How to Choose Between Healthcare-Specific AI and General-Purpose AI Tools

Choose the tool for the job and its risk—not for the words “healthcare AI” on the homepage. A healthcare-specific product may still lack clinical validation, authorization, or HIPAA coverage; a general-purpose model may be reasonable for a low-risk administrative draft when your organization permits it and controls the data. For any clinical or patient-facing function, verify the product’s exact intended use, evidence, regulatory position, data practices, human oversight, and lifecycle controls before deployment.

Start with the task, not the category

“Healthcare-specific” describes positioning, not a guarantee of safety or performance. “General-purpose” describes a model’s intended breadth, not proof that it can perform every healthcare task reliably. The first decision is the function you need to perform, the people who will rely on the output, and the harm that could result from an error.

FDA’s governing principle is function-based: “The FDA does not regulate AI as such; it regulates medical devices, including AI-enabled medical devices.” Its risk-based approach considers a product’s intended use and technological characteristics. That means two tools using similar underlying models can face very different obligations if one drafts internal meeting notes and the other recommends a diagnosis or treatment.

Use this sequence before comparing vendors:

  1. Define the output. Write down exactly what the system will generate, summarize, classify, recommend, or trigger.
  2. Name the user and affected person. A clinician, call-center worker, patient, caregiver, researcher, and billing specialist have different needs and risks.
  3. Classify the consequence of error. An incorrect calendar summary is inconvenient; an incorrect medication recommendation can injure someone.
  4. Set the required evidence and controls. Higher-risk uses need evidence that matches the task, population, setting, workflow, and version, plus meaningful human review.
  5. Check the actual data flow. Determine whether identifiable health information is sent to the service, retained, reused, or exposed to subcontractors.

Only after these questions are answered should you compare a healthcare-focused product with a general-purpose model.

Clinical decision support

Clinical decision support (CDS) can include displaying patient-specific information, helping a professional evaluate options, or producing a recommendation. Document the exact action: for example, “flags possible drug interactions for a pharmacist” is materially different from “selects the patient’s medication.” Identify whether the clinician can independently review the basis for the output and whether the tool can be overridden.

FDA’s final Clinical Decision Support Software Guidance, dated January 2026, explains that some software functions can meet statutory criteria for exclusion from the device definition. Other functions that meet the device definition remain subject to existing digital-health policies. The guidance also addresses functions intended for patients or caregivers; a patient-facing feature is not outside consideration merely because a clinician is not the user.

For these uses, a product’s healthcare label is insufficient. Ask for the intended-use statement, applicable U.S. authorization or clearance, evaluation methods, known failure modes, and the release version covered by the evidence.

Patient-facing assistance

Patient portals, symptom assistants, discharge explainers, and appointment agents can affect care even when they do not make a formal diagnosis. A wrong answer may delay treatment, expose private information, or cause a person to ignore urgent symptoms. Define escalation rules, emergency language, supported languages, accessibility needs, and the point at which the system must hand off to a person.

Check whether the vendor designed and evaluated the feature for the actual patient population and communication setting. Evidence from a clinician-only workflow does not automatically establish safety for unsupervised public use.

Administrative and operational work

Drafting a nonclinical email, converting a meeting into action items, organizing supply requests, or proposing a first-pass schedule is generally lower risk than making a treatment recommendation. A general-purpose model can be a candidate for this work when organizational policy allows it, confidential data is handled appropriately, and a person verifies the result before it is sent or entered into a record.

“Lower risk” does not mean “no risk.” Administrative text can still contain protected health information, incorrect billing details, or statements that become part of a legal or clinical record. Define what may be pasted into the tool and what must remain in an approved system.

Compare products on the evidence that matches your use

Ask vendors to map each claim to a specific function, version, user, setting, and population. A generic accuracy percentage or a demonstration using selected examples cannot substitute for that mapping.

