How to Choose a Mental Health App Without Relying on an AI Chatbot

You can choose a mental health app without making an AI chatbot the center of care. Start by defining what you want the app to do, then check whether its evidence, privacy practices, safety limits and day-to-day usability fit that purpose. A non-chatbot format—such as guided audio, structured lessons, journaling or symptom tracking—is not, by itself, proof that an app is effective, private or clinically safe.

Start with what you need the app to do

Choose one specific goal before browsing: for example, building a routine, practising guided mindfulness, learning about a topic or recording symptoms to discuss with a clinician. Then identify the app’s stated purpose. Is it general wellbeing support, self-guided education, symptom tracking, or does it claim to diagnose, treat or manage a mental health condition?

That distinction matters. A product’s intended purpose and functionality can affect what evidence and regulatory checks are relevant. In the UK, the MHRA explains how digital mental health technologies may qualify as medical devices depending on those factors; see its digital mental health technology qualification and classification guidance. A wellbeing label does not prove a product is unsafe, but it may mean it has not undergone the same checks as a regulated medical device.

Choose a format that does not depend on a chatbot

Apps can deliver support in different ways: structured modules, audio exercises, journaling prompts, symptom logs, peer support, clinician-linked care or interactive AI. If you do not want to rely on a chatbot, look for a product whose useful functions are available without one, and check whether AI is used elsewhere in the app. The American Psychiatric Association’s App Evaluation Model asks whether an app uses AI and whether it explains that use.

These formats are options, not endorsements. A journaling tool or audio exercise is not automatically effective just because it avoids conversational AI. The relevant question is whether the app’s particular features suit your goal and are supported by suitable evidence.

Check whether the evidence matches the app’s claims

Look beyond a “clinically proven” label or references to therapy. Ask whether this specific app—with the features you plan to use—was evaluated for its stated purpose among people like you. A therapeutic method may have evidence in general without evidence that a particular app implements it effectively.

  • What exactly was evaluated: the whole app, one feature or a related method?
  • Who took part, and do they resemble the app’s intended users?
  • What outcome was measured, and does it match the app’s promise?
  • Does the app explain its testing or evaluation, such as a clinical study?

The MHRA’s consumer guidance on mental health apps and technologies advises checking what a product claims and what evidence supports it, and warns against big promises without support. The APA model also prompts users to consider clinical accuracy, the foundation for the app’s content, effectiveness evidence and whether intended users were represented.

No single study design is a universal pass/fail test. The APA notes that few apps will have a gold-standard randomized, double-blind, placebo-controlled study. Its framework is intended to help people weigh evidence and circumstances, not to produce a universal score or guarantee of benefit.

Read the privacy terms before sharing sensitive information

Before entering intimate details, find out what information the app collects, why it uses it, where it is stored and who may receive it. Also look for security information, options to limit collection or opt out, how to delete an account and its data, and what the company says about responding to a breach.

A clear privacy policy is a starting point, not independent proof that a company’s technical practices match its promises. The APA’s evaluation model includes questions about data collection, use, sharing and deletion; its health advisory on generative AI chatbots and wellness apps urges caution with sensitive or personally identifying information and recommends checking settings and deletion options. If the policy is missing or too vague to answer basic questions, do not treat the app as suitable for sensitive disclosures.

Do not assume that every health or wellness app is covered by HIPAA. The APA’s Digital Mental Health resource discusses privacy and HIPAA as a specific legal consideration; whether the law applies depends on the entity and context. Rules also vary by jurisdiction.

Look for clear limits and a route to human help

Check whether the app explains what it can and cannot do, how it handles safety concerns, and where it directs someone who needs human support. An app should not leave you to infer that automated features can provide care beyond their stated scope.

The MHRA says digital mental health technologies are not a replacement for professional healthcare and advises people experiencing mental health difficulties to seek support from trained professionals. The APA likewise cautions against relying on generative-AI chatbots or wellness apps for psychotherapy or psychological treatment. If you need care, use the app only as a possible adjunct—not as a substitute for a qualified professional.

Check whether it fits your everyday needs

A tool is only useful if you can access and use it. Before paying or sharing data, check the intended age group and audience, supported language, accessibility features, reading level, compatible device and operating system, and whether it needs a reliable internet connection. Confirm how to get support and what the app costs, including any paid features needed for your goal.

If a trial or sample feature is available, try a short lesson or exercise before committing. Consider whether the instructions are understandable, the activity feels manageable and the experience aligns with the purpose you chose. These are personal-fit checks, not evidence that the app works clinically.

Compare options side by side

If you are choosing between apps, use the same questions for each one. A comparison can expose trade-offs; it cannot produce a universal winner, because the right choice depends on your circumstances.

What to compare Questions to ask
Purpose and claims What does it claim to do? Is it wellbeing support or a product with a medical purpose?
Evidence What was evaluated, for whom, and against which outcome or feature?
Privacy and control What data are collected or shared? Can you limit collection, opt out or delete your data?
Safety and scope Are the app’s limits and safety steps clear? Does it point to human or professional support when needed?
Access and usability Does it fit your age, language, accessibility, device, connectivity and cost needs?
Transparency and maintenance Does the provider disclose ownership, funding, updates, support and AI use?

The APA cautions that its evaluation framework is not a universal pass/fail label: there is no minimum or maximum number of levels that makes an app “good.” Use the questions to understand the trade-offs and decide whether a particular product fits your goal and care context.
