There is no universally best treatment. Stripping cystectomy is the better-established operative option and may lower recurrence and endometriosis-related pain, but removing the cyst wall can remove or injure healthy ovarian tissue and reduce ovarian reserve. Ethanol sclerotherapy is less invasive and appears to preserve ovarian reserve better in comparative reviews, yet its evidence is newer, heterogeneous and generally low certainty. The choice should match your symptoms, ovarian reserve, fertility timeline, cyst features, previous procedures and preferences.

This article explains the evidence for shared decision-making with a gynecologist experienced in endometriosis and fertility care. It cannot determine which procedure is safe or appropriate for an individual.

What the two procedures are

Stripping cystectomy

Stripping cystectomy is an operation in which the surgeon separates and removes the endometrioma wall from the ovary. The aim is to remove the cyst lining rather than simply emptying the cyst. The 2022 European Society of Human Reproduction and Embryology (ESHRE) guideline recommends cystectomy instead of drainage and coagulation when surgery for an ovarian endometrioma is undertaken because cystectomy reduces endometrioma recurrence and endometriosis-associated pain compared with those older approaches. That recommendation is not a direct comparison with ethanol sclerotherapy; it addresses cystectomy versus drainage and coagulation. Read the guideline at ESHRE’s 2022 endometriosis guideline.

Ethanol sclerotherapy

Ethanol sclerotherapy drains the cyst and treats the remaining cavity with ethanol. It is a minimally invasive alternative to excising the cyst wall. Protocols differ in how the cyst is accessed, how long ethanol remains in contact with the cavity and how the treated ovary is followed. Those technical differences matter when comparing studies, so the label ethanol sclerotherapy does not describe one perfectly uniform procedure.

The decision in one comparison

Decision factor Stripping cystectomy Ethanol sclerotherapy How to interpret it
Purpose and approach Operative removal of the cyst wall. Drainage followed by ethanol treatment of the cyst cavity. They are different procedures, and suitability depends on cyst characteristics and the treating clinician’s experience.
Ovarian reserve Healthy ovarian tissue can be removed or injured during excision, reducing reserve. Comparative reviews generally show a more favorable reserve signal. The reserve advantage is promising but based mainly on limited, heterogeneous evidence.
Pain and recurrence ESHRE reports lower recurrence and endometriosis-associated pain than drainage and coagulation. Recurrence varies by protocol and follow-up; the available randomized comparison did not show a significant 12-month difference. No settled long-term winner between cystectomy and ethanol sclerotherapy has been established.
Before assisted reproductive technology Routine surgery solely to improve live birth is not recommended and may harm reserve. Some studies report better reserve and egg-yield outcomes, but they do not prove better live birth for every patient. The reason for intervention should be specific, such as symptoms or an individualized access issue, rather than an assumption that surgery automatically improves IVF.
Evidence maturity A more established guideline pathway when an operation is selected. A promising but less certain alternative, with variable protocols and mostly non-randomized comparative data. Uncertainty is part of the decision and should be discussed explicitly.

How fertility evidence changes the balance

Ovarian reserve is the central trade-off

Endometrioma surgery can reduce ovarian reserve by removing or damaging ovarian parenchyma. The risk is especially consequential when cysts are bilateral, large, recurrent or being treated after previous ovarian surgery. French CNGOF/HAS recommendations specifically note that reserve may be reduced by surgery and that these features heighten concern. They also state, by expert consensus, that ethanol sclerotherapy can be offered for recurrent endometriomas; this is a national recommendation, not a universal international standard.

A 2025 overview of nine systematic reviews found a generally more favorable ovarian-reserve signal with sclerotherapy than with cystectomy, but rated the overall evidence for sclerotherapy low to very low. The most useful interpretation is that avoiding excision may protect more ovarian tissue, not that every patient will experience a measurable or clinically important fertility benefit.

AMH findings are suggestive, not a guarantee of pregnancy

A 2025 meta-analysis of 28 studies involving 1,877 patients found a smaller change in anti-Müllerian hormone (AMH) after ethanol sclerotherapy than after cystectomy. AMH is a marker of ovarian reserve; a better post-treatment marker does not automatically establish a higher chance of live birth for an individual. The analysis also had the limitations common to a literature containing small and observational studies. See the abstract at the 2025 meta-analysis.

