Compare medical-device stocks across several aligned fiscal years, not on a single quarter or one headline growth rate. Separate reported from organic growth, start cash analysis with GAAP operating cash flow, subtract capital spending transparently, and test dividends against recurring cash needs, debt and liquidity. The figures below illustrate the method; they are not a buy-or-sell ranking.
Build a fair comparison before ranking companies
Choose a peer group with businesses that are reasonably comparable, then use the same fiscal periods wherever possible. Record each company’s fiscal year-end, reporting currency, business scope and whether a measure is GAAP or non-GAAP. A consolidated figure for a diversified company is not the same thing as cash generated by its medical-device segment.

Medtronic’s fiscal year ends in April, while the cited Abbott and Boston Scientific reports cover calendar years. Their figures below therefore illustrate how to read filings, not a like-for-like league table. Abbott also operates outside medical devices, so its consolidated cash flow should not be treated as device-segment cash flow.
| Company and cited period | Revenue or growth detail | Operating cash flow | Capital spending | Cash after capital spending | Dividend disclosure |
|---|---|---|---|---|---|
| Medtronic, FY2026 ended April 24, 2026 | $36.4 billion revenue; 8.4% reported growth and 5.8% organic growth (Medtronic FY2026 release) | $7,330 million (FY2026 Form 10-K) | $1,904 million additions to property, plant and equipment (FY2026 Form 10-K) | $5,426 million company-defined free cash flow: operating cash flow less capital additions (FY2026 Form 10-K) | Board increased Q1 FY2027 dividend to $0.72 per share; $2.88 annualized, not a guarantee of future payments (June 2026 release) |
| Abbott, calendar 2025 | Not stated in the cited cash-flow figures; see annual report for company-wide revenue and segment context | $9,566 million (2025 Annual Report) | $2,171 million property and equipment spending (2025 Annual Report) | $7,395 million, calculated as operating cash flow less property and equipment spending | $2.40 common dividends declared per share in 2025 (2025 Annual Report) |
| Boston Scientific, calendar 2025 | Not stated in the cited cash-flow figures; see Form 10-K for revenue and business context | $4,534 million (FY2025 Form 10-K) | Capital purchases are reported separately; a comparable figure is not stated here | Not stated here; do not infer it from operating cash flow alone | No common-stock cash dividend paid in 2023–2025; the 2025 Form 10-K said the company then did not intend to pay one |

How to assess growth without mixing unlike measures
Start with several years of revenue
Use a multi-year view to see whether growth is persistent or concentrated in one unusually strong or weak period. Read the company’s segment discussion as well as consolidated revenue: procedure volumes, new product adoption, pricing, acquisitions and divestitures can make the headline total tell an incomplete story.
Separate reported growth from organic growth
Reported revenue growth reflects the company’s published revenue comparison. Organic growth is a company-defined measure intended to adjust for specified factors, which may include currency movements or acquisitions and divestitures. Definitions and adjustment methods can differ, so read the issuer’s explanation before comparing organic rates across companies.
Medtronic reported FY2026 revenue of $36.4 billion, up 8.4% as reported and 5.8% organically. Its release separately discusses foreign-exchange and other adjustments in the organic-growth comparison. Those percentages are an example of why labels matter, not a direct ranking against other issuers’ growth rates.

Measure cash generation from the cash-flow statement
Begin with GAAP operating cash flow
Use net cash provided by operating activities (or the equivalent cash-flow-statement label) across several years. Inspect working-capital movements and tax payments: either can move cash flow materially in a year without reflecting a matching change in underlying sales or product demand.
Abbott reported operating cash flow of $9,566 million in 2025, $8,558 million in 2024 and $7,261 million in 2023. Boston Scientific reported $4,534 million in 2025, $3,435 million in 2024 and $2,503 million in 2023. These are consolidated company figures from their respective annual filings, with different business scopes and not necessarily comparable fiscal periods.
Subtract capital spending and label the calculation
A useful cash-capacity estimate is operating cash flow less additions to property, plant and equipment. Call it cash after capital spending or state the arithmetic; do not silently treat a self-calculated figure as an issuer’s reported measure. Abbott’s 2025 report lists $2,171 million of property and equipment spending, yielding $7,395 million after subtracting it from $9,566 million of operating cash flow.

