How to choose a robotic exoskeleton for rehabilitation

Choose a robotic exoskeleton with a rehabilitation clinician, not from a product list. Start with the person’s functional goal and specific impairments, then match those needs to the device’s fit, supported joints, assistance settings, step-initiation method, intended setting and current eligibility rules. A powered exoskeleton cleared for sale is not automatically appropriate for a particular patient.

What a rehabilitation exoskeleton is—and what choosing one involves

A powered lower-extremity exoskeleton is a wearable motorized orthosis that uses controllers and/or sensors to help move one or more lower-limb joints. In the United States, the Food and Drug Administration (FDA) classifies these devices as Class II prescription medical devices under regulation 890.3480, product code PHL. That classification describes a regulatory category; it does not establish that a model is suitable for a particular person or diagnosis.

For rehabilitation, the meaningful choice is not simply which machine has the most motors. It is which system can deliver the right amount and type of support for a defined therapy objective while fitting the person safely. Supported standing, repeated stepping in a clinic, gait practice with progressively less assistance and personal or community mobility are different goals. A system designed for one may be unsuitable for another.

The 2022 clinical utilization framework by Arun Jayaraman and colleagues captures the central principle: “Pairing a patient’s clinical presentation to the device design and functionality is the most critical step in effective exoskeleton utilization.” Use that principle as the starting point for every decision.

Use a four-step selection process

1. Define the functional goal

Write down what the person is expected to do with the device and where that activity will occur. A therapy team may be trying to provide upright loading, practice a high number of assisted steps, rehearse a particular gait phase, or evaluate whether a person can use a device outside the clinic. These aims should not be treated as interchangeable.

  • Supported standing: The priority may be stable upright positioning and tolerance for time on the feet, not independent forward walking.
  • Supervised stepping: A rehabilitation program may use repeated, therapist-supervised steps as part of task practice.
  • Adjustable gait training: The team may need to reduce assistance as voluntary control improves or increase it when fatigue appears.
  • Personal or community mobility: A personal-use trial adds requirements for transport, charging, donning and doffing, environmental access, caregiver support and safe use outside a trained facility.

Record a measurable target, such as a duration of supported standing, a number of supervised step cycles, a gait-phase skill or a specific transfer task. The target gives the team a basis for deciding whether a trial is useful and when to reassess it.

2. Describe the impairment and required support

Ask which body functions limit the target activity. The answer may involve trunk control, one or both legs, stance stability, swing clearance, hip or knee movement, ankle positioning, balance or the ability to initiate a step. A device that supplies strong bilateral stance support may be valuable for one presentation but unnecessarily restrictive for someone who needs only assistance with a single gait phase.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Describe the person’s available abilities as well as deficits. Can they maintain an upright trunk? Shift weight? Follow a therapist’s cue? Produce voluntary muscle activity? Tolerate repeated standing? These capacities influence how a step can be triggered and how much powered help is appropriate.

Consider the expected course of therapy. If the goal is to build voluntary control, ask whether assistance can be reduced or targeted to a joint or gait phase. If the immediate priority is safe repetition, a system that offers more comprehensive support may be considered. The clinician should connect the assistance strategy to the treatment plan rather than choosing a model by reputation.

3. Verify fit, health and model-specific eligibility

The treating clinician and trained device team should screen the person against the current instructions and eligibility criteria for the exact model. The screening is individual; manufacturer criteria are not universal rules for every exoskeleton.

Common areas for review include body dimensions, hip width, weight and height limits stated for that model, available hip and knee range of motion, upper-body and trunk strength, skin condition at every contact surface, bone health, spasticity, cardiovascular status, standing tolerance and cognition. The team may also examine pain, contractures, balance, fatigue, communication, caregiver availability and any condition that could make a fall or prolonged loading unsafe.

For example, manufacturer information for the Indego system lists height, weight, hip width, upper-body strength, bone health, intact skin, spasticity and functional range of motion among candidate considerations. It also states that medical eligibility is determined case by case and that physician approval is required. Those statements illustrate the type of screening to expect; they should not be copied as universal thresholds for other systems.

4. Compare movement assistance and plan a supervised trial

Once a person appears medically and physically eligible, compare how each candidate produces and modifies movement. Ask which joints are powered, whether assistance can be reduced, how a step starts, what feedback the user receives and how therapists program progression. A supervised trial should use the person’s stated goal and pre-agreed measures, with reassessment after enough sessions to judge tolerance and training value.

