How to Choose a Biological Age Test: Blood, DNA, and Epigenetic Options

Choose a biological-age test by the question it is designed to answer—not by whether it says “blood,” “DNA,” or “epigenetic” on the label. A blood sample might be used for routine clinical biomarkers or for a DNA-methylation clock, which are different methods. Before paying, check the exact measurements, algorithm, intended use, validation evidence, and uncertainty the provider reports. No biological-age score is a diagnosis, a reliable personal lifespan forecast, or proof that an intervention works.

What question do you want the test to answer?

“Biological age” is a model-based estimate from selected measurements, not a single standardized quantity. A result might estimate age, compare a person’s score with a reference group (sometimes called age acceleration), or estimate a pace of aging. Those outputs are not interchangeable. Start by checking what the test actually reports and what decision, if any, that result is intended to inform.

  • General curiosity: Treat the result as an estimate with limits, not as a clinical finding.
  • Health concern or symptoms: A general wellness score is not a substitute for medical evaluation or recommended screening.
  • Tracking change over time: Look for information on repeatability, uncertainty, and whether the method is intended to detect changes within an individual. A difference between two scores is not automatically evidence that your health or aging rate changed.

What do blood, DNA, and epigenetic tests measure?

Clinical blood-biomarker composites

Some approaches combine routine clinical blood biomarkers in a model to estimate aspects of aging. The particular analytes and calculation matter: “blood-based” alone does not tell you what was measured or what the score means. The National Institutes of Health’s discussion of the CALERIE caloric-restriction study describes clinical blood-biomarker measures separately from a DNA-methylation algorithm for pace of aging, illustrating why specimen type does not define the method: NIH: Can we slow aging?

DNA-methylation clocks

Epigenetic clocks use patterns of DNA methylation at selected sites to estimate age-related measures. A test may use a blood sample to measure those patterns, but it is still a methylation-based method—not simply a composite of routine clinical blood markers. Clocks differ in how they are built and what they estimate. Some second-generation clocks were designed around outcomes related to morbidity or mortality, but that does not mean every clock accurately predicts an individual’s future health or lifespan. A review discusses both the promise of these measures and challenges in validity and interpretation, including the possible influence of population and environmental factors: PubMed: Quantification of Epigenetic Aging in Public Health and Annual Review of Public Health: Quantification of Epigenetic Aging in Public Health.

Disease-specific blood tests are a different category

A blood test developed to address a particular disease question should not be treated as evidence for general-purpose biological-age testing. For example, an NIH summary of 2026 research describes a model using repeated blood p-tau217 measurements to estimate Alzheimer’s symptom onset. The study included 603 older people and reported a median absolute error of three to four years for that specific estimate. This is a disease-specific research result, not a performance statistic for biological-age tests generally: NIH: Blood test predicts start of Alzheimer’s disease symptoms.

How to evaluate a test before buying

  1. Identify the reported output. Is it an age estimate, a comparison with a reference group, a pace-of-aging estimate, or something else? Check the stated intended use rather than inferring it from the test’s name.
  2. Find out what is measured. Check the specimen, analytes or methylation sites, laboratory process, and algorithm. If the provider says only “blood test” or “DNA test,” ask for the specific method.
  3. Check validation for that exact method and claim. Look for the population studied and the outcome used. Evidence that a score is associated with an outcome in a research group does not, by itself, show that it accurately predicts an individual’s outcome or improves decisions.
  4. Look for uncertainty and repeatability information. Check whether the report explains measurement uncertainty, reference groups, factors that may affect results, and what repeat testing can—and cannot—show.
  5. Read the consumer guidance and limitations. Check whether the provider explains follow-up, medical decision-making, and how to interpret an unexpected result.

The FDA says direct-to-consumer tests vary in their supporting evidence and may or may not undergo premarket review. Its review framework considers whether a test reliably measures what it claims (analytical validity), whether the measurement predicts the stated health condition (clinical validity), the company’s claims, and whether consumers can understand the directions and results: FDA: Direct-to-Consumer Tests. FDA biomarker qualification also applies to a specific context of use; it does not necessarily qualify the measurement method itself: FDA: Focus Area—Biomarkers.

Can a score tell you how long you will live?

Not on its own. A group-level association between a model’s score and an outcome does not establish an accurate lifespan forecast for an individual. Nor does a score show that changing a behavior or following a particular intervention will improve health. Epigenetic aging measures, in particular, continue to face questions about validity and translation into practice, and their interpretation depends on the clock and context.

The FDA advises consumers not to use direct-to-consumer test results as the sole basis for medical decisions or as a replacement for recommended screening and clinical follow-up. Discuss a concerning or surprising result with a health care provider: FDA: Direct-to-Consumer Tests.

How to compare two test reports

Do not assume two providers’ scores—such as “years younger”—share a common scale. Compare the method, intended output, reference group, validation population, and uncertainty disclosures. If those details differ or are not stated, the scores cannot be meaningfully treated as equivalent just because both are called biological age.