What has been reported in assisted reproduction

The same meta-analysis reported higher clinical pregnancy rates and higher total oocyte, metaphase II oocyte and embryo counts after sclerotherapy. Fertilization rates and gonadotrophin dosage did not differ significantly. The abstract does not provide effect sizes for every outcome, so these findings should be described as suggestive associations rather than proof that sclerotherapy is superior for all IVF patients.

A 2025 BJOG review cited a meta-analysis in which clinical pregnancy rates were similar between the two procedures (odds ratio 1.63, 95% confidence interval 0.91–2.9; three studies, 214 women). The interval includes the possibility of no difference. The same review reported a higher oocyte yield with sclerotherapy (mean difference 2.7, 95% confidence interval 0.98–4.4; three studies, 178 women). These small evidence sets cannot predict an individual’s egg yield or live-birth outcome.

When IVF is planned, do not assume that surgery must come first

ESHRE does not recommend routinely operating on an ovarian endometrioma before assisted reproductive technology solely to improve live birth. The guideline reports no demonstrated live-birth benefit and warns that surgery is likely to negatively affect ovarian reserve. A clinician may still recommend treatment for a separate, individualized reason, such as significant symptoms, difficulty accessing follicles, concern about the nature or behavior of a cyst, or another finding that changes the safety of proceeding.

Spontaneous conception remains an evidence gap

The 2025 BJOG review identified no comparative studies of spontaneous conception after endometrioma surgery versus no surgery. Therefore, neither procedure should be presented as proven to improve natural conception in a general population.

What is known about recurrence and pain

Cystectomy compared with older drainage techniques

The ESHRE recommendation for cystectomy is based on lower recurrence and endometriosis-associated pain compared with drainage and coagulation. It should not be stretched into a claim that cystectomy is better than ethanol sclerotherapy on every recurrence or pain measure.

The randomized recurrence comparison

A systematic review indexed in 2026 reported that its included randomized trial found 12-month recurrence of 48.6% after ethanol sclerotherapy and 42.9% after cystectomy, with no statistically significant difference. Across the review’s studies, recurrence reports ranged from 0% to 48.6%, depending on follow-up duration and treatment method. Most evidence was observational and at risk of bias, so this range is not an individual prediction.

Why recurrence numbers vary

Recurrence estimates depend on the ethanol protocol, how recurrence is defined, the length of surveillance, whether one or both ovaries are affected and whether patients have had previous procedures. Ask the treating center which imaging schedule and definition it uses rather than comparing an isolated percentage from one study with another.

Which factors should drive your choice?

1. The reason for intervening now

Start by naming the immediate problem. It may be pain, a cyst that interferes with egg retrieval, uncertain or changing cyst characteristics, a recurrence after previous treatment, or another clinical concern. If the only proposed benefit is a higher future live-birth rate before IVF, ask how that claim is reconciled with ESHRE’s recommendation against routine surgery for that purpose.

2. Your ovarian-reserve starting point

Discuss your baseline ovarian-reserve assessment and how much reserve could be lost if ovarian tissue is removed. A patient with low reserve, bilateral endometriomas, a single functioning ovary, previous ovarian surgery or a need for more than one fertility treatment cycle may place greater weight on tissue preservation. A patient with substantial reserve and severe, recurrent pain may weigh symptom control differently.

3. Fertility timing and treatment plan

Age, the likely time available for conception, whether IVF is planned and whether eggs or embryos might be preserved before treatment all change the decision. If treatment is urgent, an operation that delays care or reduces reserve may carry a different cost than it would for someone who is not pursuing pregnancy soon. Ask whether fertility preservation should be discussed before any procedure.

4. Cyst pattern and treatment history

Record whether the endometrioma is unilateral or bilateral, its size and appearance, whether it has recurred and what was done previously. Large, recurrent and bilateral disease raises the stakes for reserve loss. A recurrent cyst may also be the setting in which a clinician considers ethanol sclerotherapy, consistent with the French expert-consensus recommendation, while explaining that this does not make it the universal first choice.