Compare devices by the features that change therapy

Selection axis Questions for the clinical team Why it matters
Intended use and setting Is the model intended for rehabilitation-institution use, personal use, or both under specified conditions? What supervision is required? Training, staffing, environmental requirements and safety procedures differ between a clinic and a home or community setting.
Support location Does it support the trunk, both legs or selected joints? The distribution of support should match the person’s trunk, stance, swing and balance deficits.
Assistance and progression Can a therapist move from high powered assistance to partial assistance, or target a joint or gait phase? Adjustability allows the program to reflect fatigue, recovery and the intended motor-learning challenge.
Step initiation Does stepping begin after a therapist cue, weight shift, trunk movement, voluntary muscle signal or another input? The initiation method must match what the person can reliably do and what the therapy is trying to train.
Feedback What visual, auditory, tactile or movement feedback does the system provide, and can the therapist change it? Feedback and programming shape the training experience; they are not merely cosmetic hardware features.
Fit and handling Can the frame be adjusted to the person’s dimensions and range of motion? Can staff safely put it on, remove it and inspect the skin? Poor fit, difficult handling or a mismatch between the frame and the body can undermine use and increase risk.
Evidence and practical access What evidence applies to this diagnosis and model? What training, maintenance, service, coverage and local availability are confirmed? Evidence is limited for some questions, and a technically suitable device may still be impractical to obtain or support.

What the commonly discussed models illustrate

The 2022 clinical framework compares ReWalk, Ekso, Indego and HAL by powered joints, support, assistance, resistance, feedback and step actuation. It is a description in that paper, not a complete current product directory or a purchasing recommendation.

Model named in the framework Support and assistance described in the paper How to use the information
ReWalk Full trunk and bilateral lower-limb support are described. Ask whether that level of support fits the person’s trunk and leg needs and the intended therapy setting.
Ekso Full trunk and bilateral lower-limb support are described; the paper reports full-to-partial assistance in some configurations. Confirm the exact configuration, current labeling, programming options and supervision requirements.
Indego Full-to-partial assistance is described in some configurations; the paper discusses its powered-joint, feedback and actuation characteristics. Use current manufacturer and clinical-team screening rather than assuming the framework’s description covers every version.
HAL Full-to-partial assistance is described in some configurations; stepping triggers and feedback differ from the other examples. Determine whether its initiation method matches the person’s voluntary control and the treatment objective.

The framework reports differences in resistance, feedback and step actuation among these examples. Those differences should prompt questions during a demonstration, not serve as a ranking. Product names, software and eligibility criteria can change, and the paper does not establish one universally best device.

Screen the person before a device demonstration

Body dimensions and range of motion

Measure the dimensions required by the model and check hip, knee and other relevant joint ranges in the position used for fitting. A frame that cannot align with the person’s joints, or that reaches a limit during stepping, may be unsafe or unable to deliver the intended movement. Confirm that the adjustment range is adequate for clothing, footwear and any orthoses used during the session.

Trunk, upper-body and balance capacity

Even when the exoskeleton powers the legs, the user may need to maintain the trunk, operate controls, use crutches or a walker, and respond to therapist instructions. Evaluate the strength and endurance required by the specific model. Do not infer that leg motors eliminate the need for upper-body capacity.

Skin, bone and musculoskeletal tolerance

Inspect skin before and after each session at cuffs, straps, pelvic supports and other interfaces. Consider sensation, pressure-injury history and the person’s ability to report discomfort. Bone health matters because upright loading and a fall can produce injury in someone with low bone density. The clinician should decide whether loading, duration and progression are appropriate.

Spasticity, contracture and movement control

Spasticity and restricted range can affect alignment, comfort and the ability to complete the programmed gait cycle. Document when tone appears, whether it changes with speed or fatigue, and whether the device offers a safe way to accommodate it. A model’s nominal joint range is not a substitute for an examination of the individual.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Cardiovascular, respiratory and standing tolerance

Upright exercise can challenge blood pressure, heart rate and endurance. Establish how the person responds to supported standing and repeated steps, what monitoring is available and which symptoms require stopping. The first session should not be treated as proof that longer or unsupervised use is safe.

Cognition, communication and assistance needs

The user must be able to understand cues or have a reliable support plan. Consider memory, attention, communication, judgment, fatigue and the availability of a trained helper. Personal-use decisions require a realistic plan for charging, transport, transfers, emergencies and maintenance, not just the ability to complete a few steps in a clinic.

Run a structured, supervised trial

  1. Set baseline measures. Record the functional goal, standing tolerance, assistance level, step count or distance used in ordinary therapy, symptoms and skin status.
  2. Confirm the team and environment. Identify the prescribing clinician, trained operators, spotters, transfer equipment, emergency procedures and a clear area suitable for the model.
  3. Fit and align the device. Follow the model’s current instructions for dimensions, harnesses, cuffs, footwear and control setup. Recheck alignment before powering the joints.
  4. Begin with the prescribed assistance. Use the manufacturer’s start-up sequence and the therapist’s plan. Do not improvise a lower-support setting on the first attempt.
  5. Evaluate step initiation. Observe whether the person can produce the required cue—such as a weight shift, trunk movement or voluntary signal—consistently and without unsafe compensations.
  6. Monitor the whole session. Watch skin, pain, alignment, blood pressure or other clinically indicated measures, fatigue, spasticity and the quality of each step. Stop for concerning symptoms or loss of safe control.
  7. Progress only when justified. If the goal includes active participation, the therapist may test less assistance, a different gait phase or altered feedback according to the model’s capabilities.
  8. Debrief and document. Record assistance settings, duration, number or quality of steps, required staff, adverse symptoms and whether the session advanced the stated goal.
  9. Reassess suitability. Continue, modify or discontinue the trial based on repeated observations, not on a single successful demonstration.

