Choose a shampoo for the problem it can actually address: use a gentle cleanser to care for fragile strands, and use a dandruff shampoo only when you have flaking, scale, or itch that calls for it. Neither kind of shampoo is a general hair-regrowth treatment. Persistent or unexplained thinning deserves evaluation by a board-certified dermatologist, because hair loss has many possible causes.
Which shampoo should I use if my hair is thinning and my scalp is itchy?
Separate scalp care from hair-loss care. If itch comes with dandruff-like flakes, choose a dandruff shampoo whose active ingredient and directions match your symptoms and hair type. If your scalp is not flaky or irritated, a gentle shampoo is a sensible starting point for cleansing. In either case, the shampoo is not a diagnosis or a treatment for the underlying cause of thinning.

Dandruff is not evidence of poor hygiene. Mona Sadeghpour, MD, FAAD, a board-certified dermatologist and American Academy of Dermatology Association (AAD) spokesperson, called that idea “a common misconception” in a December 12, 2023 AAD news release: AAD on dandruff myths.

How to compare gentle and dandruff shampoos
| Choice | What it is for | What to check | What it cannot establish |
|---|---|---|---|
| Gentle shampoo | Routine scalp cleansing while limiting rough handling of fragile hair. | Whether the formula suits your scalp and avoids ingredients you know you react to. | It does not show why hair is thinning or promise regrowth. |
| Medicated dandruff shampoo | Flaking, scale, or itch associated with dandruff or another scalp condition for which the product is appropriate. | The active ingredient, package directions, contact time, and frequency; formulas and directions differ. | It does not treat every cause of hair loss, and dandruff-like scaling can have different causes. |
For dandruff, AAD lists zinc pyrithione, salicylic acid, sulfur, selenium sulfide, ketoconazole, and coal tar among ingredient options. If one shampoo does not help, alternating products with different active ingredients may help, but follow each product’s label and seek medical advice if symptoms persist. See AAD’s dandruff guidance.

How to use shampoo without adding avoidable damage
For routine cleansing
- Wet hair and gently work shampoo over the scalp rather than scrubbing lengths aggressively.
- Rinse thoroughly, then apply moisturizing conditioner after every shampoo, especially to the lengths.
- Use a leave-in conditioner or detangler if helpful. AAD says conditioner coats strands and can reduce breakage and split ends; these measures protect hair shafts, not regrowth.
Thinning or falling hair can be fragile and easily damaged, so gentler handling matters. AAD’s supportive-care advice is at tips for caring for thinning or falling hair.
For dandruff shampoo
- Apply it to the scalp, not just the hair lengths.
- Follow the bottle’s own instructions for frequency and how long to leave it on. Some dandruff shampoos need 5–10 minutes of scalp contact, but that timing does not apply to every product. AAD explains this in its dandruff guidance.
- If your hair is curly or coily and medicated shampoo dries it, AAD suggests concentrating the product on the scalp and using regular shampoo and conditioner afterward as needed for hair care.
How often should you wash?
There is no single schedule for every scalp and texture. Fine or straight hair, or an oily scalp, may call for more frequent washing; coarse, curly, or coily hair may need less frequent medicated-shampoo use. Follow the product label and any clinician’s plan. For seborrheic dermatitis, AAD gives examples of dandruff-shampoo use 2–3 times a week for straight or wavy hair and once weekly for curly or tightly coiled hair. These are examples, not universal instructions: AAD’s seborrheic dermatitis self-care guidance.

What a product label can—and cannot—tell you
As one U.S. example, the DailyMed label for Nizoral Anti-Dandruff Shampoo lists ketoconazole 1% and says it controls flaking, scaling, and itching associated with dandruff. For adults and children aged 12 and older, the label directs use every 3–4 days for up to 8 weeks, then as needed. Children under 12 should ask a doctor. The label says to stop and ask a doctor if a rash appears, the condition worsens, or it does not improve in 2–4 weeks. Check the current package label because product labeling and availability can change; do not apply this schedule to other ketoconazole products or dandruff shampoos. See the DailyMed Nizoral label.
Can dandruff shampoo make hair loss worse?
Dandruff shampoos are not hair-regrowth products, and some medicated ingredients can dry hair. Dryness may make fragile strands harder to manage, so use the shampoo as directed, apply it to the scalp, and condition the lengths. Stop using a product and follow its label’s advice if a rash develops or irritation worsens. Do not infer from breakage or scalp symptoms alone what is causing hair loss.

When to stop experimenting and see a dermatologist
See a board-certified dermatologist if hair loss is persistent or unexplained, or if your scalp symptoms are severe, persistent, painful, or inflamed. Dandruff-like scaling can be seborrheic dermatitis, psoriasis, fungal infection, or eczema, and treatment depends on the cause. AAD advises consulting a dermatologist for severe dandruff or dandruff that does not improve with home care: AAD’s dandruff guidance.
A dermatologist can investigate the cause and discuss treatments where appropriate. AAD describes minoxidil as a scalp-applied treatment that can help some early hair loss; it is not a shampoo, and results often take 6–12 months. Details: AAD on hair-loss treatment.
