, transport, equipment, home modifications and respite separately from doctor and hospital bills.
- Check whether Medicaid or another state program could cover qualifying services in the United States; eligibility and benefits vary by state and can change.
- For India, ask the local authority, hospital social worker and insurer which public, employer or private benefits apply to the parent’s residence and policy.
Private long-term-care insurance may help in the United States, but policy triggers and covered settings differ. Do not assume that a health policy’s hospitalization benefit pays for an ongoing caregiver.

Use the parent’s health needs—not country stereotypes—as the comparison unit
Make one row for each recurring need and test the actual option against it.
| Need to compare | Evidence to collect | Decision question |
|---|---|---|
| Primary and specialist care | Named clinicians, appointment availability, network or panel status, referral rules. | Can the parent reach the clinician who knows the history without an unaffordable out-of-network bill? |
| Hospital and emergency care | Participating hospitals, emergency process, admission authorization, room and package limits. | Who coordinates and pays if admission occurs at night or far from home? |
| Rehabilitation | Covered sessions, facilities, home visits, clinical criteria and limits. | Will rehabilitation continue after discharge, and who transports the parent? |
| Prescriptions | Formulary or covered-drug list, pharmacy access, refill rules and import restrictions. | Can the parent obtain the exact medicines continuously? |
| Pre-existing conditions | Declarations, exclusions, waiting periods, underwriting and renewal wording. | Is the parent’s actual diagnosis covered now, later, or not at all? |
| Home and facility care | Home-health definitions, nursing benefits, assisted-living or facility terms, daily-care exclusions. | What happens when the parent needs help every day rather than a discrete treatment? |
| Travel and evacuation | Destination, trip duration, emergency maximum, evacuation trigger and assistance line. | Could the family fund or arrange transport if the parent cannot fly commercially? |
| Records and communication | Language support, release forms, portals, paper records, medication list and emergency contacts. | Can a distant caregiver obtain timely, accurate information? |
Plan family coordination before the first crisis
The U.S. Department of Health and Human Services’ Long-Distance Caregiving guide recommends learning about the parent’s condition, obtaining permission and required privacy releases before speaking with clinicians, discussing advance directives and healthcare proxies, and seeking benefits information through applicable programs. Program eligibility and benefits can change and vary by state.
Create a practical authorization packet
- A signed healthcare proxy or equivalent authority, where applicable.
- Privacy releases that allow the named caregiver to speak with doctors, hospitals and insurers.
- Advance directives, resuscitation preferences and the parent’s expressed goals of care.
- A one-page medical summary: diagnoses, allergies, medicines with generic names and doses, surgeries, clinicians and emergency contacts.
- Copies of identification, insurance cards, policy numbers and claim instructions.
- The name of one primary family coordinator and one backup, with time-zone-aware contact details.
Consent is central. A relative who pays the bill may still be unable to obtain clinical information without the parent’s authorization or another legally recognized basis. Ask each provider which form it accepts and how long it remains valid.
What the available Indian statistics do—and do not—tell you
NITI Aayog’s 2024 report, citing LASI findings, gives context for planning but not a quote for an individual parent:
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →- About 104 million people in India were over age 60, with a projection of 19.5% of the population by 2050.
- Seventy-five percent of older people were reported to have one or more chronic diseases.
- Twenty-four percent were reported to have at least one Activities of Daily Living limitation.
- Forty-eight percent reported at least one Instrumental Activities of Daily Living limitation.
- Eighteen percent reported being covered by any health insurance.
These are report-era findings, not current estimates for every state or a prediction of one parent’s needs. Use them to prompt questions about chronic-care access, daily assistance and insurance gaps; verify current local data and eligibility before making a financial decision.

A repeatable comparison process for families
- Define the home base. Record residence, likely care locations, citizenship or immigration status and travel frequency.
- Inventory the parent’s health. List diagnoses, recent admissions, medicines, mobility, cognition and daily-living assistance.
- Collect the documents. Obtain insurance cards, policy wording, formularies, provider directories, bills and authorization forms.
- Build the care map. Name the primary doctor, specialists, hospital, pharmacy, rehabilitation provider and potential daily-care setting.
- Test coverage with specific scenarios. Ask what happens for a routine visit, specialist referral, emergency admission, surgery, rehabilitation, a lost prescription and a 30-day overseas stay.
- Price total exposure. Add premiums, deductibles, copays, coinsurance, uncovered medicines, transport, home help and possible evacuation—not just the monthly premium.
- Confirm cross-border protection. Obtain written travel-medical and evacuation terms for each trip, including pre-existing-condition treatment.
- Assign responsibility. Name the authorized family coordinator, backup, claims owner and person who can be physically present.
- Recheck annually and after changes. Revisit enrollment, networks, formularies, public-program rules, travel dates and the parent’s functional needs.
Common mistakes and how to avoid them
Assuming Medicare follows the parent to India
It generally does not. Investigate the narrow foreign-care exceptions, any Medigap emergency benefit and a separate travel policy before departure.
Choosing a plan from the premium alone
Compare annual cost sharing, network access, authorization rules, drug coverage and the parent’s likely hospital use.
Calling a hospitalization policy long-term-care coverage
Ask specifically about bathing, dressing, toileting, meals, supervision, transportation and ongoing home or facility assistance.

Bottom line for a U.S.–India family
Compare one parent’s complete care pathway, not two countries in the abstract. A U.S.-resident parent needs a Medicare and supplemental-coverage decision plus a separate long-term-care budget. An India-resident parent needs a current, locally usable public or private arrangement checked against actual hospitals and policy wording. A travelling parent needs destination-specific medical and evacuation protection, medicine documentation and an authorized caregiver. Because current Indian terms and individual eligibility vary, verify every decision with the relevant program, insurer and clinician before relying on it.
