For most people, the best choice is an automatic upper-arm blood pressure monitor with a cuff that fits their measured arm and a model validated for clinical accuracy. Wrist monitors are a fallback when an upper-arm cuff cannot be used for medical or fit reasons, or a correctly sized upper-arm cuff is unavailable. The American Heart Association (AHA) generally does not recommend wrist or finger monitors because their readings are less reliable.
Upper-arm or wrist: which type should you choose?
| Factor | Upper-arm monitor | Wrist monitor |
|---|---|---|
| Usual choice | Preferred for most people; choose an automatic cuff-style model. | Conditional alternative when an upper-arm cuff cannot appropriately be used or obtained. |
| Fit and placement | The cuff must fit your measured upper-arm circumference and sit on bare skin. | Positioning is especially important: keep the wrist at heart level and follow the model’s instructions. |
| What to verify | Check the exact model’s validation and confirm the cuff range includes your arm measurement. | Check the exact model’s validation and ask a healthcare professional whether it suits your circumstances. |
The AHA’s general advice is to choose an automatic upper-arm monitor. Its patient guidance says wrist and finger monitors are less reliable and are not generally recommended. That does not make wrist devices unusable in every case: the American Medical Association (AMA) advises considering one when an upper-arm cuff is clinically inappropriate or an appropriately sized upper-arm cuff is unavailable. There is no established head-to-head figure here for how many millimeters of mercury one design differs from the other, so avoid treating either type as a guaranteed substitute for correct fit and technique.

How to choose a validated monitor that fits
- Measure your upper arm. Measure around the arm and compare the result with the cuff-size range stated for the monitor. A cuff that is too small or too large can distort the reading. If the included cuff does not fit, look for a compatible cuff in the right size or a different device; compatibility is model-specific. The AHA explains the importance of choosing the correct cuff size in its home blood pressure monitoring guidance.
- Check the exact model in an independent validation listing. Search the US Blood Pressure Validated Device Listing, which the AMA describes as a resource for identifying devices validated for clinical accuracy. Do not assume that a broad marketing claim, FDA clearance, or a smartwatch feature proves independent clinical validation. The AMA listing covers multiple categories, including upper-arm and wrist devices, and its inventory can change.
- Consider whether you can use the cuff correctly. If an upper-arm cuff cannot be used for a medical or fit reason, or no correctly sized upper-arm option is available, ask a healthcare professional whether a validated wrist model is suitable. Do not choose wrist solely for convenience without considering its positioning requirements.
- Bring the monitor to an appointment. Ask a healthcare professional to check that it fits and that you are using it correctly, then compare its readings with office equipment. The AHA advises bringing it about annually or as directed by the manufacturer.
Once validation and fit are settled, consider practical features such as whether the monitor stores readings or makes them easy to record. Memory can help with recordkeeping, but it does not replace validation or correct measurement technique. The AMA reported 57 validated devices across several categories when its listing description was accessed on October 4, 2026; that is a time-sensitive count, not a current model recommendation or a comparison of prices.

- For the 30 minutes before measuring, avoid smoking, caffeinated drinks, and exercise.
- Empty your bladder, then sit quietly for at least five minutes. Do not talk or use your phone while resting or measuring.
- Sit with your back supported and feet flat on the floor. Put the cuff on bare skin, not over clothing.
- Support your arm so the cuff’s measurement point is at heart level. For a wrist monitor, carefully position the wrist at heart level as directed by its instructions.
- Take two readings one minute apart. Measure at consistent times each day where possible, and record the results.
The CDC’s measuring guidance also advises using bare skin, resting the arm with the cuff at chest height, and staying quiet during measurement. Ask your healthcare professional how often you should measure and how to interpret your readings. AHA-reported advice from HealthPartners Institute researcher Karen Margolis in 2024 described about a dozen properly taken readings over one to two weeks as potentially helpful for determining hypertension or adjusting medication; this is contextual expert advice, not a universal diagnostic protocol.

Keep a log that you can share with your healthcare professional. A monitor that stores readings may make that easier; as epidemiologist Paul Muntner told the AHA in 2020, “When blood pressure monitors cannot transmit readings to a patient’s doctor, the next best thing is to have a device that stores the blood pressure readings.” A paper or digital log works too, and you do not need to buy a logbook if you prefer another way to record results.