Decision axis Questions to ask Why it matters
Task and intended user What exact function is claimed? Is it for clinicians, patients, administrators, or another user? The same model can carry different risk and regulatory consequences in different workflows.
Evidence and scope What evaluation covers the exact task, population, setting, workflow, and version? What failures were observed? Evidence outside your context may not predict performance in it.
Regulatory context Does the function meet a device definition or fit a relevant non-device policy? What specific U.S. authorization or clearance applies, if any? Category names and marketing claims do not establish regulatory status.
Human oversight Who checks the output? What can they inspect? How do they escalate an uncertain or wrong result? Review must be practical, timely, and performed by someone qualified for the decision.
Data handling Does the service create, receive, maintain, or transmit identifiable health data? What are retention, permitted-use, deletion, security, and subcontractor terms? The data flow determines privacy obligations and contractual requirements.
Integration and operations Will it fit existing systems? How are updates, incidents, downtime, and performance changes handled? A good demo can fail when it adds unsafe work or changes behavior without notice.
Total cost and support What are licensing, implementation, training, support, monitoring, and contract costs? Official sources do not establish current head-to-head pricing; obtain comparable quotes directly.

Understand what “healthcare-specific” actually means

Vendors use the term for different things: a model trained or fine-tuned on medical text, a workflow wrapped around a general model, a product connected to an electronic health record, a clinical rules engine, or a service marketed to hospitals. Ask which of these describes the product you are considering.

  • Domain adaptation: Medical vocabulary or specialty examples may improve terminology while leaving reasoning, calibration, or rare-condition weaknesses unresolved.
  • Workflow packaging: Templates, permissions, audit logs, and escalation paths can be valuable even when the underlying model is not unique.
  • Clinical claims: A claim that a tool “supports diagnosis” or “improves outcomes” requires evidence for that exact claim, not just medical-sounding training data.
  • Data access: A connection to clinical records can make a system more useful and also increase privacy, security, and governance obligations.

Do not infer HIPAA compliance, FDA authorization, clinical validity, or superior accuracy from the label alone. Verify each property separately in the product documentation and contract.

Do not assume a general-purpose model can do everything

WHO’s guidance on large multimodal models, published March 25, 2025, notes that these systems are also called “general-purpose foundation models,” while cautioning that it has not yet been proven whether they can accomplish a wide range of tasks and purposes. Broad capability is therefore an assertion to test, not a qualification to assume.

General-purpose tools can be attractive for drafting, extraction, translation, brainstorming, and other reversible tasks because they are readily available and flexible. Their weaknesses can include confident fabrication, unstable outputs, sensitivity to prompts, missing clinical context, and changes after a model update. They may also lack medical-device documentation, specialty validation, or a workflow for urgent escalation.

For a lower-risk use, establish a bounded prompt, approved input types, a reviewer, and a way to report failures. For a clinical use, require product-specific evidence and governance rather than treating a successful informal trial as validation.

Apply the U.S. regulatory framework carefully

Clinical decision support guidance

The January 2026 FDA CDS guidance is U.S.-specific. It explains statutory criteria under which certain CDS functions may be excluded from the device definition and clarifies that functions meeting the device definition remain subject to existing digital-health policies. The determination applies to the function and how it is intended to be used, not to a company’s overall identity or a product’s “AI” label.

Record the jurisdiction in your procurement file. A conclusion about U.S. device status should not be presented as a conclusion for another country, where definitions, authorization routes, and privacy laws may differ.

Authorized AI-enabled devices and lifecycle duties

FDA describes lifecycle considerations from development and validation through deployment, monitoring, maintenance, and modification. Ask how the vendor will detect performance drift, investigate incidents, document updates, and notify customers when a change affects the evidence or workflow.

FDA reported more than 1,600 AI-enabled medical devices authorized for U.S. marketing as of September 2026 and says its searchable inventory is updated periodically. Treat that number as a dated agency figure, not a permanent total and not evidence that every listed device is suitable for your task. Verify the specific product and current entry before relying on it.

Generative-AI discussion material

FDA’s “Considerations for the Regulation of Generative AI-Enabled Medical Devices” page requests comments by October 19, 2026 on topics including risk assessment, premarket evaluation, and postmarket monitoring. FDA characterizes the paper as discussion material, not draft or final guidance, and says it does not communicate proposed regulatory expectations. Do not treat its topics as binding requirements.

