How to Choose Cold and Flu Medicine With High Blood Pressure

Choose cold and flu medicine by its active ingredients and warnings—not by brand name or the words “cold and flu.” Decongestants can raise blood pressure, and ibuprofen or naproxen may affect it too. If your blood pressure is severe or uncontrolled, Mayo Clinic says not to take decongestants. For a product that fits your symptoms and medication list, check with a pharmacist or healthcare professional.

Start with the active ingredients, not the front label

Combination products can contain several medicines even when the package emphasizes just one symptom. Read the Drug Facts panel, including active ingredients, warnings, directions, and sodium content. Formulations can vary, so check the exact product every time rather than assuming a familiar brand is unchanged.

The FDA advises choosing an over-the-counter medicine that treats only the problems you actually have. A single-ingredient option may avoid exposing you to ingredients you do not need. If you take blood-pressure medication or have other health conditions, ask a pharmacist or healthcare professional to review the specific product with your current medication list.

For congestion, check carefully for decongestants

Decongestants narrow blood vessels and can increase blood pressure. Mayo Clinic lists pseudoephedrine, ephedrine, phenylephrine, naphazoline, and oxymetazoline as ingredients to look for. Its guidance says people with severe or uncontrolled high blood pressure should not take a decongestant. FDA educational guidance also says people with high blood pressure or heart disease should talk with a doctor before using one. Mayo Clinic’s cold-remedy guidance and the FDA’s adult OTC medicine guide explain these precautions.

For a stuffy nose, saline nasal spray is a non-decongestant option suggested by Mayo Clinic and the American Heart Association. Ask a healthcare professional whether it is appropriate for you if you have questions about a particular product.

For fever and aches, weigh the ingredient and your own risks

Mayo Clinic suggests considering aspirin or acetaminophen for fever and minor aches, and cautions that ibuprofen and naproxen may lead to high blood pressure. This is general guidance, not personal clearance: age, pregnancy, liver or kidney disease, allergies, other medicines, and label directions can affect what is appropriate. The sources here do not establish individualized dosing, so follow the product label and ask a professional if you are unsure.

Check the label and your medication list before taking a product

  1. Match the medicine to your symptoms. Avoid combination products that add treatment for symptoms you do not have.
  2. Read active ingredients and warnings. Look for decongestants and warnings for people with high blood pressure or those taking blood-pressure medicine.
  3. Check for duplicates. Compare the active ingredients in all cold, flu, and pain medicines you might take, especially pain relievers such as acetaminophen.
  4. Check sodium content. The American Heart Association notes that some over-the-counter medicines contain high amounts of sodium. Its general guidance is less than 2,300 milligrams per day, with an ideal aim of less than 1,500 milligrams per day; those are general sodium targets, not cold-medicine limits or a personalized recommendation. See the AHA’s medication guidance.
  5. Follow package directions and ask when uncertain. A pharmacist or healthcare professional can check the exact product against your prescriptions and health conditions.

Do not stop prescribed blood-pressure medicine on your own. The American Heart Association advises: “Do not stop taking prescribed medications without talking to your healthcare professional.”

Know when to contact a healthcare professional

Ask a healthcare professional about OTC medicines if you have high blood pressure, take blood-pressure medication, or are unsure whether an ingredient is suitable for you. Contact one if your symptoms worsen instead of improving or last more than 10 days, as Mayo Clinic advises.
