The right choice depends on what you need now. A therapist offers private, individualized assessment and treatment; a support group offers shared experience and regular peer connection. Loneliness does not automatically mean you have a mental disorder, and you do not have to choose only one. Use the steps below to judge safety, fit, access and usefulness before committing.
Start by identifying the kind of help you want
Loneliness can mean wanting more contact, feeling disconnected despite being around people, struggling after a move or loss, or experiencing distress that is interfering with sleep, work, study or relationships. Those experiences call for different kinds of support.

| Option | Main purpose | What to evaluate |
|---|---|---|
| Individual or group psychotherapy | Professional assessment and a treatment plan tailored to your needs and medical situation | Credentials, relevant experience, treatment approach, rapport, privacy, cost and progress review |
| Peer support group | Shared experience, mutual understanding, hope and regular connection | Group focus, norms, psychological comfort, accessibility, schedule, cost and privacy practices |
| Both | Professional care alongside peer connection | Whether the two settings have compatible expectations and whether each remains useful |
The World Health Organization describes peer support as sharing experience, hope and empowerment in person-centered, rights-based services. Its guidance is for service design, not a guarantee that every group is safe or suitable. Read the WHO guidance on peer support mental health services for that framework.
When therapy may be worth considering
Consider a professional consultation when loneliness is persistent, is affecting daily functioning, or leaves you with questions that need assessment. Therapy can also help when loneliness appears alongside anxiety, low mood, grief, trauma, relationship difficulties, substance use or changes in sleep and appetite. You do not need a diagnosis to ask for an appointment.

A clinician can help determine whether loneliness is connected to a mental-health condition, a medical issue, a major life change or circumstances such as isolation and discrimination. Treatment should be tailored rather than selected from a universal “loneliness cure.” The US National Institute of Mental Health (NIMH) puts it this way: “Choosing the right treatment plan is based on a person’s individual needs and medical situation and should occur under the guidance of a mental health professional.” See NIMH’s psychotherapy guidance.
Therapy is not the only valid response. If your main need is safe company and people who understand a shared experience, a peer group may be a better first step. You can also begin with a group and seek professional care later, or use both at the same time.
How to choose a therapist
1. Define what you want help with
Before contacting providers, write a short description of the problem in your own words. Note how long it has been present, where it shows up, what makes it better or worse, and what you would like to change. Examples include “I have moved and have no social network,” “I avoid invitations because I expect rejection,” or “I feel alone even with family.” This gives a therapist enough context to explain whether their experience is relevant.

2. Verify credentials and local authorization
Ask what license, registration or professional credential the person holds and which authority issued it. Rules and protected titles differ by country, state and profession, so verify the listing with your local regulator rather than relying only on a directory profile. Ask how many years they have practiced and whether they regularly work with your age group and concerns.
NIMH recommends asking about credentials, specialty, experience with the concern, treatment approach, goals, progress checks, medication options and confidentiality. The American Psychological Association’s US-focused page, created in 2019, similarly advises checking licensure and competence. Payment and licensing rules can change, so confirm current details with the provider and your insurer. See APA’s questions for choosing a psychologist.

3. Ask what approach they use and why
Request the plain-language name of the therapy, how it is expected to help, and what sessions usually involve. Ask whether the approach has evidence for the problems you described and how it would be adapted to your goals, culture, communication needs or disability. A responsible answer should explain the rationale without promising a guaranteed result.
Ask whether the therapist works one-to-one, in groups, or both; how often sessions are usually scheduled; and whether they coordinate with a doctor or prescribe medication. In many places therapists cannot prescribe, while some medical professionals can. The important point is to know who will handle each part of care.

A short call or first appointment is an interview in both directions. NIMH says, “Once you have identified one or more possible therapists, a preliminary conversation can help you understand how treatment will proceed and if you feel comfortable with the therapist.” Prepare questions, but also notice whether you can speak freely, whether the therapist listens without rushing, and whether explanations are understandable.
Useful questions include:
- What experience do you have with loneliness, isolation, grief, social anxiety or my specific situation?
- What would we work on first, and how would we define progress?
- How often would we review goals and decide whether to continue, change approach or refer elsewhere?
- What happens if I miss an appointment or need help between sessions?
- How do you protect confidentiality, and what are its legal or safety limits?
Rapport does not mean the therapist agrees with everything you say. It means you can work together with respect, clear boundaries and enough trust to discuss difficult material.
5. Confirm cost, coverage and practical access
Ask for the full fee, cancellation policy, billing method and whether the provider accepts your insurance or public coverage. If you are paying yourself, ask about a sliding scale, reduced-fee slots, trainee clinics, community services or other lower-cost options. Confirm whether the therapist is accepting new clients, how long the wait is, and whether appointment times, travel, language, disability access and online sessions work for you.