5. Local expertise and protocol

Outcomes depend on operator skill and follow-up. Ask how often the clinician performs each procedure, which ethanol exposure protocol is used, how complications are handled and how recurrence and ovarian reserve are monitored. A theoretical advantage is less useful if the center has little experience with the technique or no clear follow-up pathway.

6. Your preferences and tolerance for uncertainty

Some patients prioritize the most established operative guideline pathway; others prioritize avoiding ovarian excision even when comparative evidence is less certain. Neither preference is irrational. The decision should state which possible harm you are most trying to avoid: persistent pain, recurrence, reserve loss, procedural complications, delay or an uncertain fertility benefit.

A practical decision process for a clinic visit

  1. Define the treatment goal. Write down whether the priority is pain relief, diagnosis, cyst access for ART, prevention of another complication or a fertility plan.
  2. Review the ovary-specific risks. Ask how unilateral or bilateral disease, prior operations and the current cyst’s size and features affect expected reserve loss.
  3. Separate established findings from hoped-for benefits. Cystectomy has guideline support against drainage and coagulation; sclerotherapy has encouraging reserve and egg-yield signals but low-certainty evidence. Neither procedure has been proven to improve live birth for every patient.
  4. Compare center-specific protocols. Obtain the proposed steps, anesthesia or sedation plan, recovery time, complications, follow-up imaging and criteria for additional treatment.
  5. Coordinate fertility care. If IVF or fertility preservation is planned, have the endometriosis surgeon and fertility specialist agree on sequencing rather than treating the procedures as isolated choices.
  6. Document a fallback plan. Decide what will happen if the cyst recurs, reserve falls, pain persists or fertility treatment produces fewer eggs than expected.

Questions to ask before consenting

  • What is the specific reason you recommend treating this cyst now?
  • What is my baseline ovarian reserve, and how could previous surgery, bilateral cysts or repeat surgery change the risk?
  • Would treatment before IVF improve access or outcomes in my particular anatomy, or is observation reasonable?
  • Which ethanol protocol is available here, and what recurrence follow-up does this center use?
  • What are your relevant outcomes and complication rates for each procedure?
  • How might each option affect the timing of egg retrieval, embryo transfer or attempts at natural conception?
  • What symptoms or imaging findings should prompt urgent reassessment after treatment?
  • If the cyst returns, what would the next treatment be and how would repeat intervention affect reserve?

Risks, adverse events and follow-up

The 2025 overview of nine systematic reviews estimated technical efficacy for ethanol sclerotherapy at 95.0%–98.3%, minor adverse events at 11.0%–12.0% and major adverse events at 1.0%–1.7%. These are pooled estimates reported by the overview’s authors, not guaranteed rates for a particular clinic or protocol. The overview found stronger support for technical efficacy and adverse-event estimates than for fertility or ovarian-reserve outcomes.

For cystectomy, the material risk is loss or injury of healthy ovarian tissue in addition to the ordinary risks of an operation. For sclerotherapy, the material uncertainties include protocol variation, incomplete or temporary treatment and recurrence. Your consent discussion should cover bleeding, infection, injury to nearby structures, anesthesia or sedation issues and the possibility of further treatment, with the exact risks tailored to the proposed approach and your medical history.

Follow-up should include the plan for symptom review, imaging and any repeat ovarian-reserve assessment that your clinicians consider useful. A single AMH result or one ultrasound cannot summarize pain control, cyst behavior and fertility potential together; interpretation requires the clinical context and your reproductive goals.

What the evidence still cannot answer

  • There are few randomized comparisons, and much of the literature is observational.
  • Ethanol sclerotherapy protocols and follow-up schedules are heterogeneous.
  • Studies do not establish one long-term winner for pain, recurrence, ovarian reserve, spontaneous conception and live birth considered together.
  • A better ovarian-reserve marker does not prove a better live-birth outcome for an individual.
  • Reported IVF advantages may reflect differences between patient groups as well as the procedure itself.

For that reason, the most defensible choice is not a universal rule. Use cystectomy when an operation is indicated and the expected benefits outweigh reserve concerns; consider ethanol sclerotherapy when preserving ovarian tissue is a dominant priority and an experienced team offers a suitable, well-monitored protocol. Make the final decision jointly with specialists who can review your cyst, reserve, fertility timeline and previous treatment in detail.