Trace the data before entering anything

Make a simple data-flow diagram: user device, application, model provider, hosting environment, integrations, logs, support staff, and subcontractors. Mark every point where identifiable health information is created, received, maintained, or transmitted.

When a business associate relationship may apply

HHS guidance says a cloud service provider that processes or stores electronic protected health information (ePHI) on behalf of a covered entity or business associate generally needs a HIPAA-compliant business associate agreement (BAA). The covered entity or business associate must understand the particular service and conduct its own risk analysis.

HHS also distinguishes a vendor that merely supplies software without access to PHI from one that needs PHI access to provide the service. Access can make the vendor a business associate. HHS gives a third-party AI chatbot on a provider portal that handles PHI as an example of a business-associate situation.

Contract questions

  • Is the vendor acting on your behalf with ePHI, and will it sign the required BAA?
  • Are prompts, outputs, attachments, and metadata used to train or improve any model?
  • How long are data and logs retained, and can you require return or deletion?
  • Where are data processed, who can access them for support, and what subcontractors are involved?
  • What encryption, access controls, audit records, breach notices, and continuity commitments apply?
  • Can the vendor change the model, hosting location, or terms without your approval?

HHS does not endorse, certify, or recommend specific technology or products. A BAA is important, but it does not replace your organization’s risk analysis, access controls, workforce training, and other HIPAA duties.

Design human oversight that works in practice

“Human in the loop” is meaningful only when the reviewer has the time, expertise, information, and authority to challenge the system. Define the review point before launch.

For clinical output

  • Assign a licensed or otherwise qualified reviewer appropriate to the decision.
  • Show the source information, assumptions, confidence or uncertainty signals when available, and relevant limitations.
  • Require independent verification for diagnoses, medication changes, triage, referrals, and other high-consequence actions.
  • Provide a documented escalation route for ambiguous, urgent, or out-of-scope cases.
  • Prevent automatic execution until the reviewer accepts the result.

For administrative output

  • Use a named owner who checks names, dates, numbers, commitments, and unintended PHI disclosure.
  • Keep generated text visibly marked as a draft until approved.
  • Retain the final human-approved version and, where appropriate, the original output for audit.

Test the review process under realistic workload. If staff routinely accept suggestions without checking them, the nominal control is not the real control.

Plan for updates, drift, and downtime

AI performance can change when the model, retrieval source, prompt template, connected data, or user population changes. Ask the vendor to distinguish ordinary maintenance from a material change that requires revalidation.

  1. Baseline before launch: Define representative cases, expected outputs, unacceptable errors, and measures such as false positives, false negatives, turnaround time, and override rates.
  2. Pilot in a controlled setting: Start with limited users and cases. Keep the system advisory while reviewers compare it with current practice.
  3. Monitor after deployment: Track errors, complaints, escalations, disparities across relevant groups, latency, outages, and changes in user behavior.
  4. Review every update: Record the version, release date, changed capabilities, new limitations, and whether existing evidence still applies.
  5. Maintain a fallback: Document how work continues when the service is unavailable, degraded, or withdrawn.

FDA’s lifecycle framing covers development and validation as well as deployment, monitoring, maintenance, and modification. Procurement should therefore include post-launch obligations, not end with an initial accuracy demonstration.

A practical selection procedure

Step 1: Write a one-page intended-use statement

Include the task, user, affected population, setting, inputs, output, decisions influenced, exclusions, and escalation path. Avoid broad language such as “improves care.” State what the system will and will not do.

Step 2: Assign a risk tier

Consider severity and reversibility of harm, whether a person can detect the error, how quickly the output is acted on, and whether the system touches ePHI. Treat patient-facing and clinical recommendations as higher risk than an internal draft that never reaches a patient record.

Step 3: Request a product evidence packet

Ask for intended use, user instructions, evaluation protocol, population and setting, performance results, limitations, known failure modes, current version, change history, and applicable regulatory documentation. Reject a packet that offers only a broad benchmark or marketing case study.

Step 4: Conduct a privacy and security review

Map data flows, determine whether a BAA is required, review retention and model-training terms, assess access controls, and complete your organization’s risk analysis. Do not paste live patient data into an unapproved public tool for a trial.