Separate supervised therapy from personal or community use

A device can be useful in a rehabilitation facility without being suitable for independent use at home or in public. Institutional therapy usually provides trained operators, transfer equipment, a controlled surface and immediate clinical monitoring. Personal use adds uneven terrain, doors, crowds, transportation, charging, storage, maintenance and the possibility of delayed help after a problem.

The 2024 expert perspective on personal powered exoskeletons after spinal cord injury recommends first evaluating interest and suitability, then ambulation capacity. It notes that specific criteria vary by device. Those recommendations address a particular population and personal-use context; they should not be generalized to every rehabilitation patient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before considering community use, ask the team to document where the device can be used, who will assist, how the user will recover from a stop or fault, how servicing is obtained and which environments are excluded by the labeling. A successful supervised session is evidence for further assessment, not permission for unsupervised walking.

Understand evidence, clearance and expected outcomes

The clinical framework reports limited evidence to guide optimal everyday utilization. The 2024 personal-use perspective also identifies gaps in candidate selection and outcomes. Therefore, an exoskeleton should be presented as one possible component of a selected rehabilitation plan—not as a guaranteed route to independent walking, neurological recovery or a particular functional result.

FDA classification and model-specific marketing authorization indicate that a device meets applicable regulatory requirements for its intended use. They do not replace an examination, prescription, fitting session or therapist-led risk assessment. Keep the geography explicit: the Class II and regulation 890.3480 description applies to the United States, while other countries use their own regulatory systems.

NICE’s 2017 briefing described Ekso GT as a therapy device for gait training rather than independent walking. That statement is historical and specific to that device and briefing; it should not be extended automatically to every current exoskeleton.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Budget and access questions to verify locally

Current prices, training fees, maintenance contracts and insurance coverage vary by model, country, provider and setting. Confirm them with the clinical facility and manufacturer rather than relying on old figures or marketplace listings. A 2017 NICE briefing in the United Kingdom printed an estimated £98,000 NHS package and a £1,650 monthly trial option. Those are historical UK figures from 2017, not present-day quotes and not evidence of what a patient will pay elsewhere.

Ask for a complete access estimate that includes:

  • evaluation and prescription appointments;
  • operator and user training;
  • fitting and follow-up programming;
  • delivery, charging equipment and storage;
  • planned maintenance, repairs and replacement parts;
  • facility or community-use requirements;
  • insurance, public or private coverage and authorization steps; and
  • the cost of a supervised trial before any long-term commitment.

Do not purchase a prescription exoskeleton or a purportedly compatible accessory through a general marketplace without the treating team confirming compatibility, labeling and service support.

Questions to take to the rehabilitation team

  • What exact functional goal will this model address, and how will we measure progress?
  • Which deficit is the device compensating for: trunk control, stance, swing clearance, balance, joint movement or step initiation?
  • Which joints are powered, and can assistance be reduced or targeted as ability changes?
  • What action starts a step, and can I reliably perform that action?
  • What feedback will I receive, and what can the therapist program?
  • Do my dimensions, range of motion, skin, bone health, spasticity, cardiovascular tolerance and upper-body capacity meet this model’s current criteria?
  • Who will fit, operate and maintain the device?
  • Is this model limited to supervised therapy, or is personal use authorized under specified conditions?
  • What training, follow-up, emergency procedure and service access are available locally?
  • What evidence applies to my diagnosis and goal, and which outcomes remain uncertain?

Common selection mistakes and safer alternatives

Choosing by brand or motor count

A recognizable name or a larger number of powered joints does not show that the system matches the person’s impairment. Start with the goal, support requirement and initiation method, then compare models that meet those needs.

Treating clearance as a prescription

Regulatory status is not an individualized medical decision. Have the prescribing clinician and trained device team complete the model-specific screening and fitting.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assuming one set of eligibility limits applies everywhere

Height, weight, hip width, range of motion, bone health and other thresholds differ by model and can change with labeling. Use the current instructions for the exact configuration under consideration.

Skipping the progression plan

A demonstration that uses maximum assistance may not show whether the device supports active practice. Agree in advance how the therapist will adjust assistance, feedback and gait-phase targets as the person responds.

Confusing a clinic result with community readiness

Controlled floors, trained staff and immediate support are not the same as home or public environments. Treat community use as a separate suitability decision with its own training and safety requirements.

Final decision framework

Proceed to a supervised trial only when the team can state the goal, identify the impairment being addressed, verify fit and health criteria for the exact model, explain how a step is initiated and define how success will be measured. If those answers are unclear, more clinical assessment is needed before comparing products. When the answers are clear, compare the available systems by support, adjustability, feedback, handling, evidence and local access—not by marketing claims or a generic ranking.
