The public tool lists reported breaches affecting 500 or more people; it is not an individual lookup service, and a missing result does not prove your records were safe. For confirmation about your own account, contact the organization directly.
Check the notice and the HHS breach listing
Look for a notice from the organization
Check postal mail and email for a breach notice from your healthcare provider, health plan, or another health-related service. Under HHS guidance, an individual HIPAA breach notice should explain what happened, the types of information involved, steps you can take to protect yourself, what the organization is doing in response, and how to contact it. The organization must notify affected individuals without unreasonable delay and no later than 60 days after discovering the breach.

If you receive a notice, read it for the incident date, the data categories involved, and any recommended protections. If it is unclear whether you were affected, use the organization’s official website, patient portal, or the phone number on your insurance card to confirm. Do not rely on contact details or links in a message you suspect may be fraudulent.

Search the HHS OCR public breach tool
Search the HHS Office for Civil Rights HIPAA Breach Reporting Tool using the provider or health plan’s legal name, system name, or details from a notice. HHS says OCR investigates reported HIPAA breaches affecting 500 or more individuals and publishes those reports in the public listing. A listed incident can show that an organization reported a large breach; it does not, by itself, confirm whether a particular person’s records were involved.

Understand what a missing result means
The public listing is not a real-time, comprehensive personal exposure checker. Its large-breach listings concern incidents affecting 500 or more people. For incidents affecting fewer than 500 people, an organization may report to HHS within 60 days after the end of the calendar year in which it discovered the incident, or report sooner. As a result, a smaller incident may not appear promptly. No result is not an all-clear.
Contact the provider or health plan for an individual answer
If you have not received a notice, cannot find a relevant listing, or need to know whether your own records were included, contact the provider or plan through an official channel. Ask:

- Did you have a security incident or data breach involving my account or records?

- When did the incident occur or get discovered, and what steps should I take?
- Where can I get help if I see unfamiliar care or insurance claims?
The organization is the appropriate source to confirm individual inclusion. There is no universal database identified here that can verify a person’s exposure across all providers and health-related services.
Check consumer health apps separately
Do not assume that every app or online service holding health information appears in the HHS HIPAA listing. HIPAA does not cover every consumer health app or personal health record service. The Federal Trade Commission’s Health Breach Notification Rule applies to some personal health record vendors and connected services outside HIPAA. If an app or service stores your health data, check its own breach notices and contact its support or privacy team through its official site.

Look for signs that someone used your medical identity
Review insurance explanations and medical records
Look through explanations of benefits from your insurer and your medical records for care, claims, providers, or services you do not recognize. An unfamiliar item may warrant follow-up with the insurer or provider to determine whether it is an error or unauthorized use. Contact them through an official number or portal and ask how to dispute or investigate it.
Use the FTC recovery route if benefits or care were used
If someone appears to have used your information to obtain medical care or use your insurance benefits, the FTC directs consumers to IdentityTheft.gov for a recovery plan. Keep copies of suspicious notices, claims, and correspondence, and follow the instructions from your insurer, provider, and the recovery plan.

What each checking route can tell you
| Route | What it can tell you | Limitation |
|---|---|---|
| Notice from the organization | Whether the organization says your information may have been involved, what types of information were affected, and what steps it recommends. | You may not have received or recognized the notice; confirm a suspicious message through official contact channels. |
| HHS OCR HIPAA breach listing | Whether a covered HIPAA organization reported a listed incident affecting 500 or more people. | It is not an individual lookup, does not cover every health app, and smaller incidents may be reported later. |
| Direct contact with the organization | An organization can address whether your account or records were included and explain its recommended next steps. | Use verified official contact details; there is no single cross-provider service established here that checks every health-data source. |