Medtronic defines its non-GAAP free cash flow as net cash provided by operating activities minus additions to property, plant and equipment. For FY2026, it reported $7,330 million of operating cash flow, $1,904 million of additions and $5,426 million of free cash flow; for FY2025, the corresponding figures were $7,044 million, $1,859 million and $5,185 million. The company says free cash flow is supplemental to, not a substitute for, U.S. GAAP results. Check each issuer’s definition and reconciliation before comparing non-GAAP free-cash-flow figures.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Boston Scientific reports capital purchases and acquisitions separately. Acquisition outlays varied materially across the cited years, so do not treat all investing cash outflow as recurring operating expense or substitute total investing cash flow for capital expenditure.
Judge dividend safety by cash capacity, not yield alone
Yield changes when a share price changes; it does not show whether the company can sustain its absolute dividend. Compare dividends paid or declared with cash remaining after capital needs, then account for debt maturities, liquidity, pensions or other obligations, and planned investment. A historical increase streak or management policy is context, not proof of future safety.
Abbott declared common dividends per share of $2.40 in 2025, $2.24 in 2024 and $2.08 in 2023. It stated in its 2025 Annual Report that it expects annual operating cash flow to continue to exceed capital expenditures and cash dividends. That is management’s expectation, not an assured outcome; compare the stated expectation with the reported cash flows and obligations.
Medtronic’s June 2026 results release said the board increased the Q1 FY2027 dividend to $0.72 per share, implying $2.88 annualized, and described it as the 49th consecutive year of increases. Annualizing a quarterly declaration is a run-rate calculation, not a promise of future payments. Boston Scientific’s 2025 Form 10-K said it had paid no common-stock cash dividend in 2023, 2024 or 2025 and then did not intend to pay one; it may be relevant to growth and cash-generation analysis, but that disclosure does not make it a current dividend-income example.

Check the risks that can change future growth and cash conversion
Past growth and cash flow do not establish future results. For each company, review the risk factors and management discussion in the latest filing for developments that could affect product uptake, sales, margins, cash collection or capital needs.

- Operations: foreign exchange, trade policy and supply-chain disruption can affect reported results, costs and availability.
- Portfolio and obligations: acquisitions and integration, divestitures, litigation, debt and planned investment can change cash needs or risk.
Boston Scientific’s 2025 Form 10-K identifies currency, competition, reimbursement and regulatory conditions, trade policy, supply-chain disruption, procedure volumes, new-product adoption, pricing, acquisitions and integration, and litigation among factors that may cause actual results to differ from forward-looking statements. Review each peer’s own disclosure rather than assuming the same risk has the same effect at every company.
A practical filing-based comparison workflow
- Set the comparison period: choose at least several fiscal years, note each fiscal year-end and align periods as closely as the filings allow.
- Map the businesses: record segments and material acquisitions or divestitures; distinguish consolidated results from device-segment results.
- Compare growth on stated terms: capture reported revenue growth, organic growth where provided, and the issuer’s definition and adjustments.
- Build a cash series: record GAAP operating cash flow, property and equipment additions, and your clearly labeled subtraction for each year; note working-capital effects.
- Read non-GAAP reconciliations: verify each free-cash-flow definition instead of treating similarly named metrics as interchangeable.
- Test dividend capacity: record cash dividends paid or declared, compare with recurring cash after capital expenditure, and assess debt, liquidity and other obligations.
- Recheck risks and dates: use the latest annual and subsequent quarterly filings before drawing conclusions, because issuer results and dividend declarations change.
This framework supports a disciplined comparison, not a prediction or investment recommendation. A company with faster revenue growth is not automatically the stronger cash generator, and a long dividend history does not by itself establish that a payout is safe.