6. Clarify privacy and information sharing
Ask where sessions take place, who can access records, whether telehealth uses a secure platform, and when information may be disclosed without your permission. Confidentiality exceptions commonly involve immediate safety concerns or legal requirements, but the exact rules depend on your jurisdiction and profession. Ask for the provider’s written policy before sharing sensitive information.
7. Review progress instead of waiting indefinitely
Agree on a reasonable point to review whether therapy is helping. Progress may involve less distress, better functioning, more confidence in relationships, or a clearer understanding of your needs; it is not always a rapid disappearance of loneliness. If you are not improving after a period that feels reasonable, raise that directly. NIMH advises discussing the lack of improvement with the therapist and considering another professional or approach.
How to choose a support group
Match the group to your experience and purpose
Groups vary widely. Some focus on a shared life experience, bereavement, disability, cultural identity, caregiving, recovery or a particular mental-health concern. Others are organized around an activity or interest and create connection without asking members to discuss symptoms. Choose a focus that feels relevant rather than assuming a group labeled “loneliness” will fit everyone.
SAMHSA describes national and local groups, with many offering online and in-person meetings. Its examples include NAMI, Alcoholics Anonymous, Narcotics Anonymous and SMART Recovery; these appear on a broad mental-health, drugs and alcohol resource page and are not endorsements or evidence that every group addresses loneliness. State, county and local agencies may list additional options. See SAMHSA’s support-group and local-program page.

Ask about format, schedule and accessibility
Find out whether meetings are online, in person or hybrid; how often they occur; how long they last; and whether registration is required. Check location, transport, hearing or mobility access, language, captions, childcare and technology requirements. Ask whether there is a cost, membership requirement or suggested donation.
If attending a room full of strangers feels overwhelming, an online or activity-based group may be a gentler starting point. Participants and stakeholders in a UK government report described online connections as a way to decide whether and when to meet in person, while also noting that some people need practical or emotional help to access groups. These are reported experiences, not a universal solution. The report, Mental health and loneliness: the relationship across life stages, also records that time and cost can block attendance.
Learn the group’s rules before you disclose
Ask the organizer how meetings are facilitated, who may attend, whether speaking is optional, and how disruptive, discriminatory or unsafe behavior is handled. Ask whether members are allowed to contact one another privately and what happens if someone breaches boundaries. Do not assume that a peer group offers the same legally protected confidentiality as a clinical service.
A well-run group should explain its purpose, participation expectations and referral process when a member needs professional or emergency help. You can attend once, listen without speaking, and leave if the tone or boundaries do not feel right.
Judge psychological comfort, not just topic match
A group can be relevant yet still feel intimidating or stigmatizing. The UK report includes accounts of anxiety about attending, fear of judgment and groups that did not match people’s circumstances. Look for signs of respect: members are not pressured to tell personal stories, different viewpoints can be expressed safely, and facilitators stop harassment or advice that could cause harm. It is reasonable to try more than one group.

Finding options and making first contact
For US readers
NIMH points to professional directories, insurer provider lists, universities, medical schools and local or state health departments. Directory listings may be incomplete and do not constitute NIMH endorsement, so verify credentials and availability directly. SAMHSA’s pages can help locate care and groups, including lower-cost services.
For readers elsewhere
Use your country’s professional regulator, public-health service, insurer or primary-care provider to confirm credentials and payment arrangements. Libraries, community centers, universities and local voluntary organizations may know about peer groups. Ask who runs the group and how current the meeting information is; schedules and eligibility can change.
A message you can send
Keep the first inquiry factual and brief:
Hello. I am looking for support with ongoing loneliness and would like to know whether you work with this concern. Could you tell me your credentials, approach, fees or insurance arrangements, current availability, session format, and how you review progress? Thank you.
For a group, ask about its focus, meeting format, cost, accessibility, participation rules, facilitator and what a first visit involves.
Common obstacles and ways around them
- Cost: Ask about public services, sliding-scale places, training clinics, group therapy or free peer meetings. Confirm the total cost, not only the advertised session fee.
- Waiting lists: Contact several qualified providers, ask about cancellations, and use a support group or primary-care appointment while waiting if that feels safe.
- Anxiety about attending: Start with an online meeting, an activity-based gathering or a trusted person who can help you make the first contact. You can listen without speaking.
- Poor fit: State what is not working. A therapist may adjust the plan or refer you; a group organizer may suggest another meeting. Leaving a group that feels judgmental or unsafe is appropriate.
- Privacy worries: Ask explicit questions before disclosure, use a private space for telehealth, and learn how records and member information are handled.
When to seek urgent help
Loneliness can coexist with severe distress or thoughts of self-harm. If you may act on suicidal thoughts, cannot stay safe, or face an immediate emergency, contact local emergency services or a crisis service now. In the United States, NIMH lists the free, confidential, 24/7 988 Suicide & Crisis Lifeline, available by call or text. 988 is a US service; readers elsewhere should use their country’s emergency number or crisis line.
A decision checklist
Before choosing a therapist
- I can describe what I want help with and what improvement would look like.
- I verified the provider’s current local credential.
- I asked about relevant experience, approach, goals and progress reviews.
- I know the fee, coverage, cancellation policy, availability and format.
- I understand confidentiality and its limits.
- I felt able to ask questions without being dismissed or pressured.
Before joining a group
- The group’s purpose and member focus match what I am seeking.
- I know when and where it meets, what it costs and what access it requires.
- I understand whether participation is optional and who facilitates.
- I know how privacy, contact between members and harmful behavior are handled.
- I have an exit plan if the meeting feels unsafe or overwhelming.
Choose the option that meets your immediate need, then reassess based on your experience rather than on a promise of a guaranteed outcome. Professional care and peer connection can complement each other, and changing providers or groups is part of finding a workable fit.