Step 5: Test the workflow, not just the model

Use representative cases, including ambiguous, incomplete, rare, and out-of-scope inputs. Measure whether staff can review results, whether alerts are actionable, and whether the tool adds unsafe delays or duplicate documentation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 6: Approve governance and contracts

Name an accountable owner, clinical or operational reviewer, privacy and security contacts, incident route, update-approval process, and retirement plan. Put service levels, breach notice, data deletion, audit access, and change notification into the contract where appropriate.

Step 7: Reassess at defined intervals

Set review dates and triggers such as a major model update, new population, new integration, safety incident, regulatory change, or material drift in performance.

How the choice changes by scenario

Scenario Reasonable starting point Required safeguards
Drafting an internal, nonclinical meeting summary A general-purpose tool may be considered if policy permits. Exclude unnecessary PHI, verify names and actions, mark as draft, and use an approved service with suitable data terms.
Summarizing a patient record for a clinician A healthcare workflow may offer useful integration, but neither category is automatically appropriate. Evidence for the exact record type and population, clinician review, source visibility, audit trail, and downtime procedure.
Suggesting a diagnosis or treatment Require product-specific clinical evidence and regulatory analysis before deployment. Qualified oversight, independent verification, escalation, monitoring, and documented authorization or device status where applicable.
Patient symptom or triage assistant Evaluate a patient-facing product specifically designed and tested for that interaction. Urgency rules, safe handoff, clear limits, accessibility, privacy controls, and testing with representative users.
Billing or coding draft Either category can be considered if the function is bounded. Check codes and payer rules, protect ePHI, retain human approval, and monitor error and denial rates.

Common failure modes and fixes

“It is healthcare-specific, so it is compliant.”

Why it fails: A marketing category does not prove HIPAA coverage, authorization, or clinical validity.

Fix: Verify intended use, evidence, regulatory position, data terms, and oversight independently.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“It passed a general benchmark.”

Why it fails: A benchmark may not represent your users, records, language, prevalence, or workflow.

Fix: Request task- and population-specific evaluation and run a controlled local pilot.

“A clinician can look at it later.”

Why it fails: Delayed or overloaded review can become automatic acceptance.

Fix: Put review before consequential action, show supporting information, and measure overrides and missed errors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“We removed names, so privacy is solved.”

Why it fails: Free text, dates, rare conditions, images, identifiers in metadata, and linked records can still identify a person.

Fix: Use approved de-identification methods and services, minimize inputs, and complete the required risk analysis.

“The model is general-purpose, so we can expand later.”

Why it fails: Breadth of interface does not establish capability for new clinical tasks.

Fix: Treat every new function, population, or setting as a new intended-use decision requiring evidence and governance.

“The product was validated once.”

Why it fails: Updates, data shifts, and workflow changes can alter performance.

Fix: Require version records, change notices, monitoring, revalidation triggers, and a rollback or fallback plan.

Buyer’s pre-deployment checklist

  • Exact task, user, population, setting, inputs, outputs, exclusions, and consequences are documented.
  • Evidence covers the intended workflow and current product version.
  • Known limitations, failure modes, uncertainty behavior, and out-of-scope cases are available to users.
  • U.S. device or non-device analysis is documented where relevant; other jurisdictions are assessed separately.
  • Patient-facing and clinical functions have qualified review and escalation before consequential action.
  • Data flows, retention, training use, subcontractors, security, deletion, and support access are understood.
  • A BAA is in place when the vendor handles ePHI on behalf of a covered entity or business associate.
  • The organization has completed its own privacy and security risk analysis.
  • Integration, permissions, audit logs, downtime, incident response, and update procedures are tested.
  • Costs include implementation, training, monitoring, support, and contract obligations—not only the subscription.
  • An accountable owner and review schedule are assigned.

The decision rule

Choose a healthcare-specific tool when its documented intended use, evidence, regulatory context, data controls, workflow integration, oversight, and lifecycle support fit the clinical or patient-facing job. Choose a general-purpose tool only for a clearly bounded use whose risk your organization can control, with approved data handling and human verification. If neither option can demonstrate a fit for the intended use, do not deploy it for that use; redesign the task or keep the existing process.
